1.Comparison of Embolization Coils and Patent Ductus Arteriosus Occluders for Coronary Artery Fistula Transcatheter Closure: A Single Centre Experience

Background and Objectives:
There is no dedicated occlusive device for closing coronary artery fistulas (CAFs), and specific efficacy and safety data of various off-label occlusive devices for CAFs closure are scarce.
Methods:
Patients undergoing transcatheter closure of CAFs from January 2011 to December 2022 were included in the single-center retrospective study. The study population was divided into 2 groups: coils group (n=35) and patent ductus arteriosus (PDA) occluders group (n=66).
Results:
No significant intergroup differences were observed in demographic characteristics except age. The presence of multiple CAF origins (54.3% vs. 4.5%, p<0.001) and multiple draining sites (51.4% vs. 3.0%, p<0.001) were more common in the coils group. In contrast, the presence of aneurysm (72.7% vs. 14.3%, p<0.001), and large fistula (75.8% vs. 37.1%, p<0.001) were more prevalent in the PDA occluders group. The acute procedural success rate of the PDA occluders group was higher compared to that of the coils group (87.9% vs.62.9%, adjusted odds ratio [OR], 7.20; 95% confidence interval, 1.59–32.64; p=0.01).In addition, no significant intergroup differences were noted in both the recanalization rate (7.8% vs. 20%, p=0.107) and the reintervention rate (3.1% vs. 8.6%, p=0.342).
Conclusions
Transcatheter closure of CAFs using PDA occluders was associated with significantly higher acute procedural success rates compared to coil embolization with comparable late outcomes.

5.Correlation between the level of serum IL-6 and the severity of infection in patients with diabetic foot

To explore the correlation between the level of serum interleukin-6 (IL-6) and the severity of infection in patients with diabetic foot (DF) and the auxiliary value of IL-6 in DF diagnosis, and aim to provide reference for clinical treatment. Based on the hospital medical record system and the laboratory system, a retrospective analysis with case-control study was conducted on the data of patients in Liyuan Hospital affiliated to Tongji Medical College, Huazhong University of Science and Technology from January 2020 to September 2024. A total of 377 patients′ information was collected, including 31 cases in control group with 17 males and 14 females from 31 to 91 years old and a median age of 71, 63 cases in NDF group with 43 males and 20 females from 37 to 96 years old, with a median age of 71, and 283 patients in DF group with 197 males and 86 females from 36 to 96 years, with a median age of 67. According to classification of infection severity, paints in the group of diabetes with DF were divided into mild infection subgroup (72 cases), moderate infection subgroup (143 cases) and severe infection subgroup (68 cases). The results showed that there were no statistically significant differences in age and gender among the three groups in the study ( F=1.795/ χ 2=2.81, P>0.05). The non parametric test results showed that there were statistically significant differences in IL-6, C-reactive protein (CRP), procalcitonin (PCT), white blood cell (WBC) and glucose (GLU) among the three groups of patients ( H=12.480, 36.277, 12.432, 12.838, 18.334, P<0.05). The pairwise comparison results showed that compared with the control group, the NDF group had higher levels of CRP, PCT and GLU ( H=20.259, 20.118, 20.056, P<0.05), and the levels of IL-6, CRP, PCT and WBC( H=14.934,14.933,14.829,14.934, P<0.05) were higher in the DF group. Both of the differences were statistically significant. But the difference of IL-6, CRP, PCT, WBC and GLU between the NDF and DF group was not statistically significant( H=1.202,0.622,0.737,1.036,1.899, P>0.05). In DF group, there were statistically significant differences in IL-6, CRP, PCT, WBC, and GLU levels among patients in the three infection subgroups ( H=11.174, 15.136, 8.657, 8.348, 3.698, P<0.05).Compared to the mild subgroup, the levels of IL-6, CRP, PCT, WBC and GLU were higher in the severe subgroup were higher( H=111.789,237.066,74.683,83.203,15.328, P<0.05) and the levels of IL-6, CRP, PCT in the moderate subgroup were higher ( H=6.877, 8.846, 5.183, P<0.05). And both of the differences were statistically significant. But there was no statistically significant difference in WBC and GLU level between the mild and the moderate subgroup( H=1.684, 1.039, P>0.05). The severity of infection in diabetic foot patients was positively correlated with the IL-6 level ( OR=1.033, 95% CI: 0.024-0.043, P<0.05). ROC curve showed that AUC of IL-6 for diagnosis in diabetic foots were 0.635. And joint testing showed that the AUC of IL-6+CRP, IL-6+PCT and IL-6+WBC were 0.718, 0.621, and 0.638 respectively. In conclusion,the level of serum IL-6 may be positively correlated with the severity of infection in diabetic foot patients, which may have auxiliary diagnostic value in predicting diabetic foot infection. And it may provide a reference for the grading of infection severity in diabetic foot patients to gauging serum IL-6 level.
6.Analysis of the correlation between article content and user feedback on hospital wechat public numbers

Objective To analyze the relationship between article content and user feedback on WeChat public numbers in hospitals,and to provide data support and theoretical basis for optimizing public number content and health communication strategy.Methods 1 279 tweets of WeChat public number of a tertiary public hospital from 2023 to 2024 were selected to ana-lyze the relationship between article categories and user feedback behavior,interactive behavior and article dissemination effect,and to assess the important influencing factors of user attention conversion behavior.Results The articles in the health science popularization category show significant advantages in the indicators of users' reading and sharing behaviors;the interactive indica-tors of users' liking and commenting have a significant positive effect on the dissemination effect of the articles,while the number of"watching"hurts the number of readings;the assessment of the random forest model shows that the number of readings genera-ted by sharing and the number of readings brought by each sharing are the most important factors influencing the users' attention.The random forest model assessment showed that the number of reads generated by sharing and the number of reads per share were the most important factors influencing users' attention.Conclusion Health science articles dominate users' reading and sharing behaviors,user interaction significantly improves the dissemination effect and attention conversion of the articles,and the number of reads generated by sharing is the key factor determining users' attention.This suggests that content dissemination strategies need to be combined with the content characteristics of the article,focusing on improving the interaction and sharing guidance of con-tent with high dissemination potential.
7.mRNA display-enabled discovery of proximity-triggered covalent peptide-drug conjugates.

Peptide-drug conjugates (PDCs) have emerged as a promising modality in precision oncology, enabling targeted delivery of cytotoxic payloads while minimizing off-target toxicity. The integration of covalent warheads, such as those based on sulfur(VI) fluoride exchange (SuFEx) chemistry, enhances drug-target residence time and tumor accumulation. However, existing screening methods for covalent peptide (CP) libraries require post-translational warhead conjugation, limiting throughput. Here, we present an integrated mRNA display platform that incorporates covalent warheads during ribosomal synthesis, enabling efficient screening of ultra-diverse covalent macrocyclic peptide libraries (>1013 variants). This approach, using site-specific incorporation of N-chloroacetyl-d-phenylalanine and fluorosulfate-l-tyrosine, accelerated the discovery of irreversibly binding (K i = 3.58 μmol/L) Nectin-4-targeting peptide CP-N1-N3 via proximity-triggered SuFEx. The peptide was further conjugated to cytotoxic payloads, yielding the covalent PDC CP-N1-MMAE with potent cytotoxicity (IC50 ≈ 43 nmol/L) against MDA-MB-468 cells. This platform establishes a new paradigm for precision covalent drug discovery.

10.Clinical effects of combined tissue flap transplantation for repairing giant chest wall defects

Objective:To investigate the clinical effects of combined tissue flap transplantation in repairing giant chest wall defects.Methods:This study was a retrospective observational study. From August 2013 to December 2020, 31 patients with chest wall tumor or radiation ulcer after radical resection of chest wall tumor and conformed to the inclusion criteria were admitted to the Department of Breast Oncoplastic Surgery of Hunan Cancer Hospital, including 12 males and 19 females, aged 25-71 years. After resection of tumor or ulcer and wound debridement, the area of secondary chest wall defect was 300-600 cm 2 with length of 16-35 cm and width of 16-32 cm. According to the actual situation of the patients and the preoperative design, the chest wall defects were repaired with the flexible combination of perforator flaps and myocutaneous flaps from different donor sites, and the area of the combined tissue flap was 260-540 cm 2 with length of 20-30 cm and width of 13-20 cm. Free posteromedial thigh perforator flap+free anterolateral thigh myocutaneous flap were used in 2 patients, free deep inferior epigastric artery perforator flap+free anterolateral thigh myocutaneous flap were used in 5 patients, free deep inferior epigastric artery perforator flap+pedicled rectus abdominis myocutaneous flap+free anterolateral thigh myocutaneous flap were used in 7 patients, free deep inferior epigastric artery perforator flap+pedicled rectus abdominis myocutaneous flap+pedicled latissimus dorsi myocutaneous flap were used in 2 patients, and bilateral free anterolateral thigh myocutaneous flaps were used in 15 patients. For the remaining small area of superficial tissue defect after being repaired by combined tissue flaps, skin graft was used to repair or delayed local flap transfering was performed after the tissue flaps survived and edema subsided. The appropriate blood vessels in the donor and recipient sites were selected for anastomosis to reconstruct the blood supply of tissue flaps. The wounds in the donor sites of tissue flaps that can be directly sutured were sutured directly; for those that cannot be sutured directly, the skin grafting or delayed suture was performed. The anastomosis of blood vessels in the recipient sites, operation length, and postoperative hospital stay were recorded. The survivals of tissue flaps and skin grafts, the shape and texture of reconstructed chest wall, the wound healing, scar formation, and function of donor sites of tissue flaps, and the scar formation of the donor sites of skin grafts were observed after operation. Tumor recurrence and death of recurrent patients were followed up after operation. Results:The blood vessels in the recipient sites were anastomosed as follows: proximal internal thoracic vessels for 24 times, distal internal thoracic vessels for 12 times, trunk of thoracodorsal vessels for 4 times, anterior serratus branches of thoracodorsal vessels for 8 times, and thoracoacromial vessels for 12 times. The operation length was 6.0 to 8.5 hours, and the postoperative hospital stay was 9 to 21 days. Necrosis at the edge of partial tissue flaps occurred in 4 patients after operation, which healed after dressing change, and the tissue flaps and skin grafts of the other patients survived completely. The shape and texture of the reconstructed chest wall were good. Four patients had poor wound healing in the donor sites of abdominal tissue flaps, which healed after dressing change and local drainage. Only linear scar was left in the donor sites of all tissue flaps, and there was no obvious dysfunction in the donor sites of tissue flaps. Mild hypertrophic scar was left in the donor sites of skin grafts. During follow-up of 9 to 36 months after operation, 6 patients had tumor recurrence, and the recurrence time was 5 to 20 months after operation. After comprehensive treatment for patients with tumor recurrence, 3 patients died.Conclusions:Transplantation of combined tissue flaps in repairing the giant chest wall defects can shorten the time of total operation and hospital stay, and avoid multiple operations. After operation, patients had good chest wall appearance, with reduced tumor recurrence in patients with chest wall tumor.
