1.Effect of ultrasound-guided needle-knife release of the ligamentum flavum on the expression of integrin alpha5 and beta1 in degenerative rabbit lumbar intervertebral discs

BACKGROUND:Needle-knife release of the ligamentum flavum can effectively improve symptoms in patients with lumbar degeneration,and ultrasound guidance can increase the precision of needle-knife release;however,the specific effects of needle-knife release of the ligamentum flavum on the degenerated intervertebral discs and the possible mechanisms remain to be clarified. OBJECTIVE:To investigate the effect of ultrasound-guided needle-knife release of the ligamentum flavum. METHODS:Twenty-four New Zealand rabbits were randomized into control(n=6)and model(n=18)groups.A rabbit model of lumbar disc degeneration model was established in the model group by cutting the supraspinous and interspinous ligaments of the L5/6 and L6/7 segments to maintain a standing posture and apply axial load to the lumbar spine.After successful modeling,the model rabbits were subdivided into a control group,a model group,an ultrasonic needle-knife group,and a sham needle-knife group according to a random number table method,with six animals in each group.The ultrasonic needle-knife group underwent ultrasound-guided needle-knife release of the right yellow ligament of L7/S1,once every week,for a total of four times.The needle-knife approach in the sham needle-knife group was the same as that in the ultrasound needle-knife group,but the ligamentum flavum was not released.At 30 days after the intervention,MRI was used to observe the changes in the signal intensity of the nucleus pulposus within the L7/S1 segment.Hematoxylin-eosin staining was used to observe the morphological changes of the L7/S1 segment.Immunohistochemical staining was used to detect the expression of type I and II collagen in the nucleus pulposus of the L7/S1 segment.RT-PCR and western blot were used to detect the expression of integrin α5 and β1,p38,and nuclear factor κB in the L7/S1 segment. RESULTS AND CONCLUSION:MRI findings indicated that the nucleus pulposus of the intervertebral disc of rabbits in the model group was gray-black in color,and the gray value of the nucleus pulposus was significantly lower than that of the control group(P<0.01).The brightness of the nucleus pulposus of the intervertebral disc of the rabbits in the ultrasonic needle-knife group was elevated compared with that of the model group,and the gray value of the nucleus pulposus was higher than that of the model group(P<0.01).Results from hematoxylin-eosin staining showed that in the model group,the shape of the nucleus pulposus was irregular,the number of nucleus pulposus cells was reduced,the extracellular matrix was compressed,the fibrous ring was ruptured,the structure and boundary of the end plate were unclear,and the chondrocytes were arranged disorderly.Compared with the model group,the ultrasonic needle-knife group showed an increase in the number of the nucleus pulposus,an improvement in the rupture of the fibrous ring,and more regular arrangement of cartilage endplate cells.Results from immunohistochemical staining showed an increase in positive expression of type I collagen(P<0.01)and a decrease in positive expression of type II collagen in the nucleus pulposus of the model group compared with the control group as well as a decrease in positive expression of type I collagen and an increase in positive expression of type II collagen in the nucleus pulposus of the ultrasonic needle-knife group compared with the model group(P<0.01).RT-PCR and western blot assays showed that the mRNA and protein expression of integrin α5,integrin β1,p38,and nuclear factor κB in the intervertebral discs of rabbits in the model group were increased compared with that in the control group(P<0.01);the mRNA and protein expression of integrin α5,integrin β1,p38,and nuclear factor κB in the intervertebral discs of rabbits in the ultrasonic needle-knife group was decreased compared with that in the model group(P<0.01).To conclude,ultrasound-guided needle-knife release of the ligamentum flavum can improve the degree of lumbar disc degeneration in rabbits,which may be related to the inhibition of p38 and nuclear factor-κB expression by modulating integrin α5 and β1 expression.
2.Best evidence summary of anxiety and depression management in adults with coronary heart disease

Objective:To summarize the best evidence for anxiety and depression management in adults with coronary heart disease (CHD).Methods:Evidence regarding anxiety and depression management of adults with CHD was retrieved from the British Medical Journal best practice, UpToDate, Guidelines International Network, National Institute for Health and Care Excellence, Scottish Intercollegiate Guideline Network, Registered Nurses Association of Ontario, Cochrane Library, the Joanna Briggs Institute (JBI), PubMed, Embase, Medline, Web of Science, China National Knowledge Infrastructure, and WanFang Data. These publications included guidelines, systematic reviews, expert consensus, and evidence summaries. The data retrieval time was from the establishment of the databases to September 13, 2021. The search terms included coronary heart disease, psychology, anxiety and depression, and the data was sorted on September 14-23, 2021. Two researchers evaluated the quality of the literature and extracted data independently according to both AGREE Ⅱ and the evaluation tools of the JBI evidence-based healthcare center. The evidence level was graded by using the evidence preliminary classification system and the evidence recommendation level system of the JBI evidence-based healthcare center. The recommended strength of the evidence was determined with the guidance of JBI′s evidence structure and the discussion within the research group and expert opinions, and then the best evidence for anxiety and depression management in adults with CHD were extracted, summarized and analyzed. Results:A total of 22 articleswere included, including 5 clinical guidelines, 14 systematic reviews, 2 expert consensuses and 1 evidence summary. Three articles of the guidelines were graded B with the up-mentioned recommendtation standard, and the other two were graded A. In the systematic reviews, five articles were rated as grade B, the other nine articles as grade A, and 95% of the items were evaluated as “yes”. The evaluation results of all items in the expert consensuses were “yes”, and the quality grade was A. The evidence summary involved 32 pieces of best evidence regarding five aspects (mental health management mode, risk factor assessment and prevention, psychological problem identification and screening, psychological adjustment and intervention, drug treatment), of which 60% were class 1 evidence, and 34% were recommended with strength A.Conclusions:The overall quality of the literature and the level of evidence regarding anxiety and depression management in adults with CHD are high. The key to anxiety and depression management in these patients is innovating the mode of mental health management and implementing effective risk factor assessment and prevention, psychological problem identification and screening, psychological adjustment and intervention, and drug treatment.
3.Analysis of the risk factors related to prolonged drainage time in patients with selective abdominal drainage after laparoscopic cholecystectomy

Objective:To determine the risk factors of drainage time longer than 1 day in patients with selective abdominal drainage after laparoscopic cholecystectomy.Methods:The clinical data related to patients with selective abdominal drainage undergoing laparoscopic cholecystectomy from November 2009 to November 2019 at Chinese PLA General Hospital were retrospectively analyzed. Of 233 patients enrolled into this study, there were 147 males and 86 females, with a median aged 59.0 (47.5, 65.5) years old. The patients were divided into drainage time 1 day group of 65 patients and longer than 1 day group of 168 patients according to postoperative drainage time. The baseline data and perioperative data were collected, the risk factors correlated with drainage time longer than 1 day were analyzed.Results:The drainage time was 1 in the 1 day group and 2~8 in another group. Among the 233 patients, there was one with biliary leakage and 14 patients had abdominal bleeding, all of them healed after 2~3 days. All of the 233 patients were recovered when discharged. Independent risk factors related to drainage time longer than 1 day include BMI≥28 kg/m 2 ( OR=3.443, 95% CI: 1.411-8.405, P=0.007), operation time ≥65 min ( OR=2.570, 95% CI: 1.310-5.045, P=0.006), thickness of gallbladder wall ≥0.5 cm ( OR=12.720, 95% CI: 1.350-5.478, P=0.005), postoperative stomachache ( OR=13.537, 95% CI: 1.685-108.748, P=0.014) and postoperative fever ( OR=8.156, 95% CI: 1.035-64.249, P=0.046). Conclusion:For patients undergoing selective abdominal drainage after laparoscopic cholecystectomy with BMI ≥28 kg/m 2, operation time ≥65 min, gallbladder wall thickness ≥0.5 cm, postoperative abdominal pain and fever, clinicians should appropriately prolong the drainage time to ensure medical safety.

Objective To evaluate preoperative oral carbohydrate in patients receiving LC day surgery.Methods 117 patients undergoing LC day surgery in PLA General Hospital from Oct 2016 to Mar 2017 were evenly divided into three groups.Patients in group A took oral carbohydrate preoperatively,group B by iv glucose,group C by overnight fasting.The preoperative thristy and the indicators of inflammation,heart rate,blood glucose,serum insulin,insulin resistance index postoperatively were collected and compared.Results No patients suffered aspiration.There were no complications above Clavien Ⅰ level.Differences in insulin resistance index,fasting serum insulin were statistically significant between group A and group B (Z =-5.60,-4.78,P < 0.05).That of insulin resistance index,fasting serum insulin and procalcitonin between group A and group C were statistically significant (Z =-2.65,-2.49,-4.02,P <0.05).Procalcitonin between group B and group C were statistically different (Z =-2.183,P < 0.05).The preoperative thristy between group A and group B,group C were statistically different (x2 =6.47,P < 0.05).Conclusions Preoperative oral carbohydrate intake is safe and feasible with the benefits of improving the quality of life in patients operated on LC day surgery.
6.Clinical significance of combined detection of multiple cytokines in serum of patients with gastric cancer

Objective To detect the expression levels of serum ITAC, Fractalkine, IL-8, IL-17A, IL-7 and TNF-α in patients with gastric cancer, and to explore correlation among them, as well as their association with different clinical characteristics.Methods The levels of the 6 kinds of cytokines in serum of 46 gastric cancer patients (gastric cancer group) and 30 healthy people (healthy control group) were detected.Results Compared with those in healthy control group, the levels of serum ITAC, Fractalkine, IL-8, IL-17A, IL-7 and TNF-α in gastric cancer group were significantly increased [22.26 (32.83) pg/ml vs 11.95 (9.99) pg/ml, P =0.001;62.21 (82.23) pg/ml vs 26.47 (50.87) pg/ml, P =0.050;4.50 (10.38) pg/ml vs 2.06 (3.17) pg/ml, P =0.002;0.83 (2.01) pg/ml vs 0.21 (0.85) pg/ml, P=0.013;3.46 (1.90) pg/ml vs 2.11 (1.48) pg/ml, P=0.001;1.21 (1.13) pg/ml vs 0.79 (0.37) pg/ml, P < 0.001].There were correlations between cytokines (all P < 0.05).The level of serum cytokines was no significant difference between gastric cancer patients with lymph node metastasis and those without lymph node metastasis (P > 0.05).Conclusions The high level of serum ITAC, Fractalkine, IL-8, IL-17A, IL-7 or TNF-α may be related to the occurrence and development of gastric cancer.High level of serum IL-8 may be a marker of poor prognosis of gastric cancer, and interaction between the various cytokines also has a certain association with tumorigenesis.

BACKGROUND:To in vitro isolate neural stem cel s with high purity and uniform biological properties and to establish a complete set of neural stem cel culture system is the basis for neural stem cel research.
OBJECTIVE:To establish an isolation and culture system for neural stem cel s from newborn mouse hippocampus, olfactory bulb and cortex and to analyze the biological properties of cel s.
METHODS:Neural stem cel s were isolated from the hippocampus, olfactory bulb and cortex tissue of newborn Kunming mice by mechanical separation and trypsin digestion. Serum-free culture technology, mechanical pipetting and trypsin digestion were used for subculture of neural stem cel s. 10%fetal bovine serum was used to induce differentiation of neural stem cel s. Neural stem cel s and their differentiated products were identified by
immunofluorescent staining of Nestin, CD133,β-TubulinIII, glial fibril ary acidic protein.
RESULTS AND CONCLUSION:The neural stem cel obtained from newborn mouse hippocampus, olfactory bulb and cortex had the capacity of self-renewal and differentiation which were positive for Nestin and CD133. After induction with fetal bovine serum, neural stem cel could differentiation toβ-tubulinIII or glial fibril ary acidic protein positive cel s that were neurons and astrocytes. This experiment has successful y established the neural stem cel isolation, culture, identification and induction system, providing experimental basis for subsequent studies of neural stem cel s.

10.Expression of TMEM16A in gastric carcinoma and its clinical implications.

OBJECTIVETo investigate the expression of TMEM16A in gastric carcinoma and its clinical implications.
METHODSA total of 72 surgical specimens of gastric carcinoma were collected for examination of TMEM16A expression with immunohistochemical staining.
RESULTSTMEM16A expression was detected in the cytoplasm and cell membrane of the tumor cells. Of the 72 specimens of the tumor tissues, the total positivity rate of TMEM16A expression was 80.56% (58/72), significantly higher than the rate in the adjacent tissues (4.17%, 3/72, P<0.005).
CONCLUSIONAberrant expression of TMEM16A occurs in the majority of gastric carcinoma cases. TMEM16A can be used as a new candidate target for diagnosis and treatment of gastric carcinoma.
