[摘 要] 目的：探讨含溴结构域蛋白1（BRD1）通过调控PI3K/AKT信号通路影响非小细胞肺癌（NSCLC）进展的作用机制。方法：利用TIMER 3.0数据库评估BRD1作为免疫治疗标志物的预测价值。利用慢病毒体系构建稳定过表达BRD1及N端截短突变体BRD1-ΔN的人肺癌细胞A549和NCI-H1299。采用CCK-8实验、克隆形成实验、划痕愈合实验及Transwell实验检测过表达BRD1/BRD1-ΔN对细胞增殖和迁移能力的影响；流式细胞术和Western blotting检测细胞凋亡水平及凋亡相关蛋白的变化。RNA-seq筛选受BRD1/BRD1-ΔN影响的信号通路，Western blotting及体外激酶实验验证BRD1/BRD1-ΔN过表达对AKT磷酸化的调控作用。采用AKT抑制剂LY294002处理过表达BRD1/BRD1-ΔN的NCI-H1299细胞，结合功能实验（CCK-8实验、划痕愈合实验）明确过表达BRD1/BRD1-ΔN促进细胞的增殖和迁移是否依赖AKT活化。通过RNA-seq联合KEGG/GSEA分析、免疫共沉淀（Co-IP）、RT-qPCR，鉴定整合素α2（ITGA2）是促进AKT活化的关键分子；通过Co-IP、敲低ITGA2、Western blotting及功能实验明确ITGA2在BRD1调控AKT活化中的具体作用。结果：数据库分析显示，BRD1可作为NSCLC免疫治疗的中等预测标志物（AUC = 0.625）。过表达BRD1/BRD1-ΔN显著增强了NSCLC细胞的增殖和迁移能力（P < 0.01或P < 0.001），但不影响细胞凋亡及凋亡相关蛋白的表达（均P > 0.05）；RNA-seq分析表明过表达BRD1/BRD1-ΔN主要富集于PI3K/AKT信号通路。Western blotting及体外激酶实验显示，BRD1/BRD1-ΔN过表达不影响AKT上游激酶（PDK1）的磷酸化，也不改变AKT的乙酰化修饰水平（均P > 0.05）；AKT抑制剂LY294002处理过表达BRD1/BRD1-ΔN细胞，发现BRD1/BRD1-ΔN促细胞增殖和迁移的作用部分依赖AKT的活化；RT-qPCR及Western blotting证实，BRD1过表达可上调ITGA2的mRNA和蛋白水平，敲低ITGA2则可逆转BRD1诱导的AKT激活及细胞增殖和迁移能力的增强。结论：BRD1是NSCLC的一种新型促癌因子，其通过上调ITGA2表达激活AKT信号通路，进而促进肺癌细胞的增殖和迁移。

Definitive chemoradiotherapy (dCRT) has become a cornerstone in the treatment of locally advanced esophageal cancer; however, local control remains suboptimal, and persistent lesions or locoregional recurrences after treatment are not uncommon. For patients without distant metastases but with local failure, whether surgical intervention can still offer curative potential remains a major clinical dilemma. Salvage esophagectomy (SE) offers potential long-term survival for selected patients, but this procedure is performed in the context of severe fibrosis, impaired local blood supply, and obscured anatomical planes following chemoradiotherapy, resulting in significantly higher perioperative risk compared to primary esophagectomy. Consequently, controversies exist regarding patient selection, preoperative restaging, choice of surgical approach, extent of lymphadenectomy, gastrointestinal reconstruction, and perioperative management. In recent years, with the refinement of restaging modalities such as PET/CT, the accumulation of experience in high-volume centers, and emerging evidence from clinical studies, the clinical role of SE has gradually shifted from a "high-risk salvage measure" to a "selective curative strategy aimed at achieving long-term survival in carefully selected patients". Nevertheless, standardized guidelines for patient selection, technical approaches, and perioperative management are still lacking. Based on current evidence and clinical experience, experts organized by the Integrated Esophageal Cancer Committee of Chinese Anti-Cancer Association systematically reviewed key issues regarding SE, including its definition, indications, preoperative evaluation, choice of surgical approach, lymphadenectomy, gastrointestinal reconstruction, and perioperative management, and formulated a Chinese expert consensus. This consensus aims to provide guidance for standardized assessment, appropriate referral, individualized surgical decision-making, and optimized perioperative management of patients with locoregional failure after dCRT. Ultimately, this will increase the likelihood of R0 resection, reduce the risk of severe complications, and promote the safer, more judicious, and standardized implementation of SE in high-risk scenarios.

Objective To analyze the screening results of prostate cancer(PCa)based on the serum prostate-specific antigen(PSA)level in middle-aged and elderly men in Kaifeng,so as to provide reference for optimizing the early screening strategy of PCa.Methods A multicenter cross-sectional design was adopted to analyze the serum PSA test results of male residents aged 52 to 94 collected from 14 community health service stations in Kaifeng during Jul.2024 and Apr.2025. Participants with PSA ≥4 μg/L were recommended to undergo prostate biopsy at a tertiary grade A hospital for a confirmed diagnosis.Multivariate logistic regression was used to analyze the influencing factors of referral compliance.The PSA positivity rate,referral loss rate,puncture willingness rate,and biopsy positivity rate were calculated.Results A total of 2812 men received free PSA screening.Among them,248 had abnormal PSA(≥4 μg/L),with a positive rate of 8.8%(248/2812).Only 105 cases were successfully referred to a tertiary grade A hospital,with a referral rate of 42.3%(105/248),and the referral loss rate reached 57.7%.Fifty-seven patients were advised to undergo prostate biopsy,but only 23 actually underwent prostate biopsy,and the willingness rate was 40.4%(23/57).Fourteen cases were diagnosed with PCa and the positive rate of prostate biopsy was 60.9%(14/23).The overall detection rate of PCa was 0.5%(14/2812).In the screening stage,the median PSA was 6.7(4.8-11.2)μg/L.The Spearman correlation analysis showed a positive correlation between participant age and PSA level(r_s=0.28,P＜0.001).In the diagnostic stage,only 40.4% of the referred patients underwent prostate biopsy,and the loss rate reached 59.6%.The median PSA in the PCa group was significantly higher than that in the non-PCa group(9.8 μg/L vs. 5.1 μg/L,P=0.002).Multivariate logistic regression analyses showed that advanced age(OR=1.08)and educational level of primary school or below(OR=3.32)were independent influencing factors for referral disorders(P＜0.001).In terms of screening efficacy,the overall biopsy positivity rate was 5.6%(14/248).Conclusion Conducting PCa screening based on PSA testing among middle-aged and elderly male residents can significantly increase the detection rate of clinically meaningful PCa,thereby improving the overall prognosis of patients.However,insufficient referral compliance(only 42.3%)and low puncture willingness rate(only 40.4%)have limited the screening benefits.Therefore,the research suggests that measures such as strengthening health education,popularizing relevant knowledge,and providing referral support are needed to reduce the churn rate.

Objective:To investigate the influencing factors of adult dental fluorosis in drinking water-borne endemic fluorosis areas of Inner Mongolia Autonomous Region.Methods:A case-control study was conducted in January 2024 to select adult fluorosis patients (case group) and healthy individuals (control group) from the drinking water-borne endemic fluorosis areas in Helinger County, Hohhot City, Inner Mongolia Autonomous Region as the survey subjects. Urine samples were collected to determine urinary fluoride concentration. A questionnaire survey was conducted. SPSS 25.0 software was used for χ 2 test and multivariate logistic regression analysis. Restricted cubic spline (RCS) was used to analyze the association between urinary fluoride concentration and the risk of dental fluorosis in adults. Results:A total of 161 individuals were included in the survey, including 100 in the case group and 61 in the control group. The results of univariate analysis showed that there were statistically significant differences in the distribution of gender, smoking, and urinary fluoride concentration between the case group and the control group (χ 2 = 7.54, 5.02, 9.69, P < 0.05). The results of multivariate logistic regression analysis indicated that gender ( OR = 0.36, 95% CI: 0.18 - 0.73, P = 0.005) and urinary fluoride concentration ( OR = 3.08, 95% CI: 1.46 - 6.67, P = 0.003) were the influencing factors of adult fluorosis. RCS analysis showed a significant linear dose-response relationship between the risk of dental fluorosis and urinary fluoride concentration ( Poverall trend = 0.001, Pnonlinear = 0.071). When the urinary fluoride concentration was greater than 1.57 mg/L, the risk of dental fluorosis increased with the increase of urinary fluoride concentration. Conclusion:Gender and urinary fluoride concentration are the risk factors of dental fluorosis in adults in drinking water-borne endemic fluorosis areas of Inner Mongolia Autonomous Region.

OBJECTIVE: To evaluate the efficacy and safety of Shexiang Tongxin Dropping Pill (STDP) in treating stable angina patients with phlegm-heat and blood-stasis syndrome by exercise duration and metabolic equivalents. METHODS: This multicenter, randomized, double-blind, placebo-controlled clinical trial enrolled stable angina patients with phlegm-heat and blood-stasis syndrome from 22 hospitals. They were randomized 1:1 to STDP (35 mg/pill, 6 pills per day) or placebo for 56 days. The primary outcome was the exercise duration and metabolic equivalents (METs) assessed by the standard Bruce exercise treadmill test after 56 days of treatment. The secondary outcomes included the total angina symptom score, Chinese medicine (CM) symptom scores, Seattle Angina Questionnaire (SAQ) scores, changes in ST-T on electrocardiogram and adverse events (AEs). RESULTS: This trial enrolled 309 patients, including 155 and 154 in the STDP and placebo groups, respectively. STDP significantly prolonged exercise duration with an increase of 51.0 s, compared to a decrease of 12.0 s with placebo (change rate: -11.1% vs. 3.2%, P<0.01). The increase in METs was significantly greater in the STDP group than in the placebo group (change: -0.4 vs. 0.0, change rate: -5.0% vs. 0.0%, P<0.01). The improvement of total angina symptom scores (25.0% vs. 0.0%), CM symptom scores (38.7% vs. 11.8%), reduction of nitroglycerin consumption (100.0% vs. 11.3%), and all domains of SAQ, were significantly greater with STDP than placebo (all P<0.01). The changes in Q-T intervals at 28 and 56 days from baseline were similar between the two groups (both P>0.05). Twenty-five participants (16.3%) with STDP and 16 (10.5%) with placebo experienced AEs (P=0.131), with no serious AEs observed. CONCLUSION STDP could improve exercise tolerance in patients with stable angina and phlegm-heat and blood stasis syndrome, with a favorable safety profile. (Registration No. ChiCTR-IPR-15006020).

To explore the differences in biological gas production in the waterlogged zone of a coal seam fire-affected area, in this study the in-situ gas production experiment was conducted with the mine water from aquitard layers in coal seam fire zones in Xinjiang. The results showed that the biogas production first increased and then decreased with the increase in distance, and the highest gas production reached 216.55 mL. The changes in key metabolic pathways during the anaerobic fermentation of coal were analyzed, which showed that as the distance from the aquitard layer in the coal seam fire zone increased, the methanogenesis pathways gradually shifted from acetic acid decarboxylation and carbon dioxide reduction to acetic acid decarboxylation and methylamine methanogenesis. The significant variability in the in-situ mine water reservoir conditions contributed to the differences. In addition, the reservoir pressure and temperature increased as the distance from the fire zone became longer, and the salinity of the farthest mine water in the reverse fault was the highest due to the lack of groundwater supply. Pearson correlation analysis revealed significant correlations of microbial communities with key functional genes and the types and concentrations of ions. The ions significantly influencing microbial enzymatic metabolic activities included Al³⁺, Fe²⁺, Co²⁺, Ni²⁺, Cu²⁺, Zn²⁺, Mg²⁺, PO₄^3-, and Mo⁶⁺. The differences in metabolic pathways were attributed to the integrated effects of a co-occurring environment with multiple ions. The gas production simulation experiments and metagenomic analyses provide data support for the practical application of in-situ biogas experiments, laying a foundation for engineering applications.

The communication between professional health caregivers, which mainly composed of medical staff, and patients and their families plays an important role in hospice care. The effective doctor-patient communication can guide terminal patients and their families to establish a correct medical view, reduce excessive medical treatment, and improve the quality of life for terminal patients. This paper briefly described the communication context of hospice care, the communication subject and the current situation of hospice care, analyzed the problems and challenges encountered by professional health caregivers, which mainly including medical staff, in the communication context of hospice care diagnosis and hospice care, and made the prospects of the development of effective communication measures.

Objective To investigate the clinical effect of transhepatic arterial chemoembolization(TACE)combined with tyrosine kinase inhibitors(TKIs)and programmed death receptors-1(PD-1)inhibitors(TACE+TKIs+PD-1 antibody)in the treatment of moderate advanced unresectable hepatocellular carcinoma(HCC).Methods The clinical data of 65 patients with moderate advanced unresectable hepatocellular carcinoma admitted to the Affiliated Hospital of North Sichuan Medical College from January 2020 to January 2022 were analyzed retrospectively.65 patients were treated with TACE+TKIs+PD-1 antibody.The observation indexes were tumor response,objective response rate(ORR),disease control rate(DCR),total survival time,progression free survival time,conversion operation rate and adverse drug reaction.Results The ORR of 65 p-atients with hepatocellular carcinoma was 49.2％(32/65),and the DCR was 89.2％(58/65).Among them,there were 2 patients with complete remission(CR),30 patients with partial remission(PR),26 patients with stable disease(SD),and 7 patients with progression disease(PD).Among 65 patients with hepatocellular carcinoma,18 patients were transformed into resectable hepatocell-ular carcinoma and underwent RO surgery.The conversion rate was 27.6％(18/65).65 patients were followed up for 3 to 22.4 months,The median follow-up time was 16.5 months.The median overall survival time and median disease progression free survival time of 65 patients were 14.5 months(95％CI:12.3～16.6 months)and 8.8 months(95％CI:6.9～10.6 months),respectively.After treatment,65 patients all had post embolism syndrome(abdominal pain,fever,nausea,vomiting and other symptoms),and some patients had transient abnormal liver function.Adverse drug reactions below grade 3 recovered within a few days.Some patients were associated with multiple adverse drug reactions.1 patient(1.5％)stopped using TACE because of stubborn vomiting,and 5 patients(7.6％)stopped using Lenvatinib because of severe liver function damage during treatment,2 patients(3％)stopped using Camrelizumab because of severe reactive capillary hyperplasia,one patient(1.5％)stopped using Tislelizumab because of severe hypothyroidism,one patient(1.5％)stopped the treatment of Lenvatinib and Sintilimab due to severe gastrointestinal bleeding.The adverse drug reactions of grade 3～4 occurred in other patients were alleviated after drug reduction,symptomatic treatment and hormone treatment.Conclusion TACE+TKIs+PD-1 antibody can obtain reliable clinical efficacy and anti-tumor activity in the treatment of moderate advanced unresectable hepatocellular carcinoma.

Objective To investigate the correlations of serum zinc finger E-box-binding protein 1 (ZEB1) and DNA methyltransferase 1 (DNMTl) with neurological deficit and prognosis in patients with acute cerebral hemorrhage (ACH). Methods A total of 105 ACH patients in the authors′hospital from July 2019 to July 2022 were selected as ACH group, and they were divided into mild group (n=34), moderate group (n=41) and severe group (n=30) according to the National Institutes of Health Stroke Scale (NIHSS) score at admission. ACH patients were divided into good prognosis group (n=65) and poor prognosis group (n=40) according to the score of modified Rankin Scale (mRS). Another 105 healthy individuals with physical examination in the same period were included as control group. A total of 45 diagnosed ACH patients treated in authors′ hospital from July 2021 to July 2022 were selected to verify the constructed receiver operating characteristic (ROC) curve for predicting the prognosis of ACH. Serum ZEB1 and DNMT1 levels were detected by enzyme-linked immunosorbent assay; the Spearman method was used to analyze the relationship of ZEB1 and DNMT1 levels with the NIHSS score in ACH patients; the multivariate Logistic regression model was used to analyze the influencing factors for prognosis of ACH patients, and the ROC curve was used to evaluate the predictive value of serum ZEB1 and DNMT1 levels for the prognosis of ACH patients. Results Compared with the control group, serum levels of ZEB1 and DNMT1 were significantly higher in the ACH group (P < 0.05). Compared with the mild group, the ACH patients in moderate group and severe group had significant higher serum levels of ZEB1 and DNMT1 as well as NIHSS score (P < 0.05); compared with the moderate group, ACH patients in severe group had significant higher serum levels of ZEB1 and DNMT1 as well as NIHSS score (P < 0.05). Spearman analysis result showed that serum levels of ZEB1 and DNMT1 were positively correlated with NIHSS scores in ACH patients (r=0.569, 0.763, P < 0.001). Univariate analysis revealed that compared to the good prognosis group, patients in the poor prognosis group had significant higher or larger NIHSS score, hematoma volume, and serum levels of ZEB1 and DNMT1 (P < 0.05). Logistic regression analysis demonstrated that increases in ZEB1 and DNMT1, NIHSS score, and hematoma volume were the risk factors for poor prognosis in ACH patients (P < 0.05). Values of area under the curve (AUC) of serum ZEB1 and DNMT1 iin predicting prognosis of ACH patients were 0.834 and 0.854 respectively, and the AUC of ZEB1 combined with DNMT1 for prediction of prognosis of ACH patients was 0.944, with sensitivity of 95.0% and specificity of 86.2%. The ROC curve for predicting prognosis was validated by the validation group, and the results showed the AUC was 0.903 (95%CI, 0.854 to 0.951), indicating that the prognosis prediction curve has good discrimination. Conclusion The levels of serum ZEB1 and DNMT1 are high in patients with ACH, both of which are significantly related to neurological defect and prognosis in patients with ACH. The combination of serum ZEB1 and DNMT1 may be more helpful for clinical evaluation of prognosis in patients with ACH.
