Objective:To explore the clinical efficacy of modified Chaihu Jia Longgu Muli Decoction in patients with liver and kidney Yin deficiency syndrome of Parkinson's disease (PD) and the effects on phosphatase and tensin homolog-induced putative kinase 1 (PINK1) /E3 ubiquitin ligase (Parkin) signaling pathway.Methods:A randomized controlled trial study was conducted. A total of 120 PD patients from the Department of Encephalopathy of Xi'an Hospital of Traditional Chinese Medicine from May 2020 to May 2024 were selected as the observation objects and divided into 2 groups through random number table method, with 60 cases in each group. The control group took dopashydrazine tablets orally, while the combined group took Chaihu Jia Longgu Muli Decoction on the basis of the control group. Two groups were treated continuously for 2 months. The levels of glial cell-derived neurotrophic factor (GDNF), nerve growth factor (NGF), brain-derived neurotrophic factor (BDNF), and basic fibroblast growth factor 2 (FGF2) were detected by ELISA. The expression levels of PINK1, Parkin and chelate 1 (SQSTM1) mRNA were detected by real-time fluorescence quantitative PCR. The adverse reactions during the treatment were observed and recorded to evaluate the clinical efficacy.Results:The total effective rate was 95.00% (57/60) in the combined group and 83.33% (50/60) in the control group, with statistical significance ( χ2=4.23, P=0.040). After treatment, the levels of serum GDNF [(510.78±31.57) ng/L vs. (431.89±30.45) ng/L, t=13.93], BDNF [(36.47±3.72) ng/L vs. (27.84±3.55) ng/L, t=13.00], NGF [(57.24±4.17) ng/L vs. (50.72±4.11) ng/L, t=8.63] and FGF2 [(26.21±3.71) ng/L vs. (21.63±3.54) ng/L, t=6.92] in the combined group were higher than those in the control group ( P<0.001). After treatment, the mRNA expression levels of PINK1 (2.38±0.07 vs. 2.19±0.05, t=17.11), Parkin (1.89±0.03 vs. 1.74±0.05, t=19.93), and SQSTM1 (1.82±0.07 vs. 1.61±0.05, t=18.91) in the combined group were higher than those in the control group ( P<0.001). During the treatment period, the incidence of adverse reactions was 6.67% (4/60) in the combined group and 10.00% (6/60) in the control group, without statistical significance ( χ2=0.44, P=0.509). Conclusion:Modified Chaihu Jia Longgu Muli Decoction can increase the levels of neurotrophic factors in patients with PD of liver and kidney yin deficiency syndrome, regulate the PINK1/Parkin pathway, protect neurons, improve mitochondrial autophagy function, and enhance clinical efficacy.

【Objective】 To investigate the central sedative and hypnotic effects and mechanisms of Shumian Capsule. 【Methods】 The blank control group, the positive drug diazepam group, and the low-, medium-, and high-dose groups of Shumian Capsule were designed in the experiments. Male Kunming mice were administered orally by gavage. We conducted the experiment of mouse autonomous activity, and assessed supra- and sub-threshold dose sleep with pentobarbital sodium, respectively. The levels of γ-aminobutyric acid (GABA) and glutamic acid (Glu) in the brain tissue of the mice were measured by enzyme-linked immunosorbent assay (ELISA) or colorimetric method. Male SD rats were injected intraperitoneally with p-chlorophenyl alanine [PCPA, 350 mg/(kg·d)] to establish an insomnia rat model, after which rats were continuously administered intragastrically for 7 days. The general status and body weight of the rats were observed. Total distance and standing times of rats were measured by open field test. Western blotting and immunohistochemistry assays were used to evaluate the protein expression of 5-HT1A receptor (5-HT1AR) in the rat hippocampus. 【Results】 Compared with the blank control group, the three dose groups of Shumian Capsule [1.6, 3.2, 6.4 mg/(kg·d)] for 14 consecutive days had significantly reduced the number of spontaneous activities and standing times in the mice (P<0.01). The high dose significantly prolonged the sleep duration of the mice induced by the supra-threshold dose of pentobarbital sodium (P<0.01). Compared with the blank control group, each dose group of Shumian Capsule had an increased GABA level in the mouse brain tissue in a dose-dependent manner (P<0.05); the medium- and high-dose groups had significantly reduced Glu levels in the hippocampus (P<0.05), while the high-dose group had significantly reduced Glu level in the cortex (P<0.05). In insomnia model rats, their activity was sluggish; their circadian activity rhythm disappeared; the hair became hard, rough and dull, with severe hair loss; and their weight reduced. After 7 consecutive days of the drug administration, the rats’ mental state in each Shumian Capsule dose group was improved. Compared with the blank control group, the rats’ body weight gains were significantly reduced after intraperitoneal injection of PCPA (P<0.01). Compared with the model group, each rat group of Shumian capsule treatment had significantly increased body weight gains (P<0.01). Compared with the blank control group, the distance of the rats in the model group increased in the open field test, and the expression level of 5-HT1AR protein in the hippocampus decreased. However, the distance of rats in diazepam group and each dose group of Shumian Capsule was significantly reduced (P<0.01), and the expression level of 5-HT1AR protein in the hippocampus of the high- and medium-dose groups of Shumian Capsule and diazepam group increased significantly (P<0.01). 【Conclusion】 Shumian Capsule has obvious effects of sedation, hypnosis and insomnia improvement, and its underlying mechanisms may be related to the increased GABA and decreased Glu contents in brain tissues, as well as up-regulated 5-HT1AR protein expression in the hippocampus.

Objective:To explore the effect of risk assessment and follow-up management combined with early ultrasound intervention on reducing perioperative pressure ulcer in infants.Methods:A total of 469 perioperative infants and young children admitted to Children's Hospital Affiliated to Zhengzhou University from June 2019 to June 2020 were selected as the research subjects. They were divided into observation group ( n=243) and control group ( n=226) by random number table. Children in the control group were treated with routine care for preventing pressure ulcers. Children in the observation group were treated with risk assessment and follow-up management combined with ultrasound on the basis of the control group. The incidence of pressure ulcers was compared between the two groups at 3 and 7 days after surgery. Results:The incidence of pressure ulcers in the observation group was significantly lower than that of the control group at 3 days and 7 days after operation, and the difference was statistically significant ( P<0.05) . It could be seen that the severity of pressure injury in the observation group was lower than that in the control group. Conclusions:Risk assessment and follow-up management combined with ultrasound in early intervention can significantly reduce the risk of pressure ulcers in infants and young children during the perioperative period, which is worthy of promotion and application.

Objective:To understand the characteristics of scientific research cooperation network, research hotspots, evolution paths and development trends of advance care planning (ACP) , so as to provide reference for follow-up related research.Methods:With the theme of "advance care planning", the retrieval results based on Web of Science core collection database were respectively imported into VOS viewer and CiteSpace for visual analysis, and the retrieval time was from database construction to February 3, 2021.Results:A total of 2 456 articles meet the selection criteria. The annual publications were all less than 50 from 1993 to 2009, which showed a rapid upward trend from 2010 to now. The distribution trend of citation frequency was basically the same as that of publications. Most of the authors and institutions are European and American. Chan Helen Yue-lai of the Chinese University of Hong Kong ranked first in China and 19th in the world in terms of publicationsand Chinese scholars' research in this field is still in its infancy. Research hotspots included ACP alternative decision-making skills training, and the research frontiers and evolutionary paths included ACP, severe mental illness, alternative decision-making, life planning, end-stage renal disease, doctor's orders, ACP model, advanced dementia, chronic heart failure, etc.Conclusions:Since 2010, the number of articles published in this field and the frequency of citations have shown a rapid upward trend. The United States, Belgium, Canada and other countries have developed well in this field and Scholars of China have a wide space for development in this field. The advanced research experience reported in the foreign ACP literature can be used for reference to deepen and speed up the clinical practice research in the field of ACP in China.

Objective:To explore the impact of pediatric nurses' presenteeism behavior and emotional commitment on psychological capital, so as to provide a reference for promoting pediatric nurses' positive organizational behaviors.Methods:From July to August 2020, convenience sampling was used to select pediatric nurses from three ClassⅢ hospitals in Henan Province as the research objects. The general information questionnaire, Presenteeism Behavior Questionnaire, Emotional Commitment Scale and Psychological Capital Scale were used for investigation. Pearson correlation and structural equation model were used to analyze the relationship between presenteeism behavior, emotional commitment and psychological capital. A total of 897 questionnaires were distributed, and 673 valid questionnaires were returned with the effective response rate of 75.03%.Results:Among 673 pediatric nurses, the scores of the Presenteeism Behavior Questionnaire, Emotional Commitment Scale and Psychological Capital Scale were (6.16±1.52) , (28.26±5.67) and (81.40±10.60) respectively. The results of Pearson correlation analysis showed that presenteeism behavior was negatively correlated with emotional commitment and psychological capital ( P<0.05) , and emotional commitment was positively correlated with psychological capital ( P<0.01) , and the differences were statistically significant. The structural equation model showed that presenteeism behavior and emotional commitment had a direct effect on psychological capital (effect values were -0.32 and 0.42, respectively) , and presenteeism behavior also had an indirect effect on psychological capital through emotional commitment (effect value was -0.48) . Conclusions:Emotional commitment acts as an intermediary between presenteeism behavior and psychological capital. Nursing managers should take effective measures to create a good organizational atmosphere, reduce presenteeism behavior, and improve the psychological capital of pediatric nurses.

Objective:To analyze the current status of recessive absence of pediatric nurses and to explore the relationship between recessive absence of pediatric nurses and magnetic nursing work environment and psychological empowerment.Methods:A cross-sectional survey method was adopted, pediatric nurses from Henan Children's Hospital and Henan Provincial People's Hospital from March to May 2019 were selected as research objects by the convenience method. The general information questionnaire, the Chinese version of the Essentials of Magnetism ScaleⅡ (EMOⅡ) , the Chinese version of the Psychological Empowerment Scale (PES) and the Chinese version of the Stanford Presenteeism Scale (SPS-6) were used to investigate them. Pearson correlation analysis and hierarchical regression analysis were used to analyze the relationship between recessive absence of pediatric nurses and magnetic nursing work environment and psychological empowerment. A total of 465 questionnaires were distributed in this study, and 452 valid questionnaires were recovered, with a recovery rate of 97.2%.Results:The total scores of the Chinese version of EMO Ⅱ, the Chinese version of PES and SPS-6 of 452 pediatric nurses were respectively (129.21±11.64) , (38.91±7.04) and (17.42±4.21) . The results of Pearson correlation analysis showed that recessive absence of pediatric nurses was negatively correlated with magnetic nursing work environment and psychological empowerment ( P<0.05) . The univariate analysis showed that there were statistically significant differences in the scores of the Chinese version of the SPS-6 scale for pediatric nurses with different ages, marital statuses, job titles and average night shifts per month ( P<0.05) . The results of hierarchical regression analysis showed that gender, marital status and the average night shifts per month in the demographic data all affected the recessive absence of pediatric nurses. After controlling demographic data, the better the working environment of magnetic nursing, the lower the level of recessive absence, which could independently explain 35.7% of the total variation. And the higher the level of psychological empowerment, the lower the level of recessive absence, which can independently explain 40.4% of the total variation. Conclusions:The phenomenon of recessive absences in pediatric nurses is more obvious, which is closely related to magnetic nursing work environment and psychological empowerment. Nursing managers should improve the magnetic nursing working environment and the level of psychological empowerment to reduce the level of recessive absence of pediatric nurses and improve the quality of nursing.

Objective: To analyze the processing situation of 124 batches of scorpions and optimize the salt-water processing.Methods: The character, extract, moisture, total ash, acid insoluble ash, amino acids content and heavy metals in the marketed scorpions were determined.Using extract and amino acids contents as the indices, the salt-water processing was optimized.Results: The results of the above determinations showed that the quality difference was very obvious for the marketed scorpion, and the extract and amino acids contents were the highest in scorpion boiled in 2% sodium chloride aqueous solution for 5 min.Conclusion: The item of extract test should be revised in the current standard, and the items such as moisture, total ash, acid insoluble ash, amino acids content and heavy metals should be taken into consideration in order to control the quality of scorpion more comprehensively.The best salt-water processing of scorpion is boiled in 2% sodium chloride aqueous solution for 5 min.
