1.Role of Intestinal Microbiota in "Correspondence Between Prescription and Syndrome"

"Correspondence between prescription and syndrome" is the foundation of the objectification of traditional Chinese medicine diagnosis and treatment, and the objectification of traditional Chinese medicine diagnosis is the key to the modernization of traditional Chinese medicine. Combining modern theories and technological means to explore objective indicators or biomarkers of "correspondence between prescription and syndrome" can help clarify quantitative indicators of syndromes. There is a characteristic microbial population in the intestine that is related to syndromes and prescription efficacy. Intestinal microbiota plays an important indicative and discriminative role in determining the formation and evolution of disease patterns and is also an important characterization and pathway of the therapeutic effect of prescriptions. Therefore, intestinal microbiota can serve as an important biological indicator for the objectification of "correspondence between prescription and syndrome", and the research content related to "intestinal microbiota-syndrome" and "intestinal microbiota-prescription" is relatively mature. On this basis, the team proposed a research approach and method of "correspondence among intestinal microbiota, syndrome, and prescription", exploring the biological mechanism of "correspondence between prescription and syndrome" from the perspective of intestinal microbiota. A complete analysis of the "intestinal microbiota-syndrome-prescription" data content of six traditional Chinese medicine diarrhea syndromes was conducted, and the results of intestinal dominant and characteristic microbiota for different syndromes of diarrhea and corresponding formula interventions were obtained. The correlation of key intestinal microbiota and metabolites with the interaction between different syndromes and formula interventions was studied, and the microbiological mechanism of "treating the same disease with different methods" for diarrhea was explored. The research ideas and results provide the possibility for the establishment and application of the theory of "correspondence among intestinal microbiota, syndrome, and prescription" and also provide a research paradigm for other diseases in traditional Chinese medicine. "Correspondence among intestinal microbiota, syndrome, and prescription" is conducive to revealing the micro-mechanisms of the formation and evolution of traditional Chinese medicine syndromes and the efficacy of prescriptions. It establishes an objective diagnosis and treatment system for traditional Chinese medicine syndromes based on intestinal microbiota, providing new methods and ideas for the objective diagnosis and treatment of traditional Chinese medicine syndromes and promoting the modernization and internationalization of traditional Chinese medicine.
2.Molecular characterization of newly emerging goose astrovirus variants and novel strains prevailed in part regions of China in 2022

Goose astrovirus(GAstV)has become one of the most important pathogens endangering the goose farming industry in China.To discover the genomic information of prevalent strains in China in 2022 and reveal their biological characteristics,the whole genomes of 9 strains of GAstV-1 and 12 strains of GAstV-2 isolated from parts of China in 2022 were sequenced by the Chromo-some Walking and analyzed by bioinformatics software and websites.The analysis results of gene structure showed that the sizes of the coding genes ranged from 6 977 to 7 215 bp.The open read-ing frame 1(ORF1)of all strains contained the characteristic motifs of serine protease,nuclear lo-calization signal(NLS)and RNA-dependent RNA polymerase(RdRp).The ribosomal frameshift signals(RFS)were all in the form of stem-loop structures.However,the numbers of stem and loop nucleotides in GAstV-1 were"14+11"configuration,while those in GAstV-2 were"12+14"configuration,and the loop of GAstV-2 was in the"8+6"double-loop configuration.The results of homological analysis for ORF1b showed that the homology of the GAstV-1 isolats compared with the representative strains of avian astrovirus types 1,2,and 3(AAstV-1,AAstV-2,AAstV-3)ranged from 7%to 64%,and the homology of the GAstV-2 isolates ranged from 8%to 68%.The average genetic distances of ORF2 were 0.9,1.2,and 0.9 respectively compared with the represent-ative strains of AAstV-1,AAstV-2 and AAstV-3.While for the GAstV-2 isolates,the highest ho-mologies were 68%and 56%,the lowest homologies were 8%and 36%respectively.And the aver-age genetic distances of ORF2 were 1.0,1.2,and 0.5 respectively.B-cell-conformational epitopes screening results of ORF2 showed that,there were four common epitopes in the GAstV Group 1 i-solates,namely PRE,LALQSQSVNTFA,AAG and YQQVTSDQSI except for N-145,N-287 and N-314 in the two strains G1FJ267 and G1JS277.And there were seven common conformational epitopes in the GAstV-2 isolates,namely NQE,RAN,GPE,PRQ,TRAQ,SNS,and AVPPNTPL except for N-83,N-136,N-331 and N-351 in the strain G2FJ283-3B.The above results indicated that GAstV maybe belong to a new type of AAstV because of the difference between the clinical i-solates and the known strains of AAstV.And there was pathogenic diversity among the isolates.All the isolates of GAstV-1 and GAstV-2 belong to two different serotypes,and the possible serologi-cal subtypes among the isolates of GAstV-1 or GAstV-2 are remained to be further identified by serological tests.

Periodontal bone defects, primarily caused by periodontitis, are highly prevalent in clinical settings and manifest as bone fenestration, dehiscence, or attachment loss, presenting a significant challenge to oral health. In regenerative medicine, harnessing developmental principles for tissue repair offers promising therapeutic potential. Of particular interest is the condensation of progenitor cells, an essential event in organogenesis that has inspired clinically effective cell aggregation approaches in dental regeneration. However, the precise cellular coordination mechanisms during condensation and regeneration remain elusive. Here, taking the tooth as a model organ, we employed single-cell RNA sequencing to dissect the cellular composition and heterogeneity of human dental follicle and dental papilla, revealing a distinct Platelet-derived growth factor receptor alpha (PDGFRA) mesenchymal stem/stromal cell (MSC) population with remarkable odontogenic potential. Interestingly, a reciprocal paracrine interaction between PDGFRA+ dental follicle stem cells (DFSCs) and CD31+ Endomucin+ endothelial cells (ECs) was mediated by Vascular endothelial growth factor A (VEGFA) and Platelet-derived growth factor subunit BB (PDGFBB). This crosstalk not only maintains the functionality of PDGFRA+ DFSCs but also drives specialized angiogenesis. In vivo periodontal bone regeneration experiments further reveal that communication between PDGFRA+ DFSC aggregates and recipient ECs is essential for effective angiogenic-osteogenic coupling and rapid tissue repair. Collectively, our results unravel the importance of MSC-EC crosstalk mediated by the VEGFA and PDGFBB-PDGFRA reciprocal signaling in orchestrating angiogenesis and osteogenesis. These findings not only establish a framework for deciphering and promoting periodontal bone regeneration in potential clinical applications but also offer insights for future therapeutic strategies in dental or broader regenerative medicine.

5.Single-cell level analysis of the immune microenvironment characteristics of dental follicle tissues from the human third molars

Objective:To analyze the immune microenvironment characteristics of human dental follicle tissues from the third molars and to explore the mutual communication and the effects of innate immune cells and adaptive immune cells within the dental follicle.Methods:Sequencing data(GSA-Human:HRA008022)in the GSA database were analyzed.Bioinformatics tools were employed for gene identification and GO enrichment analysis was performed to define the biological function of innate and adaptive immune cells.CellChat analysis was used for explaining intercellular communication among immune cell populations.Results:Using t-SNE dimen-sionality reduction analysis for immune cell populations,innate immune cell populations were obtained,including innate lymphoid cells,dendritic cells,mast cells and macrophages,and adaptive immune cell populations including T cells and B cells.Pearson corre-lation analysis showed that innate immune cells,specifically innate lymphoid cells and macrophages,had a strong correlation with adap-tive immune cell populations.GO enrichment analysis revealed mutual coordination among innate immune cell populations and regulato-ry effects on adaptive immune cell populations.Further CellChat analysis indicated biological signal transmission between innate and a-daptive immune cell populations,with CLEC,MIF,ADGRE5,COLLAGEN and MIF signaling pathways is the most significant.Con-clusion:Dental follicle tissues are rich in immune cells and innate immune cell populations interact with adaptive immune cells to regulate immune responses and participate in maintaining the homeostasis of dental follicle.
6.Clinical analysis of extubation failure in critically ill patients with tracheotomy

Objective To analyze the clinical features of extubation failure in critically ill patients with tracheotomy in order to improve the success rate of tracheal extubation.Methods The clinical data of 52 critically ill patients with tracheotomy admitted to the Department of Emergency Internal Medicine of Chizhou People's Hospital of Anhui Province from January 2014 to January 2017 were retrospectively analyzed; there were 14 cases in failed extubation group (extubation failure: unable to plug the pipe, necessary to be cut or intubated again), and 38 cases were in successful extubation group (success criteria: after trying plugging the pipe for 24 - 48 hours, pulling out sleeving tube of tracheal incision, the incision healed well, within 3 months no dyspnea or pulmonary infection necessary to again performing tracheal intubation or incision). The differences in gender, age, within 3 days before plugging the pipe the consciousness, cough condition, the levels of serum albumin (Alb), serum potassium and serum sodium, brain natriuretic peptide (BNP), hemoglobin (Hb) were compared between the two groups, the clinical characteristics of extubation failure were analyzed, and the descriptions and their numbers for all diseases resulting in extubation failure were carried out.Results The number of conscious patients (24 cases vs. 4 cases), and the number of patients with good cough function (38 cases vs. 3 cases) were obviously more in successful extubation group than those in the failed extubation group, and the levels of serum Alb (g/L: 35.13±3.13 vs. 27.50±5.53), Hb (g/L: 112.18±13.62 vs. 94.14±17.03) in successful extubation group were significantly higher than those in failed extubation group (allP< 0.05). BNP was significantly lower in successful extubation group than that in failed extubation group (ng/L: 344.03±152.85 vs. 5887.80±2695.97,P < 0.05). The common diseases in extubation failure group were as follows: stroke in 7 cases, special severe craniocerebral injury 3 cases, respiratory failure 3 cases, after cardiac pulmonary resuscitation (CPR) operation 1 case.Conclusions The patients are fully assessed before extubation, countermeasures are taken as soon as possible in cases with coma, cough capability poor, hypoproteinemia, heart dysfunction and anemia in order to improve the successful rate of extubation.
7.Ten-year trends of mortality from cerebrovascular diseases in two resident areas in Henan

The cluster sampling method was used and a retrospective survey on mortality trends of cerebrovascular diseases from 1989 to 2008 was conducted among residents from Erqi District in Zhengzhou city and Xigong District in Luoyang city. The average mortality rate of cerebrovascular diseases in these two districts was 69. 5/100 000 in 1989 to 2008. The standardized mortality rate for men was 118. 67/100 000 in 1989 to 1993, and dropped to 44. 23/100 000 in 2004 to 2008. The standardized mortality rate for women was 68.21/100 000 in 1989 to 1993, and dropped to 30. 2/100 000 in 2004 to 2008. The declining trends of cerebrovascular disease mortality rates might be related to early diagnosis and early treatment of the disease, and the extensive health education and prevention programs.
8.Gender difference of plasma lactate levels in patients with type 2 diabetes and the effect of metformin

Objective To investigate the gender difference of the plasma lactic acid(LA) levels in type 2 diabetics with normal renal and hepatic function, and the effect of metformin on LA levels in the difference gender. Methods A total of 1 021 type 2 diabetic inpatients with normal renal and hepatic functions were collected,including metformin treatment group (213 males and 210 females) and metformin non-treatment group (299 males and 299 females). LA was measured with enzyme-electrode assay. Fasting plasma glucose ( FPG), creatinine ( Cr), and alanine aminotransferase ( ALT) levels were determined. Results LA level in metformin treatment group was significantly higher than that in metformin non-treatment group [ (1.32±0.53 vs 1.14±0.49) mmol/L,P<0.01],and 61 cases had hyperlactacidemia but no lactic acidosis was found. Spearman correlation analysis showed that LA level was positively associated with gender,metformin, and body mass index( BMI) apart from Cr and ALT( P<0.01). Multivariate logistic regression analysis showed that gender,Cr,ALT,and metformin were independent correlated factors of hyperlactacidemia. LA levels in females were higher than those of males in the whole group and two groups treated or not treated with metformin (all P<0. 05 ). LA levels in females were higher compared to male in Cr and ALT subgroups,as well as age subgroups,especially with age younger than 45 years old (P=0.021). Conclusions There is gender difference of lactate level in diabetic patients,and the effect of metformin on the plasma lactate levels of different gender is varied. The plasma LA level in females,especially those approaching menopause,are prone to hoist.
9.Association of plasma creatinine with lactic acid level in type 2 diabetic patients with normal renal function

Objective To study the relationship between plasma creatinine and lactic acid levels in type 2 diabetic patients with normal renal function, and the effect of mefformiu administration on plasma lactic acid. Methods Blood samples were collected from 723 type 2 diabetic inpatients(275 cases treated with mefformin as treatment group and 448 without mefformin as control group). Plasma lactic acid was measured with enzyme-electrode assay. The fasting plasma glucose, fasting insulin, creatinine (Cr) , alanine aminotransferase (ALT), and HbA1C levels were also determined. Results (1) The lactic acid level in treatment group was significantly higher than that in control group [(1.33±0.57 vs 1.17±0.47) mmol/L, P<0.01] and so was the incidence of hyperlactacidemia (9.45% vs 4.91%, P<0.01). No lactic acidosis was found in all patients. (2) The correlation analysis showed that Cr, blood urea nitrogen, ALT and body mass index (BMI) were positively correlated with lactic acid in control group, and the positive correlation between Cr and lactic acid still existed (r= 0.345, P<0.01) after adjustment for ALT and BMI. (3) After being divided by 20 μmol/L Cr in control group, lactic acid levels in subgroup increased with the Cr levels, especially when Cr was over 90 μmol/L. The Cr cutpoint for predicting lactatemia was 95.35 μmol/L. Conclusion It is safe to administrate metformin in type 2 diabetic patients with normal renal function, along with low risk of lactic acidemia. The incidence of lactic acidemia may increase when plasma Cr level reaches 95.35 μmol/L,
