1.A brief discussion on TCM diagnosis and treatment of myelodysplastic syndrome based on the Theory of " Sui Qi Suo De"

The theory of " Sui Qi Suo De" originates from Zhang Zhongjing's Jin Gui Yao Lue and has been further developed by later generations of practitioners, offering significant guidance for clinical practice. Myelodysplastic syndromes (MDS) are common malignant disorders of the hematopoietic system, characterized by high heterogeneity and progressive mutational changes. In Traditional Chinese Medicine (TCM), MDS falls under the category of "marrow toxin exhaustion". This article applies the theory of " Sui Qi Suo De" in TCM to analyze the pathophysiological changes during different stages of MDS. Specifically, it explores the precursor stage (focusing on health maintenance and prevention before illness, addressing the " Suo De" of "gradual decline of vital qi"), the low-risk stage (strengthening the spleen and kidneys, clearing toxic pathogens, addressing the " Suo De" of "weakened vital qi invaded by pathogens"), and the medium-to-high-risk stage (detoxifying and reinforcing the body, harmonizing physical and mental health, addressing the " Suo De" of "dominant pathogens and declining vital qi"). The goal is to provide new directions and theoretical insights for the TCM treatment of MDS.
2.Study on the separation method of lung ventilation and lung perfusion signals in electrical impedance tomography based on rime algorithm optimized variational mode decomposition.

Real-time acquisition of pulmonary ventilation and perfusion information through thoracic electrical impedance tomography (EIT) holds significant clinical value. This study proposes a novel method based on the rime (RIME) algorithm-optimized variational mode decomposition (VMD) to separate lung ventilation and perfusion signals directly from raw voltage data prior to EIT image reconstruction, enabling independent imaging of both parameters. To validate this approach, EIT data were collected from 16 healthy volunteers under normal breathing and inspiratory breath-holding conditions. The RIME algorithm was employed to optimize VMD parameters by minimizing envelope entropy as the fitness function. The optimized VMD was then applied to separate raw data across all measurement channels in EIT, with spectral analysis identifying relevant components to reconstruct ventilation and perfusion signals. Results demonstrated that the structural similarity index (SSIM) between perfusion images derived from normal breathing and breath-holding states averaged approximately 84% across all 16 subjects, significantly outperforming traditional frequency-domain filtering methods in perfusion imaging accuracy. This method offers a promising technical advancement for real-time monitoring of pulmonary ventilation and perfusion, holding significant value for advancing the clinical application of EIT in the diagnosis and treatment of respiratory diseases.

3.Application of three-dimensional turbo spin-echo (SPACE) sequence in target delineation for stereotactic radiotherapy of brain metastases

Objective:To evaluate the detection capability of the contrast-enhanced three-dimensional turbo spin-echo (CE-SPACE) sequence for brain metastases (BM), aiming to provide evidence for precise target delineation in stereotactic radiotherapy (SRT).Methods:A total of 123 BM patients who received radiotherapy at the Affiliated Cancer Hospital of Zhengzhou University from May to November 2024 were enrolled. All patients underwent contrast-enhanced (CE) MRI and CT scans in the same treatment position, with images rigidly registered in the Eclipse planning system. Two experienced radiation oncologists independently delineated BM lesions on CE-MPRAGE and CE-SPACE sequences in a blinded manner. Patients were divided into the delayed group (10 min, n=61) and a priority group (5 min, n=62) based on the time interval between gadolinium injection and CE-SPACE acquisition. The non-parametric Wilcoxon rank-sum test was used to compare the lesion counts and volume differences between the two imaging sequences. Point-biserial correlation analysis was performed to assess the correlation between the additional lesions identified by CE-SPACE and lesion volume. Results:The overall analysis demonstrated that CE-SPACE detected 421 BM lesions, achieving an 8.2% higher detection rate than CE-MPRAGE ( Z=3.78, P<0.001). In terms of lesion volume, the median BM lesions volume identified by CE-SPACE [0.30(0.07,1.53)cm 3] was 8.7% larger than that by CE-MPRAGE [0.23 (0.04, 1.34) cm 3] ( Z=12.88, P<0.001). CE-SPACE demonstrated superior sensitivity for lesions ≤ 0.06 cm3, with negative correlation between the number of additional lesions detected and lesion volume ( r=-0.104, P=0.034). Subgroup analysis revealed that in the delayed group, CE-SPACE detected significantly more lesions [median 2 (1, 3.5) vs. 2 (1, 3), P=0.002] and larger volumes [0.39 (0.08, 2.24) cm3 vs. 0.29 (0.05, 1.99) cm3, P<0.001] than CE-MPRAGE. In the priority group, CE-SPACE detected significantly larger lesion volumes [0.55 (0.09, 2.06) cm3 vs. 0.45 (0.08, 1.88) cm3, P<0.001], but no significant difference was observed in lesion counts between two sequences ( P=0.059). Conclusions:Three-dimensional CE-SPACE sequence offers superior detection sensitivity for small BM (≤ 0.06 cm3), providing crucial guidance for accurate target delineation in SRT.
4.The effects of pulsed electric field combined gemcitabine therapy on the proliferation and stemness of HCCC-9810 cholangiocarcinoma stem cells

Objective:To investigate the effects of pulsed electric field (PEF) combined with gemcitabine (GEM) on the viability and stemness of HCCC-9810 cholangiocarcinoma stem cells.Methods:HCCC-9810 cholangiocarcinoma stem cells were established in serum-free, cytokine-rich medium and divided into four groups: the control group, GEM group, PEF group, and the pulsed electric field combined with gemcitabine (PEF+ GEM) group. Cell proliferation was detected using the Cell Counting Kit-8 (CCK8) assay. Cell viability and apoptosis rate were measured by flow cytometry. Cell invasion ability was assessed using the Transwell assay. The expression of stemness marker proteins CD133 and Octamer-binding transcription factor 4 (OCT4), as well as the expression of β-catenin, was detected by Western blotting.Results:Regarding cell viability, the GEM, PEF, and PEF+ GEM groups showed significantly lower cell viability and higher apoptosis rate than the control group at 24 h, 48 h, and 72 h (all P<0.05). At 48 h and 72 h, the PEF+ GEM group showed significantly lower cell viability (7.2%±0.3% and 5.9%±0.8%, respectively) than the GEM group (50.7%±0.6% and 31.0%±1.2%, respectively) and the PEF group (12.2%±0.2% and 12.8%±0.2%, respectively) (all P<0.05). Regarding stemness inhibition, the PEF+ GEM groups showed significantly lower expression levels of CD133 and OCT4 at 24 h, 48 h, and 72 h compared with the control group (all P<0.05). Notably, at 48 h, the PEF+ GEM group showed a significantly lower expression level of the OCT4 (0.61±0.02) than the GEM group (0.87±0.08) and the PEF group (1.00±0.10) ( P<0.01). Furthermore, at 24 h and 48 h, the GEM, PEF, and PEF+ GEM groups showed significantly lower expression levels of β-catenin compared with the control group (all P<0.05). Conclusion:Pulsed electric field combined with gemcitabine therapy demonstrated more effective anti-proliferation and cancer stemness inhibition effects on HCCC-9810 cholangiocarcinoma stem cells compared with either monotherapy.
5.Effects of Tactile/Motor Stimulation Massage on Feeding Outcomes and Neuromotor Development in Preterm Infants

Objective To observe the effects of tactile/motor stimulation massage on feeding outcomes and neuromotor development levels of preterm infants.Methods A total of 136 preterm hospitalized infants in the neonatal intensive care unit of a tertiary hospital in Yunnan Province from January 2024 to July 2024 were enrolled and randomly assigned to a control group and an experimental group,with 68 cases in each group.The control group received the routine care for preterm infants,while the experimental group received tactile/motor stimulation massage as an intervention in addition to the routine care.The time taken from gavage to oral feeding(d),the rate of complete oral feeding(%),the incidence of feeding intolerance(%),the weight(g),the length(cm),the head circumference(cm),neonatal behavioral neurometric score(NBNA)and Psychomotor development index(PDI)were compared between the two groups.Results The time from gavage to oral feeding in the experimental group(12.73±1.86)days was significantly shorter than that in the control group(13.66±1.68)days,and the difference was statistically significant(P<0.05);the rate of complete oral feeding in the experimental group(89.7%)was significantly higher than that in the control group(70.6%),P<0.05,and the incidence of feeding intolerance in the experimental group(8.8%)was significantly lower than that in the control group(16.2%),P<0.05;The experimental group showed superior results in weight,length,and head circumference compared to the control group(P<0.05);The neonatal behavioral neurometry score of the experimental group(36.16±1.53)was significantly higher than that of the control group(35.10±2.66),P<0.05,and the motor development index in the experimental group(76.88±6.12)was significantly higher than that in the control group(74.34±5.07),P<0.05.Conclusion Tactile/motor stimulation massage can effectively improve the feeding status and promote the neuromotor development of preterm infants.
6.Qualitative determination of multiple components in Rhodiola crenulata based on linear calibration with two reference substances assisted HPLC

Objective:To establish a qualitative analysis of Rhodiola crenulata for determing six components inclu-ding gallic acid,salidroside,tyrosol,1,2,3,4,6-O-gallic glucose,rhodiosin,oxorlin-7-O-rhamnoside by HPLC,and to find out the feasibility of the method of linear calibration using two reference substances in qualitative analysis of chromatographic peaks.Methods:The real retention time of 6 components in Rhodiola crenulata on 19 chromatographic columns were determined.Gallic acid and rhodiosin were selected as the reference substances,and the method of linear calibration using these 2 substances was used to predict the retention time.Tyrosol was also chosen as the reference to predict the retention time with the relative retention time method(RRT method).Comparing the accuracy of these two methods.Results:Compared to the RRT method,the method of linear calibration with two reference substances was more accurate for predicting the retention time and more adapatable for many kinds of chromatographic columns.Conclusion:As a new alternative reference substance method,the method of linear calibration using two reference substances can assist chromatographic peak determination better and has broad application prospects.
7.Effects of dapagliflozin tablet combined with creatine phosphate sodium on blood glucose and cardiac function in type 2 diabetes mellitus patients with heart failure

Objective:To analyze the effect of dapagliflozin tablet combined with creatine phosphate sodium on blood glucose and cardiac function in patients with type 2 diabetes mellitus (T2DM) complicated with heart failure.Methods:A total of 80 patients with T2DM complicated with heart failure who received treatment in the Rehabilitation University Qingdao Hospital (Qingdao Municipal Hospital) from October 2021 to October 2022 were selected prospectively as the subjects, and they were randomly divided into two groups with 40 cases in each group by envelope method. The control group was treated with conventional methods, and the observation group was treated with dapagliflozin tablet combined with sodium creatine phosphate on the basis of conventional methods. Both groups completed the treatment for 3 months. The clinical efficacy, blood glucose control effect, cardiac function and serum indexes of the two groups were compared between the two groups.Results:The total effective rate in the observation group was higher than that in the control group: 95.00%(38/40) vs. 75.00%(30/40), there was statistical difference ( χ2 = 6.28, P<0.05). After treatment, the levels of fasting blood glucose, 2 h postprandial blood glucose and glycosylated hemoglobin in the observation group were lower than those in the control group: (6.98 ± 0.67) mmol/L vs. (7.81 ± 0.78) mmol/L, (7.87 ± 1.58) mmol/L vs. (9.58 ± 1.49) mmol/L, (6.95 ± 0.57)% vs. (7.41 ± 0.60)%, there were statistical differences ( P<0.05). After treatment, the levels of left ventricular end-diastolic diameter (LVEDD), left ventricular end-systolic diameter (LVESD), serum interleukin6 (IL-6), C-reactive protein (CRP) and brain natriuretic peptide (BNP) in the observation group were lower than those in the control group: (61.76 ± 3.55) mm vs. (64.01 ± 3.87) mm, (45.45 ± 3.58) mm vs. (47.71 ± 3.62) mm, (8.99 ± 0.95) ng/L vs.(10.28 ± 1.16) ng/L, (4.28 ± 1.17) mg/L vs. (5.57 ± 1.31) mg/L, (199.90 ± 34.12) ng/L vs. (250.73 ± 35.11) ng/L; the cardiac output (CO) and left ventricular ejection fraction (LVEF) in the observation group were higher than those in the control group: (4.95 ± 1.06) L/min vs. (4.13 ± 0.92) L/min, (44.51 ± 3.68)% vs. (42.15 ± 3.73)%, there were statistical differences ( P<0.05). Conclusions:Dapagliflozin tablets combined with creatine phosphate sodium can improve the effective rate of patients with T2DM complicated with heart failure, reduce the level of blood sugar, help to improve the heart function, and reduce the levels of serum inflammatory factors.
