1.Application of Micro-lecture Combined with Workshop Education Mode in Home Chinese Medicine Retention Enema for Pa-tients with Ulcerative Colitis

[Objective]To investigate the application effect of micro-lecture combined with workshop education mode in home Chinese medicine retention enema for patients with ulcerative colitis(UC).[Methods]Eighty patients with UC who needed home Chinese medicine retention enema of Compound Qingdai Enema in Zhejiang Provincial Hospital of Chinese Medicine from June 2022 to June 2024 were selected and randomly divided into control group and experimental group,40 cases in each group.Control group received the routine health education,while experimental group received micro-lecture combined with workshop education mode.The scores of Chinese medicine retention enema and satisfaction with nursing were evaluated in the two groups within 1 day after intervention.The clinical intestinal symptoms,quality of life,anxiety and depression scores of patients were evaluated before intervention and 6 months after intervention.[Results]After the intervention of micro-lecture combined with workshop mode,the theoretical score of Chinese medicine retention enema in experimental group was(98.35±0.80),the operational score was(92.48±3.86),the nursing satisfaction score was(96.93±1.70),the quality of life score was(176.11±21.11),the anxiety score was(34.56±5.69),and the depression score was(33.41±5.69).In control group,the theoretical score was(92.02±2.01),the operational score was(80.05±3.07),the nursing satisfaction score was(92.40±2.12),the quality of life score was(161.29±20.56),the anxiety score was(42.31±5.33),and the depression score was(41.63±5.12).The scores of theoretical and operational assessment,nursing satisfaction,each item of clinical intestinal symptoms,quality of life,anxiety and depression in experimental group were significantly higher than those in control group(P<0.05).[Conclusion]Compared with conventional health education,the micro-lecture combined workshop education mode can improve the cognition and operation level of home Chinese medicine retention enema in patients with UC,reduce the anxiety and depression levels of patients,promote the relief of clinical intestinal symptoms,improve quality of life and nursing satisfaction.
2.Therapeutic effect and mechanism of hordenine on ovalbumin-induced allergic rhinitis in rats

To investigate the therapeutic effect and related mechanisms of hordenine on ovalbumin (OVA)-induced allergic rhinitis (AR) in rats, HE and AB-PAS staining were used to detect the improvement of pathological damage to the nasal mucosa induced by hordenine. ELISA was employed to detect the effect of hordenine on OVA-sIgE in serum and IL-4 in the nasal mucosa supernatant of rats. IHC and Western blot experiments were undertaken to examine the effect of hordenine on Th1/Th2 cell balance. Bioinformatics analysis was performed to predict pathways, which were verified by in vivo and in vitro experiments. The experimental results showed that hordenine could alleviate the behavioral manifestations of OVA-induced AR rats, alleviate nasal mucosal pathological damage caused by AR, and reduce the secretion of OVA-sIgE and IL-4. In addition, hordenine could regulate the Th1/Th2 balance. Bioinformatics analysis results showed that the potential pathway of action of hordenine on AR was the phosphoinositide 3 kinase (PI3K)/protein kinase B (Akt) signaling pathway. The in vivo experimental results showed that the expression of PI3K and p-Akt proteins in the nasal mucosa of the model group rats was significantly increased (P < 0.01), and that the protein expression level was significantly decreased after the administration of hordenine, which was also confirmed by an in vitro experiment. This study suggests that hordenine may regulate Th1/Th2 cell balance through the PI3K/Akt signaling pathway, thereby exerting an alleviating effect on OVA-induced AR.

4.Existing tests vs. novel non-invasive assays for detection of invasive aspergillosis in patients with respiratory diseases

Background::Although existing mycological tests (bronchoalveolar lavage [BAL] galactomannan [GM], serum GM, serum (1,3)-β-D-glucan [BDG], and fungal culture) are widely used for diagnosing invasive pulmonary aspergillosis (IPA) in non-hematological patients with respiratory diseases, their clinical utility in this large population is actually unclear. We aimed to resolve this clinical uncertainty by evaluating the diagnostic accuracy and utility of existing tests and explore the efficacy of novel sputum-based Aspergillus assays. Methods::Existing tests were assessed in a prospective and consecutive cohort of patients with respiratory diseases in West China Hospital between 2016 and 2019 while novel sputum assays (especially sputum GM and Aspergillus-specific lateral-flow device [LFD]) in a case-controlled subcohort. IPA was defined according to the modified European Organization for Research and Treatment of Cancer/Mycoses Study Group criteria. Sensitivity and specificity were computed for each test and receiver operating characteristic (ROC) curve analysis was performed. Results::The entire cohort included 3530 admissions (proven/probable IPA = 66, no IPA = 3464) and the subcohort included 127 admissions (proven/probable IPA = 38, no IPA = 89). Sensitivity of BAL GM (≥1.0 optical density index [ODI]: 86% [24/28]) was substantially higher than that of serum GM (≥0.5 ODI: 38% [39/102]) ( χ2 = 19.83, P < 0.001), serum BDG (≥70 pg/mL: 33% [31/95]) ( χ2 = 24.65, P < 0.001), and fungal culture (33% [84/253]) ( χ2 = 29.38, P < 0.001). Specificity varied between BAL GM (≥1.0 ODI: 94% [377/402]), serum GM (≥0.5 ODI: 95% [2130/2248]), BDG (89% [1878/2106]), and culture (98% [4936/5055]). Sputum GM (≥2.0 ODI) had similar sensitivity (84% [32/38]) (Fisher’s exact P = 1.000) to and slightly lower specificity (87% [77/89]) ( χ2 = 5.52, P = 0.019) than BAL GM (≥1.0 ODI). Area under the ROC curve values were comparable between sputum GM (0.883 [0.812-0.953]) and BAL GM (0.901 [0.824-0.977]) ( P = 0.734). Sputum LFD had similar specificity (91% [81/89]) ( χ2 = 0.89, P = 0.345) to and lower sensitivity (63% [24/38]) ( χ2 = 4.14, P = 0.042) than BAL GM (≥1.0 ODI), but significantly higher sensitivity than serum GM (≥0.5 ODI) ( χ2 = 6.95, P = 0.008), BDG ( χ2 = 10.43, P = 0.001), and fungal culture ( χ2 = 12.70, P < 0.001). Conclusions::Serum GM, serum BDG, and fungal culture lack sufficient sensitivity for diagnosing IPA in respiratory patients. Sputum GM and LFD assays hold promise as rapid, sensitive, and non-invasive alternatives to the BAL GM test.
5.Genome-wide analysis identify novel germline genetic variations in ADCY1 influencing platinum-based chemotherapy response in non-small cell lung cancer.

To explore the pharmacogenomic markers that affect the platinum-based chemotherapy response in non-small-cell lung carcinoma (NSCLC), we performed a two-cohort of genome-wide association studies (GWAS), including 34 for WES-based and 433 for microarray-based analyses, as well as two independent validation cohorts. After integrating the results of two studies, the genetic variations related to the platinum-based chemotherapy response were further determined by fine-mapping in 838 samples, and their potential functional impact were investigated by eQTL analysis and in vitro cell experiments. We found that a total of 68 variations were significant at P < 1 × 10-3 in cohort 1 discovery stage, of which 3 SNPs were verified in 262 independent samples. A total of 541 SNPs were significant at P < 1 × 10-4 in cohort 2 discovery stage, of which 8 SNPs were verified in 347 independent samples. Comparing the validated SNPs in two GWAS, ADCY1 gene was verified in both independent studies. The results of fine-mapping showed that the G allele carriers of ADCY1 rs2280496 and C allele carriers of rs189178649 were more likely to be resistant to platinum-based chemotherapy. In conclusion, our study found that rs2280496 and rs189178649 in ADCY1 gene were associated the sensitivity of platinum-based chemotherapy in NSCLC patients.
6.Pharmacogenetic Correlates of Antipsychotic-Induced Weight Gain in the Chinese Population.

Antipsychotic-induced weight gain (AIWG) is a common adverse effect of this treatment, particularly with second-generation antipsychotics, and it is a major health problem around the world. We aimed to review the progress of pharmacogenetic studies on AIWG in the Chinese population to compare the results for Chinese with other ethnic populations, identify the limitations and problems of current studies, and provide future research directions in China. Both English and Chinese electronic databases were searched to identify eligible studies. We determined that > 25 single-nucleotide polymorphisms in 19 genes have been investigated in association with AIWG in Chinese patients over the past few decades. HTR2C rs3813929 is the most frequently studied single-nucleotide polymorphism, and it seems to be the most strongly associated with AIWG in the Chinese population. However, many genes that have been reported to be associated with AIWG in other ethnic populations have not been included in Chinese studies. To explain the pharmacogenetic reasons for AIWG in the Chinese population, genome-wide association studies and multiple-center, standard, unified, and large samples are needed.

7.Total tumor perfusion value of CT in hepatocellular carcinoma with the tumor volume and Child-Pugh classification

Objective To investigate the correlation between tumor perfusion parameters and tumor volume and Child-Pugh classification in CT of hepatocellular carcinoma (HCC).Methods Fifty-six patients with hepatocellular carcinoma were selected to perform CT perfusion imaging.The parameters of the total tumor perfusion such as hepatic artery perfusion (HAP),portal vein perfusion (PVP)and hepatic perfusion index (HAPI)were calculated according to the degree of hepatic encephalopathy,albumin,bilirubin, clotting time,ascites for liver Child-Pugh classification.The relationship between the tumor or peritumoral perfusion parameters with tumor volume and Child-Pugh classification were analyzed.Results (1)There was no correlation between tumor or peritumoral perfusion with the tumor volume.(2)The difference of HAP,PVP and HAPI between the different Child-Pugh classification groups was statistically significant (P<0.000 1).(3)With the reduction of Child-Pugh classification,the tumor body HAP and HAPI values gradually decreased, while the PVP value increased gradually.Conclusion There is no correlation between the tumor volume of hepatocellular carcinoma with total tumor perfusion parameters.The differences in perfusion measurements between different Child-Pugh classification can intuitively and quantitatively reflect the reserve function of the liver.
8.Nursing research progress on chemotherapy-induced stomatitis in children with malignant tumor

The incidence of chemotherapy-induced stomatitis is very high in the pediatric patients with malignant tumor. The pain, eating disorders and malnutrition caused induced by stomatitis influence children's quality of life seriously. They even influence the progress of chemotherapy regimens. This paper summarizes the causes, risk factors, the grading standard, prevention and nursing measures of chemotherapy-induced stomatitis. It indicates that the prevention measures should be combined with the treatment for chemotherapy-induced stomatitis at the same time. Medical staff should evaluate the oral cavity problems of the pediatric patients and make oral nursing plans for every patient according to their chemotherapy regimens, in order to accelerate the healing process of chemotherapy-induced stomatitis.
9.Color Doppler ultrasound findings of hepatic artery in patients with biliary complications after liver transplantation

Objective To investigate the relation between hepatic arterial abnormity and biliary complications after liver transplantation with the use of color Doppler ultrasonography. Methods Clinical data of 98 patients undergoing liver transplantation in the Third Afifliated Hospital of Sun Yat-sen University from October 2004 to October 2013 were analyzed retrospectively. According to whether the patients suffered from biliary complications after liver transplantation, they were divided into biliary complication group and non-biliary complication group. There were 52 cases in biliary complication group [49 males,3 females, mean age:(49±24) years old]. There were 46 cases in non-biliary complication group [41 males, 5 females, mean age:(46±29) years old]. The informed consents of all patients were obtained and local ethical committee approval had been received. Color Doppler ultrasonography was performed to examine the liver graft 90 d after liver transplantation and the blood lfow parameters of intra-, extrahepatic bile duct and hepatic artery were observed. The comparison of blood lfow parameters between two groups was conducted using t test.The comparison of incidence of hepatic arterial abnormity was conducted using Chi-square test. Results In biliary complication group, hepatic artery stenosis was observed in 6 cases, hepatic artery thrombosis in 3 cases and hepatic artery stenosis combined with intrahepatic arteriovenous ifstula in 1 case. The incidence of hepatic arterial abnormity was 19% (10/52), which was significantly higher than that in non-biliary complication group [2%(1/46)] (χ2=24.96, P<0.05). The maximum velocity of the proper hepatic artery in biliary complication group [(90±21) cm/s] was significantly higher than that in non-biliary complication group [(67±17)cm/s] and the resistance index of intrahepatic artery in biliary complication group (0.55±0.14) was signiifcantly lower than that in non-biliary complication group (0.64±0.08) (t=9.74, -16.96; P<0.05). Conclusions Biliary complications after liver transplantation are associated with hepatic arterial abnormity. Monitoring the hemodynamic variation of hepatic artery after liver transplantation with color Doppler ultrasonography helps to diagnose the biliary complications early.
10.Application of transperineal ultrasound in female typing of cystocele

Objective To investigate the application value of transperineal ultrasound in female patients with cystocele.Methods Transperineal ultrasound were performed in 36 female patients with cystocele and the bladder neck movement degrees (BND),retrovesical angle and the degree of urethral rotation were observed at maximum Valsalva.The ultrasonic manifestation of 36 female patients with cystocele were summarized.Results On maximal Valsalva bladder neck mobility all increased in 36 patients,32 patients with open retrovesical angle (≥140°) and 30 female patients with urethral rotation ≥ 45°.There were 6 cases with type Ⅰ,bladder neck was lower than inferoposterior margin of the symphysis pubis,cystocele with open retrovesical angle (≥140°) and urethral rotation <45°.There were 26 cases with type Ⅱ,bladder neck was lower than inferoposterior margin of the symphysis pubis,cystocele with open retrovesical angle (≥140°) and urethral rotation ≥45°.There were 4 cases with type Ⅲ,bladder was lower than inferoposterior margin of the symphysis pubis,cystocele with intact retrovesical angle (< 140°) and urethral rotation ≥45°.Conclusions The transperineal ultrasound can be used for classification of cystocele in female patients and which may provide more reliable information for clinical diagnosis.
