2.Monitoring analysis and model prediction of HFMD in Liangqing District,Nanning City from 2012 to 2023

Objective To analyze the epidemiological and etiological characteristics of hand,foot and mouth disease(HFMD)in Nanning City,Liangqing District from 2012 to 2023.Methods Descriptive epidemiological methods were used to describe the three distribution characteristics of HFMD,and to predict the incidence trend of HFMD.Results From 2012 to 2023,Liangqing District of Nanning City reported a total of 19 715 cases of HFMD.The incidence rates varied significantly across different years,with statistically significant differences(P<0.01).The urban area reported the highest number of cases.The disease primarily occurred from April to October,with the highest incidence among children aged 6 months to 5 years,mainly among children in daycare and preschools.From 2012 to 2023,a total of 588 cases were diagnosed through laboratory tests,with the highest detection rate of other enteroviruses,and a relatively lower detection rate of EV71.Predictions indicate that the incidence of HFMD in Liangqing District of Nanning City will decrease in 2025 compared to 2023.Conclusion Incidence of HFMD in Liangqing District,Nanning is high,especially among children aged 6 months to 5 years.It is suggested to strengthen the epidemic surveillance,continue to carry out pathogen surveillance.
3.Current status and development of robotic rectal cancer surgery

Robotic surgical systems, with advantages such as 3D visualization, rotating instruments, and stable imaging, are increasingly utilized in transabdominal and transanal approaches for rectal cancer surgery. Compared to traditional laparoscopy, robotic systems could educe the technical difficulty of total mesorectal excision (TME) and enhance pelvic autonomic nerve preservation, while demonstrating comparable long-term outcomes. However, the high cost and limited clinical benefits have led to reflection. Controversies persist regarding prolonged operative time, lack of haptic feedback and limited cost-effectiveness. Studies suggest robotic surgery may benefit patients with complex conditions such as obesity, narrow pelvis or low rectal tumors. The rapid development of domestic surgical robots is expected to break market monopolies, reduce costs and expand applications through telemedicine. Future efforts should focus on identifying optimal patient cohorts and generating high-quality evidence to refine the clinical utility of robotic surgery.

5.The relationship between the preservation of the left colonic artery and the incidence of postoperative low anterior resection syndrome during low anterior resection for rectal cancer

Objective:To investigate the correlation between whether the left colonic artery(LCA) is preserved intraoperatively and the incidence of severe low anterior resection syndrome(LARS) in patients with rectal cancer.Methods:Clinical data of 255 rectal cancer patients undergoing low anterior resection from Jan 2020 to Jan 2022 at the First Affiliated Hospital of Xi'an Jiaotong University were retrospectively analysed, and the occurrence of LARS in patients with or without preserving the LCA was compared.Results:The LARS scores and severity in patients with intact LCA were better than those without preserved LCA at 12 months postoperatively ( P=0.017, 0.002). Bowel function at 6 months versus 12 months postoperatively in both groups was mainly reflected in a reduction in the number of bowel movements per day ( P=0.003, 0.001) as well as a reduction in re-voiding of the bowel within 1 hour after defecation ( P<0.001, P=0.001). Univariate and multivariate analyses showed that the distance of the lower edge of the tumour from the anal verge was ≤6 cm ( OR=2.530, 95% CI:1.307-4.900, P=0.006), preoperative neoadjuvant therapy ( OR=13.968, 95% CI: 4.969-39.260, P<0.001), prophylactic stoma ( OR=4.051, 95% CI: 2.042-8.040, P<0.001) were also independent risk factors for severe LARS after anterior resection of rectal cancer, and preservation of the left colonic artery ( OR=0.283, 95% CI: 0.142-0.563, P<0.001) was a protective factor against severe LARS after low anterior resection in rectal cancer patients. Conclusions:Intraoperative preservation of the LCA reduces the incidence and severity of postoperative LARS in patients with rectal cancer undergoing anterior resection. The distance of the lower edge of thetumour from the anal verge and prophylactic stoma are independent risk factors affecting the occurrence of severe LARS after anterior resection of rectal cancer.
6.Current status and prospects of minimally invasive surgical treatment for gastric cancer

Over the past three decades years, the treatment of gastric cancer has shifted from traditional open surgery to minimally invasive surgery. Cutting-edge technologies such as three-dimensional and 4K ultra-high-definition imaging systems, indocyanine green fluorescence navigation, and robotic surgical systems have been widely used in clinical practice, facilitating precise intraoperative anatomy and lymph node dissection. For early gastric cancer, endoscopic submucosal dissection, as the standard surgical procedure, can improve the 5-year survival rate and reduce the local recurrence rate. For locally advanced gastric cancer, neoadjuvant chemotherapy combined with minimally invasive surgery has increased the R0 resection rate and improved the prognosis of patients. For advanced gastric cancer, conversion therapy combined with minimally invasive surgery has brought hope for extended survival to patients with stage Ⅳ disease, and the application of immune checkpoint inhibitors has further promoted the progress of advanced gastric cancer treatment. With the advancement of technology and the improvement of policies, artificial intelligence and 5G remote surgery have become important directions in the minimally invasive surgical treatment of gastric cancer. In the future, it is necessary to accumulate evidence through multi-center prospective studies, optimize the evaluation of function-preserving surgery, develop cross-platform artificial intelligence tools, conduct cost-benefit analyses, and resolve ethical and legal disputes to promote the development of minimally invasive surgical treatment of gastric cancer towards precision and intelligence, achieving a dual improvement in efficacy and accessibility.
7.Inhibitory Effect of Hyperoside on Excessive Proliferation of Retinal Endothelial Cells Induced by High Glucose

OBJECTIVE To investigate the effect of hyperoside on high glucose-induced excessive proliferation of retinal endo-thelial cells(RECs)and its possible mechanism.METHODS Diabetic retinopathy(DR)models were established in male Sprague-Dawley(SD)rats.DR rats were treated with low-and high-dose hyperoside(DR+L-HY group and DR+H-HY group).Additional-ly,the normal control(NC)group,DR non-intervention(DR)group and DR+calcium dobesilate intervention(DR+CD)group were set up.The differences in the number of RECs in retinal blood vessels were observed and compared among all groups after intervention.In addition,RECs were inoculated into cell culture plates after normal culture and subculture.They were divided into 5 groups according to different treatments:normal glucose(NG)group,high glucose(HG)group,mannitol(MT)group,high glucose+low concentration of hyperoside(HG+H100)group and high glucose+high concentration of hyperoside(HG+H400)group.The activ-ity,cell migration and tubule formation of RECs in each group were detected and compared by CCK-8,cell migration and tubule for-mation assays.Western blot and qPCR were used to detect the expression of NADPH Oxidase 4(NOX4)and thioredoxin interacting protein(TXNIP)in each group.RESULTS The number of RECs in the DR group was significantly increased compared to the NC group(P<0.01).In contrast,the DR+L-HY,DR+H-HY,and DR+CD groups all showed significant decreases in RECs number compared to the DR group(P<0.05,P<0.01),and the reduction of RECs in the DR+H-HY group was significantly greater than that in the DR+L-HY group(P<0.05).Furthermore,the cell activity,migration number and tube formation number of RECs in the HG group were significantly higher than those in the NG group(P<0.05,P<0.01).The protein and mRNA expression levels of NOX4 and TXNIP in the HG group were also significantly higher than those in the NG group(P<0.01).However,the RECs activity,RECs mi-gration number and tube formation number in the HG+H100 group and the HG+H400 group were significantly lower than those in the HG group(P<0.05,P<0.01).The expression levels of NOX4 and TXNIP in both groups were significantly lower than those in the HG group(P<0.05,P<0.01),and the RECs activity,migration number,tube formation number,and the expression of NOX4 and TXNIP in the HG+H400 group were further significantly decreased compared with those in the HG+H100 group(P<0.01).CONCLU-SION Hyperoside could significantly inhibit the high glucose-induced excessive proliferation of RECs.The mechanism may be relat-ed to the inhibition of NOX4/TXNIP activation in high-glucose environment.
8.Preparation of the neutralizing monoclonal antibody against foot-and-mouth dis-ease virus serotype O and identification of antigenic epitope

Although no cross protection was observed between different serotypes of foot-and-mouth disease virus(FMDV),there were cross-reactivity between different serotypes of antibodies produced after vaccination,the aim of this paper was to prepare the neutralizing monoclonal anti-body against Foot-and-mouth disease virus(FMDV)serotype O,and to develop the method to dis-tinguish antibody against FMDV serotype O and A based on mAb.The inactivated FMDV serotype O was used as antigen in mAb production,a series of GST fusion overlapping peptides and trun-cated peptides expressed in Escherichia coli were used to identify antigenic epitope recognized by monoclonal antibodies.In order to verify feasibility of the screened monoclonal antibodies in diag-nosis,20 positive serum of FMDV serotype O and A,20 negative serum with known background were detected by blocking ELISA.Results were as follows:five monoclonal antibodies were suc-cessfully screened.The five monoclonal antibodies showed good reactivity with FMDV serotype O,but did not react with FMDV serotype A by Western blot and IFA,these mAbs showed neutrali-zing ability to FMDV/O/MY98/GZBY/2013 by VNT.The same epitope was identified by five monoclonal antibodies,the minimum epitope was145 RGDLQVLA152,Arg145 and Gln149 were key a-mino acids of the epitope.Sequence alignment analysis revealed that the identified epitopes were conserved among most of O type FMDV strains,but Gln149 was mutated among all A,Asia 1 and SAT1-3 type FMDV strains.The mAb-8C5D3 distinguished between antibody of FMDV serotype O and FMDV serotype A by blocking ELISA.The results provided materials for development of O type FMDV antibody detection kit and evaluation of vaccine immune effect.
9.Through the fog of minimally invasive development, leading the direction of minimally inva-sive technology

Minimally invasive surgery represented by laparoscopy has been carried out for more than 30 years, and the technological development has tended to be flat. Currently, the techno-logy is still improving and innovating on the platform of laparoscopy, from traditional laparoscopy to reduced port and single port laparoscopy, from transabdominal to natural orifice transluminal tech-niques, from standard definition and high-definition to 3D and 4K ultra high definition fluorescence guided laparoscopy. The booming development of robotic surgery is also based on the platform of laparoscopy. From the perspective of disease treatment, there is no revolutionary change, such as changing the long-term efficacy of tumor treatment. In this era of technological prosperity but uncertainty, the future of minimally invasive surgery may not be a single technological innovation, but the organic integration and common development of multiple technologies. The authors will focus on the advantages and challenges of various minimally invasive technologies, and look forward to the application prospects of artificial intelligence in precision technology and decision-making in minimally invasive surgery, in order to open up new paths for the sustainable development of mini-mally invasive surgery, overcome current confusion, and move towards a more accurate, efficient, and popularized future.
