1.Status and influencing factors of early out of bed activity in patients after coronary artery bypass grafting

Objective:To analyze the status and influencing factors of early out of bed activity in patients after coronary artery bypass grafting.Methods:Using the convenient sampling method, patients who underwent coronary artery bypass grafting in Affiliated Hospital of Jining Medical University from January 2018 to December 2020 were selected as the research objects. The general situation questionnaire, the Chinese version of Brief Disease Cognition Questionnaire, Muscle Strength Rating Scale and Visual Analog Scale were used to investigate them. Binomial Logistic regression was used to analyze the influencing factors of early out of bed activity after coronary artery bypass grafting.Results:A total of 100 questionnaires were distributed in this study and 100 were recovered. The effective recovery rate of the questionnaire was 100%. 38% (38/100) of patients with coronary artery bypass grafting performed early out of bed activity within 72 hours after surgery. Binomial Logistic regression analysis showed that vascular bridge selection, postoperative pain, lower limb incision blister, lower limb muscle strength rating and disease awareness were the influencing factors of early out of bed activity after coronary artery bypass grafting ( P<0.05) . Conclusions:Vascular bridge selection, postoperative pain, lower limb incision blister, lower limb muscle strength rating and disease awareness were the influencing factors of early out of bed activity after coronary artery bypass grafting
2.Advances in the study of vasoactive-inotropic score in critically ill patients

The degree of hemodynamic support by vasoactive drugs in critically ill patients is often considered one of the markers of disease severity. The sequential organ failure assessment (SOFA), European system for cardiac operative risk evaluation Ⅱ (EuroScoreⅡ), and other scores only roughly quantify the drug support of cardiovascular system. When patients need large doses of vasoactive drugs, the mortality increases accordingly. The vasoactive-inotropic score (VIS) objectively quantifies the degree of cardiovascular support using a simple formula that standardizes the dose of different agents, and it is recommended as a simple, effective, and accurate prognostic indicator. In recent years, there are more and more clinical applications and related studies at home and abroad. This paper reviews the application and progress of VIS score in critically ill patients, providing help for doctors to judge the condition and prognosis of patients and guiding the decision-making of diagnosis and treatment.
3.Role of cerebrospinal fluid chimerism in predicating central nervous relapse surveillance for patients of acute leukemia after allogeneic hematopoietic stem cell transplantation

Objective To explore the role of cerebrospinal fluid chimerism in central nervous relapse surveillance for patients of acute leukemia after allogeneic hematopoietic stem cell transplantation (allo-HSCT).Methods The follow-up data were retrospectively collected and analyzed in 104 patients with acute leukemia after allo-HSCT.Comparisons were made between patients with complete chimerism and mixed chimerism in cerebrospinal fluid.The role of recipient DNA percentage and its changing trend in predicting central nervous relapse were also explored.Analysis was conducted for determining the risk factors of central nervous relapse.And the effectiveness of prophylaxis with intrathecal injection was also examined.Results The incidence of relapse was higher in patients with mixed chimerism (P<0.001),high percentage of recipient DNA (P<0.05) and higher mixed chimerism (P<0.001).Hyperleukocytosis at an initial diagnosis was a risk factor of central nervous relapse.Whether or not intrathecal injection prophylaxis was applied showed no significant difference in relapsing rate.Conclusions Monitoring cerebrospinal fluid chimerism can effectively help predict central nervous relapse among patients of acute leukemia after allo-HSCT.Yet intrathecal injection prophylaxis failed to benefit recipients.
4.Clinical Biomarkers of Neurodegeneration in REM Sleep Behavior Disorder.

Rapid eye movement sleep behavior disorder (RBD) is currently considered as a prodromal stage of alphasynucleinopathies neurodegeneration. The update data suggested that over 80% patients with idiopathic RBD eventually developed neurodegenerative disease after a mean of 14 years interval from the onset of RBD. A series of potential biomarkers have been identified to predict the development of neurodegeneration in idiopathic RBD, including olfactory loss, color vision deficit, depression, mild cognitive impairment, excessive daytime sleepiness, dopamine dysfunction, and tonic electromyographic activity. Early recognition of the predictive markers of neurodegeneration in idiopathic RBD is essential for development of intervention or prevention strategies at the presymptomatic stage. Nonetheless, the current literature is lacking biomarkers that might reflect the alpha-synuclein neuropathology at the earliest stages. Future studies with large samples and systematic follow-up are needed to confirm more potential markers of neurodegeneration at its early stages.

5.Effect of miR-342-3p on chemotherapy sensitivity in triple-negative breast cancer.

OBJECTIVE:
To study the effect of miR-342-3p on the chemotherapy sensitivity in triple-negative breast cancer (TNBC).
METHODS:
Tissue samples were collected from January 2011 to August 2013 samples in Jiangsu Cancer Hospital from a total of 32 triple-negative breast cancer patients with preoperative chemotherapy, with 5 cases of complete response (CR) and 27 cases of partial response (PR). We detected miR-342-3p expression of TNBC with RT-PCR. We transfected has-miR-342-3p mimic and inhibitor into breast cancer cell lines MDA-MB-231 by lipofection transfection and set up negative control mim-NC and inhi-NC. Group of mimic, mim-NC, inhibitor and inhi-NC were cultivated with 2 μmol /L paclitaxel, cisplatin or 4 μmol/L doxorubicin for 48 h. The cell growth rates were measured by CCK8 reagent kit, and the cell apoptosis rate by flow cytometry.
RESULTS:
The expressions of miRNA-342-3p in TNBC tissue of CR were higher than those of PR. The cell growth rates of mimic were lower and cell apoptosis rates were higher than those of min- NC after cultivating with paclitaxel or cisplatin for 48 h, with significant difference (P<0.05). The cell growth rates of inhibitor were higher and cell apoptosis rates lower than those of inhi-NC after cultivating with paclitaxel or cisplatin 48 h, with significant difference (P<0.05). The cell growth and cell apoptosis rates of mimic and inhibitor had no difference with those of mim-NC and inhi- NC after cultivating with doxorubicin 48 h (P>0.05).
CONCLUSION
TNBC with high expression of miR-342-3p are more sensitive to chemotherapy. miRNA-342-3p may regulate the sensitivity of breast cancer cell line MDA-MB-231 to chemotherapy drugs paclitaxel and cisplatin, but can not affect the chemotherapy sensitivity of doxorubicin.

6.Correlation between chronic hepatitis B and serum triglyceride and cholesterol levels

Objective To investigate the relationship between various stages of chronic hepatitis B virus (HBV) infection and lipid metabolism and its influencing factors.Methods Seventy-two cases of chronic hepatitis B (CHB),40 cases of liver cirrhosis and 17 cases of hepatocellular carcinoma (HCC) were enrolled.One-way ANOVA analyses were used to compare age,gender,liver function,lipid metabolism,and HBV DNA levels of each group.Pearson correlation analysis was used to study the correlation between HBV DNA and lipid metabolism.Binary Logistic regression analyses were performed to explore the risk factors of cirrhosis and HCC in patients with CHB.Results Differences of age,alanine aminotransferase (ALT),albumin (Alb),triglyceride (TG),and cholesterol(CHO) among the three groups (CHB group,cirrhosis group and HCC group) were statistically significant (all P<0.05).TG levels in cirrhosis and HCC groups were (-0.061± 0.234)lg mmol/L and (-0.061±0.253) lg mmol/L,respectively,which were both significantly lower than that of the CHB group (0.116±0.182) lg mmol/L (F=11.466,P=0.000).CHO level in cirrhosis group was (0.460±0.333) lg mmol/L,which was lower than that in CHB group (0.586±0.101) lg mmol/L (F=4.892,P=0.009).The HBV DNA levels inversely correlated with TG and CHO levels in CHB group (r=-0.266,P=0.024; r=-0.309,P=0.008,respectively).The HBV DNA levels of cirrhosis and HCC patients positively correlated with ALT levels (r=0.355,P =0.007).Old age (OR=1.096,95%CI:1.025-1.172),low Alb (OR=0.000,95%CI:0.000-0.000),and low levels of ALT (OR=0.128,95%CI:0.026-0.641) were risk factors for development of cirrhosis and HCC in CHB patients (all P<0.05).Conclusions With the progression of liver injuries,TG and CHO levels are reduced.Further studies of correlation between risk factors for the development of cirrhosis and HCC and lipid metabolism in CHB patients are needed.
7.The anti-tumor efficacy of nanosecond pulsed electric fields on the mouse with melanoma xenograft in vivo.

This study was conducted to investigate the anti-tumor efficacy of nanosecond pulsed electric fields (nsPEFs) on the mouse with A375-GFP melanoma xenograft in vivo. In vivo fluorescence image analysis system was used in this study to evaluate the effects of nsPEFs on human melanoma A375 cell xenograft. On the Day 90 af ter pulse delivery, the skin that had contained A375 cell xenograft was surgically excised and pathologically evalua ted. The changes of scar were recorded by digital camera. The experiment revealed that significant changes in fluorescence value trend and amplitude were found in the treated group from those in the control group. The fluorescence of tumor in the treated group decreased mostly 48 h after the treatment and completely disappeared 10 d after the treatment, while that in control group was increased gradually. Surgical excision of the area confirmed a complete pathologic response. Within a few days after the nsPEFs treatment, a hard scab formed at the treatment region. The scab fell off by the end of the second week. As time went on, the scar gradually became faded and all xenograft tumors were disappeared without recurrence. From the experiment, we learn that nsPEFs can bring good therapeutic effect. It may provide a new approach for the clinical treatment of superficial tumors.

8.Vascular endothelial growth factor gene polymorphisms and the risk of endometriosis: a systematic review

Objective To analyze the potential association between the single ucleotide polymorphisms(SNP) of the vascular endothelial growth factor(VEGF) gene with the risk of endometriosis by meta-analysis.Methods Published case-control studies about the influence of VEGF polymorphisms on endometriosis were searched and screened in Medline,the Cochrane library,China National Knowledge Internet(CNKI),Chinese Biological Medicine Disk ( CBM ),data base of Wanfang and Foreign Medical Journal Full-Text Serv ice(FMJS).RevMan 5.0 software was used for meta-analysis.Results Finally,there were 9 literatures including 1610 endometrisis patients and 1643 controls cases,which were eligible for the criteria to investigating the VEGF SNP about - 460C/T,+ 405C/G and + 936C/T.The results of metaanalysis showed that there was no evidence for association between endometriosis and the VEGF -460C/T SNP in the genotype or allele frequencies distribution ( P > 0.05 ).Significant differences were found between the frequencies distribution of VEGF + 405CC genotype( P =0.009,OR =1.45,95% CI:1.10 - 1.91 ) and + 405 C allele ( P =0.020,OR =1.19,95 % CI:1.03 - 1.38 ),also between the + 936CC genotype ( P =0.050,OR=0.81,95%CI:0.66-1.00) and +936T allele (P=0.040,OR=1.20,95%CI:1.01 -1.43).Conclusions The VEGF + 405C/G and + 936C/T SNP may be associated with the risk of endometriosis.Women carrying the + 405C or the + 936T allele could significantly increase the risk of developing endometriosis.
9.Vascular ultrasound combined with Seldinger technology improves the success rate of peripherally inserted central catheter and reduces potential complications

ObjectiveTo investigate the applicability and superiority of modified Seldinger technique (MST).MethodsTotally 108 patients receiving peripherally inserted central catheter (PICC) through vascular ultrasound combined with MST technology were enrolled as MST group and another 108 patients receiving PICC using traditional direct vision method as control group.The first puncture success rate,PICC succesa rate,catheterization time required,and early complication rate were compared between these two groups.ResultsThe first puncture success rate was 88.89% in MST group and 72.22% in control group (P=0.002) ; the catheterization success rate was 82.41% in MST group and 57.41% in control group ( P =0.000).In the MST group,there were two cases where the catheter within neck was ectopic ( P =0.003 ),two cases experienced bleeding at the puncture sites (P =0.017),three cases had phlebitis (P =0.001 ),and one case suffered from catheter blockage (P=0.010).In the eontrol group,the corresponding numbers were 13,10,18,and 9.However,the time required for PICC showed no significant difference between these two groups (P=0.685).In the MST group,15 cases were catheterized in less than 15 minutes,72 cases between 15 and 25 minutes,and 21 cases for more than 25 minutes.In the control group,12 cases were catheterized in less than 15 minutes,70 cases between 15 and 25 minutes,and 26 cases for more than 25 minutes.ConclusionPICC through MST can be more effective in positioning the targeting blood vessels,increasing the success rate of catheter insertion,reducing various tissue damages,and lowering potential complications.
10.Study on objective sleep disturbances in patients with Parkinson' s disease

Objective To investigate the characteristics of the objective sleep disturbances in Parkinson' s disease (PD) and the factors related to it.Methods One hundred and one PD patients and 90 age- and sex- matched controls underwent a video-polysomnography.The sleep parameters and its related factors in two groups were analyzed.Results Sleep latency was not statistically different in comparing two groups.PD patients had a higher percentage of non-rapid eye movement( non-REM ) sleep stage 1 and a lower percentage of non-REM sleep stage 2 compared with controls ( 27.9 ± 1 7.8 vs 21.2 ± 11.7,t =3.034,P =0.003 ;47.8 ± 17.4 vs 54.7 + 12.9,t =- 3.043,P =0.003 ).Reduced sleep efficiency,decreased the proportion of slow wave sleep and REM sleep,increased awake time and longer REM sleep latency occurred in PD patients.There were no significant differences of these above parameters.Some sleep parameters in PD patients were correlated with advancing age,the severity of PD,and the degree of depression.The index of periodic leg movements in sleep (PLMSI) of 41 PD patients (40.6% ) was more than 15.These PD patients didn' t complain corresponding symptoms about their legs.The PLMSI in PD patients were significantly higher than the controls.PLMSI increased with aging in the PD group( r =0.261,P <0.01 ).PD patients didn' t suffer significantly lower apnea- hypopnea index and oxygen desaturation index.The lowest SPO2 ( L-SPO2 ) increased in the PD group.REM sleep without atonia occurred in 83 patients (82.2%) with PD.Thirty-eight patients (37.6%) were diagnosed with REM sleep behavior disorder (RBD).The incidences of REM without atonia and RBD in the PD group were significantly higher than in the control s(0 and 8 patients (8.9%),x2 =42.271,102.480; both P < 0.01 ).Conclusions The sleep parameters in PD patients are changed.For PD patients,there is no difficulty in falling asleep.The PD patients also have sleep structure disorder and difficulty in maintaining sleep.The sleep parameters are correlated with advancing age,the severity of PD,and the degree of depression in PD.PLMS don' t lead to sleep disturbances in PD patients.The blood oxygen saturation don' t decrease severely when PD patients suffer apnea or hypopnea.RBD occur more frequently in PD patients.
