1.Comparison of aprepitant, dexamethasone, and ondansetron with dexamethasone and ondansetron for prevention of postoperative nausea and vomiting in high-risk patients undergoing laparoscopic surgeries:a randomized controlled trial in India

Purpose:
Following laparoscopic surgeries, patients often experience discomfort and dissatisfaction due to postoperative nausea and vomiting (PONV), despite using a multimodal pharmacological approach. The study compared the effects of triple drug prophylaxis with aprepitant-dexamethasone-ondansetron versus dual drug prophylaxis with dexamethasoneondansetron in high-risk patients undergoing laparoscopic surgeries.
Methods:
This randomized controlled trial enrolled 201 female nonsmokers who used opioids postoperatively and underwent elective laparoscopic surgery. Patients were allocated into two groups: dual prophylactic antiemetics and placebo (control group), and triple prophylactic antiemetics with preoperative administration of aprepitant 80 mg (group A). Group A received oral aprepitant 80 mg 2 hours before the surgery, and the control group received a placebo.Patients were assessed at two time points: 2 hours after surgery, either in the postanesthesia care unit or in the ward, and 24 hours after surgery in the ward.
Results:
Patients in group A reported a PONV incidence of 10% as compared to 22.7% in the control group (relative risk,0.43; 95% confidence interval [CI], 0.22–0.89; adjusted p = 0.01) at 2 hours in the postoperative period. At 24 hours postoperatively, patients in group A experienced significantly lower incidence of PONV (10%) compared to the control group (29.7%; relative risk, 0.33; 95% CI, 0.17–0.64; p < 0.001). Also, the need for rescue antiemetics was reported to be lower in patients in group A. No adverse effects were reported with any of the drugs used.
Conclusion
The study suggests that prophylaxis with a triple antiemetic regimen consisting of aprepitant-dexamethasone-ondansetron results in a lower incidence of PONV compared with a two-drug regimen using dexamethasone-ondansetron in high-risk patients with three or more risk factors.

The effects of the coronavirus disease 2019 (COVID-19) pandemic on male fertility have received considerable attention because human testes contain high levels of angiotensin-converting enzyme-2 receptors, through which severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) can enter. Early studies showed decreases in semen quality during and after recovery from COVID-19. However, no semen quality studies have examined the effects of widespread subclinical and mild disease, as well as changes in lifestyle, psychosocial behavior, intake of dietary supplements, and stress. This cross-sectional study compared semen quality parameters in male partners of infertile couples between men who underwent semen analysis before the COVID-19 pandemic (prepandemic group) and men who underwent semen analysis during the pandemic period (pandemic group); the analysis sought to clarify the overall effects of the pandemic. No participants in the pandemic group had experienced clinically overt disease. Among the 239 participants, mean body weight (P = 0.001), mean body mass index (P < 0.001), median sperm concentration (P = 0.014), total sperm count (P = 0.006), and total percentages of motile (P = 0.013) and abnormal cells (P < 0.001) were significantly greater in the pandemic group (n = 137) than those in the prepandemic group (n = 102). Among abnormal cells, the percentages of cells with excess residual cytoplasm (P < 0.001), head defects (P < 0.001), and tail defects (P = 0.015) were significantly greater in the pandemic group than those in the prepandemic group. With the exception of morphology, the overall semenogram results were better in the pandemic group than those in the prepandemic group.

4.Effect of addition of buprenorphine or dexamethasone to levobupivacaine on postoperative analgesia in ultrasound guided transversus abdominis plane block in patients undergoing unilateral inguinal hernia repair: a prospective randomized double blind controlled trial

BACKGROUND:
The transversus abdominis plane (TAP) block is an effective technique to block the thoracolumbar nerves innervating the anterolateral abdominal wall. This study was conducted to evaluate the analgesic efficacy and opioid consumption with the use of perineural buprenorphine or dexamethasone in TAP blocks after unilateral inguinal hernioplasties.
METHODS:
This prospective, randomized, double-blinded, placebo-controlled study enrolled 93 patients scheduled for unilateral inguinal hernioplasty, followed by an ultrasound-guided TAP block. The participants were randomized into 3 groups (31 patients each). Group L received 20 ml 0.25% levobupivacaine + 1 ml normal saline (NS); group LB, 20 ml 0.25% levobupivacaine + 0.3 mg (1 ml) buprenorphine; and group LD, 20 ml 0.25% levobupivacaine + 4 mg (1 ml) dexamethasone. The patients were observed postoperatively for 24 h for first rescue analgesic requirement, total rescue analgesic consumption, and pain scores on the numeric rating scale (NRS).
RESULTS:
The time to first rescue analgesic requirement was significantly longer in Group LB than in groups LD and L (688.87 ± 36.11 min, 601.45 ± 39.85 min, and 383.06 ± 36.21 min, respectively; P < 0.001). The mean total tramadol consumption in the first 24 h was the lowest in group LB (P < 0.001, L vs. LB / LD). Groups LB and LD displayed significantly lower NRS scores than group L (P < 0.001 both).
CONCLUSIONS
Levobupivacaine with perineural buprenorphine in a TAP block after unilateral open inguinal hernioplasty facilitates prolonged analgesia and reduced requirement for rescue analgesics compared to perineural dexamethasone, without significant side effects.
