1.Mechanism of Shexiang Tongxin Dripping Pills in Ameliorating AngⅡ-induced Vascular Smooth Muscle Cell Dysfunction

ObjectiveTo study the mechanism of Shexiang Tongxin Dripping pills-containing serum (STDP) in ameliorating angiotensinⅡ (AngⅡ)-induced cell phenotype transformation, proliferation, migration, and dysfunction of vascular smooth muscle cells. MethodsAn AngⅡ-induced proliferation and migration model of vascular smooth muscle cells was established. The cells were treated with STDP at 5%, 10%, and 20% for 24 h. The immunofluorescence assay was employed to detect the expression of α smooth muscle actin (α-SMA) and matrix metalloproteinase-2 (MMP-2). The cell-counting kit-8 (CCK-8) assay and 5-ethynyl-2'-deoxyuridine (EdU) staining were employed to detect the proliferation of vascular smooth muscle cells, and the scratch assay was employed to detect the migration of the cells. Western blot was employed to determine the expression levels of pathway proteins such as angiotensin-converting enzyme 2 (ACE2), angiotensin Ⅱ type 2 (AT2), angiotensin Ⅱ type 1 (AT1), as well as proliferation and migration proteins such as typeⅠ collagen (ColⅠ) and osteopontin (OPN). ResultsCompared with the model group, STDP increased the expression of α-SMA, reduced the expression of MMP-2, and inhibited the proliferation and migration of vascular smooth muscle cells (P<0.05). Furthermore, STDP up-regulated the expression levels of ACE2, AT2, and MAS1, while down-regulating the expression level of AT1, PCNA, ColⅠ, MMP-9, Rock1, Rock2, and SRF (P<0.05). Compared with the STDP group, the ACE2 inhibitor reversed the regulatory effects of STDP. ConclusionSTDP inhibits the phenotype transformation, proliferation, and migration of vascular smooth muscle cells and regulates the expression of cell proliferation and migration-related proteins to ameliorate the dysfunction of vascular smooth muscle cells. It exerts the effects by up-regulating the expression of proteins in the ACE2-AT2/MAS pathway and down-regulating the expression of proteins in the AT1-Rock signaling pathway.
2.Shexiang Tongxin Dropping Pills Ameliorate Dysfunction of Cardiac Microvascular Endothelial Cells in Rat Model of Heart Failure

ObjectiveTo study the mechanism by which Shexiang Tongxin dropping pills (STDP) ameliorate the dysfunction of coronary microvascular endothelial cells in the rat model of heart failure. MethodsThe heart failure model was established by ligation of the left anterior descending coronary artery in rats, which were then allocated into sham, model, STDP, and telmisartan (TLM) groups and treated for 21 days. The heart function was detected by echocardiography, and the levels of myocardial injury markers, nitric oxide (NO), endothelin-1 (ET1), and angiotensinⅡ (AngⅡ) were determined by enzyme-linked immunosorbent assay (ELISA). The protein levels of endothelial nitric oxide synthase (eNOS) and inducible nitric oxide synthase (iNOS) were determined by Western blot. The model of cardiac microvascular endothelial cell injury was established by AngⅡ induction and then treated with the STDP-containing serum (5%, 10%, and 20%) for 24 h. The levels of NO and ET1 were measured by ELISA. Western blot was employed to determine the protein levels of eNOS, iNOS, angiotensin-converting enzyme 2 (ACE2), and angiotensinⅡ receptor 2 (AT2). MLN-4760, an ACE2 inhibitor, was used to explore the mechanism underpinning the regulatory effect of STDP on the ACE2-AT2/MAS pathway. ResultsCompared with the sham group, the model group showed decreases in left ventricular ejection fraction (LVEF) and left ventricular fractional shortening (LVFS) (P<0.05), a decline in serum NO level, elevations in serum AngⅡ and ET1 levels, a reduction in p-eNOS/eNOS ratio, and up-regulation in iNOS expression (P<0.05). Compared with the model group, STDP increased LVEF, LVFS, and cardiac output (P<0.05), raised the level of NO and lowered the levels of AngⅡ and ET1 in the serum (P<0.05), increased the p-eNOS/eNOS value, and inhibited iNOS expression (P<0.05). Compared with the AngⅡ group, STDP increased the NO content and decreased the ET1 content in endothelial cells (P<0.05), increased the p-eNOS/eNOS ratio, and inhibited the iNOS expression (P<0.05). The ACE2 inhibitor MLN-4760 reversed the regulatory effects of STDP on p-eNOS, eNOS, and iNOS. ConclusionSTDP improves the cardiac function in the rat model of heart failure, enhances the synthesis and release of NO in cardiac microvascular endothelial cells, reduces AngⅡ and ET1 levels, and regulates the expression of p-eNOS and eNOS, thereby ameliorating the dysfunction of microvascular endothelial cells in heart failure. This mechanism is related to the upregulation of the expression of proteins in the ACE2-AT2/MAS pathway.
3.Analysis of Risk Factors for Meningeal Metastasis in Patients with Lung Adenocarcinoma Following Non-surgical Interventions.

BACKGROUND:
Meningeal metastasis (MM) is a form of malignant metastasis where tumor cells spread from the primary site to the pia mater, dura mater, arachnoid, subarachnoid space, and other cerebrospinal fluid compartments. Lung cancer is one of the most common malignant tumor types with MM. MM not only signifies that the lung cancer has progressed to an advanced stage but also leads to a range of severe clinical symptoms due to meningeal involvement. Currently, the risk factors associated with the development of MM are not fully elucidated. The aim of this study was to investigate the risk factors for MM in patients with lung adenocarcinoma (LUAD) who underwent non-surgical interventions, in order to identify LUAD patients at high risk for MM.
METHODS:
This retrospective study analyzed the clinical data of patients diagnosed with LUAD at the First Affiliated Hospital of Zhengzhou University from January 2020 to July 2024. Missing data were imputed using multiple imputation methods, and risk factors were identified through LASSO, univariate, and multivariate Logistic regression analyses.
RESULTS:
A total of 170 patients with LUAD were included in this study and divided into two groups: 87 patients with MM and 83 patients without MM. Univariate and multivariate Logistic regression analyses revealed that younger age at diagnosis (P=0.004), presence of the epidermal growth factor receptor (EGFR) L858R gene mutation (P=0.008), and concurrent liver metastasis at baseline (P=0.004) were independent risk factors for developing MM in LUAD patients who did not undergo surgical intervention. Conversely, higher baseline globulin levels (P=0.039) and the presence of the anaplastic lymphoma kinase (ALK) gene mutation (P=0.040) were associated with a reduced risk of MM development.
CONCLUSIONS
Age at diagnosis, EGFR L858R mutation status, ALK gene mutation status, concurrent liver metastasis, globulin levels at baseline were significantly associated with the risk of developing MM in patients with LUAD patients who did not undergo surgical intervention. For patients diagnosed at a younger age, carrying the EGFR L858R mutation, or presenting with baseline liver metastasis, early implementation of tertiary prevention strategies for MM is crucial. Regular monitoring of MM status should be conducted in these high-risk groups.

4.Exploration of the Thinking of Syndrome Differentiation and Treatment "Treatment According to Disease Tendency" in Traditional Chinese Medicine from the Perspective of Holism and Constant Motion

"Tendency of disease" is an important concept in the theory of traditional Chinese medicine (TCM). It was born out of and rooted in the thinking of "conducting by tendency" of ancient Chinese philosophy, meaning that the development and change of humans and nature have their own inevitable laws and dynamic tendencies. Based on the concept of holism and constant motion, the basic definition of "tendency", the origin of the idea, the influence factors and related concepts were analysed, to grasp the overall connection in the dynamic changes in space and time, and to find a scientific and comprehensive objective law of development. The "tendency" was regarded as dynamic forces in the complex system, when yin and yang harmonized, called normal tendency; when fail to keep normal, called disease tendency; the season have sequential tendency, the earth has geographical tendency, people have body tendency, medications have medical tendency, and pulse has pulse tendency. Then the "eight methods of treating tendency" were summed up as observing tendency, evaluating tendency, waiting for tendency, accumulating tendency, favoring tendency, counteracting tendency, preventing tendency, and borrowing tendency, and condenses the methodo-logical thinking of "treatment according to disease tendency", with a view to re-understanding the internal logic of TCM diagnosis and treatment, and providing metaphysical contemplation and exploration for the construction of a new paradigm of TCM syndrome differentiation and treatment.
5.Efficacy and safety assessment of polyglycolic acid and hydroxypropyl methylcellulose for postoperative suturing in a distal pancreatectomy model

【Objective】 To explore the effectiveness and safety of polyglycolic acid (PGA) and hydroxypropyl methylcellulose (HPMC) composite materials in distal pancreatectomy postoperative suturing. 【Methods】 We selected 36 healthy adult beagles and divided them randomly into observation group and control group, with 18 distal pancreatectomy surgeries in each group. The observation group used PGA+HPMC composite materials for incision reinforcement while the control group used NEOVEIL for incision reinforcement. 3 days before surgery, 3 days after surgery, and before dissection, blood routine tests were performed on each group of experimental dogs. Observation periods of 2-week, 4-week and 8-week were set, and six animals at each observation point were evaluated for histological examination of heart, liver, spleen, lung, and kidney tissues, hard tissue slice pathological diagnosis, and safety evaluation. 【Results】 There was no significant difference between the observation group and the control group in the preoperative blood routine test. Repeated measures ANOVA results showed differences in the mean values of white blood cell count (WBC) ( F=14.875, P=0.001), mean corpuscular hemoglobin concentration (MCHC) (F=5.049,P=0.009), neutrophil percentage (Neu%) (F=4.794, P=0.011), red blood cell count (RBC) (F=6.591, P=0.002), hemoglobin (HGB) (F=8.154, P=0.001), hematocrit (HCT) (F=5.281, P=0.007), platelet count (PLT) (F=6.560, P=0.014), red blood cell distribution width coefficient of variation (RDW-CV) (F=33.950, P=0.039), or lymphocyte percentage (Lym%) (F=3.299, P=0.043) at different time points. However, the observation group and the control group did not differ, and the interaction between time and group had no significant effect on the above indicators, suggesting that both groups of dogs had inflammatory response or surgical stress. For the 8-week postoperative hard tissue pathological section score, there was no significant difference between the observation group and the control group in the inflammation and necrosis related score, fibrosis, repair or other related scores and total score (P>0.05). In the dissection at 8 weeks after surgery, no obvious damage to the heart, liver, spleen, lung, kidney, or other organs was found in both groups, nor was there any residual suture material, pancreatic fistula, or pancreatitis, indicating that the suture materials in both groups had been completely absorbed and metabolized, and the incision healed well without causing adverse effects on the visceral organs. 【Conclusion】 PGA and HPMC are effective and safe postoperative suture materials, with good biodegradability, biocompatibility, suture strength, and wound healing quality. They can be comparable to traditional absorbable reinforcement materials in distal pancreatectomy postoperative suturing, thus providing scientific basis for their clinical application.
6.Measurement performance of Gafchromic film dosimeter

Objective To determine the applicable conditions and the film reading time of Gafchromic EBT3 film dosimeter through the research and test of the measurement performance such as blackening time,dosimetry range,dosimetry resolution and dose response non-uniformity.Methods Three Gafchromic films were exposure to different doses,and the optical densities were read at different moments after irradiation for analyzing and confirming the effects of blackening time on dose measurement and different doses on blackening time.The absorbed dose-optical density curve was used to determine Gafchromic film dosimetry range.The Gafchromic film dosimetry resolution was obtained by calculating the average optical densities of two groups with a dose difference of 0.01 Gy.The dose response non-uniformity of Gafchromic film was evaluated by irradiating the whole film and the cutting film.Results Relative to the optical density at the termination moment of irradiation,the optical density changes at 4 h were 1.69%(2 Gy),2.53%(4 Gy)and 2.13%(8 Gy);the changes at 24 h were 2.91%(2 Gy),3.31%(4 Gy)and 3.20%(8 Gy);and the changes at 48 h were 2.91%(2 Gy),3.31%(4 Gy)and 3.96%(8 Gy).The optical density given by Gafchromic film dosimeter within 0.1 to 10.0 Gy had a linear relationship with the absorbed dose,while the optical density of Gafchromic film dosimeter within 10.0 to 30.0 Gy had a nonlinear relationship with the absorbed dose,but there was still a one-to-one correspondence.The difference of the average optical densities of two groups was 0.004 when the inter-group dose difference was 0.01 Gy.The non-uniformity was 2.2%for the whole film and 2.8%for the cutting film.Conclusion Gafchromic EBT3 film reading time can be set at 24 h after irradiation,and the dose is independent of blackening time.Gafchromic film dosimeter can be used for point dosimetry and dose distribution measurement within 0.1 to 10.0 Gy,and point dosimetry within 10.0 to 30.0 Gy.The dosimetry resolution of the Gafchromic film dosimeter is better than 0.01 Gy.To avoid the effect of scattered rays,the cutting film is recommended for testing dose response non-uniformity.
7.Microwave sensor for recognition of abnormal nodule tissue on body surface.

For the detection and identification of abnormal nodular tissues on the body surface, a microwave sensor structure loaded with a spiral resonator is proposed in this paper, a sensor simulation model is established using HFSS software, the structural parameters are optimized, and the actual sensor is fabricated. The S21 parameters of the tissue were obtained when nodules appeared by simulation, and the characteristic relationship between the difference of S21 parameters with position was analyzed and tested experimentally. The results showed that when nodules were present in normal tissues, the curve of S21 parameter difference with position change had obvious inverted bimodal characteristics, and the extreme value of S21 parameter difference appeared when the sensor was directly above the nodules, which was easy to identify the position of nodules. It provides an objective detection tool for the identification of abnormal nodular tissues on the body surface.

8.Metaverse key technologies and their application status in medical care

9.Construction of sensitive index system for nutritional screening and evaluation of nursing quality in maintenance hemodialysis patients

Objective:To establish a scientific, sensitive and practical sensitive index system for nutritional screening and nursing quality evaluation of maintenance hemodialysis patients, so as to provide reference for objective evaluation of nutritional screening and nursing quality evaluation of MHD patients.Methods:Based on Donabedian three-dimensional quality structure theory of "structure-process-result", the first draft of the index system was constructed through literature review and semi-structured interview. From September to October 2022, a total of 20 clinical nursing and nursing management experts from the field of hemodialysis in 12 Class Ⅲ Grade A hospitals were selected for two rounds of expert letter consultation to revise and confirm the indicator system.Results:The positive coefficient of the two rounds of expert correspondence were 95.00% and 100.00%, the expert authority coefficient were 0.813 and 0.818, the Kendall's harmony coefficients were 0.121-0.257 (all P<0.05), and the coefficient of variation of each index was less than 0.30. After 2 rounds of expert letter consultation, the sensitive index system for nutritional screening and evaluation of nursing quality for maintenance hemodialysis patients was finally constructed, including 1 structural index, 7 process index and 2 outcome index. Conclusions:In this study, a sensitive index system of nutritional screening and evaluation of nursing quality for maintenance hemodialysis patients with 10 indicators is established, which was scientific, practical and specialized and could be used to standardize the nutritional screening and evaluation nursing work in blood purification centers and promote the continuous improvement of nursing quality.
10.Construction of the frailty risk evaluation index system for patients with maintenance hemodialysis

Objective:To establish an index system for frailty risk assessment of maintenance hemodialysis patients, and scientifically evaluate frailty risk status of maintenance hemodialysis patients.Methods:With the biopsychosocial medical model as the theoretical framework, semi-structured interview method and Delphi expert consultation method were used to establish and screen indicators and analytic hierarchy process was used to determine the weight of indicators, and an indicator system for frailty risk assessment of patients with maintenance hemodialysis was constructed.Results:The effective recovery rates of the two rounds of expert consultation questionnaires were respectively 100.00% (22/22) and 95.45% (21/22), the expert authority coefficients were respectively 0.85 and 0.84, the Kendall's harmony coefficients of the importance of expert opinions were respectively 0.149 and 0.201, and the Kendall's harmony coefficients of the feasibility of expert opinions were respectively 0.134 and 0.175. Finally, the frailty risk evaluation system for maintenance hemodialysis patients was established, including 3 primary indexes, 8 secondary indexes and 36 tertiary indexes.Conclusions:The established frailty risk evaluation index system for patients with maintenance hemodialysis is scientific and practical and comprehensive in content, which can better reflect the specialty characteristics and provide a basis for frailty risk assessment for maintenance hemodialysis patients.
