OBJECTIVE To compare the efficacy and safety of evolocumab and probucol in patients with ultra-high-risk atherosclerotic cardiovascular disease (ASCVD) following percutaneous coronary intervention (PCI). METHODS A retrospective analysis was conducted on 156 ultra-high-risk ASCVD patients who underwent PCI in our institution between January 1, 2023 and December 31, 2024. According to the lipid-lowering regimen, the patients were categorized into evolocumab group ( n =86) and probucol group ( n =70). Changes in lipid parameters [total cholesterol (TC), low-density lipoprot ein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), triglycerides, lipoprotein (a), and lipid goal achievement rate ] , inflammatory markers [interleukin-6 (IL-6) and C-reactive protein (CRP) ] , and cardiac function indices (left ventricular ejection fraction, left ventricular end-systolic diameter, left ventricular end-diastolic diameter, and N-terminal pro-B-type natriuretic peptide) were compared between two groups at baseline and after 6 months of treatment. The incidence of adverse clinical events during treatment, including acute myocardial infarction, in-stent restenosis, acute heart failure, cerebral hemorrhage, and stroke, was also evaluated. RESULTS No statistically significant differences were observed between the two groups at baseline ( P >0.05). After 6 months of treatment, both groups demonstrated significant improvements in lipid profiles (except HDL-C) and inflammatory markers compared to those at baseline ( P <0.05). The evolocumab group exhibited greater reductions in TC, LDL-C, IL-6, and CRP, along with a higher lipid target achievement rate, compared with the probucol group ( P <0.05). There were no statistically significant differences in the cardiac function-related indicators before and after treatment between the two groups, nor in the incidence of adverse events during the treatment ( P >0.05). CONCLUSIONS For ultra-high-risk ASCVD patients after PCI, both of the above treatment options are associated with improvements in blood lipid and inflammatory response, with good safety during short-term follow-up. Evolocumab shows superior efficacy in TC, LDL-C and inflammatory markers reduction and lipid target achievement, compared to probucol.

Objective:To evaluate the clinical efficacy of a novel surgical approach of debridement, antibiotics, and implant retention (DAIR) with flap transfer, for treating chronic implant infections in bone tumor patients. Methods:A retrospective review was conducted on nine consecutive patients [6 males, 3 females; median age 35(27, 51) years, range 9-71] who underwent a modified procedure of DAIR plus flap transfer between November 2022 and January 2024. The cohort included six cases of chronic periprosthetic joint infection and three cases of chronic plate and screw infection. Tumor diagnoses included seven primary malignant tumors (osteosarcoma=5, undifferentiated pleomorphic sarcoma of bone=1, synovial sarcoma=1) and two bone metastasis of renal cell carcinoma. The procedure involved wide, radical debridement, meticulous removal of biofilm from implants and surrounding soft tissue, followed by the transfer of a well vascularized musculocutaneous flap to fully envelope the contaminated interface. Pre-operative clinicopathological data, surgical details, postoperative complications and infection recurrence were analyzed. Results:The median interval between initial implantation and debridement was 10.0(3.3, 14.8) months. Median follow-up after debridement was 15.9(15.4, 18.2) months. All nine surgeries were completed as planned: six musculocutaneous flaps, two fasciocutaneous flaps and one muscle-only flap. Implants were preserved in six patients; two required subsequent removal for recurrent infection, and one patient later underwent amputation for tumor recurrence. Infection-free implant survival at 3, 6 and 12 months was 88.9%, 87.5% and 87.5%, respectively. Major complications included one donor-site hematoma, one donor-site sensory deficit and one wound healing delay. All the complications were well management. Both reinfections occurred in proximal tibial prostheses, likely due to limited flap coverage options and local anatomical constraints. Conclusion:Although reinfections happened in two cases DAIR with flap transfer provides promising short-term infection control in patients with chronic implant-associated infections following bone tumor surgery.

[Objective]To develop a classification model based on complete blood count(CBC)parameters combined with clinical factors to predict severe respiratory infections caused by Human adenovirus(HAdV)in pediatric patients.[Methods]From September 2023 to September 2024,the CBC parameters and related clinical data from pediatric patients diagnosed with HAdV infection were collected.Principal component analysis and random forest models were used to identify potential predictors of severe cases.[Results]A total of 668 pediatric patients were included, with 564 cases assigned to the training cohort and 104 cases to the validation cohort.Severe cases were defined as pneumonia and/or fever lasting≥5 days(pneumonia or prolonged fever,PorPF).Principal component analysis and feature importance analysis(Mean Decrease Gini value)identified the monocytosis ratio(PMono),red blood cell count(RBC),and platelet count(PLT)as the most critical CBC parameters.Logistic regression analysis revealed that oxygen therapy(OR=4.367,95%CI:1.568-12.161)and increased work of breathing(OR=3.904,95%CI:2.146-7.101)were relative risk factors for PorPF.Meanwhile, higher PMono(OR=0.696,95%CI:0.640-0.757),RBC(OR=0.201,95%CI:0.124-0.325),and PLT(OR=0.990,95%CI:0.987-0.994)were protective factors. When PMono was used as a predictive marker for PorPF,the area under the receiver operating characteristic curve(AUC)was 0.648 and 0.705,respectively.A random forest model incorporating four risk factors[PMono,RBC,PLT,and hematocrit(HCT)]was constructed to classify PorPF and general cases, achieving AUCs of 0.688 and 0.768,respectively.[Conclusions]PMono,RBC,and PLT may serve as characteristic CBC indicators for predicting pneumonia or prolonged fever in children with HAdV infection.A risk factor model built using PMono,RBC,PLT,and HCT offers a relatively simple and accurate approach to predicting severe cases in pediatric HAdV infections.

Chinese Journal of General Practitioners 2024;23(4):354-360
Objective:To investigate the health status and needs of the occupational population in an industrial park in Shanghai, and to explore the causes. Methods:The study was a mixed method study. First, a list of health needs of the occupational population was developed through literature review. Then, a questionnaire was prepared on the basis of the list of health needs of the occupational population, which consisted of 4 parts: basic personal information, health status, medical care, and health needs. From September 16 to October 9, 2022, 3 enterprises of representative size and industry in Shanghai Xinzhuang Industrial Park were selected by convenience sampling method, and the target sample size was allocated in equal proportions according to the total number of employees, and the Quick Response code of the electronic questionnaire was distributed (Questionnaire Star). Finally, an interview outline was created based on the results of the questionnaire survey to gain a deeper understanding of the connotations and reasons for the health needs of employees, etc. The interviews were conducted from October 15 to 22, 2022.Results:A total of 819 enterprise employees were surveyed, of whom 580 (70.82%) were male, and their age was mainly concentrated in the 19-44 age group (621(75.82%)). Among them, 348 (42.49%) were overweight or obese, 388 (47.37%) had regular medical check-ups, and only 224 (27.35%) had active health interventions. The top 5 health needs of the surveyed population were relief of visual fatigue (365(44.57%)), improvement of sleep (355(43.35%)), relief of physical fatigue (343(41.88%)), oral health (333(40.66%)), and immunity enhancement (332(40.54%)). The interviewees were from 2 large enterprises, totaling 12 people. The results of the interviews showed that excessive use of mobile phones, high stress, interpersonal and social confusion, retaliatory late nights, sedentary lifestyles, and socializing were the main causes of health problems. Conclusions:The health status of the occupational population in Shanghai Xinzhuang Industrial Parks is not optimistic, and their health needs are obvious. Their health problems and needs are related to the characteristics of their work patterns.

Chinese Journal of Information on Traditional Chinese Medicine 2024;31(6):167-170
"Xuanfu",as the microscopic structure in TCM theory of visceral manifestations, spreads all over the human body. The microscopic pathological changes of acute ulcerative colitis are manifested as cryptic abscesses or even structural disorders of the colon. Its damp heat accumulates in the large intestine, and the pathogenic characteristics of the large intestine coincides with the concept of"Xuanfu"that likes opening and avoiding closing. The functions of the mechanical barrier and mucus barrier in the intestinal crypt are very similar to the characteristics of the"Xuanfu",which can provide morphological basis for the"Xuanfu"theory of the intestine. The method of clearing away and opening the"Xuanfu"can inhibit the"Xuanfu closed"state of the intestine caused by the accumulation of damp and heat, and promote the guiding role and theoretical support of the"Xuanfu-crypt"theory in the treatment of early crypt lesions of ulcerative colitis.
