1.A Single-Arm Clinical Trial of Fuyuan Tiansui Decoction in the Treatment of Cerebellar Ataxia Syndrome

[Objective]To observe the efficacy of Fuyuan Tiansui Decoction in the treatment of cerebellar ataxia syndrome(CAS).[Methods]A total of 29 patients with CAS treated at Wenzhou Hospital of Integrated Traditional Chinese and Western Medicine outpatient clinics from February to October 2024 were enrolled.All participants received Fuyuan Tiansui Decoction treatment for 12 weeks.Before and after treatment,the changes of International Cooperative Ataxia Scale(ICARS),Scale for the Assessment and Rating of Ataxia(SARA),Activities of Daily Living(ADL)score and traditional Chinese medicine(TCM)syndrome scale were compared.[Results]A total of 29 patients were enrolled,with 27 completing the 12-week treatment course(2 withdrawals).Post-treatment analysis revealed an ICARS score improvement rate of 55.6%and a stabilization rate of 25.9%.The ICARS score was 35.85±12.90 before treatment and 34.33±13.10 after treatment(P<0.05).The SARA score and TCM syndrome scale score were significantly decreased(P<0.05).During the treatment,the adverse reaction rate was 11.1%,including 3 cases of diarrhea.[Conclusion]Fuyuan Tiansui Decoction can relieve the ataxia symptoms and the syndrome of kidney deficiency and marrow deficiency in patients with CAS.

The occurrence and development of type 2 diabetes mellitus(T2DM)are closely as-sociated with defects in insulin secretion.Synaptosomal-associated protein 25(SNAP25),as a core component of the soluble N-ethylmaleimide-sensitive factor attachment protein receptor(SNARE)complex,directly mediates the fusion of insulin secretion granules with the cell membrane and regu-lates the dynamic balance of insulin secretion stimulated by glucose.The expression defect of SNAP25 in patients with T2DM and model animals directly leads to vesicle fusion disorder,constituting the core pathological link of insulin secretion disorder and exacerbating the deterioration of β-cell func-tion.SNAP25 may serve as a key hub for multi-target synergistic intervention,and its genetic poly-morphism and the plasticity of its regulatory network offer novel strategies for precision therapy.This article innovatively integrated multidimensional regulatory mechanisms,including calcium channel ac-tivity,G-protein-coupled signaling and epigenetic modifications,to systematically analyze the spatio-temporal-specific regulatory network of SNAP25 in insulin secretion,providing a theoretical basis for T2DM therapeutic strategies targeting vesicle fusion.
3.Application of green treatment in older patients with cancer

With the aging of the population,the proportion of older patients with cancer is increasing.The treatment of older patients with cancer is challenging owing to frailty,comorbidities,and the exclusion from most clinical studies.Green therapy,using low-injury,sustainable,repeatable,and individualized treatment methods—such as minimally invasive surgery and traditional Chinese medicine—provides a new option for patients with cancer who are intolerant or have failed in conventional treatment.This article summarizes the safety and effectiveness of minimally invasive surgery,such as radiofrequency ablation,microwave ablation,cryoablation,and vascular interventional therapy,in older patients with tumors,as well as external treatment of traditional Chinese medicine in clinical practice to provide a basis for green treatment of tumors.Prospective clinical trials should be performed on older patients with cancer to provide high-level evidence-based medical evidence for the treatment of these patients.Furthermore,a clinical prediction model of green treatment based on real-world data should be constructed for older patients with cancer to provide a foundation for clinical medical decision-making.Basic researches should be conducted to explore the mechanism of green tumor treatment.

6.Study on the influence of reconstruction algorithms of image on the image quality and precision of automatic registration of imaging system with megavolt grade

Objective:To compare and study the improvement of different iterative reconstruction(IR)algorithms of the tomotherapy(TOMO)Radixact system on the image quality of megavoltage computed tomography(MVCT)imaging system,and the influence of that on the precision of automatic registration,and to explore the reconstruction algorithm that is suitable for clinical application.Methods:Using the MVCT imaging system to respectively scan the Tomo-Phantom HE phantom and the Catphan 604 phantom,and to analyze three groups of images were generated by three kinds of reconstruction algorithms,which included the Standard(STD)algorithm,IR General(IR-G)algorithm and IR Soft Tissue(IR-ST)algorithm,in MVCT image.The noise index(NI),uniformity index(UI)of image,modulation transfer function(MTF),low contrast visibility(LCV)index and contrast-to-noise ratio(CNR)of three groups of images were calculated respectively.The Lucy phantom was used to test the accuracy of automatic registration algorithm.The registration data of four dimensions,included left-right(X-axis),head-foot(Y-axis),vertical(Z-axis)and free rotation(Roll)around the Y-axis,were used to conduct verification analysis for the influences of them on the precision of automatic registration.Results:The NI values of IR-G and IR-ST reconstruction algorithms were respectively 39.58±0.10 and 14.62±0.26,which were better than 39.58±0.10 of STD algorithm,and the UI values of them were respectively 19.87±0.83 and 15.84±2.51,which were better than 24.51±1.81 of STD algorithm,and LCV values of them were respectively 2.50±0.03 and 1.74±0.11,which were better than 3.67±0.04 of STD algorithm.All of them appeared significant increase,but the resolution with high contrast of MTF were respectively 0.23 and 0.21,which were lower than 0.32 of STD.The overall image quality of the IR algorithms was superior to that of the STD algorithm.In the accuracy test of the automatic registration algorithm,the registration precisions of IR-G and IR-ST algorithms on Y-axis were respectively(0.360±0.142)mm and(0.245±0.050)mm,which were significantly higher than 0.145±0.136 of STD algorithm,and the differences of them were significant(Z=6.0,15.0,P<0.05).The differences of registration precisions of other directions were not significant(P>0.05).Conclusion:The IR algorithm has advantages in terms of noise,uniformity and resolution with low-contrast,however,it shows reduction on resolution with high-contrast.This reduction of resolution with high-contrast do not lead to the decrease of the precision of automatic registration.
7.Analysis of image quality based on different imaging algorithms of the Ethos adaption radiotherapy system

Objective:To quantitatively analyze the differences in image quality and generated synthetic-computed tomography(sCT)between cone beam CT(CBCT)and iteration CBCT(iCBCT),which based on filtered back projection(FBP)and iterative reconstruction(IR)techniques of Varian Ethos adaption system.Methods:Using the special Catphan604 phantom of the test of CT image quality,five times of scanning series of CBCT and iCBCT were respectively performed based on thoracic and abdominal scanning protocols,so as to obtain four sets of images,and each set of image included the CBCT image,the composited sCT and iCBCT images based on CBCT,the composited sCT bases on iCBCT.The noise power spectrum(NPS),image uniformity(UI),modulation transfer function(MTF),low-contrast visibility(LCV)and contrast-to-noise ratio(CNR)of them were analyzed.Additionally,a retrospective analysis was conducted on images of six patients who underwent clinical radiotherapy,and the images of them were obtained by using thoracic and abdominal scanning protocols.The sCT images,which were generated by deformable registrations on the basis of CBCT and iCBCT,were analyzed,and they were compared with located CT images,and their mean absolute error(MAE)and root mean square error(RMSE)were further quantitatively analyzed.Results:Compared to CBCT,iCBCT significantly reduced NPS(thoracic images reduced by 0.08,abdominal images reduced by 0.06).The UI values of thoracic images and abdominal images of iCBCT were respectively 0.37 and 0.28,and those of CBCT were respectively 0.65 and 0.39.The LCV values of thoracic images and abdominal images of iCBCT were respectively 0.67 and 0.56,and those of CBCT were respectively 0.93 and 0.90.The MTF50%were 0.28 lp/mm except abdominal image of iCBCT was 0.29 lp/mm.The resolution rate of high contrast between iCBCT and CBCT was basic consistency.The differences of similarity analysis of the generated sCT and located CT bases on CBCT and iCBCT was no significant(P>0.05).Conclusion:The image quality of iCBCT is superior to conventional CBCT.The accuracy of CT value of generated sCT bases on CBCT and iCBCT can meet requirement,and both of them can be used in the dose calculation of adaptive radiotherapy.
8.Researches on multi-level rehabilitation service system in China:a bibliometrics analysis

Objective To analyze the trends,cooperation,topics and hotspots of researches about multi-level rehabilitation service system in China. Methods The literature on multi-level rehabilitation service system in China was searched and screened in databases of CNKI from 1983 to 2023.The number of the articles was described,and the cooperation,research hotspots and changing trend were analyzed using VOSviewer. Results A total of 4 643 articles were included.The number of the articles tended to increase and developed in stages.Nine groups with five or more researchers were found,and seven of them cooperated with each other.The most frequent keywords were community-based rehabilitation(occurrence 1 251 with connection strength 1 780),stroke(occurrence 674 with connection strength 1 126),family rehabilitation(occurrence 412 with connection strength 514),rehabilitation nursing(occurrence 178 with connection strength 240)and quality of life(occur-rence 156 with connection strength 311).The researchers initially focused on disability rehabilitation,then fo-cused on community-based rehabilitation and family rehabilitation,and gradually focused on the quality of life,activities of daily living,satisfaction,mental health,negative emotion and healthcare consortium in recent years. Conclusion The researches about multi-level rehabilitation service system are developing in China,focusing on commu-nity-based rehabilitation,stroke,family rehabilitation,rehabilitation nursing and quality of life.The cooperation among scholar groups need to be strengthened.Quality of life,activities of daily living,satisfaction,mental health,negative emotion and healthcare consortium may be the hotspots in the future.
9.Correlations of serum Apelin-13 and fatty acid binding protein 4 levels with postmenopausal osteoporosis

Objective To investigate the correlations of serum Apelin-13 and fatty acid binding protein 4 (FABP4) levels with metabolic and bone metabolic indicators in postmenopausal women with different bone mass. Methods A total of 145 postmenopausal women were selected as subjects and divided into three groups based on bone mineral density (BMD) test results: normal bone mass group(49 cases), osteopenia (ON) group(51 cases), and osteoporosis (OP) group(45 cases). Serum Apelin-13, FABP4 levels, bone metabolic indicators, and biochemical indicators were measured and compared among the three groups. Spearman correlation analysis was used to analyze the correlations of Apelin-13, FABP4, and other indicators with BMD. Multivariate Logistic regression analysis was performed to analyze the risk factors for OP, and the receiver operating characteristic (ROC) curve was plotted to analyze the predictive value of serum Apelin-13 for postmenopausal osteoporosis (PMOP). Auditory neuropathy spectrum disorder (ANSD) represents a variety of sensorineural deafness conditions characterized by abnormal inner hair cells and/or auditory nerve function, but with the preservation of outer hair cell function. ANSD represents up to 15% of individuals with hearing impairments. Through mutation screening, bioinformatic analysis and expression studies, we have previously identified several apoptosis-inducing factor (AIF) mitochondria-associated 1 (AIFM1) variants in ANSD families and in some other sporadic cases. Here, to elucidate the pathogenic mechanisms underlying each AIFM1 variant, we generated AIF-null cells using the clustered regularly interspersed short palindromic repeats (CRISPR)/CRISPR-associated protein 9 (Cas9) system and constructed AIF-wild type (WT) and AIF-mutant (mut) (p.T260A, p.R422W, and p.R451Q) stable transfection cell lines. We then analyzed AIF structure, coenzyme-binding affinity, apoptosis, and other aspects. Results revealed that these variants resulted in impaired dimerization, compromising AIF function. The reduction reaction of AIF variants had proceeded slower than that of AIF-WT. The average levels of AIF dimerization in AIF variant cells were only 34.5%?49.7% of that of AIF-WT cells, resulting in caspase-independent apoptosis. The average percentage of apoptotic cells in the variants was 12.3%?17.9%, which was significantly higher than that (6.9%?7.4%) in controls. However, nicotinamide adenine dinucleotide (NADH) treatment promoted the reduction of apoptosis by rescuing AIF dimerization in AIF variant cells. Our findings show that the impairment of AIF dimerization by AIFM1 variants causes apoptosis contributing to ANSD, and introduce NADH as a potential drug for ANSD treatment. Our results help elucidate the mechanisms of ANSD and may lead to the provision of novel therapies.
