1.Molecular mechanisms of hyperlipidemic acute pancreatitis comorbid with fatty liver disease

Both hyperlipidemic acute pancreatitis and fatty liver disease are associated with lipid metabolism disorders and are commonly comorbid with each other in clinical practice. The pathogenesis of such comorbidity involves the interaction between multiple factors such as hypertriglyceridemia, metabolic syndrome, obesity, and insulin resistance, and these factors may form a vicious cycle and jointly promote disease progression. In clinical practice, hyperlipidemic acute pancreatitis is characterized by severe disease conditions, a high incidence rate of complications, a high mortality rate, and a tendency for recurrence, and it can easily lead to multi-organ damage and even multiple organ failure without timely treatment, posing a serious threat to the life of patients. Starting from the various signaling pathways associated with hyperlipidemic acute pancreatitis comorbid with fatty liver disease, this article discusses the potential molecular mechanisms of synergistic pathogenesis between hyperlipidemic acute pancreatitis and fatty liver disease, so as to provide a reference for the early prevention and treatment of such comorbidity.
2.Association between intergenerational parent-child separation and internalizing and externalizing behavioral problems among rural preschool children

Objective:
To explore the effects of intergenerational parent-child separation on internalizing and externalizing behavioral problems among rural preschoolers aged 3-6 years, so as to provide empirical evidence for formulating targeted intervention strategies and blocking the intergenerational transmission of parent-child separation.
Methods:
From March to June, 2024, ten kindergartens were selected from rural areas of Nanling County, Wuhu City, Anhui Province, and 2 279 preschool children aged 3- 6 years, along with their fathers, mothers and primary caregivers, were recruited for survey by cluster convenient sampling method. Children s fathers and mothers reported their own childhood parent-child separation information separately. Information on the children s history of psychological and behavioral problems was reported by the primary caregivers. Information of internalizing and externalizing behavioral problems in preschool children was reported with Strengths and Difficulties Questionnaire (SDQ).The Chi square test was used to compare the differences in the detection rates of psychological and behavioral problems among children with different intergenerational continuity of parent-child separation. Binary Logistic regression model was applied to analyze the association between intergenerational continuity of parent-child separation and internalizing and externalizing behavioral problems in preschool children.
Results:
The detection rate of internalizing behavioral problems was 27.5% and externalizing behavioral problems was 19.1% among rural preschool children. There were significant differences in internalization behavior, externalization behavior, hyperactivity impulse and peer communication problems among preschool children in different types of parent-child separation groups( χ 2=14.29, 8.80, 17.26, 13.84, all P <0.05). Binary Logistic regression analysis showed that after adjusting for covariates such as children s gender, birth weight, and delivery mode, compared with children without separation, intergenerational continuity of parent-child separation group( OR =1.95), only child separation( OR =1.74) were associated with significant higher risk of conduct problems(both P <0.05). Parent-child separation intergenerational continuation groups( OR =1.68), only child separation( OR = 1.47) were associated with significant higher risk of peer problems(both P <0.05). Both types of parent-child separation experiences significantly increased the risk of children s prosocial behavior problems,among them, the intergenerational continuation group was 1.99 times that of the control group ( OR =1.99), and the childhood only separation group was 2.09 times that of the control group ( OR =2.09)(both P <0.05). Compared with children whose parents and children had no experience of parent-child separation, children in the intergenerational continuation group of parent-child separation had increased risks of both internalizing and externalizing behavior problems ( OR =1.53, 1.65, both P <0.05).
Conclusion
All types of parent-child separation are associated with internalizing and externalizing behavioral problems among rural children aged 3 to 6 years.
3.Using of Lawnest,an intravascular snare in retrograde percutaneous coronary intervention for chronic total occlusion:a case report

Coronary chronic total occlusion(CTO)remains a significant challenge in the field of interventional therapy for coronary artery disease.With advancements in interventional techniques,particularly retrograde approaches,the success rate of CTO interventions has improved.The key steps of retrograde intervention include traversing collateral channels with the retrograde guidewire,wiring the occlusion segment,and establishing antegrade access.In this case,the patient had a heavily calcified occlusion at the left circumflex artery(LCX)ostium jailed by the prior stent implanted in the left main and anterior descending arteries,making it difficult to establish antegrade access using conventional methods after retrograde guidewire crossing.The procedure was successfully completed by employing a domestically developed intravascular snare system combined with a pre-assembled extension catheter technique to capture the retrograde guidewire and establish antegrade access,followed by stent implantation at the LCX ostium.This innovative approach provides a new strategy for complex CTO retrograde interventions,particularly in cases where retrograde guidewire entry into the antegrade guiding catheter is challenging.
4.Effect of Qishen Yixin Granules on microcirculatory endothelial dysfunction induced by Ang Ⅱ and high-fat diet in mice and its mechanism

Aim To clarify the mechanism by which Qishen Yixin Granules improved microcirculation vas-cular endothelial dysfunction(VED)in mice,through activating the Nrf2/HO-1 signaling pathway to regulate oxidative stress.Methods C57 mice were randomly divided into six groups:blank group,model group,pos-itive drug group,and low-,medium-,and high-dose groups of Qishen Yixin Granules.The VED model was established by long-term infusion of Ang Ⅱ combined with a high-fat diet.Each treatment group received the corresponding drug intervention.After four weeks of drug intervention,cardiac function was assessed by echocardiography.Carstairs staining was used to ob-serve the formation of microthrombi in myocardial tis-sue.The micro vascular ischemia was evaluated by Hei-denhain staining.The ultrastructure of endothelial cells was observed by electron microscopy.The levels of EMPs,ROS,NO,ET-1,TF,TM,VWF,and TXA2 in serum were measured by ELISA.The expression levels of MDA,SOD,and GSH-Px in mouse heart tissue were determined by chemical methods.Cardiac microvascu-lar density and the expression of Nrf2,Keap1,and HO-1 proteins were detected by Immunohistochemical stai-ning.The protein expressions of Keap1,cytoplasmic Nrf2,nuclear Nrf2,and HO-1 in myocardial tissue were detected by Western blot.Results Qishen Yixin Granules could effectively improve the cardiac function of mice,alleviate the damage of endothelial cells and endothelial function.They could up-regulate serum NO levels and the activities of antioxidant enzymes SOD and GSH-Px,while down-regulating the expression of ROS and vascular inflammatory injury factors such as ET-1,VWF,TXA2,TF,TM,and EMPs.Qishen Yixin Granules also increased the positive counts of CD34,Nrf2,and HO-1,as well as microvessel density.Fur-thermore,they inhibited the expression of MDA,Keap1,and cytoplasmic Nrf2 protein in myocardial tis-sue,while increasing the expression of nuclear proteins HO-1 and Nrf2.Conclusions Qishen Yixin Granules may inhibit oxidative stress and inflammatory response by regulating the Nrf2/HO-1 signaling pathway,thereby improving vascular endothelial damage and cardiac function in VED mice.
5.Jiawei Xiaoyao Pills improves depression-like behavior in rats by regulating neurotransmitters,inhibiting inflammation and oxidation and modulating intestinal flora

Objective To explore the bioactive components in Jiawei Xiaoyao Pills(JWXYP)and their mechanisms for alleviating depression-like behaviors.Methods The active compounds,key targets,and pathways of JWXYP were identified using TCMSP and TCMIP databases.Thirty-six SD rats were randomized equally into 6 groups including a control group and 5 chronic unpredictable mild stress(CUMS)-induced depression groups.After modeling,the 5 model groups were treated with daily gavage of normal saline,1.8 mg/kg fluoxetine hydrochloride(positive control drug),or JWXYP at 1.44,2.88,and 4.32 g/kg.The depression-like behaviors of the rats were evaluated using behavioral tests,and pathological changes in the liver and hippocampus were examined with HE staining.The biochemical indicators in the serum and brain tissues were detected using ELISA.Serum metabolomics analysis was performed to identify the differential metabolites using OPLS-DA,and gut microbiota changes were analyzed using 16S rDNA sequencing.Results Network pharmacology revealed that menthone and paeonol in JWXYP were capable of penetrating the blood-brain barrier to regulate inflammatory pathways and protect the nervous system.In the rat models subjected to CUMS,treatment with JWXYP significantly improved body weight loss,sucrose preference and open field activities,reduced liver inflammation,alleviated structural changes in the hippocampal neurons,decreased serum levels of TNF-α,IL-1β,IL-6 and LBP,and increased 5-HT and VIP concentrations in the serum and brain tissue,and these effects were the most pronounced in the high-dose group.Metabolomics analysis showed changes in such metabolites as indole-3-acetamide and acetyl-L-carnitine in JWXYP-treated rats,involving the pathways for bile acid biosynthesis and amino acid metabolism.16S rDNA analysis demonstrated increased gut microbiota diversity and increased abundance of Lactobacillus species in JWXYP-treated rats.Conclusion JWXYP alleviates depression-like symptoms in rats by regulating the neurotransmitters,inhibiting inflammation and oxidation,and modulating gut microbiota.
6.Analysis of curative effect of Shexiang Tongxin dropping pills combined with"Quadruple therapy"on elderly patients with heart failure with preserved ejection fraction

Objective To evaluate the clinical efficacy of Shexiang Tongxin dropping pills combined with"Quadruple therapy"in elderly patients with heart failure with preserved ejection fraction(HFpEF).Methods A total of 193 elderly HFpEF patients admitted to Affiliated Hospital of Yangzhou University from January to June 2022 were selected as study objects,and were divided into observation group(97 cases)and control group(96 cases)according to random number table method.The control group was treated with sacubitril valsartan sodium tablets+dapagliflozin tablets+metoprolol tartrate tablets+spironolactone tablets,and the observation group was treated with Shexiang Tongxin dropping pills on the basis of control group treatment,and both groups were treated for 6 months.The changes of cardiac function indexes,serum myocardial injury markers,endothelial function and inflammation indexes before and after treatment were compared between two groups.The average length of hospitalization,readmission rate within 30 days and the effective rate of treatment were analyzed.After 6 months of follow-up,the 6-minute walk test,6-month readmission rate and the incidence of major adverse cardiovascular event(MACE)were compared between two groups.Results After 30 days of treatment,the total effective rate of observation group was significantly higher than that of control group(93.8%vs.90.6%,x2=3.982,P=0.031).After 30 days of treatment,left ventricular end diastolic diameter,left ventricular end systolic diameter,N-terminal pro-brain natriuretic peptide,cardiac troponin I,nucleotide-binding domain leucine-rich repeat and pyrin domain-containing receptor 3 in observation group were significantly lower than those in control group,left ventricular ejection fraction,early peak mitral valve diastolic blood flow velocity(E)/advanced peak mitral valve diastolic blood flow velocity(A)and nitric oxide were significantly higher than those in control group,and 6 min walking distance was significantly longer than that in control group(P<0.05).After 6 months of follow-up,the average length of hospital stay in observation group was significantly lower than that in control group,and the readmission rate within 30 days was significantly lower than that in control group(P<0.05).There was no significant difference in the 6-month readmission rate and the incidence of MACE between two groups(P>0.05).Conclusion Shexiang Tongxin dropping pills can inhibit cardiac inflammation.Combined with the"Quadruple therapy",the clinical effect of HFpEF in elderly patients is better than that of western medicine alone,and it is worthy of clinical promotion and application.
7.Clinical effects of Huayu Ditan Linao Decoction combined with Suhexiang Pills on elderly patients with cerebral hemorrhage in convalescent stage

AIM To explore the clinical effects of Huayu Ditan Linao Decoction combined with Suhexiang Pills on elderly patients with cerebral hemorrhage in convalescent stage.METHODS One hundred and ten patients were randomly assigned into control group(55 cases)for 1-month intervention of both conventional treatment and rehabilitation training,and observation group(55 cases)for 1-month intervention of Huayu Ditan Linao Decoction,Suhexiang Pills,conventional treatment and rehabilitation training.The changes in clinical effects,TCM syndrome score,NIHSS score,FMA score,BI index,cerebrovascular function indices(dynamic resistance,average flow velocity,peripheral resistance,average flow rate),serum inflammatory indices(IL-6,hs-CPR,TNF-α)and serum oxidative stress indices(SOD,MDA)were detected.RESULTS The observation group demonstrated higher total effective rate than the control group(P<0.05).After the treatment,the two groups displayed decreased TCM syndrome score,NIHSS score,dynamic resistance,peripheral resistance,serum inflammatory indices,MDA(P<0.05),and increased FMA score,BI index,average flow velocity,average flow rate,SOD(P<0.05),especially for the observation group(P<0.05).CONCLUSION For the elderly patients with cerebral hemorrhage in convalescent stage,Huayu Ditan Linao Decoction combined with Suhexiang Pills can enhance clinical effects,improve limb functions and nerve functions,regulate serum inflammatory and oxidative stress indice,and promote prognosis.
8.Quality evaluation of benchmark sample of Zexie Decoction based on HPLC fingerprints and content determination

AIM To evaluate the quality of benchmark sample of Zexie Decoction.METHODS HPLC fingerprints were established,after which the content determination of epoxy alisma ene,23-acetyl alisol B,23-acetyl alisol C,alisol A,alisol B,atractylenolide Ⅰ,atractylenolide Ⅱ and atractylenolide Ⅲ was performed,and the transfer rate and paste yield were calculated.RESULTS There were 20 common peaks in the fingerprints for 15 batches of benchmark samples with the similarities of more than 0.95.The average contents of various effective constituents were 180.86 μg/g for alisol B 23-acetate,18.65 μg/g for alisol C 23-acetate,34.74 μg/g for alismoxide,17.65 μg/g for alisol A,238.19 μg/g for alisol B,2.85 μg/g for atractylenolide Ⅰ,6.38 μg/g for atractylenolide Ⅱ,and 15.42 μg/g for atractylenolide Ⅲ,respectively.In the decoction piece-benchmark sample,alisol B 23-acetate,alisol C 23-acetate,atractylenolide Ⅰ,atractylenolide Ⅱ and atractylenolide Ⅲ demonstrated the average transfer rates of 12.09%,16.45%,3.93%,12.17%and 34.37%respectively.The paste yields in various batches of benchmark samples were 15.2%-20.2%.CONCLUSION HPLC fingerprints combined with content determination can be used for the quality control of benchmark sample of Zexie Decoction,thus provides a reference for the development of its compound preparations.
9.Modulation of cardiac inflammation by Qifu Yixin Granules in rats with heart failure via TLR4/MyD88/NF-κB signaling pathway

AIM To investigate the effects of Qifu Yixin Granules on cardiac inflammation in a rat model of heart failure.METHODS The rats were induced into chronic heart failure(CHF)models by 6-week intraperitoneal injection of doxorubicin followed by the random assignment of the successful rat models into the model group,the captopril group(22.5 mg/kg),and the low-dose,medium-dose,and high-dose Qifu Yixin Granules groups(2.84,5.67,11.34 g/kg),in contrast to the normal rats of the blank group.The rats had their body weight monitored;their cardiac function assessed by echocardiography;their serum levels of NT-proBNP,TNF-α,IL-6,IL-1 and CRP measured by ELISA;their cardiac morphological alterations observed by HE and Masson staining;their cardiac protein expressions of TLR4,MyD88 and NF-κB detected by immunohistochemistry and Western blot;and their cardiac mRNA expressions of TLR4,MyD88 and NF-κB measured by RT-qPCR.RESULTS Compared to the blank group,the model group exhibited significantly reduced body weight,LVEF and LVFS(P<0.01),alongside significantly elevated LVEDD,LVESD,and serum concentrations of NT-proBNP,TNF-α,IL-6,IL-1 and CRP(P<0.01).Additionally,the model group displayed greater myocardial inflammatory cell aggregation,increased collagen deposition(P<0.01);and upregulated myocardial protein and mRNA expressions of TLR4,MyD88 and NF-κB(P<0.01).Compared to the model group,the groups intervened with captopril or medium/high dose Qifu Yixin Granules demonstrated significantly increased body weight,LVEF and LVFS(P<0.05,P<0.01);significantly reduced LVEDD,LVESD,and serum levels of the aforementioned indicators(P<0.05,P<0.01);mitigated inflammation and collagen deposition(P<0.05,P<0.01);and downregulated myocardial protein and mRNA expressions of TLR4,MyD88 and NF-κB(P<0.05,P<0.01).CONCLUSION Qifu Yixin Granules attenuate cardiac inflammation and improve cardiac function in doxorubicin-induced CHF rats;this therapeutic effect is mediated by inhibiting the activation of the TLR4/MyD88/NF-κB signaling pathway.
10.Characterization of a Physiological Mock Circulation System Based on Capillary-Bundle Resistance Element

Objective To investigate the feasibility of parallel capillary bundle arrays for physiomimetic impedance modeling and establish a parametric quantification framework,thereby providing a customizable impedance characterization methodology for diverse in-vitro mock circulation researches.Methods Based on the parallel flow resistance and Poiseuille equation,a tube resistance element with multiple parallel-aligned capillary glass tubes was designed and fabricated.The resistance values of the capillary-bundle and a ball valve were measured through constant flow experiments analogous to electrical resistance measurement method.Moreover,a simple lumped-parameter mock circulation loop was constructed and the pressure and flow rate for each node of the loop were measured under different input flow waveforms.An 0D-Windkessel model corresponding to the experiment was developed.The impedance and compliance were adjusted to match the simulated and experimental pressure and flow waveforms.The accuracy of the capillary bundle impedance in pulsatile experiments was verified by using the computational resistance values.Results The constant-flow impedance calibration experiments revealed that the capillary bundle impedance remained unaffected by flow rate variations over a wide flow range.When the capillary bundle impedance was integrated into the pulsatile circulatory system and the same impedance value obtained from the constant-flow calibration was applied in the computational model,the resulting pressure and flow waveforms showed good agreement with those measured in the pulsatile experiments.However,when the ball valves with nominally identical impedance values were inserted in the pulsatile system,the calculated impedance exhibited a two-fold difference,and significant discrepancies were observed between the simulated and experimental terminal flow waveforms.Conclusions The capillary bundle impedance maintains a constant value regardless of flow rate variations.Once the calibrated resistance value is determined through constant flow experiments,it can be directly applied to pulsatile systems.This approach can provide quantitative pulsatile flow conditions for testing various medical devices.
