1.Effect of Bazhen Lihe Kangxian prescription on rats with liver fibrosis induced by carbon tetrachloride

ObjectiveTo investigate the therapeutic effect of Bazhen Lihe Kangxian prescription on rats with liver fibrosis induced by carbon tetrachloride (CCl4), and to explore its mechanism of action against liver fibrosis. MethodsA total of 50 male Sprague-Dawley rats were randomly divided into blank group, model group, silybin group (43.19 mg/kg), and low-, middle-, and high-dose Bazhen Lihe Kangxian prescription groups (7.96, 15.93, and 31.86 g/kg, respectively). Subcutaneous injection of 40% CCl4-olive oil solution (twice a week for 8 consecutive weeks) was performed to establish a model of liver fibrosis. After successful modeling, the rats in the blank group and the model group were given normal saline by gavage, while those in the treatment groups were given the corresponding drug by gavage, once a day for 4 consecutive weeks. HE staining and Masson staining were used to observe liver histopathological changes; an automatic biochemical analyzer was used to measure the serum levels of albumin (Alb), aspartate aminotransferase (AST), and alanine aminotransferase (ALT); ELISA was used to measure the serum levels of hyaluronic acid (HA), collagen Ⅳ (Col-Ⅳ), procollagen Ⅲ N-terminal peptide (PⅢNP), and laminin (LN); Western blotting was used to measure the protein expression levels of phosphoinositide 3-kinase (PI3K), phosphorylated PI3K (p-PI3K), protein kinase B (Akt), phosphorylated Akt (p-Akt), mammalian target of rapamycin (mTOR), and phosphorylated mTOR (p-mTOR) in liver tissue; RT-qPCR was used to measure the mRNA expression levels of collagen Ⅰ (Col-Ⅰ) and α-smooth muscle actin (α-SMA) in liver tissue. A one-way analysis of variance was used for comparison of continuous data between multiple groups, and the least significant difference t-test was used for further comparison between two groups. ResultsCompared with the blank group, the model group had disrupted hepatic lobular structure, disordered arrangement of hepatic cell cords, hepatocyte fatty degeneration, severe inflammatory cell infiltration, and deposition of massive blue collagen fibers, as well as significant increases in the levels of AST, ALT, HA, LN, Col‑Ⅳ, and PⅢNP and a significant reduction in the level of Alb (all P<0.01). Compared with the model group, the silybin group and the three Bazhen Lihe Kangxian prescription groups had varying degrees of improvement in inflammatory cell infiltration, hepatocyte fatty degeneration, and collagen deposition in liver tissue, as well as significant reductions in the levels of AST, ALT, HA, LN, Col‑Ⅳ, and PⅢNP and a significant increase in the level of Alb (all P<0.01). Compared with the blank group, the model group had significant increases in the protein expression levels of p‑PI3K/PI3K, p-Akt/Akt, and p‑mTOR/mTOR and the mRNA expression levels of Col-I and α-SMA in liver tissue (all P<0.01); compared with the model group, the silybin group and the high- and middle-dose Bazhen Lihe Kangxian prescription groups had significant reductions in the protein expression levels of p-PI3K/PI3K (all P<0.01), and the silybin group and the three Bazhen Lihe Kangxian prescription groups had significant reductions in the protein expression levels of p-Akt/Akt and p-mTOR/mTOR and the mRNA expression levels of Col-Ⅰ and α-SMA in liver tissue (all P<0.01). ConclusionBazhen Lihe Kangxian prescription can improve CCl4-induced liver fibrosis in rats and alleviate liver injury, possibly by inhibiting the PI3K/Akt/mTOR pathway.
2.Effect of modified Baduanjin exercise on cardiopulmonary function, motor function and activities of daily living for stroke patients

ObjectiveTo investigate the effect of modified Baduanjin exercise, as an rehabilitation exercise, on cardiopulmonary function, motor function and activities of daily living in patients with stroke. MethodsFrom January to September, 2023, 42 stroke patients in the Nanjing Qixia District Hospital were randomly divided into control group (n = 21) and experimental group (n = 21). The control group received routine rehabilitation, and the experimental group received modified Baduanjin exercise in addition, for four weeks. They were assessed with peak oxygen uptake (VO2peak), anaerobic threshold (AT), peak oxygen pulse (VO2peak/HR), forced vital capacity (FVC), forced expiratory volume in the first second (FEV1), peak expiratory flow (PEF), Fugl-Meyer Assessment-upper extremities (FMA-UE), Berg Balance Scale (BBS) and modified Barthel Index (MBI) before and after intervention. ResultsVO2peak, AT, and the scores of FMA-UE, BBS and MBI improved in the control group after intervention (|t| > 2.256, |Z| > 2.936, P < 0.05); while VO2peak, AT, VO2peak/HR, FVC, FEV1, PEF, and the scores of FMA-UE, BBS and MBI improved in the experimental group (|t| > 4.390, |Z| > 3.451, P < 0.001); and all the indexes were better in the experimental group than in the control group (|t| > 4.136,|Z| > 2.751,P < 0.01), except the scores of BBS and MBI. ConclusionModified Baduanjin exercise can improve the cardiopulmonary function and upper limb motor function for stroke patients.
3.Effect of galvanic vestibular stimulation on stroke patients with lateropulsion

ObjectiveTo investigate the effect of galvanic vestibular stimulation on stroke patients with lateropulsion. MethodsFrom February to October, 2023, 30 stroke patients with lateropulsion in the First Affiliated Hospital of Nanjing Medical University were divided into control group (n = 15) and experimental group (n = 15) randomly. The control group received routine rehabilitation and sham galvanic vestibular stimulation, and the experimental group received routine rehabilitation and true galvanic vestibular stimulation, for two weeks. They were assessed with Scale for Contraversive Pushing (SCP), subjective visual vertical (SVV), Line Cancellation Test (LCT), Star Cancellation Test (SCT), Berg Balance Scale (BBS), Holden Functional Ambulation Category (FAC) and Barthel Index (BI) before and after treatment. ResultsAll the indexes improved in both groups after intervention (|t| > 2.461, Z > 3.000, P < 0.05), except the SVV orientation, SVV certainty and SCT in the control group; while the SCP, SVV certainty, LCT and FAC were better in the experimental group than in the control group (|t| > 2.189, Z = -2.862, P < 0.05), and the differences before and after intervention were better in the experimental group than in the control group (|t| > 2.382, P < 0.05), except LCT. SCP was correlated with SVV orientation, SVV certainty, SCT, BBS, BI and FAC (|r| > 0.381, P < 0.05). ConclusionGalvanic vestibular stimulation can improve the lateropulsion, balance, walking function and activities of daily living in stroke patients, which may be related to improvement for spatial cognitive function, especially vertical perception.
4.Nosocomial Infection in ICU

OBJECTIVE To investigate the site of nosocomial infection in ICU,distribution and resistance of bacteria in order to make the intervention strategy.METHODS Antimicrobial susceptibility of pathogenic bacteria isolated from nosocomial infection patients in ICU from Jul 2006 to Jul 2008 was performed by Kirby-Bauer method.RESULTS The main pathogens of nosocomial infection in ICU were Gram-negative organisms(48.09%),Gram-positives(38.93%),and fungi(12.98%).The most common pathogens were Staphylococcus aureus,Acinetobacter baumannii,Pseudomonas aeruginosa,fungi,and S.haemolyticus.All strains of S.aureus and S.haemolyticas were antimicrobial sensitive to vancomycin,teicoplanin and linezolid.All A.baumannii strains were antimicrobial sensitive to cefoperazone/sulbactam.They were resistant to other antimicrobial agents.CONCLUSIONS Gram-negative organisms are the main pathogenic bacteria of nosocomial infection in ICU,but the percentage of Gram-positives and fungi is increasing,S.aureus is the most main pathogenic bacterium of nosocomial infection in ICU.S.haemolyticus is also a main pathogenic bacterium.The pathogenic bacteria of nosocomial infection in ICU are highly resistant to the most antimicrobial agents.
