1.Analyses of serotypes and antibiotic resistance of Salmonella in diarrheal diseases in Baoshan District of Shanghai from 2023 to 2024

ObjectiveTo retrospectively analyze the correlation among serotypes, antimicrobial resistance phenotypes, and resistance genotypes of Salmonella isolates from diarrheal cases in Baoshan District of Shanghai from 2023 to 2024, and to provide a reference for the prevention and control of Salmonella infections and the rational use of antibiotics in clinical practice. MethodsSalmonella isolates collected from diarrheal cases under surveillance in Baoshan District from 2023 to 2024 were serotyped. Antimicrobial resistance phenotypes were determined using the broth microdilution method, and whole-genome sequencing was performed to identify resistance genes. Positive predictive value and Kappa were calculated to evaluate the agreement between phenotypic and genotypic resistance profiles of Salmonella. ResultsA total of 64 Salmonella isolates belonged to 17 serotypes, with the predominant ones being Salmonella Typhimurium (25.00%) and Salmonella Enteritidis (18.75%). The tested strains exhibited high resistance rates to ampicillin (60.94%), streptomycin (59.38%), and ampicillin/sulbactam (45.31%). All isolates remained susceptible to ceftiofur and ceftazidime/avibactam. Forty different resistance profiles were identified, and 39 isolates accounting for 60.94%, were multidrug-resistant. Overall, 80 resistance genes belonging to 13 categories were detected, with the most prevalent being blaTEM-1 (57.81%), aac(6')⁃Iy (54.69%), and aph(6)⁃Id (46.88%). No carbapenem or polymyxin resistance genes were found. The types and numbers of resistance genes varied significantly across serotypes. A high concordance was observed between genotype and phenotype for penicillins (positive predictive value: 94.59%, Kappa: 0.81), cephalosporins (90.91%, 0.46), cephamycins (100.00%, 0.65), macrolides (77.78%, 0.86), tetracyclines (100.00%, 0.93), and chloramphenicol (95.83%, 0.93). ConclusionSalmonella isolates from diarrheal diseases in Baoshan District exhibit high antimicrobial resistance. Whole-genome sequencing provides valuable support for resistance surveillance, however, it still needs to be integrated with phenotypic susceptibility testing for comprehensive assessment. Further studies are warranted to elucidate the mechanisms and transmission patterns of resistance in Salmonella.
2.Mechanism of Feiyanning Formula in Suppressing Invasion and Metastasis of Lung Cancer Cells via Modulating Exosomal miR-328 and STAT3/c-Fos/NFATc1 Pathway

ObjectiveTo investigate the molecular mechanism by which Feiyanning formula(FYN) regulated exosomal miR-328 to suppress the invasion and metastasis of lung cancer. MethodsIn vitro models using low-metastatic 95C and high-metastatic 95D lung cancer cell lines, along with isolated exosomes, were established. Initially, 95C and 95D cells were cultured, and exosomes were isolated from their supernatants. The exosomes were identified using transmission electron microscopy (TEM), nanoparticle tracking analysis (NTA), and Western blot. Exosome uptake assays confirmed that both 95C and 95D cells could internalize exosomes derived from lung cancer patients' plasma. Cell counting kit-8 (CCK-8) assays were performed to assess the inhibitory effects of FYN (0, 0.1, 0.2, 0.4, 0.8, 1.6, 3.2, 6.4 g·L-1) on 95C and 95D cell proliferation. Subsequent experiments were divided into three groups of control, low-dose FYN (1 g·L-1), and high-dose FYN (2 g·L-1) groups. Wound-healing and Transwell assays were used to evaluate the inhibitory effects of FYN on the migration and invasion of 95C and 95D cells. Western blot was employed to detect the protein expression levels of Janus kinase 2 (JAK2), signal transducer and activator of transcription 3 (STAT3), cellular oncogene Fos (c-Fos), and nuclear factor of activated T cells 1 (NFATc1). The mRNA expression of miR-328 was measured by Real-time quantitative polymerase chain reaction (Real-time PCR). ResultsLung cancer plasma-derived exosomes (EXO) enhanced the migration, invasion, and miR-328 expression levels in 95C and 95D cells (P<0.05, P<0.01). FYN inhibited the proliferation, invasion, and metastasis of 95C and 95D cells in a dose-dependent manner (P<0.01), with more pronounced effects on 95C cells. Additionally, FYN downregulated exosomal miR-328 expression and suppressed the protein levels of JAK2, STAT3, and its downstream target genes c-Fos and NFATc1 (P<0.01). ConclusionFYN may inhibit lung cancer metastasis by downregulating exosomal miR-328 expression, potentially through the STAT3/c-Fos/NFATc1 signaling pathway. Further studies are warranted to elucidate its detailed mechanisms and facilitate clinical application, providing insights into traditional Chinese medicine-based strategies for lung cancer treatment.
3.Difference of compensatory mechanisms in bilateral knee osteoarthritis patients of varying severity.

OBJECTIVE:
To investigate the load distribution on the more painful and less painful limbs in patients with mild-to-moderate and severe bilateral knee osteoarthritis (KOA) and explore the compensatory mechanisms in both limbs among bilateral KOA patients with different severity levels.
METHODS:
A total of 113 participants were enrolled between July 2022 and September 2023. This cohort comprised 43 patients with mild-to-moderate bilateral KOA (Kellgren-Lawrence grade 2-3), 43 patients with severe bilateral KOA (Kellgren-Lawrence grade 4), and 27 healthy volunteers (healthy control group). The visual analogue scale (VAS) score for pain, the Hospital for Special Surgery (HSS) score, passive knee range of motion (ROM), and hip-knee-ankle angle (HKA) were used to assess walking pain intensity, joint function, and lower limb alignment in KOA patients, respectively. Motion trajectories of reflective markers and ground reaction force data during walking were captured using a gait analysis system. Musculoskeletal modeling was then employed to calculate biomechanical parameters, including the peak knee adduction moment (KAM), KAM impulse, peak joint contact force (JCF), and peak medial/lateral contact forces (MCF/LCF). Statistical analyses were performed to compare differences in clinical and gait parameters between bilateral limbs. Additionally, one-dimensional statistical parametric mapping was utilized to analyze temporal gait data.
RESULTS:
Mild-to-moderate KOA patients showed the significantly higher HSS score (67.7±7.9) than severe KOA patients (51.9±8.9; t=8.747, P<0.001). The more painful limb in all KOA patients exhibited significantly greater HKA and higher VAS scores compared to the less painful limb ( P<0.05). While bilateral knee ROM did not differ significantly in mild-to-moderate KOA patients ( P>0.05), the severe KOA patients had significantly reduced ROM in the more painful limb versus the less painful limb ( P<0.05). Healthy controls showed no significant bilateral difference in any biomechanical parameters ( P>0.05). All KOA patients demonstrated longer stance time on the less painful limb ( P<0.05). Critically, severe KOA patients exhibited significantly higher peak KAM, KAM impulse, and peak MCF in the more painful limb ( P<0.05), while mild-to-moderate KOA patients showed the opposite pattern with lower peak KAM and KAM impulse in the more painful limb ( P<0.05) and a similar trend for peak MCF.
CONCLUSION
Patients with mild-to-moderate KOA effectively reduce load on the more painful limb through compensatory mechanisms in the less painful limb. Conversely, severe bilateral varus deformities in advanced KOA patients nullify compensatory capacity in the less painful limb, paradoxically increasing load on the more painful limb. This dichotomy necessitates personalized management strategies tailored to disease severity.

4.Etiological characteristics of two strains of human-origin Escherichia albertii in Baoshan District, Shanghai

ObjectiveTo analyze the etiological characteristics of two strains of Escherichia albertii first identified in Baoshan District, Shanghai, in 2024, to explore their antibiotic resistance characteristics, virulence characteristics, and multilocus sequence typing (MLST) to elucidate their genetic evolutionary relationships, so as to provide a scientific basis for the surveillance and control of Escherichia albertii in the local area. MethodsA total of 457 anal swabs were collected from patients with diarrhea in 2024, from which two suspected Escherichia albertii strains were isolated. The isolated strains were identified through mass spectrograph analysis, biochemical identification, and polymerase chain reaction (PCR) test. Microdilution broth method was used for the antimicrobial susceptibility testing, and whole-genome sequencing was performed for molecular characteristic analysis. ResultsMass spectrograph analysis and PCR test initially identified the two strains as Escherichia coli (E. coli), but their biochemical profiles were inconsistent with those of E. coli. Genomic sequencing identified the strains as Escherichia albertii, and which were named as S1 and S2, respectively. The antimicrobial susceptibility test showed that both strains were susceptible to β‑lactam antibiotics and moderately susceptible to polypeptide antibiotics. S1 was resistant to chloramphenicol (CHL), streptomycin (STR), tetracyclin (TET), florfenicol (FFC) and nalidixic acid (NAL), and carried five resistance genes including APH(6),APH(3), gyrA, tet and flor, whereas S2 was susceptible to the five antibiotics mentioned above. Neither of the two strains contained the stx virulence gene, but S2 harbored the astA virulence gene. The two Escherichia albertii strains had the MLST types of ST4638 and ST4606, respectively. The genome size of the two strains was 4.5 Mb, and the closest relative strain was GTC14781 (Escherichia albertii). The average nucleotide identity (ANI) values to the reference genome of E. coli were both below 90%. S1 did not have the H serotype defined in the current database. S1 and S2 differed by 28 267 single-nucleotide polymorphisms (SNPs), and no homologous strains were found in comparison with the data in the GeneBank database. ConclusionTwo strains of Escherichia albertii are initially identified in Baoshan District of Shanghai. Strain S1 shows strong multidrug resistance, while strain S2 carries diarrheagenic virulence genes. The two strains have a distant genetic relationship and exhibits a pattern of sporadic occurrence. It is recommended to utilize whole-genome sequencing technology to enhance the epidemiological surveillance and laboratory detection of Escherichia albertii.

Objective To investigate the relationship between tooth loss and the occurrence of esophageal cancer in a natural village in Wenfeng District, Anyang City, Henan Province. Methods A prospective cohort study was conducted to observe the occurrence of tooth loss and esophageal cancer among the asymptomatic residents of the natural village for 16 years from January 2008 to July 2024. Data were analyzed by chi-square test, binary logistic regression, and restricted cubic spline. Results Among the total population of 711 cases, 136 cases were lost to follow-up and 575 cases were included in the final statistics, including 45 cases with esophageal cancer. Significant statistical difference was found between esophageal cancer patients with and without tooth loss (P<0.05). ObjectiveTo analyze the drug resistance and the molecular typing characteristics through pulsed field gel electrophoresis (PFGE) of Campylobacter spp. isolated from patients with infectious diarrhea in Baoshan District of Shanghai, and to provide a basis for Campylobacter spp. prevention and control and clinical medication. MethodsCampylobacter spp. was isolated, cultured and identified from stool samples of diarrheal patients collected from medical institutions at two monitoring sites in Baoshan District from 2019 to 2022. Antimicrobial susceptibility testing for 12 antibiotics was conducted on the isolated Campylobacter jejuni (C. jejuni) and Campylobacter. Coli (C. coli), and molecular typing was performed using PFGE. ResultsA total of 179 strains of Campylobacter spp. were isolated from 1 786 samples of diarrheal patients, with a positive rate of 10.02%. The highest resistance rate of C. jejuni was to ciprofloxacin (98.63%), followed by tetracycline (97.26%) and nalidixic acid (89.73%). C. coli was completely resistant to ciprofloxacin and nalidixic acid (100.00%), followed by tetracycline (90.91%). The multidrug resistance rates of C. jejuni and C. coli were 89.73% and 100.00%, respectively. 142 strains of C. jejuni produced 122 PFGE bands, while 33 strains of C. coli produced 33 PFGE bands, and the distribution of the bands was relatively dispersed. ConclusionFrom 2019 to 2022, the detection rate of Campylobacter in diarrheal patients was relatively high in Baoshan District of Shanghai, the multidrug resistance rate of Campylobacter isolates from diarrheal patients was relatively serious, in addition, the drug resistance pattern was complex, and the PFGE band pattern displayed a polymorphic distribution.
8.Prospective Study of Disease Occurrence Spectrum in Asymptomatic Residents in Areas with High Incidence of Esophageal Cancer: 16-year Observation of 711 Cases in Natural Population

Objective To understand the disease spectrum of a natural village in an area with high incidence of esophageal cancer to provide a reference for precise prevention and control. Methods From 2008 to 2024, 711 asymptomatic people over the age of 35 years in a natural village with high incidence of esophageal cancer in China were surveyed, and 171 of them were subjected to gastroscopy, biopsy, and pathological examination. All participants were followed up for a long time, and their disease history was recorded. Results A total of 16 years of follow-up were performed, and 703 people were effectively followed up. In 2008, 171 people underwent gastroscopy, and 160 people had biopsy and pathological results in endoscopic screening. By 2024, 76 people had been diagnosed with malignant tumors of 12 different types, and among these people, 45 had esophageal cancer. Conclusion Esophageal cancer remains a significant cause of morbidity and mortality from malignant tumors in this region. Biopsy and pathological examination should be strengthened during gastroscopy, and follow-ups and regular check-ups should be given high importance to reduce the incidence and mortality rates of esophageal cancer.
9.Incidence and Mortality of Gastric Cancer in Hefei City of Anhui Province in 2020 and the Trend from 2013 to 2020

[Purpose]To analyze the incidence and mortality of gastric cancer in Hefei City of Anhui Province in 2020 and the trend from 2013 to 2020.[Methods]The incidence and mortality data of gastric cancer in Hefei City from 2013 to 2020 were collected from sofeware CanReg4.The incidence(mortality)rate,age-standardized incidence(mortality)rate by Chinese standard popula-tion and world standard population(ASIRC/ASIRW,ASMRC/ASMRW),age-specific incidence(mortality)rate and average annual percentage change(AAPC)of the incidence(mortality)rate were calculated.[Results]In 2020,the crude incidence rate of gastric cancer in Hefei was 47.14/105,the ASIRC was 27.10/105,the ASIRW was 26.78/105,ranking the second in all cancer types.The crude mortality rate of gastric cancer was 41.12/105,the ASMRC was 21.64/105,the ASMRW was 21.25/105,ranking the second in all cancer types.The incidence rate and mortality rate of gastric cancer increased with age,and reached the peak in the age group of 80~84 years old.The incidence rate and mortality rate of men were higher than those of women,and rates in rural areas were higher than those in urban areas.The ASIRC and ASMRC of gastric cancer showed downward trends from 2013 to 2020 in Hefei(AAPC were-7.19%and-4.71%,both P<0.05).[Conclusion]The ASIRC and ASMRC of gastric cancer show downward trends from 2013 to 2020 in Hefei,but are still higher than the average level in China,so the prevention and control work should be fur-ther strengthened.
10.Application of progressive exercise training based on mMRC grading in respiratory rehabilitation for patients with chronic obstructive pulmonary disease in a primary healthcare setting

Objective To explore the efficacy of progressive exercise training based on the modified Medical Research Council dyspnea scale (mMRC) grading in respiratory rehabilitation for patients with chronic obstructive pulmonary disease (COPD) at a primary healthcare setting. Methods A total of 106 patients with COPD admitted to Zhuanqiao Community Health Service Center in Shanghai from Aug.1, 2022 to Jul. 30, 2024 were selected as research subjects. They were randomly divided into a study group and a control group in a 1∶1 ratio, with 53 patients in each group. The control group received conventional treatment, while the study group received conventional treatment combined with progressive exercise training. After 4 weeks of continuous treatment, the changes in the 6-minute walk test (6MWT), COPD assessment test (CAT) score, mMRC grading, Global Initiative for Chronic Obstructive Lung Disease (GOLD) grading and pulmonary function were compared between the two groups. Results Patients in both groups showed improvements in 6MWT distance, CAT score, mMRC grading, GOLD grading, and pulmonary function compared to baseline (P<0.05). Moreover, the study group had better improvements in 6MWT distance, CAT score, mMRC grading, GOLD grading, and pulmonary function than the control group (P<0.05). Conclusions Conventional treatment combined with progressive exercise training based on mMRC grading can enhance the effect of respiratory rehabilitation in patients with COPD, particularly in improving pulmonary function and exercise tolerance.
