1.Retrospective analysis of adverse events associated with traditional Chinese medicine formula granules and decoction pieces in hospitalized patients using the global trigger tool

OBJECTIVE To provide technical support for improving recognition rate of adverse drug events (ADEs) related to traditional Chinese medicine (TCM) formula granules and decoction pieces among inpatient patients. METHODS By referencing the global trigger tool (GTT) whitepaper, literature on adverse reactions to TCM, and expert review opinions, ADE trigger items for TCM formula granules and decoction pieces used in the inpatients were established. GTT was applied to analyze ADEs in inpatients who had used TCM formula granules and decoction pieces in our hospital from August 2013 to August 2023, utilizing the Chinese Hospital Pharmacovigilance System. The effectiveness of GTT and the characteristics of these ADEs were analyzed. RESULTS A total of forty-eight triggers were established, including thirty-two laboratory test indexes, thirteen clinical symptoms, and three antidotes. Among the 1 682 patients included, GTT identified 652 potential ADEs, 284 true positive ADEs,with a trigger rate of 38.76% and a positive predictive value of 43.56%. After review by the auditor, 278 cases of ADEs were finally confirmed, with an incidence rate of 16.53%, significantly higher than the number of spontaneously reported ADEs during the same period (0). The 278 cases of ADEs were mostly grade 1 (223 cases), mainly involving hepatobiliary system, gastrointestinal system, blood- lymphatic system, etc;a total of 219 types of TCMs are involved,and the top five suspected TCMs used at a frequency higher than 1% were Poria cocos, Codonopsis pilosula, Atractylodes macrocephala, fried Glycyrrhiza uralensis, and Scutellaria baicalensis. CONCLUSIONS The established GTT can improve the recognition rate of ADEs for hospitalized patients using traditional Chinese medicine formula granules and decoction pieces.
2.Clinical analysis of CT angiography responsible vessel detection in children with hemoptysis by location setting of different regions of interest

Objective To explore the ability of different regions of interest(ROI)location settings for the detection of bronchial artery and pulmonary vascular lesions in children with hemoptysis via computed tomography angiography(CTA)examination.Methods The hemoptysis group(79 cases)who underwent chest CTA examination and the control group(79 cases)with negative CTA results were retrospectively selected.The ROI of the hemoptysis group were placed in the pulmonary trunk,left ventricle,ascending aorta and descending aorta,which were defined as groups 1,2,3 and 4 respectively.Clinical data(age,sex)and CT parameters,including bronchial artery diameter,ascending aorta CT value,pulmonary artery CT value,△CTA-PA,bronchial artery image score,pulmonary artery image score,were recorded and analyzed,respectively.Results There was a statistically significant difference in the diameter of the left and right bronchial arteries between the hemoptysis group and the control group(P<0.05).In the hemoptysis group,there were statistically significant differences in the image scores of the left and right bronchial arteries,pulmonary artery image scores,and△CTA-PA among the four subgroups(P<0.05).According to post-hoc comparison results,there was a statistically significant difference in the overall mean scores of the right bronchial artery image score(x2=11.333,P<0.05)and left bronchial artery image score(x2=8.111,P<0.05)between groups 2 and 3.Conclusion When ROI is placed in the ascending aorta,bronchial artery lesions and pulmonary vascular lesions can be detected simultaneously in CTA examination,which is helpful for the diagnosis of hemoptysis vascular etiology in children.
3.Application value of machine learning prediction model for neural invasion in gallbladder cancer based on enhanced CT and clinical characteristics

Objective:To explore the application value of machine learning prediction model for neural invasion in gallbladder cancer based on enhanced computed tomography (CT) and clinical characteristics.Methods:The retrospective cohort study was conducted. The clinical and imaging data of 502 patients with gallbladder cancer who were admitted to The First Affiliated Hospital of Xinxiang Medical University from January 2010 to June 2024 were collected. There were 171 males and 331 females, aged 65(range, 35?91)years. All patients underwent preoperative abdominal enhanced CT and radical resection. The 502 patients were randomly divided into a training set of 351 cases and a test set of 151 cases at a 7:3 ratio. The training set was used to construct prediction model, and the test set was used to validate prediction model. Observation indicators: (1)neural invasion in gallbladder cancer and influencing factor analysis; (2) construction and validation of machine learning prediction models for neural invasion in gallbladder cancer. Comparison of count data between groups was conducted using the chi-square test. Comparison of ordinal data between groups was conducted using the Mann-Whitney U test. Logistic regression model was performed for univariate and multivariate analyses. Independent influencing factors were incor-porated to construct machine learning models using the standard library modules based on Python 3.9. Receiver operating characteristic (ROC) curves were plotted, and the accuracy, sensitivity, specificity, area under the curve (AUC), precision, F1 score, positive predictive value, negative predic-tive value, and Kappa value were calculated to evaluate the predictive performance of the models. The Delong test was used to assess the differences in AUC among different models in the test set. The Hosmer-Lemeshow test and Brier score were used to evaluate the calibration of the models. Results:(1) Neural invasion in gallbladder cancer and influencing factor analysis. Of the 502 patients with gallbladder cancer, 131 cases had neural invasion, and 371 cases had no neural invasion. Results of multivariate analysis showed that total bilirubin, carcinoembryonic antigen, CA199, CA125, neutrophil-lymphocyte ratio, liver invasion detected by CT, vascular invasion detected by CT, hilar or retroperi-toneal lymph node metastasis detected by CT, and tumor stages T3 and T4 were independent influencing factors for neural invasion in patients with gallbladder cancer [ odds ratios=3.747, 2.395, 3.917, 3.596, 2.805, 2.377, 3.523, 2.774, 5.080, 6.809, 95% confidence interval ( CI) as 1.890?7.430, 1.154?4.971, 2.054?7.472, 1.807?7.155, 1.506?5.225, 1.241?4.553, 1.666?7.449, 1.483?5.189, 2.050?12.589, 2.552?18.168, P<0.05]. (2) Construction and validation of machine learning predic-tion models for neural invasion in gallbladder cancer. Based on the independent influencing factors, seven machine learning models were constructed, including logistic regression, K-nearest neighbors, support vector machine, random forest, decision tree, back-propagation neural network, and gradient boosting machine. The ROC curves of seven machine learning models in the test set were plotted, and the AUC were 0.900(95% CI as 0.851?0.948), 0.741(95% CI as 0.646?0.829), 0.836(95% CI as 0.762?0.895), 0.782(95% CI as 0.701?0.855), 0.839(95% CI as 0.770?0.901), 0.817(95% CI as 0.738?0.887), 0.843(95% CI as 0.770?0.909), respectively. Results of Delong test showed that the logistic regression model had the highest AUC. The sensitivity and specificity of the logistic regression model were 0.868 and 0.805 respectively, indicating the best balance. Results of Hosmer-Lemeshow test showed that the logistic regression model had a good goodness-of-fit ( χ2=5.320, P>0.05). The Brier score of the logistic regression model was relatively low, as 0.168, which verified its calibration advantage. Conclusion:Total bilirubin, carcinoembryonic antigen, CA199, CA125, neutrophil-to-lymphocyte ratio, liver invasion detected by enhanced CT, vascular invasion detected by enhanced CT, hilar or retroperitoneal lymph node metastasis detected by enhanced CT, and tumor stages T3 and T4 are independent influencing factors for nerve invasion in patients with gallbladder cancer. Seven machine learning models are constructed based on enhanced CT and clinical characteristics to predict neural invasion in gallbladder cancer, of which the logistic regression model demonstrates good predictive performance.
4.Artificial intelligence-assisted quality control of anal sphincter ultrasound:a multicenter clinical study

Objective:To develop a quality control model for anal sphincter ultrasound images and validate its diagnostic performance across multiple centers.Methods:A retrospective analysis was conducted on anal sphincter ultrasound images from seven medical centers in China between May 2019 and June 2022. A total of 7 040 images from 3 116 patients were included and divided into a training set(4 912 images)and a validation set(2 128 images). The images were classified as standard or non-standard images by three experts. Three models were developed based on different image feature extraction methods:a single-branch model,a multi-branch weighted model,and a multi-branch ensemble model. The diagnostic performance of each model was evaluated using the area under the ROC curve(AUC),sensitivity,specificity,accuracy,positive predictive value,and negative predictive value,respectively. The optimal model was selected and compared with the performance of 4 doctors with varying experience levels. Sixty days later,the images with the assistance of the model's output were reassessed by the doctors to evaluate its impact on manual quality control.Results:① Among the 3 models,the multi-branch ensemble model demonstrated the highest AUC and sensitivity,with an AUC of 0.966(95% CI=0.958 - 0.974),a sensitivity of 91.83%,and a specificity of 91.41%. This model was named M quality. ② M quality's AUC was slightly lower than that of Senior A and B(0.966 vs. 0.976,0.976,and P<0.05),its sensitivity was slightly lower than that of Senior A(91.83% vs. 95.61%, P<0.001)but comparable to Senior B(91.83% vs. 92.89%, P=0.315),its specificity was slightly lower than Senior A and B(91.41% vs. 94.44%,98.18%,and P<0.05). However,M quality significantly outperformed Junior A and B in AUC and sensitivity(AUC:0.966 vs. 0.850,0.818;sensitivity:91.83% vs. 84.90%,61.46%;all P<0.001),its specificity was higher than that of Junior A(91.41% vs. 80.28%, P<0.001)but lower than that of Junior B(91.41% vs. 95.96%, P<0.001). ③ With model assistance,Senior B's sensitivity(92.89% vs. 94.20%, P=0.001)and Senior A's specificity(94.44% vs. 96.56%, P<0.001)improved significantly. Junior A and B showed significant improvements in AUC and sensitivity(AUC:0.931 vs. 0.850,0.914 vs. 0.818;sensitivity:91.83% vs. 84.90%,89.53% vs. 61.46%;all P<0.001). After model assistance,Junior A's specificity increased(93.62% vs. 80.28%, P<0.001),while Junior B's specificity decreased(91.60% vs. 95.96%, P=0.013). Conclusions:This study develops a quality control model for anal sphincter ultrasound images with robust diagnostic performance,approaching the level of seniors. The model significantly enhances the image quality assessment capabilities of juniors,demonstrating promising clinical application potential.
5.Clinical effect of precapsular pocket reposition in correcting implant malposition after breast augmentation

Objective:To investigate the clinical outcomes of precapsular pocket repositioning for correcting implant malposition following augmentation mammoplasty.Methods:A retrospective analysis was conducted on 29 female patients aged 25-37 (28.8±3.4) years who underwent precapsular pocket repositioning at the Affiliated Friendship Plastic Surgery Hospital of Nanjing Medical University from December 2015 to August 2024. The surgical technique involved preserving the original capsule, creating a new implant pocket anterior to the capsule, and closing the original capsular space. Postoperative complications were recorded, and breast satisfaction was evaluated preoperatively and at 6 months postoperatively using the BREAST-questionnaire (BREAST-Q).Results:All 29 patients successfully underwent precapsular pocket repositioning with primary wound healing. During the follow-up period, all patients were satisfied with the correction of implant malposition. The mean BREAST-Q score improved significantly from (43.56±3.17) scores preoperatively to (72.56±13.49) scores at 6 months postoperatively ( P<0.001). No hematoma, implant rupture, recurrent malposition, capsular contracture, or surgical site infection occurred in any patient. Conclusion:Precapsular pocket repositioning provides favorable clinical outcomes for patients with implant malposition after augmentation mammoplasty, and there are no severe complications .
6.Effects of dance movement therapy on psychological symptoms and psychological craving in methamphetamine-dependent women

Objective:To investigate the intervention effect of dance therapy on the psychological symptoms and psychological craving in methamphetamine-dependent women.Methods:A total of 60 methamphetamine de-pendents in a women's drug rehabilitation center were divided into intervention group and control group(1 person dropped out).The intervention group was trained for 8 dance movements,during which the control group did not in-tervene.The Zhang Tuoji and Chen Huichang's Temperament Scale,Symptom Self-Rating Scale(SCL-90)and Amphetamine Craving Scale(DSQ)were used to evaluate the temperament type,psychological symptoms and psy-chological craving.Results:The differences of SCL-90 depression and anxiety scores(post-intervention score-base-line score)and DSQ total scores were higher in the intervention group than in the control group(Ps<0.05).The choleric temperament scores were negatively correlated with the differences of DSQ total scores(r=-0.45,P<0.05),and the melancholic temperament scores were positively correlated with the differences of SCL-90 hostility dimension scores(r=0.41,P<0.05).Conclusion:Dance therapy could improve the psychological symptoms and psychological craving of women dependent on methamphetamine,with varying effects across different temperament types.
7.Evaluation of the reliability and validity of the Chinese listening self-efficacy questionnaire

Objective To develop and translate the Chinese listening self-efficacy questionnaire(C-LSEQ)and to test its reliability and validity.Methods A total of 172 subjects aged≥60 years with age-related hearing loss completed the C-LSEQ questionnaire via direct interviews.Pure tone hearing threshold test,Mandarin hearing in noise test,and the hearing handicap inventory for elderly-screening(HHIE-S)were evaluated.Twenty subjects were randomly selected from the original group to complete the C-LSEQ questionnaire in 2 weeks after the initial evaluation.Results ① Reliability:The Cronbach's a coefficients of the three sub-dimensions and the overall ques-tionnaire were all>0.8,and the test-retest reliability coefficients of the three sub-dimensions and the overall ques-tionnaire were all>0.9(P<0.001).② Validity:The experts collectively evaluated the representativeness of the statements with good content validity.The convergent validity test showed that the composite reliability(CR)of the 2 sub-dimensions and the overall questionnaire were all>0.7,and the average variance extracted(AVE)were all>0.5.The CR value of the complex listening dimension was 0.655,and the AVE value was 0.937.The criterion validity test showed that the overall questionnaire and the three sub-dimensions of the C-LSEQ were significantly correlated with the pure-tone average,speech recognition thresholds in noise,and HHIE-S(P<0.001).Conclusion The C-LSEQ exhibits stable structure,with good reliability and validity.It can be used to assess listen-ing self-efficacy in adults with hearing loss,especially in the elderly with age-related hearing loss.
8.Severity of SARS-CoV-2 infection in children with kidney disease undergoing immunosuppressive therapy

Objective:To investigate the impact of immunosuppressive therapy on the severity of SARS-CoV-2 infection and cytokine levels in pediatric patients with kidney diseases.Methods:A retrospective analysis was conducted on the clinical data of 40 hospitalized pediatric patients who were diagnosed with SARS-CoV-2 infection at the 900th Hospital of PLA Joint Logistic Support Force from December 2022 to February 2023. Based on their immunosuppressive status prior to SARS-CoV-2 infection, these patients were categorized into immunosuppressive group and non-immunosuppressive group. Independent sample t-tests, Mann-Whitney U tests, and χ2 test were employed to compare the clinical baseline characteristics and laboratory data, the severity of SARS-CoV-2 infection, and the levels of cytokines between the 2 groups. Results:Among the 40 patients, 11 were in the immunosuppressive group (aged 13 (8, 14) years, 9 males and 2 females) and 29 in the non-immunosuppressive group (aged 2 (1, 4) years, 15 males and 14 females). In the immunosuppressive group, 2 were asymptomatic cases, 8 were mild cases, and 1 was moderate case, and there was no severe or critical cases. In the non-immunosuppressive group, 8 were mild cases, 5 were moderate, 15 were severe cases, 1 was critical case, and no asymptomatic cases. The underlying diseases in the immunosuppressive group included nephrotic syndrome (6 cases), IgA vasculitis nephritis (2 cases), lupus nephritis (1 case), post-renal transplantation (1 case), and renal failure (1 case), with a mean total immunosuppression score (TIS) of (3.6±1.4) points. In the non-immunosuppressive group, 2 patients had a history of epilepsy, and the remaining 27 cases had no underlying conditions, all with TIS scores of 0. Compared to the children in the non-immunosuppressive group, those in the immunosuppressive group were more likely to exhibit asymptomatic or mild infection, with lower risks of severe disease, cytokine storm, fever, and cough, but a higher risk of fatigue ( OR=1.22, 2.66, 0.48, 0.12, 0.12, 0.13, 1.22; 95% CI 0.93-1.62, 0.99-7.15, 0.33-0.70, 0.03-0.57, 0.03-0.57, 0.03-0.65, 0.93-1.62; all P<0.05). The levels of cytokine IL-6, interferon-α and interferon-γ in the immunosuppressive group were all lower than those in the non-immunosuppressive group ( Z=2.23, 2.51, 2.92, respectively; all P<0.05). Conclusion:Pediatric patients with kidney diseases receiving appropriate immunosuppressive therapy may mitigate the severity of SARS-CoV-2 infection by suppressing the expression of cytokines.
