1.Research Progress on Small Animal Models of Extracorporeal Membrane Oxygenation

Extracorporeal membrane oxygenation(ECMO)provides blood circulation with assisted breathing for patients with severe cardiopulmonary failure,and buys valuable time for the rescue of critical patients.However,extracorporeal membrane oxygenation is often associated with serious complications.Small animal models have the advantages of low price,wide source,high flexibility and good reproducibility,and are an effective platform for evaluating strategies for prevention and treatment of ECMO complications.In recent years,more and more experimental studies have been conducted using small animal ECMO models.In this paper,the current status of the construction and application of small animal ECMO models at home and abroad is summarized,in order to optimize the related strategies of small animal ECMO model construction and promote the application and development of small animal ECMO models.
2.Reconstruction of Lumbar Vertebrae Images from Abdominal CT Examinations Using Deep Learning Image Reconstruction Algorithms

Purpose To evaluate the effectiveness of deep learning image reconstruction(DLIR)algorithms in reconstructing lumbar vertebrae images from abdominal CT scans,aiming to reduce radiation dose and eliminate the need for repeat lumbar CT examinations.Materials and Methods A retrospective collection was conducted from March to May 2024 in the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine.Thirty-two patients who underwent both abdominal and lumbar CT scans in a supine head-first position were enrolled.The abdominal CT(DLIR group)utilized a tube voltage of 120 kVp and a current of 200 mA with high-intensity DLIR for lumbar reconstruction.The standard lumbar CT(lumbar group)used the same voltage with a tube current of 260 mA and was reconstructed using 60%weighted adaptive statistical iterative reconstruction.Objective assessments was used to measure the CT values,noise(standard deviation,SD value),signal-to-noise ratio and contrast-to-noise ratio(excluding adipose tissue)at the third lumbar vertebral pedicle level and the L2/L3 intervertebral disc level for muscle,adipose tissue,cancellous bone,intervertebral discs,dura mater and cortical bone.Subjective assessments employed a five-point scale to evaluate image contrast,noise and sharpness.Results The volume CT dose index in lumbar group and DLIR group were 15.25 mGy and 11.74 mGy,respectively.There was no statistical difference in CT values between the structures of both groups(all P>0.05).Compared with the lumbar group,the DLIR group showed significant reductions in SD values across the measured tissues by 31.09%,35.66%,13.48%,27.82%,24.93%and 15.09%(t=5.09-7.21,all P<0.05).The signal-to-noise ratio improved by 36.40%,52.31%,16.56%,34.13%,38.39%and 18.81%,and the contrast-to-noise ratio improved by 51.70%,51.32%,36.24%,34.47%and 53.56%(t=-9.58--4.23,all P<0.001).The DLIR group significantly outperformed the lumbar group in image contrast[4.45(4.00,5.00)points vs.4.75(4.00,5.00)points],image noise[4.06(4.00,4.00)points vs.4.39(4.00,5.00)points],and spatial resolution of fine structures[4.00(4.00,4.00)points vs.4.27(4.00,5.00)points](Z=-3.80,-4.38,-3.55,all P<0.001).Conclusion Using high-intensity DLIR for abdominal examinations can achieve high-quality lumbar CT images with a 25%reduction in radiation dose,enabling simultaneous abdominal and lumbar scanning in a single session.
3.Synthetic MRI for differentiating cervical squamous carcinoma and cervical adenocarcinoma

Objective To observe the value of synthetic MRI(SyMRI)MAGnetic resonance image Compilation(MAGiC)sequence parameters for differentiating cervical squamous cell carcinoma and cervical adenocarcinoma.Methods Sixty-six patients with pathologically confirmed cervical cancer were retrospectively enrolled and divided into cervical squamous cell carcinoma group(n=56)and cervical adenocarcinoma group(n=10).Quantitative MAGiC parameters were collected and compared between groups,and those being significantly different were combined to construct a logistic regression model.The performance of each parameter alone and their combination for differentiating cervical squamous cell carcinoma and cervical adenocarcinoma was evaluated with receiver operating characteristic(ROC)curve and the area under the curve(AUC).Results In cervical adenocarcinoma group,lesions's T1 and T2 were higher,while R1 and R2 were lower than those in cervical squamous cell carcinoma group(all P<0.05).No statistically significant difference of proton density was found between groups(P>0.05).The AUC of T1,T2,R1,R2 alone and their combination for differentiating cervical squamous cell carcinoma and cervical adenocarcinoma was 0.959,0.945,0.961,0.942 and 0.996,respectively,and no significant difference was found between each two ones(Z=0.267 to 1.396,all P>0.05).Conclusion SyMRI had high value for differentiating cervical squamous cell carcinoma and cervical adenocarcinoma.
4.Application of Micro-lecture Combined with Workshop Education Mode in Home Chinese Medicine Retention Enema for Pa-tients with Ulcerative Colitis

[Objective]To investigate the application effect of micro-lecture combined with workshop education mode in home Chinese medicine retention enema for patients with ulcerative colitis(UC).[Methods]Eighty patients with UC who needed home Chinese medicine retention enema of Compound Qingdai Enema in Zhejiang Provincial Hospital of Chinese Medicine from June 2022 to June 2024 were selected and randomly divided into control group and experimental group,40 cases in each group.Control group received the routine health education,while experimental group received micro-lecture combined with workshop education mode.The scores of Chinese medicine retention enema and satisfaction with nursing were evaluated in the two groups within 1 day after intervention.The clinical intestinal symptoms,quality of life,anxiety and depression scores of patients were evaluated before intervention and 6 months after intervention.[Results]After the intervention of micro-lecture combined with workshop mode,the theoretical score of Chinese medicine retention enema in experimental group was(98.35±0.80),the operational score was(92.48±3.86),the nursing satisfaction score was(96.93±1.70),the quality of life score was(176.11±21.11),the anxiety score was(34.56±5.69),and the depression score was(33.41±5.69).In control group,the theoretical score was(92.02±2.01),the operational score was(80.05±3.07),the nursing satisfaction score was(92.40±2.12),the quality of life score was(161.29±20.56),the anxiety score was(42.31±5.33),and the depression score was(41.63±5.12).The scores of theoretical and operational assessment,nursing satisfaction,each item of clinical intestinal symptoms,quality of life,anxiety and depression in experimental group were significantly higher than those in control group(P<0.05).[Conclusion]Compared with conventional health education,the micro-lecture combined workshop education mode can improve the cognition and operation level of home Chinese medicine retention enema in patients with UC,reduce the anxiety and depression levels of patients,promote the relief of clinical intestinal symptoms,improve quality of life and nursing satisfaction.
5.Analysis on the clinical application effects and development of intelligent control system for airbag pressure of tracheal catheter

Objective:To analyze the research and development of control system for airbag pressure of intelligent tracheal catheter,and its application effect in clinical practice.Methods:The hospital designed an intelligent self-adaptive control system for cuff pressure of tracheal catheter,which can rationally apply main components such as single chip microcomputer,display screen,control panel,pressure monitoring module,inflation device and solenoid valve,etc..This control system connected the measuring module of pressure through single chip microcomputer,so as to continuously detect the airbag pressure of tracheal catheter.An inflation and deflation algorithm was independently researched and developed to control air pump or solenoid valve to inject or release part of the gas to the airbag,so as to achieve intelligent control for the pressure of airbag.Sixty patients underwent endotracheal intubation who admitted to the department of general surgery of Wuhan Hospital of Traditional Chinese Medicine from January 2022 to January 2023 were selected as the study objects,and they were divided into control group(n=30)and observation group(n=30)according to random number table method.The cuff pressure of endotracheal intubation of patients in control group were managed by conventional method.The intelligent adaptive control system was adopted to manage the cuff pressure of endotracheal intubation for patients in the observation group.The complications,compliance rate of airbag pressure,the instantaneous maximum value of airbag pressure during sputum aspiration,insertion of gastric tube,and turning over of body were compared between the two groups after intubation.Results:The incidence of complication included cough,trachyphonia,dysphagia and bloody sputum after tracheal intubation in the observation group were 6.66%,which was significantly lower than 33.33%in the control group,and the difference was statistically significant(x2=6.667,P<0.05).The compliance rate of the pressure(25-30cmH2O)of airbag in the control group was 92.83%,which was significantly lower than 97.78%in the observation group,and the difference was statistically significant(x2=14.698,P<0.05).The instantaneous maximum values of airbag pressure during sputum aspiration,insertion of gastric tube and turning over were(28.24±3.65)cmH2O,(27.98±4.25)cmH2O and(28.65±4.87)cmH2O in observation group,respectively,which were significantly lower than those in control group,and the differences were significant(t=17.930,19.208,16.485,P<0.05).Conclusion:The application of intelligent adaptive control system for cuff pressure of tracheal catheter can maintain the airbag pressure of tracheal catheter at normal level(20-30 cmH2O),and reduce complications,and improve the compliance rate of airbag pressure.
6.Staged pressure therapy can improve scar hyperplasia and motor function after a deep hand burn

Objective:To explore the effect of staged pressure therapy, in addition to rehabilitation training, on scar hyperplasia and motor function after a deep hand burn.Methods:Sixty patients with deep hand burns were randomly divided into a control group ( n=30) and an observation group ( n=30). In addition to specialized burn treatment and early rehabilitation training, the control group was given routine pressure treatment, while the observation group was provided with staged pressure therapy for 6 months. Before the treatment and after 3 and 6 months of treatment, scar hyperplasia and hand function in both groups was evaluated using the Vancouver scar scale (VSS), grip strength and pinch strength tests, and the Jebsen hand function test (JHFT). Total active motion (TAM) and total passive motion (TPM) of the finger joints was quantified, and any complications during the treatment were recorded. Results:After 3 and 6 months of the treatment, all of the observations in both groups had improved significantly, but the improvements were significantly better among the treatment group, on average.Conclusions:Staged pressure therapy can more effectively inhibit scar hyperplasia after a deep hand burn. In conjunction with rehabilitation training, it better improves the mobility of the finger joints, hand muscle strength and the recovery of hand function. It is worthy of clinical promotion and application.
7.Research on the Prediction of the Pathological Grade of Invasive Lung Adenocarcinoma by the CT Signs Model of Pulmonary Nodules

Objective A binary Logistic regression model was developed to forecast the pathological grade of invasive adenocarcino-ma by utilizing the CT characteristics of lung nodules.Methods A retrospective analysis was conducted on the clinical data,pathological types,and imaging findings of 303 cases of ground-glass nodules diagnosed with postoperative pathological infiltrative adenocarcinoma at the First Affiliated Hospital of Henan Polytechnic University and the First Affiliated Hospital of Xinxiang Medical College from January 2021 to February 2023.Based on the pathological results,these lesions were categorized into two groups:the low-grade group(compri-sing 262 cases characterized by adherent,acinar,or papillary types as predominant forms of adenocarcinoma with no more than 20%high-grade pattern)and the high-grade group(consisting of 41 cases exhibiting any form of adenocarcinoma with over 20%high-grade components).The Mann-Whitney U test was employed to compare quantitative parameters between both groups,while qualitative parameters were analyzed using the x2 test.Additionally,binary Logistic regression models were utilized to identify independent predictors;further evaluation included area under curve(AUC)values,calibration curves,and decision analysis curves to assess model differentia-tion,calibration accuracy,and clinical applicability.Results Univariate analysis revealed that gender,air bronchial sign,vacuole sign,vascular cluster sign,pleural depression sign,long diameter,short diameter,and CT-enhanced net increment exhibited statistical signifi-cance(P<0.05),whereas location,burr sign,and solid component ratio did not demonstrate statistical significance(P>0.05).Binary Logistic regression analysis identified long diameter,CT-enhanced net increment,vascular cluster sign,pleural depression sign,and vacu-ole sign as independent predictors of the pathological grade model for invasive adenocarcinoma.The results of ROC curve analysis indicated that the AUC value of the Logistic regression model was 0.846 with a sensitivity of 81.25%and specificity of 86.52%.Conclusion The logistic regression model based on CT signs has excellent ability and stability in predicting the pathological grade of invasive adenocarcinoma.
8.A Case of Adult-onset Still's Disease Presented with Fever and Neutrophil-dominant Pleural Effusion

We report a case of adult-onset Still's disease(AOSD)presenting with fever,myalgia,and pleural effusion.The patient exhibited significantly elevated inflammatory markers and exudative pleural effusion with markedly elevated leukocyte count showing neutrophilic predominance.After comprehensive exclusion of infectious etiologies,the diagnosis of AOSD was established.Clinical symptoms resolved following anti-inflam-matory therapy with glucocorticoids and tocilizumab.This case highlights a critical clinical insight:while infec-tion is a common cause of elevated inflammatory markers,it is not an invariable etiology.In such presentations,meticulous clinical observation,thorough diagnostic screening,and systematic inference are es-sential for accurate diagnosis.
9.Effects of peiminine B on Streptococcus pneumoniae-induced alveolar epithelial cell injury and its mechanism

OBJECTIVE To investigate the effects of peiminine B (PEI) on Streptococcus pneumoniae (SP)-induced alveolar epithelial cell injury by regulating the Ras-related C3 botulinum toxin substrate 1 in nucleus accumbens (Rac1)/protein kinase B (Akt)/nuclear factor κB (NF-κB) signaling pathway. METHODS Human alveolar epithelial cells (HPAEpiC) were taken and randomly divided into the Control group, SP group (1×108 cfu/mL SP bacterial solution), low-, medium-, and high-concentration PEI groups (1×108 cfu/mL SP bacterial solution+0.05, 0.10, 0.20 mmol/L PEI), and high-concentration PEI+Akt activator group (P-H+SC79 group, 1×108 cfu/mL SP bacterial solution+0.20 mmol/L PEI+10 μmol/L SC79). Except for the Control group, the other groups of cells were treated with SP bacterial solution and/or corresponding drug solution. After 24 h of treatment, the levels of inflammatory factors (interleukin-6, -18, -1β) in the supernatant solution, the contents of oxidative stress indexes [lactate dehydrogenase (LDH), reactive oxygen species (ROS) and superoxide dismutase (SOD)], apoptosis rate, as well as the expressions of proliferation/apoptosis-related proteins [cyclin-dependent kinase 1 (CDK1), B cell lymphoma-2 related X protein (Bax)] and pathway-related proteins (Rac1, Akt, phosphorylated Akt, NF-κB and phosphorylated NF-κB) were detected in each group. RESULTS Compared with the Control group, the levels of inflammatory factors in supernatant solution, LDH and ROS contents, apoptosis rate, the protein expressions of Bax and Rac1 and the phosphorylation levels of Akt and NF-κB in the SP group were significantly increased or up-regulated, while SOD content and the protein expression of CDK1 were significantly decreased or down-regulated (P<0.05). Compared with the SP group, the above indexes in PEI groups were significantly improved in a concentration-dependent manner (P<0.05). SC79 could significantly reverse the improvement effect of the high concentration of PEI (P<0.05). CONCLUSIONS PEI can alleviate SP-induced inflammation and oxidative stress damage of alveolar epithelial cells and inhibit apoptosis, which may be achieved by inhibiting Rac1/Akt/NF-κB signaling pathway.
10.Retrospective analysis of adverse events associated with traditional Chinese medicine formula granules and decoction pieces in hospitalized patients using the global trigger tool

OBJECTIVE To provide technical support for improving recognition rate of adverse drug events (ADEs) related to traditional Chinese medicine (TCM) formula granules and decoction pieces among inpatient patients. METHODS By referencing the global trigger tool (GTT) whitepaper, literature on adverse reactions to TCM, and expert review opinions, ADE trigger items for TCM formula granules and decoction pieces used in the inpatients were established. GTT was applied to analyze ADEs in inpatients who had used TCM formula granules and decoction pieces in our hospital from August 2013 to August 2023, utilizing the Chinese Hospital Pharmacovigilance System. The effectiveness of GTT and the characteristics of these ADEs were analyzed. RESULTS A total of forty-eight triggers were established, including thirty-two laboratory test indexes, thirteen clinical symptoms, and three antidotes. Among the 1 682 patients included, GTT identified 652 potential ADEs, 284 true positive ADEs,with a trigger rate of 38.76% and a positive predictive value of 43.56%. After review by the auditor, 278 cases of ADEs were finally confirmed, with an incidence rate of 16.53%, significantly higher than the number of spontaneously reported ADEs during the same period (0). The 278 cases of ADEs were mostly grade 1 (223 cases), mainly involving hepatobiliary system, gastrointestinal system, blood- lymphatic system, etc;a total of 219 types of TCMs are involved,and the top five suspected TCMs used at a frequency higher than 1% were Poria cocos, Codonopsis pilosula, Atractylodes macrocephala, fried Glycyrrhiza uralensis, and Scutellaria baicalensis. CONCLUSIONS The established GTT can improve the recognition rate of ADEs for hospitalized patients using traditional Chinese medicine formula granules and decoction pieces.
