1.Five-year survival analysis and influencing factors of elderly lung cancer patients with chronic obstructive pulmonary disease in Mianyang City

Objective To study the five-year survival status and influencing factors of elderly patients with lung cancer complicated with chronic obstructive pulmonary disease (COPD). Methods A cohort study was conducted to follow up 450 patients with lung cancer and chronic obstructive pulmonary disease who were hospitalized in our hospital from January 2018 to December 2023. The endpoint of the follow-up was the end of a five-year period or death. The Life Tables method was used to calculate survival rates and plot survival curves. The Cox proportional hazards model was used to analyze the influencing factors of five-year survival. Results The results indicated that the overall five-year survival rate of patients was 4.89%, and it decreased year by year. Cox regression analysis showed that age, gender, family functioning, and psychological status significantly influenced patient survival rate (all P<0.05). Stratified analysis found that the smoking status, family functioning, and psychological status of male patients all had an impact on survival rate (all P<0.05), while the psychological status of female patients had a more significant impact on survival (P=0.008). Conclusion This study provides a scientific basis for comprehensive intervention of elderly lung cancer patients with COPD. It is recommended that clinical attention should be paid to psychological and family factors to improve patient prognosis.
2.Comparison of the efficacy of remimazolam and propofol in the induction and maintenance of general anesthesia in elderly patients undergoing thoracoscopic lobectomy

OBJECTIVE To compare the clinical efficacy and safety of remimazolam and propofol in general anesthesia induction and maintenance for elderly patients undergoing thoracoscopic lobectomy. METHODS A total of 86 elderly lung cancer patients who underwent thoracoscopic lobectomy at Chongqing University Three Gorges Hospital from February to July 2024 were selected and divided into the propofol group and the remimazolam group according to the randomized numerical table method, with 43 cases in each group. During anesthesia induction, patients in the propofol group and the remimazolam group were intravenously administered 2 mg/kg of Propofol medium- and long-chain fat emulsion injection or 0.25 mg/kg of Remimazolam tosilate for injection, respectively; during anesthesia maintenance, the two groups received intravenous infusion of 6-10 mg/(kg·h) of Propofol medium- and long- chain fat emulsion injection or 1-3 mg/(kg·h) of Remimazolam tosilate for injection, respectively. The anesthesia effects, anesthesia-related indicators, intraoperative opioid and muscle relaxant dosages, Ramsay sedation score, numerical rating scale (NRS) score, and hemodynamic parameters were compared between the two groups, and the occurrence of adverse drug reactions was recorded. RESULTS A total of 41 patients in the propofol group and 43 patients in the remimazolam group completed the trial. The proportion of patients with grade Ⅰ anesthesia effect in the remimazolam group was significantly higher than that in the propofol group, while the proportion of patients with grade Ⅱ anesthesia effect was significantly lower than that in the propofol group (P<0.05). In this group, the disappearance time of eyelash reflex, the time taken for the bispectral index to drop to 60, and the Ramsay sedation scores (2 and 6 hours after operation) were all significantly prolonged or increased, while the recovery time, NRS scores (2 and 6 hours after operation), and the incidence of intraoperative hypotension were all significantly shortened or reduced; moreover, the improvements of the above sedation/NRS scores exhibited a time-dependent pattern within 2 to 24 hours after operation (P<0.05). Compared with before anesthesia induction (T0), the heart rate [except at 2 min after medication (T1), 60 min after anesthesia (T4), and at the end of surgery (T5) in the remimazolam group] and mean arterial pressure [except at T1 in the remimazolam group] of patients in both groups significantly decreased at T1, 5 min after medication (T2), at the start of surgery (T3), T4, and T5 (P<0.05). Meanwhile, regional cerebral oxygen saturation significantly increased in both groups. Furthermore, the heart rate and mean arterial pressure of patients in the remimazolam group were significantly higher than those in the propofol group at T1, T2 and T4 (P<0.05). No statistically significant differences were observed between the two groups in terms of postanesthesia care unit stay time, dosage of opioids and muscle relaxants, regional cerebral oxygen saturation, or peripheral oxygen saturation at various time points (P>0.05). CONCLUSIONS Compared to propofol, remimazolam demonstrates superior anesthesia effects when used for the induction and maintenance of general anesthesia in elderly patients undergoing thoracoscopic lobectomy. It not only provides more stable intraoperative hemodynamics and shortens the postoperative recovery time but also effectively reduces the incidence of intraoperative hypotension.
3.Study on Quantitative Evaluation Method of Balance Ability in Cancer Patients Based on Gait Features.

The importance of gait assessment in the rehabilitation of cancer patients is gradually being recognized. However, quantitative analysis of balance ability in cancer patients is still limited. A total of 102 cancer patients meeting the inclusion criteria were recruited from Hefei Cancer Hospital, Chinese Academy of Sciences. Their balance ability was evaluated using the Berg Balance Scale (BBS). Gait data were collected by an electronic walkway and an IMU sensor system, including spatial-temporal and kinematic gait features such as step length, cadence, support time, and range of motion. Recursive feature elimination was used for feature selection. Ridge, Elastic Net, SVR, RF, and AdaBoost models were used to predict balance ability scores. Five-fold cross-validation was used to evaluate the performance of these models. Results show that the SVR model achieves the best performance with fifteen features (RMSE=3.22, R 2=0.91), followed by Ridge (RMSE=3.63, R 2=0.89). A method for evaluating balance ability based on gait features is proposed, providing a quantitative tool for personalized rehabilitation interventions in cancer patients.

4.Treatment for lower extremity arteriosclerosis obliterans solely using intravascular ultrasound-guided endovascular intervention:A case report and literature review

Lower extremity arteriosclerosis obliterans(LEASO)is a disease characterized by the formation of atherosclerotic plaques in the lower extremity arteries,leading to arterial stenosis,occlusion,and consequent chronic limb ischemia.This article analyzes the clinical data of a patient with LEASO complicated by an iodine contrast agent allergy,along with the treatment involving intravascular ultrasound(IVUS)-guided endovascular intervention performed without contrast agent,and reviews relevant literature.The patient,a 53-year-old female,presented with coldness and numbness in the left lower extremity and pain after walking for 2 years,symptoms which had worsened over the past 2 months,with a maximum claudication distance of less than 50 meters.The lower extremity arterial CT angiography(CTA)results indicated"left iliac artery occlusion and left superficial femoral artery stenosis."After examination,the patient exhibited allergic symptoms.Upon admission,other diseases were ruled out.Given that the lower extremity symptoms severely impacted the patient's daily life and considering the potential for contrast agent allergy,the use of contrast agent was abandoned.IVUS guidance alone was employed to assess the intravascular condition for the interventional procedure.The surgical interventions chosen were drug-coated balloon angioplasty of the left superficial femoral artery and stent implantation in the left iliac artery.Postoperatively,the patient was able to walk normally,the discomfort resolved,and the treatment outcome was satisfactory.For LEASO patients with contrast agent allergy or other contraindications to contrast use,the sole use of IVUS guidance combined with invasive blood pressure monitoring to intraoperatively assess efficacy can effectively avoid contrast agent administration,providing a valuable reference for clinicians managing such cases.
5.The Effect of Mentality-Controlling on Local Microcirculation of Acupuncture at Hegu (LI 4) by Application of Laser Speckle Contrast Imaging

ObjectiveTo explore the effect of mentality-controlling on local microcirculation of acupuncture at Hegu (LI 4). MethodsForty healthy subjects were randomly divided into simple acupuncture group and mentality-controlling acupuncture group, with 20 cases in each group. The simple acupuncture group was acupunctured at Hegu (LI 4) on the right hand of the patient. In mentality-controlling acupuncture group, an audio clip was played to guide the participants to control their mentality when needle retention for 5 min after performing acupuncture twirling mani-pulation. The microcirculatory blood perfusion units (MBPU) was captured and compared between groups of participants using laser speckle contrast imaging (LSCI) for 5 mins in each of the four stages, state before acupuncture (R1), after needle insertion (A1), after needle twirling (A2) and after needle withdrawal (R2). The participants' feelings such as relaxation, concentration, drowsiness, energy flow at acupoints were compared between groups, and the adverse reactions were recorded. ResultsThe MBPU values of R1, A1, A2 and R2 stages in mentality-controlling acupuncture group were 67.92±18.40 PU, 168.13±46.03 PU, 144.19±44.40 PU, 159.73±65.21 PU, respectively. The simple acupuncture group were 79.85 (66.80, 99.40)PU, 193.31 (173.15, 224.80)PU, 147.68 (128.12, 198.30)PU, 158.09 (126.05, 207.59)PU respectively. There was a statistically significant difference in MBPU between groups of the four stages (P<0.01). Among them, the MBPU in the A1 stage of both groups was significantly larger than that in the R1 stage, the MBPU in the A2 stage significantly smaller than that in the A1 stage, and the MBPU in the R2 stage was significantly larger than that in the R1 stage (P<0.01). MBPU in the R2 stage of the mentality-controlling acupuncture group was larger than that in the A2 stage (P<0.05). The difference of MBPU between A2 and A1 in mentality-controlling acupuncture group was significantly smaller than that in the simple acupuncture group, and the difference was statistically significant (t = 3.79, P<0.01). In mentality-controlling acupuncture group, 18 participants felt relaxed (90%), 15 concentrated (75%), 3 felt sleepy (15%), and 14 felt energy flow at acupoints (70%); while in the simple acupuncture group, 13 participants felt relaxed and sleepy (65%), 6 concentrated and 6 felt energy flow at acupoints (30%). The number of participants with concentration and energy flow at acupoints in mentality-controlling acupuncture group was higher than that in the simple acupuncture group, and the number felt sleepy was lower than that in the simple acupuncture group (P<0.05). No needle fainting, needle site infection, local haematoma and any other adverse reactions were found in both groups. ConclusionAcupuncture could affect the changes of local microcirculation, and controlling mentality during acupuncture can slow the rate of MBPU reduction and enhance the post-acupuncture effect with a favourable safety profile.
6.Development and evaluation of a rapid and sensitive POC chemiluminescent assay for β-human chorionic gonadotropin

Objective:To develop and evaluate a rapid and sensitive point-of-care chemiluminescent assay(POC-CLIA)for β-human chorionic gonadotropin(β-HCG).Methods:POC-CLIA was constructed based on alkaline phosphatase(Alp)-AMPPD lumi-nescence system and magnetic particles(Mps)carrier.Performance of POC-CLIA,including sensitivity,precision,accuracy,linear dilution,specificity,stability,hook effect and clinical application were evaluated.Results:Detection limit of β-HCG was 0.71 mU/ml,linear detection range was 0.710~1.092×104 mU/ml,and was no hook effect up to 1.7×105 mU/ml.Intra and inter batch coefficients of variation were less than 10%,and could be stored stably at 37℃ for 10 days.Accuracy deviation was within±10%,so results were reliable.There was no cross-reactivity between interfering substances and anti-β-HCG antibdies.For detecting β-HCG in 100 clinical serum samples,results were highly correlated with those that were tested by clinical standard methods(R2=0.997 0).Turnaround time for single sample was less than 15 min and throughput could reach 200 T/h.Conclusion:This method is adequate that can be widely used in grassroots communities to help large-scale screening of pregnancy and related diseases.
7.Research on the construction of core medical skill index system for helicopter medical rescue personnel

Objective:To construct core medical skill index system for helicopter medical rescue personnel by Delphi expert consensus method.Methods:This study combined literature review and two rounds of Delphi expert correspondence to construct the core skill index system. Total of 22 experts from relevant disciplines were selected purposively to conduct Delphi expert consultation.Results:The effective questionnaire recovery rates of the two rounds of Delphi expert correspondence was 100%; the authority coefficient of experts was 0.87; the Kendall coordination coefficients of the importance of the first and second level indexes were 0.380 and 0.338 in the first round of correspondence, and then 0.415 and 0.357 in the second round. the Kendall coordination coefficients of the feasibility of the second level indexes were 0.347 in the first round of correspondence and 0.354 in the second round. The final core skill index system for helicopter medical rescue personnel includes 4 primary indicators: core medical skill for diseases or trauma in respiratory system, circulatory system, sports system and other system, and 32 secondary indicators.Conclusions:The core medical skill index system for helicopter medical rescue personnel constructed through two rounds of Delphi expert correspondence is scientific and reasonable, which can provide a scientific basis to accurately understand, describe, analyze, and evaluate the competence level of helicopter medical rescue personnel, as well as serve as a content framework for training programs.
8.Research Progress on Traditional Chinese Medicine in Treating Viral Hepatitis: A Review

Viral hepatitis is an important cause of liver cirrhosis and liver cancer, which has become a major public health problem in the world. Traditional Chinese medicine has unique advantages in treating viral hepatitis, which can inhibit virus replication and enhance immunity. It can effectively prevent liver fibrosis and canceration, improving liver function and symptoms significantly with definite clinical curative effects, a high level of safety, and seldom drug resistance. In addition, it reduces the side effects of western medicine, achieving the effect of synergy and attenuation while reducing the recurrence rate of patients after drug withdrawal. Attention has been paid to the research on the treatment of viral hepatitis with traditional Chinese medicine, and great progress has been made in experimental research and clinical practice. In this paper, the research progress of traditional Chinese medicine in the treatment of viral hepatitis at home and abroad in recent five years was systematically reviewed. Modern research has confirmed that traditional Chinese medicine can play a role in the treatment of viral hepatitis by directly or indirectly inhibiting the virus, anti-inflammatory, anti-fibrosis, anti-oxidation, regulating immunity, regulating autophagy, and other signal pathways. In clinics, traditional Chinese medicine compound or combined with western medicine is often adopted to ameliorate the clinical symptoms of patients such as fatigue and loss of appetite, improve the immune mechanism of the body, enhance the antiviral ability, shorten the treatment course of patients and improve their quality of life. The research provides a reference for pharmacological research, clinical research, and new drug development for viral hepatitis.
9.Role and significance of deep learning in intelligent segmentation and measurement analysis of knee osteoarthritis MRI images

BACKGROUND:MRI is important for the diagnosis of early knee osteoarthritis.MRI image recognition and intelligent segmentation of knee osteoarthritis using deep learning method is a hot topic in image diagnosis of artificial intelligence. OBJECTIVE:Through deep learning of MRI images of knee osteoarthritis,the segmentation of femur,tibia,patella,cartilage,meniscus,ligaments,muscles and effusion of knee can be automatically divided,and then volume of knee fluid and muscle content were measured. METHODS:100 normal knee joints and 100 knee osteoarthritis patients were selected and randomly divided into training dataset(n=160),validation dataset(n=20),and test dataset(n=20)according to the ratio of 8:1:1.The Coarse-to-Fine sequential training method was used to train the 3D-UNET network deep learning model.A Coarse MRI segmentation model of the knee sagittal plane was trained first,and the rough segmentation results were used as a mask,and then the fine segmentation model was trained.The T1WI and T2WI images of the sagittal surface of the knee joint and the marking files of each structure were input,and DeepLab v3 was used to segment bone,cartilage,ligament,meniscus,muscle,and effusion of knee,and 3D reconstruction was finally displayed and automatic measurement results(muscle content and volume of knee fluid)were displayed to complete the deep learning application program.The MRI data of 26 normal subjects and 38 patients with knee osteoarthritis were screened for validation. RESULTS AND CONCLUSION:(1)The 26 normal subjects were selected,including 13 females and 13 males,with a mean age of(34.88±11.75)years old.The mean muscle content of the knee joint was(1 051 322.94±2 007 249.00)mL,the mean median was 631 165.21 mL,and the mean volume of effusion was(291.85±559.59)mL.The mean median was 0 mL.(2)There were 38 patients with knee osteoarthritis,including 30 females and 8 males.The mean age was(68.53±9.87)years old.The mean muscle content was(782 409.18±331 392.56)mL,the mean median was 689 105.66 mL,and the mean volume of effusion was(1 625.23±5 014.03)mL.The mean median was 178.72 mL.(3)There was no significant difference in muscle content between normal people and knee osteoarthritis patients.The volume of effusion in patients with knee osteoarthritis was higher than that in normal subjects,and the difference was significant(P<0.05).(4)It is indicated that the intelligent segmentation of MRI images by deep learning can discard the defects of manual segmentation in the past.The more accuracy evaluation of knee osteoarthritis was necessary,and the image segmentation was processed more precisely in the future to improve the accuracy of the results.
10.Genetic stability analysis of two recombinant rotaviruses harboring foreign gene insertions within the NSP3 region

Objective:To analyze the genetic stability of two recombinant rotaviruses (rLLR/NSP3 NLuc) and (rLLR/NSP3 CoV2/RBD) that are inserted and express exogenous genes for continuous passage and proliferation on MA104 cells.Methods:After measuring the titers of two recombinant rotavirus strains, they were transferred to the P2 generation according to MOI0.01. Subsequently, the previous generation of virus lysate was diluted and activated at 1∶100, and MA104 cells were continuously infected for 18 generations (P20). The virus titers of the P1, P5, P10, P15, and P20 generation of cell lysate were measured using indirect immunofluorescence, and RT-PCR identification and dsRNA PAGE silver staining were performed. The luciferase activity of rLLR/NSP3-NLUc was also detected.Results:No inserted fragment loss was found in the recombinant rotavirus rLLR/NSP3 NLuc within 20 generations, with recombinant virus titers ranging from 3.85~5.16 × 10 6 FFU/ml, with strong luciferase signals in each generation. The recombinant rotavirus rLLR/NSP3 CoV2/RBD showed loss of inserted fragments in the 6th generation, with infectivity titers ranging from 2.6 to 3.36 in the first 5 generations of the recombinant virus × 10 6 FFU/ml. Conclusions:The recombinant rotavirus with 582 bp NLuc inserted at the end of the NSP3 gene has good genetic stability, while the recombinant rotavirus with 885 bp RBD inserted at the end of the NSP3 gene was only stable in the first 5 generations, indicating that foreign genes can be inserted at the end of the NSP3 gene of the recombinant rotavirus, and the insert can express, but its stability requires more in-depth research.
