Nipah virus (NiV), a pathogen with pandemic potential, lacks approved treatments or vaccines,
highlighting the urgent need for research on immune-targeted antigenic determinants. A significant
gap persists in NiV research, as studies on the fusion (F) protein critical for viral entry as well as the B
cells epitopes, have primarily focused on computational prediction rather than experimental validation
of immunogenic epitopes obtained through immunoinformatics approach. This study focused on the
conserved F protein across NiV human isolates and employed an immunoinformatics approach to
predict linear B-cell epitopes capable of direct immune activation. Predicted epitopes were screened
for antigenicity, toxicity, and allergenicity. Molecular docking analysis was performed to evaluate
its binding affinity with B-cell receptors (BCRs). To validate its immunogenicity, the LF6 peptide was
synthesized and used in an indirect ELISA to test sera from cohort previously infected with NiV as well
as with negative cohort. Epitope LF6 (LISNIEIGFCL) was identified as a strong candidate based on its
immunogenic properties. Molecular docking showed favorable binding of LF6 with BCR. ELISA results
revealed that one sera from the survival cohort showed positive response which is an IgG antibody
response 2-fold higher (0.343) than the cut-off value (0.0171). This study provides the first reported
evidence linking computational predictions with functional immune reactivity for a B-cell epitope of
NiV. The findings suggest that LF6 has the potential to elicit specific immune responses. However, given
the small sample size, further validation in larger cohorts is essential to confirm LF6’s vaccine relevance.
2.Asymptomatic lymphatic filariasis in an elderly patient from Bako, Sarawak: A case report and public health implications

Lymphatic filariasis (LF) remains a significant public health challenge in many tropical regions where the
disease is endemic. In Malaysia, LF is found in small pockets across the country. Asymptomatic carriers
play a critical role in transmission but are often undetected. This report details an investigation of an
asymptomatic filariasis reported by local health authorities involving an 83-year-old female patient
residing in the Bako area, Sarawak. Despite being immobile due to a stroke, routine screening identified
an infection with Brugia malayi through microscopy and a rapid diagnostic test. Interestingly, the patient
exhibited no acute or chronic symptoms typically associated with filariasis. Contact tracing among
her family members revealed that her son was also infected. Both patients received treatment with
diethylcarbamazine (DEC) at a dosage of 6 mg/kg, along with albendazole 400 mg and ivermectin 12
mg. Preventive measures included health education, entomological studies, and the implementation of
a ‘Test & Treat Filariasis’ program in the village. By documenting both the index case and a secondary
asymptomatic case within the same household, the study provides a strong example of how routine
screening and contact tracing can identify hidden sources of infection. This adds significant value to
LF elimination strategies and emphasizes the importance of community-level surveillance programs.
Coordinated efforts by health authorities, including contact tracing, environmental assessments, and
targeted treatment, are essential for controlling the spread of LF and safeguarding public health.
3.Blood transfusion in elective gynecological surgeries in the Philippines: A multicenter study

BACKGROUND
Intraoperative transfusion for gynecologic surgery, when appropriately used, can improve patient outcomes. However, when utilized incorrectly, blood transfusion can worsen patient outcomes and increase patient cost. This study aimed to evaluate the blood transfusion practices of tertiary hospitals in the Philippines.
METHODSThe study utilized a cross-sectional design wherein prospective data were gathered through multiple sources across seven tertiary-level hospitals. Women admitted to undergo gynecologic surgery were recruited based on a set of criteria. A chart review was conducted, and blood utilization indices were calculated. Outcomes were compared between public versus private facilities and transfused versus nontransfused patients.
RESULTSAmong 514 patients, 79.7% underwent cross-matching and 75.1% received transfusions. Adverse events were rare, with no transfusion-related deaths. The overall crossmatch-to-transfusion ratio (C/T ratio) was 2.8, exceeding the 2.5 optimal benchmark; all public hospitals recorded a C/T ratio >2.5, whereas private centers had more efficient usage. Six hospitals met acceptable benchmarks for transfusion probability and transfusion index. Open abdominal procedures, particularly hysterectomy, accounted for the most blood used. Transfused patients had longer operative times, greater blood loss, lower preoperative hemoglobin, and more frequently involved resident physicians in training. Public hospitals recorded higher cross-match and transfusion rates, greater resident physician participation, and broader use of general anesthesia.
CONCLUSIONResults of the study highlight the importance of monitoring blood transfusion parameters to optimize blood utilization. The observed differences between public and private institutions in the country highlight the urgent need for standardized and evidence-based practice to ensure efficient transfusion protocols nationwide.

BACKGROUND
Bronchial asthma is one of the most common chronic childhood diseases encountered in the primary care setting. Adherence to recommendations from clinical practice guidelines on asthma can be utilized as an indicator of quality of care when evaluating the implementation of the universal health care in the Philippines.
OBJECTIVESTo determine the clinical profile of pediatric patients with bronchial asthma; and to evaluate the prescription patterns for asthma treatment in a primary care setting.
METHODSThis was a retrospective cohort study that involved review of the electronic medical records in a rural site of the Philippine Primary Care Studies (PPCS). All patients less than 19 years old who were diagnosed with asthma from April 2019 to March 2021 were included. Quality indicators for asthma care were based on adherence to recommendations from the 2019 Global Initiative for Asthma (GINA) Guidelines.
RESULTSThis study included 240 asthmatic children with mean age of 6 years (SD ± 4.9) and a slight male preponderance (55.4%). Majority (138 children or 57.5%) were less than 6 years old. Out of the 240 children, 224 (93.3%) were prescribed inhaled short-acting beta-agonists (SABA) and 66 (27.5%) were prescribed oral SABA. Only 14 children (5.8%) were prescribed inhaled corticosteroids (ICS), with 13 children (5.4%) given ICS with longacting beta-agonists (LABA) preparations, and one child (0.4%) given ICS alone. Quality indicators used in this study revealed underutilization of ICS treatment across all age groups, and an overuse of SABA-only treatment in children 6 years old and above. Moreover, 71.3% of the total patients were prescribed antibiotics despite the current GINA recommendation of prescribing antibiotics only for patients with strong evidence of lung infection, such as fever or radiographic evidence of pneumonia.
CONCLUSIONThere were 240 children diagnosed with asthma over a 2-year period in a rural community, with a mean age of 6 years old and a slight male predominance. This quality-of-care study noted suboptimal adherence of rural health physicians to the treatment recommendations of the GINA guidelines, with overuse of SABA and underuse of ICS for asthma control.

OBJECTIVE
This study compared the demographics, clinical profile, treatment, and outcomes of retinoblastoma patients seen at medical institutions in the Philippines between two time periods: 2010 to 2015 and 2016 to 2020.
METHODSThis was a multicenter, analytical, cohort study using review of medical charts and databases of retinoblastoma patients seen in 11 medical institutions from 2010 to 2020.
RESULTSThere were 636 patients (821 eyes) included in this study: 330 patients were seen in 2010 to 2015 while 306 in 2016 to 2020. More cases per annum were seen in the latter timeline. The number of patients with unilateral disease was not significantly different between the two time periods (p=0.51). Age at onset of symptom, age at initial consultation, and delay in consult were also not significantly different between the two time periods (p > 0.05). Patients had significantly different distributions of intraocular grades (p < 0.0001) and systemic staging (p < 0.0001) between the two time periods. Enucleation was the most common surgical treatment performed in both timelines. There was significant difference in the status of patients based on the need for systemic chemotherapy (p < 0.01). There was significant difference in outcome between the two time periods, including the proportions of living and deceased patients.
CONCLUSIONThis study compared the most comprehensive data on retinoblastoma patients in the country. There was no improvement in early health seeking behavior based on similar age at initial consult and delay in consult. Enucleation remained the most common treatment mode as opposed to chemotherapy due to similar percentage of patients with unilateral disease, an indication for enucleation rather than chemotherapy.

BACKGROUND
Multisystem inflammatory syndrome in children (MIS-C) is the most severe pediatric disease associated with COVID19 for which the optimal therapeutic strategy remains unknown. There have been studies that aim to describe treatment outcomes but have conflicting findings. Evidence is urgently needed to support treatment decisions for MIS-C.
OBJECTIVEThis study aims to compare the effectiveness of intravenous immunoglobulins (IVIG) plus corticosteroids versus IVIG alone as initial therapy in MIS-C, in terms of decrease in left ventricular dysfunction, decrease in inotropic support, reduced need for adjunctive immunomodulatory treatment, favorable fever course, decrease in mechanical ventilator use, decrease in admission to the pediatric intensive care unit, and decrease in coronary artery aneurysm.
METHODOLOGYPublished articles reporting MIS-C treatment outcomes were searched through various databases. A structured data extraction was employed and risk of bias was assessed with Newcastle-Ottawa Scale. Corticosteroid effects were reported as pooled odds ratio and forest plots were generated for each outcome.
RESULTSThe effect on the components of hemodynamic support showed no significant difference between the two treatment groups: left ventricular dysfunction (P -value= 0.86), inotrope use (P -value = 0.65), mechanical ventilator use (P -value = 0.21), and admission at the PICU (P -value = 0.87). However, initial treatment with IVIG plus corticosteroids, was associated with a more favorable fever course (P -value = < 0.02), less use of adjunctive immunomodulatory therapy (P -value = < 0.00001), and less incidence of coronary artery aneurysm (P -value = < 0.04).
CONCLUSIONInitial treatment with IVIG plus glucocorticoids was associated with a more favorable fever course, less use of adjunctive immunomodulatory therapy, and less incidence of coronary artery aneurysm than IVIG alone.

Positive-sense RNA viruses modify intracellular calcium stores, endoplasmic reticulum and Golgi apparatus (Golgi) to generate membranous replication organelles known as viral factories. Viral factories provide a conducive and substantial enclave for essential virus replication via concentrating necessary cellular factors and viral proteins in proximity. Here, we identified the vital role of a broad-spectrum antiviral, peruvoside in limiting the formation of viral factories. Mechanistically, we revealed the pleiotropic cellular effect of Src and PLC kinase signaling via cyclin-dependent kinase 1 signaling leads to Golgi-specific brefeldin A-resistance guanine nucleotide exchange factor 1 (GBF1) phosphorylation and Golgi vesiculation by peruvoside treatment. The ramification of GBF1 phosphorylation fosters GBF1 deprivation consequentially activating downstream antiviral signaling by dampening viral factories formation. Further investigation showed signaling of ERK1/2 pathway via cyclin-dependent kinase 1 activation leading to GBF1 phosphorylation at Threonine 1337 (T1337). We also showed 100% of protection in peruvoside-treated mouse model with a significant reduction in viral titre and without measurable cytotoxicity in serum. These findings highlight the importance of dissecting the broad-spectrum antiviral therapeutics mechanism and pave the way for consideration of peruvoside, host-directed antivirals for positive-sense RNA virus-mediated disease, in the interim where no vaccine is available.
10.Transtubular Transoral Approach for Irreducible Ventral Craniovertebral Junction Compressive Pathologies: Surgical Technique and Outcome

Introduction: To investigate the use of a tubular retractor to
provide access to the craniovertebral junction (CVJ) sparing
the soft palate with the aim of reducing complications
associated with traditional transoral approach but yet
allowing adequate decompression of the CVJ.
Materials and methods: Twelve consecutive patients with
severe myelopathy (JOA-score less than 11) from ventral
CVJ compression were operated between 2014-2020 using a
tubular retractor assisted transoral decompression.
Results: All patients improved neurologically statistically
(p=0.02). There were no posterior pharynx wound infections
or rhinolalia. There was one case with incomplete removal of
the lateral wall of odontoid and one incidental durotomy.
Conclusions: A Tubular retractor provides adequate access
for decompression of the ventral compression of CVJ. As the
tubular retractor pushed away the uvula, soft palate and
pillars of the tonsils as it docked on the posterior pharyngeal
wall, the traditional complications associated with traditional
transoral procedures is completely avoided.
