Objective:In order to reduce the trauma associated with fetal cardiopulmonary bypass(F-CPB), Our team plans to develop a minimally invasive F-CPB through a small incision in the right axilla. The efficacy of this technique will be verified by using a big experimental animal model, thereby laying the foundation for fetal cardiac surgery supported by F-CPB in the future. Methods:Ten pregnant sheep were divided into F-CPB group(n=5) and control group(n=5). After fasting for 24 h, fetal lambs in the F-CPB group underwent a right axillary incision to establish F-CPB running for 1 h; The control group of fetal lambs only expose heart 1 h without F-CPB. Collect blood sample for laboratory test at the CPB vehicle before(T0), 30 min(T1), and 1 h after F-CPB running(T2) for the F-CPB group and through Superior Vena Cava before(T0), 30 min(T1), and 1 h after F-CPB running(T2) for the control group. Results:The blood routine indicators such as RBC, HCT, and Hb in the F-CPB group of fetal lambs decreased significantly during F-CPB, and their distribution showed significant statistical differences compared with the control group( P<0.05). There were no significant statistical differences in blood gas indicators such as pH, PO 2, PCO 2, and lactate concentration between the F-CPB group and the control group( P>0.05). There was no statistically significant difference in the concentration of cTnI in fetal lamb serum at each time point( P>0.05). There were significant statistical differences( P<0.05) in the distribution of fetal lamb Alb, γ-GGT, CK concentration and cholesterol concentration at various time points in the F-CPB group compared with the control group in liver function examination. In addition, the distribution of BUN in fetal lambs showed a significant difference between the two groups( P=0.006). Conclusion:A minimally invasive F-CPB via small incision in the right axilla is safe and feasible. The experimental animal model has demonstrated that this technique has minimal impact on the vital organ functions and internal environment of fetal lambs, thereby laying the foundation for clinical fetal cardiac surgery in the future.

Background and Aims:Gastric cancer remains a common malignancy worldwide with a poor prognosis and limited response to current therapies.Ferroptosis,a novel form of regulated cell death, has emerged as a promising therapeutic target in cancer.Terminal nucleotidyltransferase 5B(TENT5B)is downregulated in various tumors, but its role in gastric cancer and ferroptosis remains unclear. This study aimed to investigate the expression pattern and biological function of TENT5B in gastric cancer and to elucidate its underlying mechanisms in regulating ferroptosis. Methods:The expression of TENT5B in gastric cancer was analyzed using TCGA and GEO datasets, and further validated in gastric cancer tissues and cell lines by qRT-PCR and Western blotting.CCK-8,colony formation, wound healing, and Transwell assays were performed to evaluate the effects of TENT5B overexpression on cell proliferation and migration.Ferroptosis was assessed by measuring cell viability, lipid ROS,and MDA levels.Bioinformatics analysis,mRNA stability assays, and rescue experiments were conducted to explore the molecular mechanisms.A subcutaneous xenograft mouse model was used to validate the in vivo effects.Results:TENT5B was significantly downregulated in gastric cancer tissues and cells.Overexpression of TENT5B inhibited cell proliferation and migration while promoting ferroptosis.Mechanistically,TENT5B enhanced PRKAA2 mRNA stability and upregulated its expression, thereby exerting tumor-suppressive effects. In vivo,TENT5B overexpression suppressed tumor growth and elevated PRKAA2 expression.Conclusion:TENT5B functions as a tumor suppressor in gastric cancer by stabilizing PRKAA2 mRNA,promoting ferroptosis, and inhibiting cancer progression. These findings suggest that TENT5B may serve as a promising molecular target for ferroptosis-based therapeutic strategies in gastric cancer.

This study was performed to investigate the variation characteristics and functions of Siglec-9 immune checkpoint in pulmonary tuberculosis.R language was used for bioinformatics analysis of GSE83456 chip data.Total of 48 patients with confirmed pulmonary tuberculosis(PTB)and 46 healthy controls were included.Real-time fluorescence quantitative PCR(qRT-PCR)was used to detect the mRNA expression of Siglec-9 in peripheral blood;flow cytometry was used to detect the proportion of Siglec-9+CD4+T cells. The levels of TNF-α,IL-6,IFN-γ and IL-4 were detected by Cytometric Bead Array(CBA).Furthermore, the correlation of the proportion of Siglec-9+CD4+T cells with TNF-α and other cytokines were analyzed.Bioinformatics analysis showed that Siglec-9 was significantly increased in patients with PTB(P<0.001),and was related to TNF-α/NF-κB,IL-6/JAK/STATA3,PI3K/AKT/mTOR signal pathways.Experimental data showed that the proportion of Siglec-9+CD4+T cells was significantly increased in patients with PTB(P<0.001)and negatively correlated with the levels of TNF-α and other cytokines. In conclusion, the higher levels of Siglec-9 mRNA and Siglec-9+CD4+T cells in PTB may inhibit the function of CD4+T cells and participate in the occurrence and development of PTB.

Pulmonary vein stenosis (PVS) is an extremely rare and lethal disease caused by multiple etiologies. PVS has a bimodal distribution in the population, affecting children and adults. Congenital PVS is the usual PVS type in children, which sometimes develops after cardiothoracic surgery. Acquired PVS, in turn, is the most common PVS type in adults. A review of the relevant literature has shown that PVS after radiofrequency ablation of atrial fibrillation is the most common, as well as that caused by compression of proliferative fibrous tissues or tumor in the mediastinum (eg, PVS caused by fibrosing mediastinitis, lung tumors, metastases, etc). This article provides a comprehensive review of PVS in terms of embryology and anatomy, etiology and triggers, classification, clinical symptoms and signs, treatment, and prognosis, intending to promote the understanding and treatment of this disease.

Chinese Journal of Primary Medicine and Pharmacy 2021;28(4):636-640
Circular RNAs are a group of novel non-coding RNA molecules, which form covalently closed continuous loop structures, lacking the terminal 5′ and 3′ ends. They are abundant in eukaryotes and have recently become an increasing area of research in the field of RNA. Circular RNAs have many functions, such as abundant expression, cell- or tissue-specific expression, and high resistance to RNase-induced degradation. They are closely related to tissue development and the occurrence and development of diseases, play an important role in the pathogenesis and development of a variety of diseases, and can be used as a novel molecular marker of some diseases. Therefore, this review summarizes the latest research progress of circular RNAs in the regulation of endocrine diseases to better understand the biological mechanisms underlying endocrine diseases.
