1.Diagnotic value of endoscopic retrograde cholangiopancreatography combined with modified biopsy forceps for suspected malignant biliary stricture (with video)

To explore the diagnostic value of endoscopic retrograde cholangiopancreatography (ERCP) combined with modified biopsy forceps for suspected malignant biliary obstruction, 72 patients with suspected malignant biliary obstruction who underwent ERCP using modified biopsy forceps from January 2017 to April 2023 in the First Affiliated Hospital of Henan University of Chinese Medicine were selected as the modified group, while 61 contemporaneous patients who underwent ERCP with traditional biopsy forceps were selected as the control group at the same time. The stenosis site was identified during the operation, and the effective pathological tissue was obtained by biopsy. Benign or malignant tumor was determined according to the medical history and clinical follow-up data. The sensitivity and specificity of the two groups were compared. The success rate of biopsy was 100.00% in the two groups. All patients had no serious complications and were diagnosed histologically. Sixty-seven cases were finally diagnosed as malignant and 5 cases were benign in the modified group. In the control group, 58 cases were malignant and 3 cases were benign. The sensitivity for diagnosis of malignant biliary stricture was 79.10% (54/67) in the modified group, and 60.34% (35/58) in the control group, with significant difference ( χ2=6.218, P= 0.013). The specificity of the two methods for the diagnosis of malignant stenosis was 100.00%. Therefore, it is safe and effective to apply ERCP combined with the modified biopsy forceps in the diagnosis of extrahepatic malignant bile duct stenosis.
2.Serum testosterone and estradiol levels correlate with disease severity and prognosis in male patients with liver failure

Objective:To investigate and explore the serum levels of testosterone and estradiol in correlation with disease severity and prognosis in male patients with liver failure.Methods:Sixty male cases with liver failure who received treatment from April 2022 to December 2023 were selected as the research subjects. Forty healthy subjects who underwent physical examination in the physical examination center during the same period were enrolled as the control group. The levels of sex hormones (serum testosterone and estradiol) were compared between the two groups. Logistic regression was used to analyze the diagnostic value of testosterone and estradiol for the grading of male patients with liver failure. The prognostic factors for predicting disease severity were analyzed using COX regression. The area under the ROC curve (AUC) was used to evaluate the predictive value.Results:The testosterone level was significantly higher in the healthy group than that in the liver failure group [(5.11±3.00) nmol/L vs. (2.22±2.78) nmol/L, t=4.934, P<0.001], while the estradiol level was significantly lower in the liver failure group [37.46±13.21) nmol/L vs. (113.45±67.70) nmol/L, t=-8.457, P<0.001]. Multiple discriminant logistic regression analysis results showed that estradiol and testosterone were independent predictors of the model for end-stage liver disease. Multivariate Cox regression analysis showed that testosterone was an independent prognostic factor for the 1-year mortality rate in male patients with liver failure. The area under the curve predicting the 1-year mortality rate was 0.745 after adjusting for other factors. Conclusion:Testosterone and estradiol levels are significantly altered in male patients with liver failure. Testosterone and estradiol levels in peripheral blood can effectively reflect the degree of liver function impairment and the 1-year mortality rate in male patients with liver failure, which is helpful for accurately assessing the severity of the disease and its prognosis.
3.Comparative efficacy of whole blood transfusion versus component therapy in the resuscitation of patients with traumatic shock: a Meta-analysis

Objective:To compare the efficacy of whole blood (WB) versus component therapy (COMP) in the resuscitation of patients with traumatic shock.Methods:A systematic literature search was conducted using PubMed and Embase databases for English literature published from January 2006 to December 2023. Studies on the comparative efficacy of WB versus COMP in resuscitation of patients with traumatic shock were all included. Patients were categorized according to the initial transfusion strategies into WB group and COMP group. Primary outcome indicators were extracted and subjected to meta-analysis, including transfusion requirements after admission (4-hour red blood cell requirement, 4-hour plasma requirement, 4-hour total transfusion volume, 24-hour red blood cell requirement, 24-hour plasma requirement, and 24-hour total transfusion volume), length of hospital stay, length of ICU stay, and mortality rates (early mortality, 24-hour mortality, late mortality, and in-hospital mortality, and 24-hour mortality with multiple variables adjusted).Results:Twenty-seven studies involving 15 176 patients (4 692 patients in WB group and 10 484 in COMP group) were included. Meta-analysis results indicated that the 4-hour red blood cell requirement (SMD=-0.44, 95% CI -0.69, -0.19, P<0.01), 4-hour plasma requirement (SMD=-0.25, 95% CI -0.50, -0.01, P<0.05), 24-hour red blood cell requirement (SMD=-0.22, 95% CI -0.36, -0.09, P<0.01), and 24-hour mortality with multiple variables adjusted ( OR=0.78, 95% CI 0.68, 0.91, P<0.05) were significantly reduced in WB group. No significant differences were observed between the two groups regarding 4-hour total transfusion volume, 24-hour plasma requirement, 24-hour total transfusion volume, length of hospital stay, length of ICU stay, early mortality, 24-hour mortality, late mortality, and total in-hospital mortality ( P>0.05). Conclusions:WB demonstrates certain advantages over COMP in the resuscitation of patients with traumatic shock by reducing 4-hour red blood cell requirement, 4-hour plasma requirement, 24-hour red blood cell requirement, and 24-hour mortality with multiple variables adjusted. However, WB and COMP demonstrate comparable effectiveness in reducing 4-hour total transfusion volume, 24-hour plasma requirement, 24-hour total transfusion volume, early mortality rate, 24-hour mortality rate, late mortality rate, and in-hospital mortality.
4.Development and validation of clinical prediction model for post-treatment recurrence in high-risk non-muscle invasive bladder cancer after BCG intravesical instillation

Objective To investigate the factors influencing the efficacy of intravesical Bacille Calmette-Guérin(BCG)instillation after transurethral resection of bladder tumor(TURBT)in patients with intermediate-and high-risk non-muscle invasive bladder cancer(NMIBC),and to construct a prediction model for recurrence after BCG treatment.Methods A retrospective cohort study was conducted on the subjected patients diagnosed with intermediate-and high-risk NMIBC undergoing TURBT followed by standard BCG instillation.The 110 patients treated in Department of Urology of Army Medical Center of PLA from January 2018 to December 2023 were assigned into a training set,while the 52 patients treated at Department of Urology of General Hospital of Central Theater Command from January 2015 to December 2020 were into an external validation set.A total of 17 variables were included and analyzed.Univariate and multivariate Cox regression analyses were performed to identify factors associated with recurrence after BCG instillation,and nomograms were plotted to predict 1-year,3-year,and 5-year recurrence-free survival(RFS).Calibration curve,decision curve analysis(DCA),and receiver operating characteristic(ROC)curve analysis were conducted for internal and external validation to evaluate the predictive performance and clinical utility of the model.Results In the training set,26 patients(23.64%)experienced recurrence during the follow-up period,with a median RFS of 32.00(18.00~50.50)months.Univariate Cox regression analysis suggested that platelet count,eosinophil to lymphocyte ratio(ELR),neutrophil to lymphocyte ratio(NLR),platelet to lymphocyte ratio(PLR),systemic immune inflammation(SII)index,and neutrophil-monocyte to lymphocyte ratio(NMLR),pathological T1 stage(pT1)tumor and hemoglobin,albumin,lymphocyte,and platelet(HALP)score were potential factors influencing recurrence after BCG instillation.Multivariate Cox regression analysis identified high HALP score(HR=0.185,95%CI:0.046~0.736,P=0.017)as an independent protective factor,while high ELR(HR=3.599,95%CI:1.505~8.608,P=0.004)and pT1 stage(HR=3.240,95%CI:1.191~8.818,P=0.021)were independent risk factors for recurrence.Based on this,a nomogram prediction model was constructed.The calibration curves demonstrated good agreement between predicted and actual 1-,3-,and 5-year recurrence risks.Decision curve analysis indicated clinical utility across a wide threshold probability range.In the training set,the model showed strong predictive performance for 1-(AUC=0.842),3-(AUC=0.847),and 5-year(AUC=0.887)recurrence risks,which was further validated in the external cohort.Conclusion Higher HALP score prior to BCG instillation therapy is a protective factor against tumor recurrence,while higher ELR and pT1 stage are risk factors.Our nomogram prediction model based on HALP score,ELR and pathological T stage,can identify individuals at high risk of recurrence after BCG instillation therapy.

Objective To investigate the effect of drug-eluting bead DACE(DEB-TACE)combined with systemic treatment for hepatocellular carcinoma(HCC)in different locations.Methods A total of 204 HCC patients who underwent DEB-TACE combined with systemic therapy(targeted and immunotherapy)were retrospectively collected.According to the anatomical location of HCC,86 cases with lesions located at the main trunk of portal vein(PV)or within 1 cm of the first PV branch were classified into central type group,while 118 cases with lesions located at the other areas were classified as peripheral type group.Follow-up was regularly performed after DEB-TACE until August,2024.The objective response rate(ORR)and disease control rate(DCR)at 1,3,6 and 12 months after DEB-TACE,also patients'progression-free survival(PFS)and overall survival(OS)were compared between groups.Results All patients were followed up for a median of 32.6 months,during which 164 cases died.Significant differences of ORR at 1 and 3 months after DEB-TACE(77.91%[67/86]vs.89.83%[106/118],34.88%[30/86]vs.54.24%[64/118])and DCR at 3 and 6 months after DEB-TACE(51.16%[44/86]vs.66.95%[79/118],34.88%[30/86]vs.50.00%[59/118])were found between groups(all P<0.05).Patients'PFS(30.18[9.12,48.54]months)and OS(37.36[17.79,56.68])in peripheral type group were better than those in central type group(20.11[11.35,28.87]months and 23.24[3.11,43.47]months,x2=3.971,4.162,P=0.048,0.041).Conclusion The effect of DEB-TACE combined with systemic treatment for peripheral type HCC was better than for central type HCC.
6.Comparison of potential profiles analysis of psychosocial adaptation and social alienation differences in nsoriasis natients

Objective To explore the potential categories of psychosocial adaptation in psoriasis patients and their differences in social alienation.Methods Using a cross-sectional survey design,convenience sam-pling was used to select 376 psoriasis patients from multiple hospitals in Shandong Province from September to December 2022.Participants completed the general information questionnaire,Psychosocial Adaptation to Illness Scale(PAIS-SR),Acceptance and Action Questionnaire-Ⅱ(AAQ-2),and General Alienation Scale(GAS).Latent profile analysis was performed using Mplus8.0 software to identify psychosocial adaptation patterns of psoriasis patients,and SPSS25.0 was used to compare social alienation differences among different adaptation groups.Results Psoriasis patients could be divided into two latent profiles:moderate psychosocial adaptation group(31.38%)and low psychosocial adaptation group(68.62%).Medical payment method,dis-ease recurrence,psoriasis subtype,disease duration,family history,skin lesion exposure,and AAQ-2 scores were identified as main influencing factors(P<0.05).Significant differences in total GAS scores were found between the two groups(P<0.05).Conclusion The psychosocial adaptation of psoriasis patients shows het-erogeneity and could be classified into two latent profiles.Targeted interventions should be implemented to improve psychosocial adaptation levels.
