1.Quality evaluation of Qingwen hufei granules based on fingerprints combined with multi-component content determination

OBJECTIVE To provide a scientific basis for the quality evaluation and clinical application of Qingwen hufei granules. METHODS Fourteen batches of Qingwen hufei granules were used as samples to establish high-performance liquid chromatography (HPLC) fingerprints using the Similarity Evaluation System for Chromatographic Fingerprint of Traditional Chinese Medicine (2012 Edition). The chromatographic peaks were identified and the similarity was evaluated. Cluster analysis (CA), principal component analysis (PCA), and orthogonal partial least squares-discriminant analysis (OPLS-DA) were used to conduct chemical pattern recognition analysis on the 14 batches of samples. Meanwhile, the contents of neochlorogenic acid (NGA), chlorogenic acid (CHA), cryptochlorogenic acid (CGA), forsythoside A (FTA), 3,5-O-dicaffeoylquinic acid (3,5-O- DA), 4,5-O-dicaffeoylquinic acid (4,5-O-DA), and angoroside C (AGC) in the samples were determined by HPLC. RESULTS The methodological investigation results of both the fingerprint and the content determination complied with the relevant requirements. Fourteen common peaks were indicated in the HPLC fingerprints of the 14 batches of samples, and 7 of them were identified [NGA (peak 2), CHA (peak 3), CGA (peak 5), FTA (peak 11), 3,5-O-DA (peak 12), 4,5-O-DA (peak 13), and AGC (peak 14)]; the similarity of each sample was greater than 0.94. The results of CA and PCA showed that the samples could be classified into 3 categories; the results of OPLS-DA indicated that peak 4 (unknown), peak 11 (FTA), peak 8 (unknown), peak 9 (unknown), and peak 1 (unknown) were the differential components. The content ranges of NGA, CHA, CGA, 3,5-O-DA, FTA, 4,5-O-DA and AGC in the 14 batches of samples were 0.210 4-0.458 7, 0.269 1-0.506 3, 0.228 1-0.461 1, 0.443 9-1.044 6, 0.066 7-0.155 7, 0.062 8-0.143 8, and 0.057 4-0.105 7 mg/g, respectively. CONCLUSIONS The HPLC fingerprint and multi-component content determination methods established in this study are efficient and reliable, and can be used for the quality evaluation of Qingwen hufei granules.

In July 2024,the Diagnosis Related Groups(DRG)2.0 is released based on the Notice from the National Healthcare Security Administration on Issuing the DRG 2.0 and Deepening the Relevant Work.Compared with DRG 1.1,version 2.0 was established based on a wider range of suggestions regarding the Adjacent Diagnosis Related Groups(ADRG),Major Comorbidity or Complication(MCC),and Comorbidity or Complication(CC)from various institutions.A list of disease diagnoses and surgical operations that are not used as grouping rules was compiled,and grouping efficacy was further improved by upgrading the algorithms for MCC and CC with the help of AI.Meanwhile,it is necessary to pay more attention to the number of cases of ADRG,the better methods to list the MCC/CC,the suggestions of various doctors and continuously standardize the data and update the grouping scheme of DRG.
3.Clinical characteristics and risk factors of second asparaginase-associated pancreatitis in children with acute lymphoblastic leukemia

Objective:To investigate the clinical features and risk factors of second asparaginase-associated pancreatitis (AAP) in children with acute lymphoblastic leukemia (ALL).Methods:In this case-control study, the clinical data of ALL children who were diagnosed with pancreatitis and received asparaginase chemotherapy at Shanghai Children′s Medical Center, Shanghai Jiao Tong University School of Medicine from November 2013 to November 2023 were retrospectively analyzed.Multivariate Logistic regression was used to determine independent risk factors for the occurrence of second AAP.Results:A total of 78 children with AAP were included, of whom 32 were re-exposed to asparaginase.Ten of the 32 cases (31.25%) suffered second AAP, and they were rated at intermediate risk, with an age of (8.26±0.87) years.The number of asparaginase re-exposures before second AAP was (2.10±0.99) times.Second AAP developed during the intensification period in 2 cases and during the re-induction period in 8 cases.There were 3 cases (30.00%) of Grade 1 pancreatitis and 7 cases (70.00%) of Grade 2 pancreatitis.No patient died of second AAP.Two patients (20.00%) experienced AAP-related long-term complications, including chronic pancreatitis and insulin-dependent diabetes mellitus.After 2 children with no clinical data related to the first AAP were excluded, 30 re-exposed children were divided into a control group (22 cases) and a case group (8 cases) according to the presence or absence of second AAP.The univariate analysis showed that there were significant differences in activated partial thromboplastin time (APTT)[(29.80±5.25) s vs.(38.47±1.42) s, t=-0.45, P=0.023] and total bilirubin (TBIL)[18.40 (11.00, 25.53) μmol/L vs.24.30 (19.93, 46.48) μmol/L, Z=-0.32, P=0.020] between the 2 groups.The further multivariate Logistic regression analysis showed that after adjusting for age and immunophenotype, TBIL ( HR=1.115, P=0.048) was an independent risk factor for second AAP.Comparisons of the prognosis revealed that there were no significant differences in long-term complications ( P=0.168) and final survival ( P=0.716) between children with only once AAP (68 cases) and those with second AAP (10 cases). Conclusions:Second AAP mostly occurs after the second dose of asparaginase, and the re-induction period is the peak period of incidence of second AAP.Hyperbilirubinemia may be a risk factor of second AAP.Compared with once AAP, second AAP does not increase the mortality and the incidence of AAP-related long-term complications.
4.Efficacy and safety of endoscopic retrograde cholangiopancreatography for pediatric patients with asparaginase-associated pancreatitis

Objective:To preliminarily investigate the efficacy and safety of endoscopic retrograde cholangiopancreatography (ERCP) for recurrent acute pancreatitis and refractory pancreatic pseudocysts following asparaginase-associated pancreatitis (AAP) in children.Methods:A retrospective analysis was conducted on the clinical data of 19 pediatric patients who underwent ERCP for recurrent aute pancreatitis and refractory pancreatic pseudocysts following AAP at Shanghai Children's Medical Center between November 2018 and July 2024. Data primarily included ERCP procedure outcomes, ERCP-related complications. Pseudocyst size, frequency of pancreatitis episodes, and body mass index (BMI) before and after the ERCP intervention were compared.Results:A total of 30 ERCP procedures were performed in 19 children, with a technical success rate of 96.7% (29/30). Among the 13 children with recurrent acute pancreatitis, 11 remained symptom-free post-ERCP. One clild experienced 3 further episodes of acute mild pancreatitis following asparaginase administration, though the severity and duration of abdominal pain were reduced compared with pre-ERCP episodes. Another child developed 1 episode of aute mild pancreatitis after overeating. Both of them were resolved following medical treatment. Of the 12 children with pancreatic pseudocysts, complete resolution was observed in 8 cases, and significant reduction in size was achieved in 4 cases. All patients showed an increase in BMI postoperatively. A total of 10 procedure-related adverse events occurred, includings 5 post-ERCP pancreatitis (PEP), 5 postoperative infection, 2 hyperamylasemia, and 1 postoperative bleeding. Co-occurrence of PEP and infection was noted in 2 procedures, and PEP with bleeding in 1 procedure. All complications were managed successfully with conservative treatment.Conclusion:ERCP demonstrates favorable efficacy and acceptable safety for managing recurrent acute pancreatitis and refractory pancreatic pseudocysts following AAP in children.
5.Clinical analysis of CT angiography responsible vessel detection in children with hemoptysis by location setting of different regions of interest

Objective To explore the ability of different regions of interest(ROI)location settings for the detection of bronchial artery and pulmonary vascular lesions in children with hemoptysis via computed tomography angiography(CTA)examination.Methods The hemoptysis group(79 cases)who underwent chest CTA examination and the control group(79 cases)with negative CTA results were retrospectively selected.The ROI of the hemoptysis group were placed in the pulmonary trunk,left ventricle,ascending aorta and descending aorta,which were defined as groups 1,2,3 and 4 respectively.Clinical data(age,sex)and CT parameters,including bronchial artery diameter,ascending aorta CT value,pulmonary artery CT value,△CTA-PA,bronchial artery image score,pulmonary artery image score,were recorded and analyzed,respectively.Results There was a statistically significant difference in the diameter of the left and right bronchial arteries between the hemoptysis group and the control group(P<0.05).In the hemoptysis group,there were statistically significant differences in the image scores of the left and right bronchial arteries,pulmonary artery image scores,and△CTA-PA among the four subgroups(P<0.05).According to post-hoc comparison results,there was a statistically significant difference in the overall mean scores of the right bronchial artery image score(x2=11.333,P<0.05)and left bronchial artery image score(x2=8.111,P<0.05)between groups 2 and 3.Conclusion When ROI is placed in the ascending aorta,bronchial artery lesions and pulmonary vascular lesions can be detected simultaneously in CTA examination,which is helpful for the diagnosis of hemoptysis vascular etiology in children.
6.Relapse-related candidate genes and their clinicopathological connections of diffuse large B cell lymphoma

Objective:To explore the relapse-related genes and their clinicopathological connections of diffuse large B cell lymphoma (DLBCL).Methods:Targeted panel sequencing was conducted on 32 eligible DLBCL samples; the patients were diagnosed, treated, and went into complete remission at the First Affiliated Hospital of Nanjing Medical University from January 2015 to December 2019, including 14 cases with recurrence (relapsed group) and 18 cases with long-term complete remission of over five years (remission group). Clinical and pathological data were further reviewed. Fisher′s exact test was employed to compare the differences in clinicopathological characteristics and mutation patterns between the two groups.Results:Among the 32 patients, there were 18 males and 14 females, with a male to female ratio of 1.3∶1.0 and a median age of 53 (45.5, 67.0) years. In the relapsed group, PIM1 (11/14), KMT2D (7/14), PRDM1 (6/14), MYD88 (6/14), DTX1 (6/14) emerged as the most frequently mutated genes. In the remission group, while recurrent PIM1, KMT2D and MYD88 mutations were also observed, the TP53 gene exhibited the highest mutation frequency (6/18). Compared to the remission group, relapsed group showed elevated mutation frequencies of PIM1 ( P=0.013) and FAT4 ( P=0.010), alongside a reduced incidence of TP53 mutations. In all 32 patients, DLBCL with CD79B, CCND3, DTX1, KMT2D and PRDM1 mutations demonstrated a propensity towards advanced clinicopathologic stage. Conclusions:Relapsed DLBCL has distinctive clinicopathological and genetic features. PIM1 and FAT4 may be served as potential biomarkers for screening relapsed DLBCL-NOS and as targets for novel therapeutic strategies.

8.Self-guided attention network for detecting responsible lesions related to cerebral palsy in children with periventricular white matter injury

Objective To observe the efficacy of self-guided attention network for detecting responsible lesions related to cerebral palsy(CP)in children with periventricular white matter injury(PVWMI).Methods Totally 383 children with PVWMI were retrospectively enrolled and divided into CP group(n=243)and non-CP group(n=140),while 214 children without obvious brain abnormality on brain MRI were taken as control group.ROI of 4 key anatomical structures related to CP,i.e.centrum semiovale,posterior limb of internal capsule,cerebral peduncle and thalamus were delineated on T1WI,while responsible lesions related to CP within the key anatomical structures were labeled on T2WI,and the images were then registrated and used as input of the networks.ResNet34 network was adopted combined with attention and self-guided networks to train the network for detecting responsible lesions related to CP in children with PVWMI,and their efficacies were evaluated.The optimal network was screened,and its efficacy for segmenting the key anatomical structures was evaluated.Results Self-guided attention network was the optimal network,its area under the curve(AUC)for detecting lesions was 0.794-0.914,and the Dice similarity coefficient for segmenting the key anatomical structures was 0.702-0.764.Conclusion Self-guided attention network could be used for preliminarily detecting responsible lesions related to CP in children with PVWMI.
