1.Analysis of liver histological characteristics and clinically related factors in patients with inactive HBsAg carriers

Objective:To analyze the liver histological characteristics and clinically related factors in inactive hepatitis B surface antigen (HBsAg) carriers (IHC), and also explore whether antiviral treatment is necessary for IHC, as defined in the 2022 version of the hepatitis B prevention and treatment guidelines.Methods:A multicenter, retrospective cohort study was conducted. Two hundred and thirty-one IHC cases who underwent liver biopsy histopathological examination in nine medical institutions, including Beijing Youan Hospital affiliated with Capital Medical University, from January 2018 to December 2023 were included. General informative data, clinical serological markers, and transient elastography (TE) examination results were collected. Patients were divided into a positive (148 cases) and a negative group (83 cases) according to the results of hepatitis B virus (HBV) DNA detection. The differences in liver pathological inflammatory activity (G) and liver fibrosis stage (S) were analyzed between the two groups to explore the correlation between liver tissue conditions and clinically related factors. Comparsions of normally distributed continwous data, skeukd continuous data, and categorical data between groups are performed using t tests, Mann-Whitney U tests and χ2 tests, respectively. Results:The age of 231 IHC cases was 43 (38, 51) years old, with 95.2% (220/231) aged ≥30 years, and males accounted for 64.9% (150/231). HBsAg and HBV DNA levels were 131.9 (20.8, 400.9) IU/mL and 94.0 (0, 448.5) IU/mL, respectively, of which 35.9% (83/231) were HBV DNA negative (<20 IU/mL). The remarkable proportions of G≥2, S≥2, and liver injury (G≥2 and/or S≥2) in liver tissue were 16.5% (38/231), 29% (67/231), and 35.9% (83/231), respectively. The S≥2 proportion was significantly higher in the HBV DNA-negative group than the positive group (42.2% vs. 21.6%, P<0.001), and it mainly occurred in the population cohort over 30 years old (44.9% vs. 31.0%, P=0.04). The liver stiffness measurement (LSM), aspartate transaminase to platelet ratio index (APRI), and platelet (PLT) were significantly higher in the S≥2 group than the S<2 group ( P<0.05). Conclusion:Clinicians can comprehensively evaluate the degree of liver fibrosis in IHC based on clinical factors such as age, PLT, APRI, and LSM, even if the liver histological results are lacking. The China 2022 version guidelines define that nearly half of IHC has histological indications for antiviral therapy, and liver biopsy and prompt treatment can be recommended.
2.The effects of tele-rehabilitation with a comprehensive exercise program on pain and function in patients with non-specific neck pain

Objective:To compare the effects of tele-rehabilitation delivered via a WeChat mim-program with traditional outpatient-home combined rehabilitation for non-specific neck pain patients.Method:Individuals with non-specific neck pain were recruited from the Rehabilitation Medicine Department of West China Hospital between January 2021 and August 2021,were randomly assigned to either tele-rehabili-tation treatment group(TRG)or outpatient-home combined rehabilitation group.Both groups received compre-hensive exercise therapy at home or in the clinic for 4 weeks.Assessments were conducted at baseline,4 weeks,and 8 weeks post-intervention,using the Neck Disability Index(NDI),Numeric Pain Rating Scale(NPRS),and Pain Self-efficacy Questionnaire(PSEQ).Result:A total of 60 patients completed the intervention,with 30 in each group.There were no significant de-mographic differences between groups.At the 4-week mark,both groups showed significant improvements in NDI,NPRS and PSEQ scores compared to baseline,but no significant differences were found between groups.By the 8th week,the tele-rehabilitation group showed further improvements in all three metrics,with NDI and NPRS scores significantly better than those of the outpatient-home combined rehabilitation group.Conclusion:Both remote and outpatient-home combined comprehensive exercise therapies demonstrate short-term effects on non-specific neck pain.However,tele-rehabilitation exhibited more sustained improvements,sug-gesting that it holds promise for further clinical application.
3.Influencing factors of intraoperative blood transfusion and prognosis in lung transplant patients

Objective: To explore the risk factors of allogeneic blood transfusion during lung transplant surgery and prognostic effects of transfusion by analyzing the basic data, surgical details, laboratory tests results, and intraoperative blood transfusion details during the perioperative period of lung transplant, so as to guide clinical blood use. Methods: A retrospective analysis was conducted on the data of 319 patients who underwent lung transplantation surgery in our hospital from January 2022 to December 2023. The patients were divided into a non-transfusion group (n=70) and a transfusion group (n=249) based on their intraoperative blood transfusion status. The clinical data, surgical details, perioperative laboratory results and other relevant preoperative and postoperative parameters were compared between the two groups, and the postoperative prognosis (improvement, non-recovery, and death) was analyzed. Results: After comparison between the two groups of patients, it was found that the non-transfusion group had higher levels of preoperative Hb (g/L)(144.41±17.66 vs 129.78±20.44), preoperative Hct [43.25(40.23, 47.5) vs 40.7(37, 43.55)], preoperative TBIL (μmol/L)[11.45(9.15, 15.3)vs 9.9(6.88, 13.33)], and postoperative PLT (×10
/L)(167.74±64.43 vs 132.37±54.84) than the transfusion group (all P<0.05). The non-transfusion group had lower levels of preoperative pCO
(mmHg)[41.4(37.4, 45.8)vs 45.3(40, 52.48)], postoperative TBIL (μmol/L)[25.45(17.68, 33.95)vs 30.8(21.55, 43.05)], postoperative pH (7.41±0.09 vs 7.45±0.10), bilateral lung transplantation [27(38.6%) vs 157(63.1%)], surgical duration (h) [5(4, 7)vs 6.5(5, 8)], use of ECMO [52(74.3%) vs 232(93.2%)], and intraoperative blood loss (mL)[600(500, 800)vs 1 000(800, 1 500)] compared to the transfusion group (all P<0.05). The items with P<0.1 in the compared indicators were included in the binary logistic regression analysis, and the results showed that bilateral lung transplantation, intraoperative blood loss, preoperative TBIL, postoperative PLT, postoperative TBIL, preoperative pCO2, and postoperative pH were significantly correlated with whether blood transfusion was performed (P<0.05). The P values of the Chi-square test for postoperative improvement and mortality in the non transfusion group and transfusion group were both greater than 0.05, indicating no statistically significant difference in the prognosis rate between the two groups of patients. Conclusion: Bilateral lung transplantation, intraoperative blood loss, preoperative TBIL, and preoperative pCO
are risk factors for blood transfusion during lung transplantation. Intraoperative blood transfusion has a significant impact on postoperative PLT, postoperative TBIL, and postoperative pH indicators, but has no significant effect on prognosis. A comprehensive evaluation of laboratory indicators and surgical details can help developing blood transfusion strategies more effectively.
4.Assessment of glymphatic system function in mild traumatic brain injury patients based on diffusion tensor imaging analysis along the perivascular space index

Objective To investigate the function of glymphatic system in patients with mild traumatic brain injury via diffusion tensor imaging analysis along the perivascular space(DTI-ALPS)index and its relationship with clinical features.Methods The clinical and imaging data from 163 patients with mild traumatic brain injury were retrospectively collected,including 31 cases with MRI-negative findings and 132 cases with MRI-positive findings.In addition,29 healthy individuals underwent physical examinations were included as a control group.The differences in DTI-ALPS index and clinical features between mild traumatic brain injury and control group,as well as among subgroups of traumatic brain injury were analyzed.Results The DTI-ALPS index in patients with mild traumatic brain injury were significantly higher than those in control group(P<0.05).Subgroup analysis showed that DTI-ALPS index of traumatic brain injury patients with MRI-negative findings and MRI-positive findings were all significantly higher than those of control group(P=0.023 and 0.004,respectively).Compared to control group,DTI-ALPS index in mild traumatic brain injury patients gradually increased with the progression of injury,and the difference was statistically significant(P=0.008).Conclusion Patients with mild traumatic brain injury exhibit increased functional activity of glymphatic system,as indicated by increased DTI-ALPS index,which further increase with the progression of injury.
5.Clinical efficacy analysis of hemorrhoidal artery ligation combined with ultrasonic knife excision suture tethering suspension for the treatment of circumferential prolapsed mixed hemorrhoids

Objective To investigate the clinical effect of hemorrhoidal artery ligation combined with ultrasonic knife resection,suture,ligature and suspension in the treatment of mixed hemorrhoids with annular prolapse.Methods A retrospective analysis was conducted to select 80 patients with annular prolapse mixed hemorrhoids admitted to the anorectal Department of our hospital from January 2022 to April 2023.According to different treatment plans,they were divided into control group(n=40 cases)who received external stripping and internal ligation combined with tape ligation,and study group(n=40 cases)who received hemorrhoidal artery ligation combined with ultrasonic knife resection,suture,ligation and suspension.The operative effect,operative status,postoperative anal pain,anal edema,stool blood score and quality of life were evaluated.Results The effective rate of the study group was 97.50%,which was higher than that of the control group(77.50%)(P<0.05).The operation time,intraoperative blood loss,hospital stay and wound healing time of the research group were(25.03±10.00)minutes,(10.00±5.32)ml,(7.25±1.54)days and(12.21±2.00)days respectively.[is better than that of control group(29.85±14.00)minutes,ml(17.85±10.25),(11.87±2.35),(18.89±4.85)days],two groups of comparison,the difference was statistically significant(P<0.05).The VAS scores of anal pain in the study group on 1 day,3 days and 7 days after the operation were(4.25±0.85)points,(2.89±0.54)points and(2.00±0.30)points,respectively.The scores of perianal edema were(1.87±0.36)points,(1.41±0.30)points,and(1.00±0.20)points,respectively.The hematochezia scores were(1.85±0.21)points,(1.34±0.18)points,and(0.85±0.13)points,respectively.Which were better than that of control group[(7.56±1.10),(6.54±1.03),(4.87±1.00)][(2.40±0.58),(2.03±0.49),(1.87±0.45)][(2.45±0.55),(2.03±0.47),(1.88±0.25)],and the differences were statistically significant(P<0.05).The recurrence rate was 2.50%in the study group and 17.50%in the control group(P<0.05).The scores of SF,MH,PF and GH of the Modified Health Survey Summary Form(SF-36)in the study group were(88.54±10.22,87.87±10.58,88.97±10.00,89.95±10.05)higher than those of the control group(67.52±10.00,70.10±11.25,71.10±9.85,70.00±10.00)(P<0.05).Conclusion Hemorrhoidal artery ligation combined with ultrasonic knife resection,suture,ligature and suspension in the treatment of patients with annular prolapse mixed hemorrhoids has ideal surgical effect and little trauma,which is beneficial to promoting postoperative rehabilitation,alleviating anal pain,improving anal edema and hematostoecium,and improving quality of life.

10.Three-dimensional finite element analysis of effects of partial anterior cruciate ligament rupture on knee joint stress

BACKGROUND:Anterior cruciate ligament injury tends to lead to secondary meniscus injury and osteoarthritis.At present,there are few studies on the mechanics of meniscus and articular cartilage injury caused by anterior cruciate ligament injury. OBJECTIVE:To study the effect of partial rupture of the anterior cruciate ligament on the stress of medial and lateral meniscus and articular cartilage of knee joint by finite element analysis. METHODS:The CT and MRI images of the knee joint of a healthy volunteer were selected,and the scan data were imported into Mimics,Geomagic and SolidWorks software.After registration and fusion,four kinds of three-dimensional knee joint models were established:models of intact anterior cruciate ligament,rupture of the posterior external tract of anterior cruciate ligament,rupture of the anterior internal tract of anterior cruciate ligament,and absence of anterior cruciate ligament.Finally,data were imported into Ansys software to apply four different modes of loads to the knee joint:Longitudinal loads of 750 N were applied to the top of the femur;longitudinal load of 750 N to the top of the femur and forward thrust of 134 N behind tibia;a longitudinal load of 750 N and a varus moment of 10 Nm were applied to the top of the femur to simulate genu varus;750 N longitudinal load and 4 Nm internal rotation moment were applied to the proximal end of the femur to simulate knee internal rotation.The finite element analysis of biomechanical stress changes of the meniscus and articular cartilage of the knee joint was carried out. RESULTS AND CONCLUSION:(1)In the straight position of the knee joint,when the anterior medial tract of the anterior cruciate ligament was broken and the anterior cruciate ligament was missing under longitudinal loads of 750 N at the top of the femur,the total stress and peak value of meniscus increased significantly,but the stress distribution of the meniscus and the stress of articular cartilage did not change significantly.In longitudinal load of 750 N to the top of the femur and forward thrust of 134 N behind tibia,the fracture of the anterior internal tract of the anterior cruciate ligament increased the tibia forward,the compressive stress of posterior angle of the meniscus increased,and the stress of the articular cartilage did not change significantly.During simulating genu varus,the posterior angular stress of the lateral meniscus decreased,the stress of the medial meniscus increased,and the stress of articular cartilage slightly decreased when anterior cruciate ligament injuries were complete.When the anterior internal tract of the anterior cruciate ligament was broken or absent under knee internal rotation,the equivalent stress peak value of femoral cartilage and tibia cartilage shifted from medial cartilage to lateral cartilage,and the stress peak value of meniscus increased significantly.At this time,the anterior internal tract of the anterior cruciate ligament played a leading role in the rotational stability of the knee joint.(2)These results indicate that the risk of secondary meniscus injury in patients with anterior and medial anterior cruciate ligament band rupture was much higher than that in patients with posterior and external anterior cruciate ligament band rupture when the knee was in the upright standing position,varus and pronation,and there was no significant difference in the impact on articular cartilage.
