1.Annual review of clinical research on lung transplantation worldwide in 2025

Lung transplantation is currently the only effective treatment for end-stage lung disease. However, the field of lung transplantation still faces numerous challenges, including donor shortage, primary graft dysfunction, rejection, infection and others. In 2025, scholars worldwide have made remarkable progress in clinical research on lung transplantation. This article systematically reviews the cutting-edge developments in clinical research on lung transplantation in 2025, focusing on the application of novel evaluation tools in donor lung screening, optimization of lung perfusion techniques, exploration of extracorporeal membrane oxygenation and robot-assisted minimally invasive surgery, as well as mechanistic research and intervention strategies for primary graft dysfunction, acute kidney injury and chronic lung allograft dysfunction. Meanwhile, hot topics such as the management of special populations, prevention and control of postoperative infections, and improvement of quality of life are summarized. This review aims to conclude the important annual research findings and provide references for the clinical practice and research development of lung transplantation in China.
2.Pathogenesis Evolution Characteristics and Intervention Strategies for Alternation Between Active and Remission Phases of Inflammatory Bowel Disease:Based on the Theory of "Grease-Turbidity-Heat Toxin"

Based on the theoretical framework of "grease-turbidity-heat toxin", this paper elucidates the patholo-gical evolution pattern underlying the alternation between active and remission phases of inflammatory bowel disease (IBD). It is proposed that the active phase of IBD represents a progressive and explosive process characterized by internal accumulation of grease turbidity, constraint transforming into heat, and exuberant heat generating toxin. During the remission phase, the pathogenic process manifests as subsidence of heat toxin, depletion of qi and yin, latent retention of residual turbidity, and a state of coexistence between deficiency of healthy qi and lingering pathogenic factors. Accordingly, a staged therapeutic strategy with dynamic intervention and targeted modulation has been established. During the active phase, treatment follows the sequential principle of unblocking, clearing, and resolving pathogenic factors. In the early stage, the therapeutic approach focuses on regulating qi movement, fortifying the spleen, and resolving turbidity, using Tongxie Essential Formula (痛泻要方) and Shenling Baizhu Powder (参苓白术散) to regulate qi and fortify spleen. In the middle stage, the strategy emphasizes clearing heat, eliminating dampness, and dispersing pathogenic accumulation through downward and outward pathways, and Gegen Qinlian Decoction (葛根芩连汤) combined with Banxia Xiexin Decoction (半夏泻心汤) is used to clear and resolve damp-heat. In the late stage, treatment aims to resolve toxin, restrain sore, cool blood, and stabilize collaterals, using Baitouweng Decoction (白头翁汤), Shaoyao Decoction (芍药汤), and Xijiao Dihuang Decoction (犀角地黄汤) to eliminate toxins and cool blood. During the remission phase, the principle of reinforcing, consolidating, and penetrating is adopted to restore internal balance and clear the source, using Sishen Pills (四神丸) and Sijunzi Decoction (四君子汤) to reinforce healthy qi and venting pathogens.
3.Syndrome Differentiation and Treatment of Diarrhea-Predominant Irritable Bowel Syndrome:Based on the Theory of "Zi Wu Liu Zhu (子午流注)"

Based on the theory of Zi Wu Liu Zhu (子午流注, the ebb and flow of midnight and midday) and the clinical characteristics of diarrhea-predominant irritable bowel syndrome (IBS-D) that its symptoms mainly occur during chou (丑) period (01:00—03:00), mao (卯) period (5:00—7:00), and si (巳) period (9:00—11:00), this paper proposes a temporal pathogenesis chain of "liver constraint during chou period, intestinal stagnation during mao period, and spleen deficiency during si period". Within this chain, liver constraint serves as the initiating factor, and intestinal stagnation manifests as the branch, while spleen deficiency constitutes the root of the disease. Accordingly, a chronotherapeutic principle of "soothing the liver, regulating the intestines, and fortifying the spleen" is proposed, establishing a corresponding time-based syndrome differentiation and treatment system. During chou period, the treatment should focus on soothing the liver to relieve constraint and calming the mind to tranquilize the spirit, thereby blocking the mechanism of liver constraint overacting on the spleen. During mao period, the focus is regulating qi to resolve dampness and harmonizing the intestine to alleviate the branch symptoms of morning abdominal pain and diarrhea. During si period, the suggested method is fortifying the spleen and supplementing qi to drain dampness and stop diarrhea, thereby restoring the pivot of spleen-stomach transportation and transformation. These methods are complemented by strategies to optimize medication compliance, thereby providing a new chronotherapeutic approach for clinical diagnosis and treatment of IBS-D.

5.Analysis of Changes in Key Nuclei of Dopamine System in Early Parkinson's Disease via Free Water Imaging

Purpose To investigate the changes of free water(FW)values in the key basal ganglia of dopamine system in the brain of early Parkinson's disease(PD)by free water imaging,and to reveal the potential relationship between the changes and early PD.Materials and Methods From February to December 2023,62 patients with early PD and 27 healthy controls were enrolled in the Parkinson's Disease Progression Markers Project database,and 25 age-and sex-matched healthy controls were self-recruited from Henan Provincial People's Hospital.All subjects underwent clinical scale assessment and MRI data collection.Free water imaging was used to calculate the FW values of key nuclei in dopamine system,and the difference of FW values between PD group and healthy controls group,the correlation between FW values of PD group and clinical scale scores were analyzed.Results The FW values of compact part of substantia nigra(SNc)and ventral tegmental area(VTA)in PD group were significantly higher than those in healthy controls group(Z=2.458-3.914,all P<0.05).Regression analysis showed that FW values of SNc were correlated with the movement disorder society-sponsored revision unified Parkinson′s disease rating scale(MDS-UPDRS)Ⅲ scores and state-trait anxiety inventory(STAI)scores on both sides(r2=0.223,P<0.001;r2=0.125,P=0.018;r2=0.151,P=0.003;r2=0.128,P=0.017),the FW value of VTA on the left was significantly correlated with MDSUPDRS Ⅲ score(r2=0.143,P=0.004)and the FW value of the right VTA was significantly correlated with the STAI score(r2=0.125,P=0.019).Conclusion The FW values of SNc and VTA in PD patients are significantly increased in the early stage,and are correlated with motor and non-motor symptoms in PD patients.FW values may reflect brain degeneration,and this change may be related to the decline of motor and non-motor function in patients.

7.Value of magnetic resonance imaging-proton density fat fraction in evaluating the degree and distribution characteristics of hepatic steatosis in patients with chronic hepatitis B virus infection

Objective To investigate the value of magnetic resonance imaging-proton density fat fraction(MRI-PDFF)in evaluating hepatic steatosis in patients with chronic hepatitis B virus(HBV)infection.Methods The patients,aged>16 years,who visited the outpatient service or were hospitalized in Beijing Tsinghua Changgung Hospital from January 2018 to December 2022 and were diagnosed with chronic HBV infection were enrolled,and all patients underwent MRI examination of the liver in our hospital.The patients were divided into groups based on the presence or absence of liver cirrhosis,and the consistency in PDFF between different hepatic segments was compared between groups.The Kappa consistency test and intraclass correlation coefficient(ICC)were used for consistency analysis.Results A total of 76 patients treated with nucleoside analogues were enrolled,among whom 23(30.26%)had liver cirrhosis.For all patients,the simple arithmetic average of PDFF fluctuated between 1.49%and 30.93%.According to MRI-PDFF≥5%as the diagnostic criterion for fatty liver disease,there were 29 patients(38.16%)with fatty liver disease among all patients.For all 76 patients,the simple arithmetic average of PDFF was lower than the weighted average of PDFF for the whole liver,and there was no significant difference between the simple arithmetic average of PDFF,the weighted average of PDFF,and the PDFF values of the left and right lobes of the liver(F=0.39,P=0.76).The consistency test showed that the PDFF values of each hepatic segment and the left and right lobes of the liver had strong consistency with the weighted average and simple arithmetic average of PDFF,with an ICC of>0.75,but the consistency between the PDFF value of the right lobe and the weighted average of PDFF was higher than that between the PDFF value of the left lobe and the weighted average of PDFF.In the consistency test of differentiating fatty liver disease in patients with liver cirrhosis,there was poor consistency between the PDFF value of segment Ⅶ and the weighted average of PDFF(Kappa=0.39),with moderate consistency for the left lobe and the Ⅰ,Ⅱ,Ⅲ,Ⅴ,Ⅵ,and Ⅷ segments.For the patients with liver cirrhosis,the lowest consistency was observed between the PDFF value of Ⅶ segment and the weighted average of PDFF for the whole liver,and the highest consistency was observed between the PDFF value of Ⅵ segment and the weighted average of PDFF for the whole liver.For the patients without liver cirrhosis,the lowest consistency was observed between the PDFF value of Ⅱ segment and the weighted average of PDFF for the whole liver,and the highest consistency was observed between the PDFF value of Ⅴ segment and the weighted average of PDFF for the whole liver.Conclusion MRI-PDFF is more comprehensive in evaluating hepatic steatosis in patients with chronic HBV infection,and for the patients with liver cirrhosis,there is poor consistency between the PDFF value of each segment and the weighted average of PDFF.
8.Satisfaction survey on the preliminary assessment of unified elderly care needs evaluation in Shanghai: from the perspective of service recipients

ObjectiveTo analyze the satisfaction of service recipients with the preliminary assessment service of the unified elderly care needs evaluation in Shanghai, so as to provide reference for further optimization of the service system. MethodsA multi-stage random sampling method was employed to recruit participants. A total of 110 individuals who had received unified elderly care needs evaluation service in six districts of Shanghai were investigated using a structured questionnaire. SPSS 25.0 was used to analyze the data. Univariate analyses were conducted using the Chi-square test and Fisher’s precision probability test, while multivariate analysis was performed using ordinal regression. ResultsThere were differences in satisfaction with various aspects of the preliminary assessment process among participants from different areas, with those from outer suburbs generally exhibiting lower satisfaction (P<0.01). Multivariate analysis results indicated that satisfaction with the timing of the preliminary assessment was related to the service recipients’ residential area and their primary source of income; satisfaction with the service attitude and capability of the assessment personnel was only related to the residential area. Satisfaction with the preliminary evaluation results was related to the participants’ marital status, educational level, income level, and the specific rating results. ConclusionThe satisfaction level with the preliminary assessment of the unified elderly care needs assessment in Shanghai is relatively high. In the process of evaluation, a deeper analysis and exploration of individual needs based on various personal information are essential to ensure the appropriateness of the provided elderly care services, and thereby effectively enhancing recipient satisfaction.
9.Analysis of the perception and satisfaction levels of unified elderly care needs assessment personnel in Shanghai

ObjectiveTo clarify the perception of the work and satisfaction levels among personnel conducting unified elderly care needs assessment in Shanghai, to analyze the factors influencing their job satisfaction, and to provide references for optimizing the job satisfaction of unified elderly care needs assessment personnel in Shanghai. MethodsA survey was conducted among 93 assessment personnel randomly selected from 29 elderly care needs assessment agencies in Shanghai. The personnel’s perception of their work and their levels of job satisfaction were examined. Chi-square test and Fisher’s precision probability test were used to compare regional differences, while the two-factor theory was employed to analyze the factors influencing job satisfaction. ResultsRegarding perception of the work, 92.5% of the surveyed personnel believed their work was important. Moreover, 87.1% were interested in their assessment tasks, and 84.9% believed they made a positive contribution to the assessment work and the assessment position was in line with their personal preferences. Regarding job satisfaction, 83.9% of the participants were satisfied with their assessment work, 78.5% considered the workload appropriate, and 74.2% found the working hours suitable. Additionally, 65.6% expressed satisfaction with the level of cooperation from the assessment subjects, while 94.6% and 91.4% of the personnel were satisfied with the support provided by the assessment agencies and had a sense of belonging, respectively. Furthermore, 65.6% of the personnel expressed satisfaction with their job compensation and benefits. Hygiene factors affecting job satisfaction included salary and compensation, workload, time spent on the job and institutional support related to external assistance, job suitability, teamwork, and client cooperation (P<0.05), while motivational factors included recognition of the significance of the job, sense of achievement, interest in the job, sense of belonging, and opportunities for personal growth (P<0.05). ConclusionThe overall work cognition and satisfaction of unified elderly care needs assessment personnel in Shanghai towards their work are relatively positive. Main issues include the heavy workload and actual job subsidies not meeting expectations. It is recommended to improve the standardization of the assessment process, establish a professional team of assessment personnel, and enhance the compensation and incentive mechanisms to continuously increase staff satisfaction.
10.AZD1775 and anti-PD-1 antibody synergistically sensitize hepatoma to radiotherapy

Background::Radiation (IR)-induced DNA damage triggers cell cycle arrest and has a suppressive effect on the tumor microenvironment (TME). Wee1, a cell cycle regulator, can eliminate G2/M arrest by phosphorylating cyclin-dependent kinase 1 (CDK1). Meanwhile, programed death-1/programed death ligand-1 (PD-1/PDL-1) blockade is closely related to TME. This study aims to investigate the effects and mechanisms of Wee1 inhibitor AZD1775 and anti-PD-1 antibody (anti-PD-1 Ab) on radiosensitization of hepatoma.Methods::The anti-tumor activity of AZD1775 and IR was determined by 3-(4,5-dimethylthiazol-2-y1)-2,5-diphenyltetrazolium bromide (MTT) assay on human and mouse hepatoma cells HepG2, Hepa1-6, and H22. The anti-hepatoma mechanism of AZD1775 and IR revealed by flow cytometry and Western blot in vitro. A hepatoma subcutaneous xenograft mice model was constructed on Balb/c mice, which were divided into control group, IR group, AZD1775 group, IR + AZD1775 group, IR + anti-PD-1 Ab group, and the IR + AZD1775 + anti-PD-1 Ab group. Cytotoxic CD8 + T cells in TME were analyzed by flow cytometry. Results::Combining IR with AZD1775 synergistically reduced the viability of hepatoma cells in vitro. AZD1775 exhibited antitumor effects by decreasing CDK1 phosphorylation to reverse the IR-induced G2/M arrest and increasing IR-induced DNA damage. AZD1775 treatment also reduced the proportion of PD-1 +/CD8 + T cells in the spleen of hepatoma subcutaneous xenograft mice. Further studies revealed that AZD1775 and anti-PD-1 Ab could enhance the radiosensitivity of hepatoma by enhancing the levels of interferon γ (IFNγ) + or Ki67 + CD8 T cells and decreasing the levels of CD8 + Tregs cells in the tumor and spleen of the hepatoma mice model, indicating that the improvement of TME was manifested by increasing the cytotoxic factor IFNγ expression, enhancing CD8 + T cells proliferation, and weakening CD8 + T cells depletion. Conclusions::This work suggests that AZD1775 and anti-PD-1 Ab synergistically sensitize hepatoma to radiotherapy by enhancing IR-induced DNA damage and improving cytotoxic CD8 + T cells in TME.
