1.Injectable Drugs in Diabetes Treatment: Insulin versus Incretin

Type 2 diabetes mellitus (T2DM) is a chronic condition characterized by hyperglycemia. While lifestyle modifications and oral medications are initial treatments, many patients require injectable therapies like insulin and incretin-based drugs. Insulin therapy, a cornerstone treatment for T2DM, can effectively control blood sugar but is associated with hypoglycemia and weight gain. Incretin-based therapies, such as glucagon-like peptide-1 (GLP-1) receptor agonists offer improved glycemic control, weight loss, and reduced cardiovascular risk. Recent studies have shown the superiority of incretin-based therapies over basal insulin. Among these, GLP-1/GIP (glucose-dependent insulinotropic polypeptide) receptor dual agonists like tirzepatide have demonstrated significant weight loss and improved glycemic control. Selection of injectable therapy should be individualized based on factors like glycemic goals, cardiovascular risk, hypoglycemia risk, and patient preference. While insulin remains a useful option, incretin-based therapies, especially newer agents, offer promising therapeutic benefits for many patients with T2DM.
2.Predictive value and optimal cut-off level of high-sensitivity troponin T in patients with acute pulmonary embolism

Background/Aims:
Elevated troponin levels predict in-hospital mortality and influence decisions regarding thrombolytic therapy in patients with acute pulmonary embolism (PE). However, the usefulness of high-sensitivity troponin T (hsTnT) regarding PE remains uncertain. We aimed to establish the optimal cut-off level and compare its performance for precise risk stratification.
Methods:
374 patients diagnosed with acute PE were reviewed. PE-related adverse outcomes, a composite of PE-related deaths, cardiopulmonary resuscitation incidents, systolic blood pressure < 90 mmHg, and all-cause mortality within 30 days were evaluated. The optimal hsTnT cut-off for all-cause mortality, and the net reclassification index (NRI) was used to assess the incremental value in risk stratification.
Results:
Among 343 normotensive patients, 17 (5.0%) experienced all-cause mortality, while 40 (10.7%) had PE-related adverse outcomes. An optimal hsTnT cut-off value of 60 ng/L for all-cause mortality (AUC 0.74, 95% CI 0.61–0.85, p < 0.001) was identified, which was significantly associated with PE-related adverse outcomes (OR 4.07, 95% CI 2.06–8.06, p < 0.001). Patients with hsTnT ≥ 60 ng/L were older, hypotensive, had higher creatinine levels, and right ventricular dysfunction signs. Combining hsTnT ≥ 60 ng/L with simplified pulmonary embolism severity index ≥1 provided additional prognostic information. Reclassification analysis showed a significant shift in risk categories, with an NRI of 1.016 ± 0.201 (p < 0.001).
Conclusions
We refined troponin’s predictive value in patients with acute PE, proposing a new cut-off value of hsTnT ≥ 60 ng/L. Validation through large-scale studies is essential to offer clinically useful guidance for managing patient population.
3.Clinical approach to sexually transmitted infections and pelvic inflammatory disease in women with acute pyelonephritis

Background/Aims:
This study aimed to investigate co-occurrence and clinical characteristics of sexually transmitted infections (STIs) and pelvic inflammatory disease (PID) in women hospitalized for acute pyelonephritis (APN).
Methods:
This single-center retrospective study reviewed medical records of inpatients with APN from January 2019 to February 2023 and identified records of 142 patients who were referred to a gynecologist to evaluate gynecological diseases including STIs.
Results:
Of the 142 patients, 47 were tested positive for sexually transmitted pathogens in nucleic acid amplification testing, confirming the presence of STIs. In patients with APN, those with STIs were more likely to have lower abdominal pain or cervical motion tenderness (CMT) on pelvic examination and leukocytosis (> 14.5 × 109/L) than those without STIs. Of the 93 patients who underwent pelvic examination, 34 had CMT with one or more of additional criteria for the clinical diagnosis of PID, such as abnormal vaginal discharge and leukorrhea confirmed by microscopic examination, which could be clinically diagnosed as PID.
Conclusions
In sexually active women with APN, it is important to evaluate the possibility of STIs and PID, considering several risk factors such as lower abdominal pain, abnormal vaginal discharge, CMT, and leukocytosis.

5.Effects of obtaining plain radiographs in pediatric patients with radial head subluxation

Purpose:
This study investigated the rate and factors of spontaneous reduction (SR) during plain radiography in pediatric patients with radial head subluxation (RHS).
Methods:
We retrospectively reviewed patients aged 5 years or younger with RHS who visited a tertiary hospital emergency department (ED) from March 2015 through February 2022. They were classified into the SR during X-ray, manual reduction (MR) after X-ray, and MR-first groups. We compared the clinical variables, such as ED length of stay, among the 3 groups, and investigated factors associated with SR during radiography.
Results:
Among a total of 550 enrolled patients, 153 (27.8 %), 177 (32.2%), and 220 (40.0%) belonged to the SR during X-ray, MR after X-ray, and MR-first groups, respectively. Among the groups, no difference was found in the proportions of indoor injury (SR during X-ray, 92.6% vs. MR after X-ray, 79.0% vs. MR-first, 89.0%; P = 0.066). The mean ED length of stay was shorter in the MR-first group than in the equivalent values of the other groups (23.1 minutes vs. 49.0-53.0 minutes; P < 0.001), without differences in the other time intervals. The sole factor associated with SR during radiography was the indoor injury (odds ratio, 3.32; 95% confidence interval, 1.01-10.88; P = 0.048).
Conclusion
Based on the results of this study, emergency physicians or pediatricians might consider obtaining radiographs first in patients with RHS injured indoors, which can cause SR and exclusion of complications such as iatrogenic injury.

9.Comparative Evaluation of Pre-Test Probability Models for Coronary Artery Disease with Assessment of a New Machine Learning-Based Model

Purpose:
This study aimed to validate pivotal pre-test probability (PTP)-coronary artery disease (CAD) models (CAD consortium model and IJC-CAD model).
Materials and Methods:
Traditional PTP models-CAD consortium models: two traditional PTP models were used under the CAD consortium framework, namely CAD1 and CAD2. Machine learning (ML)-based PTP models: two ML-based PTP models were derived from CAD1 and CAD2, and used to enhance predictive capabilities [ML-CAD2 and ML-IJC (IJC-CAD)]. The primary endpoint was obstructive CAD. The performance evaluation of these PTP models was conducted using receiver-operating characteristic analysis.
Results:
The study included 238 participants, among whom 157 individuals (65.9% of the total sample) had CAD. The IJC-CAD model demonstrated the highest performance with an area under the curve (AUC) of 0.860 [95% confidence interval (CI): 0.812– 0.909]. Following this, the ML-CAD2 model exhibited an AUC of 0.814 (95% CI: 0.758–0.870), CAD1 showed an AUC of 0.767 (95% CI: 0.705–0.830), and CAD2 had an AUC of 0.785 (95% CI: 0.726–0.845). Each of the PTP models was adjusted to have a CAD score cutoff that classified cases with a sensitivity of over 95%. The respective cutoff values were as follows: CAD1 and CAD2 >12, MLCAD2 >0.380, and IJC-CAD >0.367. All PTP models achieved a CAD sensitivity of over 95%. Similar to the AUC performance, the accuracy of the PTP models was highest for IJC-CAD, reaching 80.3%. The accuracy of ML-CAD2 was 77.7%, while that for CAD1 and CAD2 was 74.8% and 75.2%, respectively.
Conclusion
ML-CAD2 and IJC-CAD showed superior performance compared to traditional existing models (CAD1 and CAD2)
10.Differences in Treatment Outcomes Depending on the Adjuvant Treatment Modality in Craniopharyngioma

Purpose:
Adjuvant treatment for craniopharyngioma after surgery is controversial. Adjuvant external beam radiation therapy (EBRT) can increase the risk of long-term sequelae. Stereotactic radiosurgery (SRS) is used to reduce treatment-related toxicity.In this study, we compared the treatment outcomes and toxicities of adjuvant therapies for craniopharyngioma.
Materials and Methods:
We analyzed patients who underwent craniopharyngioma tumor removal between 2000 and 2017. Of the 153 patients, 27 and 20 received adjuvant fractionated EBRT and SRS, respectively. We compared the local control (LC), progression-free survival (PFS), and overall survival between groups that received adjuvant fractionated EBRT, SRS, and surveillance.
Results:
The median follow-up period was 77.7 months. For SRS and surveillance, the 10-year LC was 57.2% and 57.4%, respectively. No local progression was observed after adjuvant fractionated EBRT. One patient in the adjuvant fractionated EBRT group died owing to glioma 94 months after receiving radiotherapy (10-year PFS: 80%). The 10-year PFS was 43.6% and 50.7% in the SRS and surveillance groups, respectively. The treatment outcomes significantly differed according to adjuvant treatment in nongross total resection (GTR) patients. Additional treatment-related toxicity was comparable in the adjuvant fractionated EBRT and other groups.
Conclusion
Adjuvant fractionated EBRT could be effective in controlling local failure, especially in patients with non-GTR, while maintaining acceptable treatment-related toxicity.
