1.Methodological establishment of red blood cell lysis method for handling Rh typing double group samples

Objective: To establish an accurate and rapid typing method for Rh typing of samples from patients who have received recent blood transfusions by utilizing the difference in osmotic fragility between fresh and old red blood cells. Methods: A lysing solution suitable for destroying old RBCs was prepared. Sixty-one samples collected in our hospital in 2024 with Rh typing of double groups were treated with the lysing solution to remove the old allogeneic red blood cells while preserving the patient's own fresh red blood cells, followed by repeat Rh typing tests. Results: For 61 samples with Rh typing in double groups, 41 were accurately detected identified through the red blood cell lysis method, yielding an identification rate of 67.21%. No significant difference was observed compared to the detection rate of the commonly used capillary centrifugation modified method (χ
=0.103, P>0.05). Conclusion: The red blood cell lysis method provides a novel and rapid experimental approach for clinical use in processing Rh-typed samples that are of double groups, thereby offering a basis for Rh compatibility blood transfusion.
2.Quercetin inhibits proliferation and migration of clear cell renal cell carcinoma cells by regulating TP53 gene.

OBJECTIVES:
To identify potential molecular targets of quercetin in the treatment of clear cell renal carcinoma (ccRCC).
METHODS:
The therapeutic targets of quercetin were screened from multiple databases by network pharmacology analysis, and the targets significantly correlated with ccRCC were screened from 4907 plasma proteins using a Mendelian randomization method. The drug-disease network model was constructed to screen the potential key targets. The functions of these targets were evaluated via bioinformatics analysis, and the screened targets were verified in cultured ccRCC cells.
RESULTS:
Network pharmacology analysis combined with Mendelian randomization identified TP53 (OR=3.325, 95% CI: 1.805-6.124, P=0.0001), ARF4 (OR=0.173, 95% CI: 0.065-0.456, P=0.0003), and DPP4 (OR=0.463, 95% CI: 0.302-0.711, P=0.0004) as the core targets in quercetin treatment of ccRCC. Bioinformatics analysis showed that TP53 was highly expressed in ccRCC, and patients with high TP53 expressions had worse survival outcomes. Molecular docking studies showed that the binding energy between quercetin and TP53 was -5.83 kcal/mol. In cultured 786-O cells, CCK-8 assay and wound healing assay showed that treatment with quercetin significantly inhibited cell proliferation and migration. Quercetin treatment also strongly suppressed the expression of TP53 at both the mRNA and protein levels in 786-O cells as shown by RT-qPCR and Western blotting.
CONCLUSIONS
TP53 may be the key target of quercetin in the treatment of ccRCC, which sheds light on potential molecular mechanism that mediate the therapeutic effect of quercetin.

This paper focuses on extreme environments and systematically analyzes sleep disorders caused by multiple pathogenesis,including circadian rhythm disorder,yin-yang imbalance and qi-blood disorder under weightlessness,emotional depression due to environmental changes,abnormal diet and excretion,and six excesses pathogenic factors,in accordance with the theory of correspondence between nature and human.It proposes harmonizing yin and yang as the core therapeutic principle and guiding framework,with specific methods including calming rebellious qi-blood,regulating qi movement,harmonizing heart and kidney,and dredging and regulating blood vessels,thus forming the"one guiding principle and four specific methods"treatment strategy.Additionally,by integrating the modified application of classic formulas,a diagnostic and therapeutic approach targeting multi-dimensional pathogenesis is established.This study aims to promote the in-depth integration of Traditional Chinese Medicine(TCM)in extreme environmental medicine through TCM theories and innovative application of prescriptions,provide TCM-characterized solutions for health management of workers in extreme environments,and facilitate the transformation of extreme environmental medicine toward the modern medical model of"prevention-treatment integration".
4.Therapeutic effect of endoscopic retrograde appendicitis therapy for the diagnosis and treatment of pediatric uncomplicated appendicitis

Objective:To compare the effects of the endoscopic retrograde appendicitis therapy (ERAT) assisted with 3 different methods in the diagnosis and treatment of children with uncomplicated appendicitis, and to explore the most effective ERAT.Methods:A case series study was made.The clinical data of 206 children with uncomplicated appendicitis who underwent ERAT in Ji′nan Children′s Hospital from January 2019 to December 2023 were retrospectively analyzed.The patients were divided into 3 groups according to the auxiliary treatment method adopted.There were 13 cases receiving X-ray guided ERAT (ERAT-X) group, 109 cases receiving B-ultrasound guided ERAT(ERAT-B) group and 84 cases receiving dong cha endoscope guided ERAT (ERAT-DC) group.Appendix cavity exploration, irrigation and fecalith removal were performed by ERAT.Perioperative data such as endoscopic manifestations, abdominal pain and the success rate of intubation and follow-up data were recorded and compared among the three groups of patients.The one-way ANOVA method, Wilcoxon sign rank sum test and Kruskal-Wallis H test were used to analyze measurement data, and the χ2 test was used to analyze enumeration data. Results:A total of 201 cases were successfully intubated, including 12 in the ERAT-X group, 106 in the ERAT-B group, and 83 in the ERAT-DC group.The mucosa of the appendix showed different inflammatory changes under the endoscope.There were no significant differences in abdominal pain score before treatment, abdominal pain score after treatment, success rate of intubation, effective rate of treatment, complication rate, antibiotic use rate and length of hospital stay among the three groups (all P>0.05).The abdominal pain score after treatment was significantly lower than that before treatment in all the 3 groups[0(1.00) points vs.4(1.50) points/0(1.00) points vs.4(2.00) points/0(1.00) points vs.4(1.75) points, Z=-3.082, -8.939, -8.009, all P<0.05].The diagnostic rate of the ERAT-DC group was lower than that of the ERAT-B group (75.0% vs.95.4%, P<0.05).The diagnostic specificity of ERAT-DC group was better than that of the ERAT-B group.The operation time of the ERAT-DC group was significantly reduced, compared with that of ERAT-X and ERAT-B groups[38.5(34.3) min vs.85.0(42.5) min vs.67.0(27.0) min, Z=80.075, 55.865, all P<0.05].The hospitalization cost of the ERAT-DC group was significantly higher than that of ERAT-X and ERAT-B groups[(17 269±1 035) yuan vs.(12 081±921) yuan vs.(11 609±1 479) yuan, Z=-89.308, -104.633, all P<0.05].Follow-up results showed that the recurrence rate of abdominal pain (9.1%, 16.7%, 14.8%) and the rate of unplanned reoperation (9.1%, 10.8%, 8.6%) showed no significant difference among the 3 groups(ERAT-X/ERAT-B/ERAT-DC) (all P>0.05). Conclusions:ERAT is a safe and effective therapy in treating children with uncomplicated appendicitis.EART-DC is more accurate and reliable method for the diagnosis and treatment of pediatric appendicitis, so it is worthy of widespread clinical application.
5.Repair effect of ginseng polypeptide thermosensitive hydrogel on heat-induced skin injury in rats and its mechanism

Objective:To prepare a novel ginseng polypeptide thermosensitive hydrogel,and to investigate its repair effect on heat-induced skin injury in the rats and explore the underlying mechanisms.Methods:Thermosensitive hydrogels were formulated using Pluronic F127 and β-sodium glycerophosphate(β-GP),and their phase transition temperatures,spatial structures,elemental compositions,and water retention capacities were evaluated.The rat models of heat-induced skin injury were established and the model rats were divided into PBS group,Gel group,and ginseng polypeptide gel(GP-Gel)group.After 11 d of treatment,the morphological changes of wound and collagen deposition in the wound of the rats in various groups were observed by HE and Masson staining.Immunohistochemistry was used to detect the expression levels of α-smooth muscle actin(α-SMA),connective tissue growth factor(CTGF),basic fibroblast growth factor(bFGF),proliferating cell nuclear antigen(PCNA),cell proliferation marker Ki67,epidermal growth factor(EGF),CD31,vascular endothelial growth factor(VEGF),P50 and P65 proteins in the skin wound tissue of the rats in various groups.Western blotting method was used to detect the expression levels of Toll-like receptor 4(TLR4)in the skin wound tissue of the rats in various groups.ELISA method was used to measure the levels of tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),interleukin-6(IL-6),interleukin-15(IL-15),and interleukin-10(IL-10)in the serum of the rats in various groups.Results:Compared with PBS and Gel groups,the wound area of the rats in GP-Gel group was reduced(P<0.01),the expression levels of PCNA,Ki67,EGF,CD31,VEGF,α-SMA,and CTGF proteins in the skin wound tissue were increased(P<0.05 or P<0.01),and the expression levels of P65 and TLR4 proteins were decreased(P<0.01);the level of anti-inflammatory factor IL-10 in serum was increased(P<0.01),while the levels of pro-inflammatory factors TNF-α,IL-1β,IL-6 and IL-15 were decreased(P<0.05 or P<0.01).Conclusion:The ginseng polypeptide thermosensitive hydrogel promotes the repair of heat-induced skin injury by enhancing cell proliferation,collagen synthesis,angiogenesis,and reducing inflammatory responses.
6.Hematopoietic stem cell and kidney transplantation from the same donor in a patient with acute myeloid leukemia and literature review

The paper reports a 32-year-old female acute myeloid leukemia patient who developed graft-versus-host disease after paternal hematopoietic stem cell transplantation, which subsequently led to renal thrombotic microangiopathy. She subsequently required a kidney transplant from the same donor 5 years later due to renal failure. Considering that both the bone marrow and kidney were from the same donor and the recovery of renal function was favorable, immunosuppressive therapy was discontinued after a short course of anti-rejection treatment, with maintained stable kidney function. This case suggests that under the condition of high chimerism, allogeneic hematopoietic stem cell transplantation and kidney transplantation from the same donor can achieve immune tolerance, potentially improving solid organ transplantation success rate. The findings provide a novel therapeutic approach for solid organ transplantation following allogeneic hematopoietic stem cell transplantation.
7.Analysis of salary satisfaction and influencing factors of medical staff in public hospitals in Guangxi

Objective:To analyze the salary satisfaction and influencing factors of medical staff in public hospitals in Guangxi under the background of salary system reform, and to provide suggestions for deepening the salary system reform of public hospitals in Guangxi.Methods:From July to December 2022, medical staff from 39 public hospitals that have carried out salary system reform were selected through stratified random sampling, and self-made questionnaires were used to survey them, which mainly included salary, salary expectations and salary satisfaction. Descriptive analysis of the questionnaire data was performed, and binary logistic regression was used to analyze the influencing factors of salary satisfaction.Results:A total of 10 299 valid questionnaires were obtained. 6 869 (66.7%) medical staff had a lower average monthly income after tax than those employed in urban non-private units in Guangxi in that year; 8 100 (78.6%) medical staff expected to be paid 30%~50% higher than the current salary level; 4 073 (39.5%) were satisfied with the overall salary satisfaction, of which 2 377 (23.1%) were satisfied with the salary increase after the reform of the salary system in 2017. The results of multivariate analysis showed that education, hospital type, hospital level, professional title, position and job type were the main influencing factors of salary satisfaction of medical staff ( P<0.05), and hospital level interacted with education, hospital type and job type respectively ( P<0.05). Conclusions:The external competitiveness of the salary of medical staff in Guangxi was insufficient, and the salary satisfaction was not high. It is suggested to pay attention to the inter-embeddedness of the reform of the salary system and the reform of the " three-medical linkage" , pay attention to the balanced development of different levels and types of hospitals, scientifically calculate the value coefficient of different positions, and effectively improve the salary level of medical staff.
8.Mechanisms of Wuzi Yuye Decoction in the treatment of diabetic oligoasthenozoospermia:Based on network pharmacology

Objective The aim of this study is to systematically analyze the therapeutic mechanisms of Wuzi Yuye Decoction in diabetic oligoasthenozoospermia based on network pharmacology and animal experiments.Methods Active components and therapeutic targets of Wuzi Yuye Decoction were screened by integrating multiple databases,and their inter-section with diabetic oligoasthenozoospermia-related targets was obtained.Common targets were identified using the Venny platform,and protein-protein interaction(PPI)networks were constructed via the STRING database.And key targets were screened through topological analysis with Cytoscape.Gene Ontology(GO)functional enrichment and Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway analyses were performed using the DAVID platform.A mouse model of diabetic oligoasthenozoospermia was established by high-fat diet combined with streptozotocin induction,followed by intervention with Wuzi Yuye Decoction.Body weight,testicular index,and sperm quality were assessed,while histopathological changes were evaluated by HE staining and transmission electron microscopy(TEM).In addition,ELISA,oxidative stress assays and Western blot were used to verify the changes in key pathway-related proteins.Results A total of 36 common targets were identified,including key nodes such as TNF,IL6 and IL1B.Enrichment analysis indicated that these targets were sig-nificantly involved in inflammatory responses,oxidative stress,autophagy-apoptosis regulation and reproduction-related pathways.Animal experiments confirmed that Wuzi Yuye Decoction markedly improved testicular mass and sperm motility in diabetic mice,reduced the levels of AGE,TNF-α,IL-6 and IL-1β in testicular tissues,alleviated oxidative stress damage,and restored autophagic flux and blood-testis barrier function by inhibiting the RAGE/NF-κB/mTOR signaling axis.Conclusion Wuzi Yuye Decoction improves the pathological progression of diabetic oligoasthenozoospermia by synergistically modulating multiple targets,including inflammation inhibition,antioxidant effects,autophagy regulation,and related pathways.
9.Research progress of tadalafil in the treatment of erectile dysfunction

Erectile dysfunction(ED)is a common sexual dysfunction in adult males.With the accelerated pace of modern life and lifestyle changes,the prevalence of ED and its associated comorbidities have been steadily rising.Problems such as premature ejaculation,hypertension,hyperlipidemia,diabetes,prostate diseases,and infertility all interact with and aggravate ED,thereby endangering the overall health of men.Phosphodiesterase type 5 inhibitors(PDE5i)is the first-line pharmacotherapy for ED.Tadalafil,currently the only long-acting PDE5i approved for clinical use,has received mar-keting authorization in China for the treatment of ED since 2005.In 2013,the once-a-day continuous regimen was intro-duced as a novel treatment paradigm.And the indication was expanded to ED coexisting with benign prostatic hyperplasia in 2019.Accumulating clinical experience and evidence-based data consistently demonstrate its efficacy and safety across ED and ED-related comorbidities.This review summarizes the pharmacological profile of tadalafil and the latest clinical evi-dence on the management of ED and ED-related comorbidities,aiming to provide a reference for clinical practice.
10.Application Value of Pancreatic Duct-to-Jejunum Stent-Bridging Internal Drainage in Robotic Central Pancreatectomy

Objective To conduct preliminary evaluation of the reliability and safety of pancreatic duct-to-jejunum stent-bridging internal drainage as a supplementary approach to pancreaticojejunostomy in central pancreatectomy.Methods The clinical data of 28 patients who underwent robotic central pancreatectomy performed by our team between January 2021 and November 2024 were retrospectively collected,and and follow-up of postoperative endocrine and exocrine functions was performed.Based on the methods of digestive tract reconstruction adopted,the patients were divided into a conventional pancreaticojejunostomy group and a pancreatic duct-to-jejunum stent-bridging internal drainage group(PancreaticoJejunal-Stent bridge group).The operative time,digestive tract reconstruction time,and short-term complications were compared between the two groups.Results Among patients undergoing robotic central pancreatectomy,the digestive tract reconstruction time was shorter(t=5.168,P<0.001)in the PancreaticoJejunal-Stent bridge group([31.1±6.3]min)than that in the conventional pancreaticojejunostomy group([49.7±8.9]min)(t=5.168,P<0.001).The total operative time was(172.7±64.6)min in the PancreaticoJejunal-Stent bridge group and(200.1±52.7)min in the conventional pancreaticojejunostomy group,showing no statistically significant difference(t=1.215,P=0.235).In the PancreaticoJejunal-Stent bridge group,one patient developed a postoperative biochemical fistula,and 14 patients developed grade B pancreatic fistulas.Among the 14 patients with grade B pancreatic fistulas,1 case was complicated by fistula-related intra-abdominal infection,and 13 cases had drainage tube retention time of more than 21 days.In the conventional pancreaticojejunostomy group,2 patients developed postoperative biochemical fistulas,and 11 patients developed grade B pancreatic fistulas.Among the 11 patients with grade B pancreatic fistulas,1 case was complicated by fistula-related intra-abdominal infection,and 1 case was complicated by fistula-related intra-abdominal bleeding and infection.No postoperative gastroparesis,pancreatitis,or grade C pancreatic fistulas occurred in either group.There were no statistically significant differences between the two groups in overall postoperative complication rate(P=0.522),postoperative pancreatic fistula rate(P=0.583),intra-abdominal infection rate(P=0.583),or bleeding rate(P=0.464).Conclusion Pancreatic duct-to-jejunum stent-bridging internal drainage optimizes the anastomosis between the distal end of the pancreas and the jejunum during central pancreatectomy,shortens digestive tract reconstruction time,and reduces surgical complexity without increasing the risk of short-term severe postoperative complications.This approach is safe and feasible.
