The clinical diagnosis and treatment of urinary tract infection has long faced the challenges of insufficient standardization of diagnosis and treatment pathways, irrational use of antimicrobial drugs and high recurrence rate. How to optimize the hierarchical diagnosis and treatment pathway of urinary tract infection, standardize the use of antimicrobial drugs, and reduce the recurrence rate have always been the focus of clinical attention. There is significant heterogeneity in the existing diagnostic criteria for urinary tract infection, which seriously affects the comparability and evidence integration of clinical and research studies. In order to solve the above problems,a consensus on global multidisciplinary diagnostic criteria for urinary tract infection has been formed by international multidisciplinary experts after three rounds of Delphi method.Breaking through the traditional classification framework, the consensus innovatively established a four-dimensional quantitative scoring system including local symptoms and signs, systemic inflammatory response, quantitative analysis of pyuria and urine culture results, and established a hierarchical standard for stepwise urinary tract diagnosis according to the scoring threshold.Based on the key citations related to the consensus, this paper interprets in detail the basis for the selection of core indicators and the establishment of thresholds for the diagnosis of urinary tract infection in the consensus, and focuses on the key issues and implementation paths of the consensus in localization practice. This consensus provides a unified standard for standardizing the clinical diagnosis and treatment of urinary tract infection, improving the homogeneity of clinical research through standardized diagnostic processes, and promoting the standardization of UTI drug research and development and the rational use of antibiotics and precision.

This study was performed to investigate the variation characteristics and functions of Siglec-9 immune checkpoint in pulmonary tuberculosis.R language was used for bioinformatics analysis of GSE83456 chip data.Total of 48 patients with confirmed pulmonary tuberculosis(PTB)and 46 healthy controls were included.Real-time fluorescence quantitative PCR(qRT-PCR)was used to detect the mRNA expression of Siglec-9 in peripheral blood;flow cytometry was used to detect the proportion of Siglec-9+CD4+T cells. The levels of TNF-α,IL-6,IFN-γ and IL-4 were detected by Cytometric Bead Array(CBA).Furthermore, the correlation of the proportion of Siglec-9+CD4+T cells with TNF-α and other cytokines were analyzed.Bioinformatics analysis showed that Siglec-9 was significantly increased in patients with PTB(P<0.001),and was related to TNF-α/NF-κB,IL-6/JAK/STATA3,PI3K/AKT/mTOR signal pathways.Experimental data showed that the proportion of Siglec-9+CD4+T cells was significantly increased in patients with PTB(P<0.001)and negatively correlated with the levels of TNF-α and other cytokines. In conclusion, the higher levels of Siglec-9 mRNA and Siglec-9+CD4+T cells in PTB may inhibit the function of CD4+T cells and participate in the occurrence and development of PTB.

Journal of Kunming Medical University 2023;44(12):177-183
The meniscus in the knee joint plays an important role in bearing load, shock absorption, joint lubrication, and increasing the stability of the knee joint in various directions.Meniscal injury is a common ailment in knee joint injuries, which can cause severe knee pain and limited knee joint movement, significantly affecting the patient's quality of life.Arthroscopic minimally invasive treatment has become the main approach for treating meniscal injuries, with the consensus being to preserve the meniscus morphology and structure as much as possible.By introducing the structure and function of the meniscus, the mechanism of injury, and focusing on several suture techniques for arthroscopic meniscal injury treatment, analyzing their advantages and disadvantages,it aims to provide some reference for clinical research on arthroscopic suture of the meniscus.

Objective:To investigate the clinical effect of the facial artery perforator flap in repairing the lower eyelid cicatricial ectropion. Methods:The clinical data of patients with lower eyelid cicatricial ectropion admitted to the Department of Burn and Plastic Surgery, Affiliated Hospital of Zunyi Medical University from January 2016 to December 2019 were retrospectively analyzed. After the lower eyelid scar was removed and contracture was released, the facial artery perforator flap was designed to repair the skin defect, and the donor site was sutured directly. The appearance and complications of the recipient and donor sites were followed up after the operation, and the operation effect was evaluated as good, general and poor. Results:A total of 22 patients with lower eyelid cicatricial ectropion were enrolled, including 15 males and 7 females, with an average age of 35.2 years (range, 27-68 years). The area of skin defect after scar release was 3.0 cm×1.5 cm-4.0 cm×2.0 cm, and the area of skin flap was 3.1 cm×1.6 cm-4.1 cm×2.1 cm. All flaps survived with good blood supply and healed well. All patients were followed up for 6-48 months, and there was no recurrence of lower eyelid ectropion, blepharon separation, local pigmentation and other phenomena. All patients showed good anatomical structure and functional improvement, and the postoperative effect evaluation of all cases was good. Conclusions:The facial artery perforator flap has the advantages of the stable perforator, good blood supply, flexible rotation, close position with the lower eyelid, and similar color, texture, and thickness. Good result can be obtained in repairing lower eyelid cicatricial ectropion by using a facial artery perforator flap.

The rapid screening of tumor markers is a challenging task for early diagnosis of cancer. This study aims to use highly sensitive chemiluminescent protein microarray technology to efficiently screen a variety of low abundance tumor related markers. A new material, termed integrated polydimethylsiloxane modified silica gel (iPDMS), was obtained by adding a surface polymerization initiator with olefin end to the conventional polydimethylsiloxane, and fixing into the three-dimensional structure of polydimethylsiloxane by thermal crosslinking through silicon hydrogen bonding. In order to make the iPDMS material resistant to non-specific protein adsorption, a poly(OEGMA) polymer brush was synthesized by surface-initiated atom transfer radical polymerization at the active initiation site. Finally, 20 tumor-related antigens were printed into the specific areas of the microarray by high-throughput spray printing technology, and assembled into 48-well detection microtiterplates of the iPDMS microarray. It was found the VEGFR and VEGF121 autoantibodies that obtained from 8 common tumors (breast cancer, lung cancer, colon cancer, gastric cancer, liver cancer, leukemia, lymphoma and ovarian cancer) can be used as potential tumor markers. The chemiluminescence labeled iPDMS protein microarray can be used for the screening of tumor autoantibodies at early stage.
