1.Modern Clinical Application and Mechanism of Action of Chaihu Guizhi Ganjiangtang: A Review

Chaihu Guizhi Ganjiangtang (CGG)is a classic prescription in the Treatise on Cold Damage,which has the effects of clearing and relieving stagnation heat in Shaoyang,warming and dissolving water drink,and relieving the pivot mechanism. It is a classic prescription for treating spleen deficiency and liver depression and stopping internal stagnation caused by water drink. The formula is exquisite and well-matched and is often modified and used by ancient and modern medical practitioners to treat various miscellaneous diseases of internal and external medicine,with significant therapeutic effects. In recent years,with the rapid development of modern pharmacology,research on the micro mechanism of CGG has been continuously developed and deepened,providing new ideas for the treatment of diseases with CGG. Therefore,the authors systematically searched databases such as China National Knowledge Infrastructure,Wanfang Data Knowledge Service Platform,VIP Database, and PubMed for literature on the clinical application and pharmacological mechanism of CGG published by Chinese and foreign scholars in recent years. This article summarized the literature from two aspects:the modern clinical application and mechanism of action of CGG and elaborated on the diseases treated by CGG in modern literature,involving digestive system,respiratory system,nervous system,endocrine system,circulatory system,urinary system,gynecology,as well as its application in reducing the side effects of radiotherapy and chemotherapy, gynecology, dermatology, ophthalmology, and orthopedics. At the same time,the mechanism of CGG in treating diseases may be related to anti-inflammatory,anti-oxidative stress, regulation of immunity, anti-fibrosis, anti-tumor, improvement of gastrointestinal flora and motility, protection of liver tissue, reduction of blood lipids and blood sugar, and regulation of hormone levels.

Xiaoji Yinzi is one of the classic prescriptions for treating urinary diseases, originated from the Yan's Prescriptions to Aid the Living (Yan Shi Ji Sheng Fang) written by YAN Yonghe in the Song dynasty. Xiaoji Yinzi is composed of Rehmanniae Radix, Cirsii Herba, Talcum, Akebiae Caulis, Typhae Pollen, Nelumbinis Rhizomatis Nodus, Lophatheri Herba, Angelicae Sinensis Radix, Gardeniae Fructus, and Glycyrrhizae Radix et Rhizoma and has the effects of cooling blood and stopping bleeding, draining water and relieving stranguria. The medical experts of later generations have inherited the original prescription recorded in the Yan's Prescriptions to Aid the Living, while dispute has emerged during the inheritance of this prescription. In this study, the method of bibliometrics was employed to review and analyze the ancient documents and modern clinical studies involving Xiaoji Yinzi. The results showed that Xiaoji Yinzi has two dosage forms: powder and decoction. According to the measurement system in the Song Dynasty, the modern doses of hers in Xiaoji Yinzi were transformed. In the prepration of Xiaoji Yinzi powder, 149.2 g of Rehmanniae Radix and 20.65 g each of Cirsii Herba, Talcum, Akebiae Caulis, stir-fried Typhae Pollen, Nelumbinis Rhizomatis Nodus, Lophatheri Herba, wine-processed Angelicae Sinensis Radix, stir-fried Gardeniae Fructus, and stir-fried Glycyrrhizae Radix et Rhizoma are grounded into fine powder with the particle size of 4-10 meshes and a decocted with 450 mL water to reach a volume of 240 mL. After removal of the residue, the decoction was taken warm before meals, 3 times a day (i.e., 7.77 g Rehmanniae Radix and 0.97 g each of the other herbs each time). In the preparation of Xiaoji Yinzi decoction, 20.65 g each of the above 10 herbs are used, with stir-fried Typhae Pollen, wine-processed Angelica Sinensis Radix, stir-fired Gardeniae Fructus, stir-fired Glycyrrhizae Radix et Rhizoma, and raw materials of other herbs. Xiaoji Yinzi is specialized in treating hematuresis and blood stranguria due to heat accumulation in lower energizer, which causes injury of the blood collaterals of gallbladder and dysfunction of Qi transformation. In modern clinical practice, Xiaoji Yinzi is specifically used for treating urinary diseases and can be expanded to treat diseases of the cardiovascular system and other systems according to pathogenesis. The comprehensive research on the key information could provide a scientific reference for the future development of Xiaoji Yinzi.
3.Prevalence of sarcopenia and influencing factors in middle-aged and older adults in Zhejiang Province

Objective:To investigate the prevalence of sarcopenia and potential influencing factors in middle-aged and elderly populations in Zhejiang Province.Methods:Data were obtained from Zhejiang Provincial Household Economic Status Survey, a cross-sectional survey was condcuted in middle-aged and olde adults selected through multi-stage sampling in three cities in Zhejiang (Huzhou, Jiaxing and Shaoxing) in July 2023. A total of 3 019 study participants, average age 62.3 years old, 53.5% men, were included according to the inclusion and exclusion criteria. Sarcopenia screening was conducted by using the questionnaire with five sarcopenia related-items. Univariable and multivariable logistic regression analysis was used to identify the factors associated with sarcopenia.Results:The prevalence of sarcopenia in the middle-aged and old study participants was 4.47%. Significant differences were observed between the participants with or without sarcopenia in terms of age, educational level, BMI, alcohol consumption status, diet habit, physical activity level, sleep quality, number of chronic diseases, childhood socioeconomic status, adulthood community socioeconomic status, muscle strength, walking assistance, ability to stand from seat, ability to climb stairs, and fall frequency ( P<0.05). Multivariable logistic regression analysis revealed that old age (≥75 years: OR=2.82, 95% CI: 1.60-4.97), low body weight ( OR=1.96, 95% CI: 1.06-3.62), unhealthy diet habit ( OR=1.57, 95% CI: 1.01-2.46), physical inactivity ( OR=5.80, 95% CI: 3.09-10.88), poor or very poor sleep quality ( OR=1.65, 95% CI:1.23-2.41), number of chronic diseases (1 chronic disease: OR=1.84, 95% CI: 1.08-3.14; 2 chronic diseases: OR=3.22, 95% CI: 1.81-5.71; 3 or more chronic diseases: OR=3.74, 95% CI: 2.11-6.65), poor childhood socioeconomic status ( OR=2.98, 95% CI: 1.23-7.20), and poor adulthood community socioeconomic status ( OR=3.87, 95% CI: 1.63-9.17) were significant risk factors for sarcopenia. Conclusions:The prevalence of sarcopenia was relatively low in middle-aged and old population in Zhejiang. Age, BMI, unhealthy diet, physical activity level, sleep quality, number of chronic diseases, childhood socioeconomic status, and adulthood community socioeconomic status were identified as significant influencing factors.
4.Prognostic value of serum thrombospondin-1 combined with alberta stroke program early CT score in patients with acute large-vessel occlusive cerebral infarction

Objective To investigate the prognostic value of serum thrombospondin-1(THBS-1)combined with Alberta Stroke Program Early CT Score(ASPECTS)in patients with acute cerebral infarction with large vessel occlusion(ACI-LVO).Methods A total of 198 ACI-LVO patients admitted to the hospital from January 2022 to October 2024 were prospectively enrolled(ACI-LVO group).Patients were categorized into mild,moderate,and severe subgroups based on National Institutes of Health Stroke Scale(NIHSS)scores,and into good or poor prognosis groups based on 90-day outcomes.Serum THBS-1 levels were measured using ELISA,and ASPECTS scores were calculated.Spearman correlation analysis was used to assess the relationship between THBS-1,ASPECTS,and NIHSS scores.Prognostic factors and predictive values of THBS-1 and ASPECTS were also analyzed.Results Among the 198 patients,58 had mild,67 had moderate,and 73 had severe ACI-LVO.Serum THBS-1 levels increased,and ASPECTS scores decreased progressively with worsening disease severity(P<0.05).NIHSS scores were positively correlated with serum THBS-1 and negatively correlated with ASPECTS scores(P<0.05).The 90-day poor outcome rate was 37.37%(74/198).Independent risk factors for poor prognosis included older age,modified thrombolysis in cerebral infarction score of 0~2a,higher NIHSS score,larger infarct volume,hemorrhagic transformation,and elevated THBS-1.A higher ASPECTS score was an independent protective factor(P<0.05).The area under the curve(AUC)for predicting prognosis was 0.794 for THBS-1,0.831 for ASPECTS,and 0.903 for their combination,with the combined model showing superior predictive value(P<0.05).Conclusions Elevated serum THBS-1 and decreased ASPECTS scores are associated with increased disease severity and poorer prognosis in ACI-LVO patients.The combination of serum THBS-1 and ASPECTS score provides high predictive value for assessing prognosis in ACI-LVO patients.
5.Effects of PPARδ agonist GW501516 on the intestinal mucosal barrier in septic mice by regulating autophagy

Objective To investigate the mechanism of action of GW501516 on the intestinal mucosal barrier of septic mice.Methods Septic mice were divided into four groups:model,GW501516,GW501516+Rap,and a sham group.Hematoxylin and eosin staining was used to observe the pathological injury of the small intestinal mucosa.D-lactate(D-LA)and diamine oxidase(DAO)levels in serum,as well as secreted immunoglobulin A(sIgA)and inflammatory factor levels in small intestinal tissues,were measured using the enzyme-linked immunosorbent assay.The number of autophagosomes in the intestinal tissue was determined using transmission electron microscopy.Peroxisome proliferator activated receptor δ(PPARδ)and autophagy-related protein expression levels in small intestinal tis-sues were assessed using Western blotting.Results Compared with mice in the sham group,those in the model group exhibited severe intestinal mucosal injury,significantly higher D-LA and DAO serum levels,elevated sIgA and inflammatory factor levels in the small intes-tinal tissues,excessive autophagy in the small intestinal tissues,and significantly lower PPARδ protein levels(all P<0.05).Compared with the model group,intestinal mucosal injury was significantly reduced in the GW501516 group,with a significant decrease in serum D-LA and DAO levels,as well as reduced sIgA and inflammatory factor levels in small intestinal tissues.Excessive autophagy in small intes-tinal tissues was inhibited,and PPARδ protein levels were significantly increased(all P<0.05).However,Rap significantly inhibited the ameliorative effects of GW501516 on the intestinal mucosal injury in septic mice.Conclusion The PPARδ agonist GW501516 improves intestinal mucosal barrier damage in septic mice by inhibiting autophagy.
6.Nano-drug delivery strategies affecting cancer-associated fibroblasts to reduce tumor metastasis.

Tumor metastasis is the leading cause of high mortality in most cancers, and numerous studies have demonstrated that the malignant crosstalk of multiple components in the tumor microenvironment (TME) together promotes tumor metastasis. Cancer-associated fibroblasts (CAFs) are the major stromal cells and crosstalk centers in the TME of various kinds of tumors, such as breast cancer, pancreatic cancer, and prostate cancer. Recently, the CAF-induced pro-tumor metastatic TME has gained wide attention, being considered as one of the effective targets for tumor therapy. With in-depth research, CAFs have been found to promote tumor metastasis through multiple mechanisms, such as inducing epithelial-mesenchymal transition in tumor cells, remodeling the extracellular matrix, protecting circulating tumor cells, and facilitating the formation of a pre-metastatic niche. To enhance the anti-tumor metastasis effect, therapeutic strategies designed by combining nano-drug delivery systems with CAF modulation are undoubtedly a desirable choice, as evidenced by the research over the past decades. Herein, we introduce the physiological properties of CAFs, detail the possible mechanisms whereby CAFs promote tumor metastasis, categorize CAFs-based nano-drug delivery strategies according to their anti-metastasis functions and discuss the current challenges, possible solutions, as well as the future directions in order to provide a theoretical basis and reference for the utilization of CAFs-based nano-drug delivery strategies to promote tumor metastasis therapy.
7.Development and application research of a lung rehabilitation education game software for patients with chronic obstructive pulmonary disease based on health belief model

Objective Taking into account the physical and mental characteristics of patients with COPD,we develop and implement a lung rehabilitation education game software,aiming to explore more effective health education practices for COPD.Methods Drawing on the Health Belief Model theory,the game content was developed by semi-structured interviews,brainstorming sessions,and expert consultations,and it was implemented via a WeChat mini program.Patients with COPD from the respiratory department of a tertiary general hospital of Luzhou city between March and October 2023 were conveniently selected as the study subjects.Among them,40 patients from July to October 2023 were designated as an experimental group,and another 40 from March to June 2023 constituted a control group.The experimental group was provided with routine health education combined with pulmonary rehabilitation games,whereas the control group received standard health education.After the intervention,the researchers compared the COPD knowledge questionnaire scores and pulmonary rehabilitation adherence between the 2 groups,and assessed the satisfaction of both patients and nurses with the software.Results No sample detachment.The score of the COPD knowledge questionnaire in the experimental group was higher than that in the control group,and the difference was statistically significant(Z=5.361,P<0.001).The proportion of patients in the experimental group with good adherence to pulmonary rehabilitation(85%)was significantly higher than it in the control group(25%),(x2=29.091,P<0.001).The patients'overall satisfaction rate with the game was 95%,with operational effectiveness receiving the highest satisfaction rating at 97%.Conclusion The pulmonary rehabilitation education game for patients with COPD can improve their knowledge of COPD,enhance their compliance with pulmonary rehabilitation.Patients have high satisfaction with the software.The software enriches the clinical health education methods,which can be used in clinical practice.

10.Visualisation and Analysis of Literature on Digilization of Traditional Chinese Medicine Culture From 2006 to 2024

[Objective]To analyze the digital research literature in the field of traditional Chinese medicine culture,to understand the hot topics and lineage of research on digitization of traditional Chinese medicine culture and analyze the future development trend.[Methods]China National Knowledge Infrastructure(CNKI)and Web of Science(WOS)databases were searched to collect relevant journal literature in the field of digitization of traditional Chinese medicine culture from January 2006 to February 2024.CiteSpace 6.1.R6 software was used to analyze the evidence visualization of the included literature,including the trend of publication,the cooperation of authors and institutions,journal distributions,keyword co-occurrence,clustering and burst word analysis,and the visualization map was drawn.[Results]A total of 523 documents were included in the analysis.The number of studies on digitization of traditional Chinese medicine culture was generally on the rise;there were 82 core authors,with a total of 184 articles(35.18%),and the core group of authors had not yet been formed.Research institutions were mainly located in Beijing and Shanghai,and there was less exchange and cooperation among different institutions and scholars.High-frequency keywords in the field of digitization of traditional Chinese medicine culture included Chinese medicine,communication,new media and international communication,etc.,of which cultural self-confidence,integrated media and popularization of science were hot and cutting-edge topics in recent years.[Conclusion]In the past years,the digital research on Chinese medicine culture had shown more obvious features of digital integration,more focused direction of digital empowerment,and a focus on digital inheritance and dissemination direction research.In the future,researchers should further improve the construction of the research system,promote the cooperation of scholars in different fields,adopt interdisciplinary research methods and promote the diversification of research contexts.
