1.Age group-related characteristics of pediatric drowning patients treated at an emergency medical center in northern Yeongseo, Gangwon Province

Purpose:
We aimed to analyze pediatric drowning cases to study age group-related clinical characteristics, such as causes, location, and outcomes, in an under-researched area in South Korea.
Methods:
From January 1, 2020 through July 20, 2025, we retrospectively analyzed medical records of patients aged 19 years or younger who had experienced drowning and visited Kangwon National University Hospital located in the northern Yeongseo region of Gangwon Province. Their clinical characteristics were compared between those with worse outcomes, defined as hospitalization to the intensive care unit or in-hospital mortality, and those with better outcomes.
Results:
During the period, 27 patients having undergone drowning accidents visited the hospital. The most frequent location was the bathtub at home in the patients younger than 1 year (all 4 patients), commercial swimming pools in those aged 1-14 years (12 of the 16 patients), and rivers in those aged 15-19 years (all 7 patients). Of the 27 cases, 11 had worse outcomes including 4 mortalities. Patients with worse outcomes had higher percentages of guardian’s negligence, altered mental status, pool or river as the location, implementation of oxygen therapy or positive pressure ventilation, and lower mean initial Korean Triage and Acuity Scale (i.e., higher acuity), as well as higher mean concentrations of glucose, urea nitrogen, aspartate aminotransferase, and base deficit.
Conclusion
This study on pediatric drowning patients in the under-researched area showed age-related differences in the primary causes of drowning and clinical features related to the worse outcomes. These findings need to be considered for prompting increased parental vigilance and more comprehensive societal preventive measures.
2.Occupation classification model based on DistilKoBERT: using the 5th and 6th Korean Working Condition Surveys

Accurate occupation classification is essential in various fields, including policy development and epidemiological studies. This study aims to develop an occupation classification model based on DistilKoBERT. This study used data from the 5th and 6th Korean Working Conditions Surveys conducted in 2017 and 2020, respectively. A total of 99,665 survey participants, who were nationally representative of Korean workers, were included. We used natural language responses regarding their job responsibilities and occupational codes based on the Korean Standard Classification of Occupations (7th version, 3-digit codes). The dataset was randomly split into training and test datasets in a ratio of 7:3. The occupation classification model based on DistilKoBERT was fine-tuned using the training dataset, and the model was evaluated using the test dataset. The accuracy, precision, recall, and F1 score were calculated as evaluation metrics. The final model, which classified 28,996 survey participants in the test dataset into 142 occupational codes, exhibited an accuracy of 84.44%. For the evaluation metrics, the precision, recall, and F1 score of the model, calculated by weighting based on the sample size, were 0.83, 0.84, and 0.83, respectively. The model demonstrated high precision in the classification of service and sales workers yet exhibited low precision in the classification of managers. In addition, it displayed high precision in classifying occupations prominently represented in the training dataset. This study developed an occupation classification system based on DistilKoBERT, which demonstrated reasonable performance. Despite further efforts to enhance the classification accuracy, this automated occupation classification model holds promise for advancing epidemiological studies in the fields of occupational safety and health.

4.Physical Activity-Induced Modification of the Association of Long-Term Air Pollution Exposure with the Risk of Depression in Older Adults

Purpose:
Evidence suggests that long-term air pollution exposures may induce depression; however, the influence of physical activity on this effect is unclear. We investigated modification of the associations between air pollution exposures and depression by the intensity of physical activity.
Materials and Methods:
This cross-sectional study included 1454 Korean adults. Depression was defined as a Geriatric Depression Scale score ≥8. Concentrations of particulate matter (PM10 and PM2.5: diameter ≤10 μm and ≤2.5 μm, respectively) and nitrogen dioxide (NO2) level at each participant’s residential address were estimated. Based on metabolic equivalents, physical activity intensity was categorized as inactive, minimally active, or health-enhancing physical activity (HEPA).
Results:
Each 1-part per billion (ppb) NO2 concentration increase was significantly associated with a 6% [95% confidence interval (CI), 4%–8%] increase in depression risk. In older adults (≥65 years), a 1-ppb NO2 increase was associated (95% CI) with a 4% (1%–7%), 9% (5%–13%), and 21% (9%–33%) increase in depression risk in the inactive, minimally active, and HEPA groups, respectively. Compared with the inactive group, the minimally active (p=0.039) and HEPA groups (p=0.004) had higher NO2 exposure-associated depression risk. Associations of PM10 and PM2.5 with depression did not significantly differ by the intensity of physical activity.
Conclusion
We suggest that older adults who vigorously exercise outdoors may be susceptible to air pollution-related depression.
5.Gallstone Dissolution Effects of Combination Therapy with n-3 Polyunsaturated Fatty Acids and Ursodeoxycholic Acid:A Randomized, Prospective, Preliminary Clinical Trial

Ursodeoxycholic acid (UDCA) is the only well-established and widely used agent for dissolving gallstones. Epidemiological and animal studies have suggested potential therapeutic benefits of n-3 polyunsaturated fatty acids (PUFA) for dissolving cholesterol gallstones. We evaluated whether adding PUFA to UDCA improves gallstone dissolution in patients with cholesterol gallstones.
Methods:
This randomized, prospective, preliminary clinical trial compared the efficacy and safety of UDCA plus PUFA combination therapy (combination group) with those of UDCA monotherapy (monotherapy group). The inclusion criteria were a gallstone diameter ≤15 mm on ultra-sonography, radiolucent stones on plain X-ray, and no to mild symptoms. Gallstone dissolution rates, response rates, and adverse events were evaluated.
Results:
Of the 59 screened patients, 45 patients completed treatment (24 and 21 in the monotherapy and combination groups, respectively). The gallstone dissolution rate tended to be higher in the combination group than in the monotherapy group (45.7% vs 9.9%, p=0.070). The radiological response rate was also significantly higher in the combination group (90.5% vs 41.7%, p=0.007). In both groups, dissolution and response rates were higher in patients with gallbladder sludge than in those with distinct stones. Four adverse events (two in each group) were observed, none of which were study drug-related or led to drug discontinuation. The incidence of these adverse events was similar in both groups (combination vs monotherapy: 9.5% vs 8.3%, p=0.890).
Conclusions
UDCA plus PUFA therapy dissolves cholesterol gallstones more effectively than UDCA monotherapy, without significant complications. Further prospective, large-scale studies of this combination therapy are warranted.

10.Persistent Gingival Swelling and Fistula Obscured Horizontal Root Fracture:A Case Report

Horizontal root fracture (HRF) is a result of trauma to teeth and periodontium, which implies severe injury to cementum, dentin, and pulp. This is a rare case of HRF in the maxillary lateral incisor of a 62-year-old male who only presented persistent gingival swelling, fistula, and dull pain at first. An apical radiolucency of unknown origin turned out to be a result of hidden HRF at the coronal third level that was later visualized radiographically during endodontic treatment. The tooth was scheduled to be extracted upon the patient’s agreement. The purpose of this report is to alert clinicians about the importance of diagnosing HRF through thorough clinical and radiographic examinations. Where there is persistent fistula without proper cause, HRF should be considered as a causative factor, and the diagnosis could be effective with aid of cone beam computed tomography, electronic root apex locator, as well as other clinical signs.
