1.Correlation between rectal colonization and bloodstream infections of carbapenem-resistant Klebsiella pneumoniae infection: a study based on molecular epidemiology

Objective:To investigate the correlation between rectal colonization of carbapenem resistant Klebsiella pneumoniae(CRKP)and bloodstream infections(BSI)using molecular epidemiological analysis. Methods:Patients admitted to the Intensive Care Unit(ICU),Hematology Department,and Neurosurgery Department of the First Hospital of Jiaxing from January 2022 to December 2024,were enrolled. Rectal CRKP colonization screening was performed for all participants,with concurrent monitoring for BSI.Whole genome sequencing of CRKP strains in the intestine and blood flow of patients with CRKP rectal colonization and CRKP-BSI was performed using the Illumina NovaSeq PE150 sequencing platform,and samples were genotyped based on the PubMLST database. MLST 2.0 was applied for multi site sequence typing,VFDB online database was used to analyze virulence genes,ResFinder was used to analyze resistance genes,and whole genome sequences were imported into BioNumerics software for core genome multi site sequence typing and clustering analysis. Using the BacWGSTdb database to construct a phylogenetic tree based on genomic SNPs,and the homology between CRKP rectal fixed plants and corresponding BSI-CRKP infected plants were analyzed.Results:A total of 772 patients were included,including 78 cases with positive results in rectal CRKP colonization screening(10.1%)and 694 cases without rectal CRKP colonization(89.9%). The CRKP-BSI rate in rectal CRKP colonization patients was significantly higher than that in non-CRKP colonization patients[19.2%(15/78) vs. 5.5%(38/694), χ2=20.749, P<0.001]. Analysis of CRKP rectal colonization strains and bloodstream infection strains in 15 patients with CRKP rectal implantation and CRKP-BSI revealed that ST11 type was the main strain( n=10),followed by ST37 type( n=3),with all carrying multiple β-lactam and carbapenem producing enzyme resistance genes.The distribution of virulence genes showed that CRKP strains carried multiple virulence genes,with iroE being ubiquitous,followed by iucA/ B/ C/ D, rmpA2,rmpA,and iroN. All ST11-type CRKP strains exhibited hypervirulent characteristics. Capsular serotyping analysis showed that the predominant type of CRKP colonization and infection strains was KL64. The results of cgMLST and SNP clustering analysis showed that CRKP rectal fixed plants exhibited homology with blood flow infected plants. Moreover,two clusters of CRKP rectal colonization strains with significant homology were found to cluster together among 15 patients. Conclusions:Rectal colonization of CRKP is an important risk factor for the occurrence of BSI-CRKP in hospitals,and ST11 hypervirulent CRKP is the main type. It is recommended to screen high-risk patients for CRKP to reduce the risk of BSI-CRKP.
2.Effect of Buyang Huanwu Decoction Combined with Tirofiban on Cerebral Oxygen Metabolism and Neurological Function in Patients with Ischemic Stroke

Objective To observe the effect of Buyang Huanwu Decoction combined with Tirofiban on cerebral oxygen metabolism and neurological function in patients with ischemic stroke.Methods A total of 110 patients with ischemic stroke of qi deficiency and blood stasis type who were treated at Rizhao Central Hospital from October 2022 to October 2024 were selected.The patients were randomly divided into a control group and a combined group using a random number table,with 55 patients in each group.The control group received intravenous infusion of Tirofiban,while the combined group was given additional Buyang Huanwu Decoction orally on the basis of treatment for the control group.The course of treatment for both groups covered 2 weeks.Before and after treatment,the changes in traditional Chinese medicine(TCM)syndrome scores,National Institutes of Health Stroke Scale(NIHSS)scores,oxidative stress response,neurological function,platelet function,and indicators associated with cerebral oxygen metabolism in both groups were observed.Results(1)After treatment,the scores of TCM syndromes of sudden dizziness,hemiplegia,shortness of breath and fatigue,and pale and dull complexion were decreased in both groups compared with those before treatment(P<0.05),and the decrease in the combined group was significantly superior to that in the control group(P<0.01).(2)After treatment,the levels of serum oxidative stress response indicators of glutathione peroxidase(GSH-Px)and superoxide dismutase(SOD)were increased(P<0.05),while the levels of serum oxidized low-density lipoprotein(ox-LDL)and malondialdehyde(MDA)were decreased(P<0.05)in both groups compared with those before treatment.The combined group showed more significant increase in serum GSH-Px and SOD levels and more significant decrease in serum ox-LDL and MDA levels than the control group(P<0.01).(3)After treatment,the platelet function indicators of platelet aggregation rate and platelet adhesion rate were decreased in both groups compared with those before treatment(P<0.05),and the decrease in the combined group was significantly superior to that in the control group(P<0.01).(4)After treatment,arterial oxygen content(CaO2)was increased(P<0.05),while the levels of cerebral oxygen extraction rate(ERO2),and cerebral arteriovenous oxygen content difference[D(a-jv)O2]were decreased in both groups compared with those before treatment(P<0.05),and the changes of the levels in the combined group were more obvious than those in the control group(P<0.01).(5)Ater treatment,the neurological function indicators of NIHSS scores and serum neuron-specific enolase(NSE)level were decreased in both groups compared with those before treatment(P<0.05),while the levels of serum brain-derived neurotrophic factor(BDNF)and nerve growth factor(NGF)were increased(P<0.05).The combined group showed significant decrease in NIHSS scores and serum NSE level and significant increase in serum BDNF and NGF levels compared to the control group(P<0.01).Conclusion For the treatment of patients with qi deficiency and blood stasis type of ischemic stroke,Buyang Huanwu Decoction combined with Tirofiban is effective on reducing TCM syndrome scores,improving cerebral oxygen metabolism and neurological function indicators,and inhibiting oxidative stress response,with significant therapeutic effects.
3.Mediating Role of Perceived Organizational Support in Sleep Quality of Nurses: in Tertiary Hospitals: A Nationwide Multicenter Cross-sectional Study

To investigate the current status of sleep quality among nurses in tertiary hospitals in China, analyze the correlations of work stress and perceived organizational support with the risk of sleep problems, and further examine the mediating effect of perceived organizational support between work stress and sleep problems in nurses. A multi-stage cluster stratified random sampling method was employed to select nurses from tertiary hospitals in different regions across China from October 2023 to April 2024 as research subjects to investigate the current status of their sleep quality. Restricted cubic spline (RCS) analysis was conducted to examine the linear/non-linear relationships between work stress, perceived organizational support, and the risk of sleep problems in nurses. The Bootstrap method was applied to test the mediating effect of perceived organizational support between work stress and sleep problems in nurses. A total of 6634 nurses from tertiary hospitals were surveyed in this study, with 6106 valid questionnaires recovered, yielding a response rate of 92.04%. The 6106 nurses were divided into eastern ( The nurses in tertiary hospitals in China usually present poor sleep quality. Perceived organizational support plays a partial mediating role between work stress and sleep problems in nurses. Therefore, enhancing the level of organizational support can help alleviate the impact of work stress on sleep quality.

5.Effect of Drug Therapy Combined with Multi-dimensional Psychological Intervention on Insomnia Medical Staff in Shift Based on the"Sunshine Hospital"Mode

Objective To explore the effects of drug therapy combined with multi-dimensional psychological intervention on the insomnia of shift medical staff in"sunshine hospital"mode,and to provide a reference for improving the physical and mental health of shift medical staff.Methods A total of 140 cases of medical staff with insomnia in shifts in Wuhan were included and divided into study group and control group by random number table method,70 cases in each group.The control group was given drug treatment,and the study group was treated combined with multi-dimensional psychological intervention.based on"sunshine hospital"model.The scale and questionnaire of PSQI,ISI,ESS and FAS were used to evaluate sleep status and HAMD,HAMA,PHQ-9 and SAS were used to evaluate psychological status,at baseline,visit 1(baseline+intervention for 1 month)and visit 2(baseline+intervention for 3 months),respectively.Results(1)Sleep status:There were differences in 7 dimensions of PSQI score,ISI,ESS,and FAS score between the study group and the control group at different periods(P<0.05);Among them,the PSQI scores of the two groups showed significant difference between the two groups at interview 2(P<0.05);There were significant differences in sleep quality,sleep efficiency scores,ESS and FAS between the two groups at visit 1 and visit 2(P<0.05).(2)Psychological status:HAMD,HAMA,PHQ-9 and SAS scores of the study group and the control group were different in different periods(P<0.05).At interview 2,there were significant differences in HAMA and SAS scores between the two groups(P<0.05).Conclusion Based on the"sunshine hospital"model of drug therapy combined with multi-dimensional psychological intervention,it can significantly improve sleepiness,fatigue symptoms,sleep quality,sleep efficiency,and anxiety and depression of first-line shift medical staff in the short term,and significantly alleviate overall insomnia and anxiety in the long term.
6.Research progress on digital dental models and the independent development of digital dental-cranio-cervi-cal model

The application of digital technology in teaching and clinical practice has been hindered by the lack of integrity,consis-tency and accuracy in current digital models.Despite the efforts of domestic and international scholars,the localization,refining and standardization of model data for digital dental models remain inadequate,which limits the promotion and application of digital dental models.A preliminergly produced precise,standardized and multi-form applicable 3D dental-cranio-cervical digital model based on Chinese anatomical data is reported in this paper.The further improvement,application and promotion of this model are expected to contribute to the improvement of the quality of dental education and clinical efficiency.
7.Study of the influence of peer support service on social function of patients with chronic schizophrenia

BackgroundThe improvement of social function in patients with schizophrenia is an important part of their rehabilitation, and peer support services, as a rehabilitation method, may be of great significance to improve the social function of patients with schizophrenia. ObjectiveTo explore the influence of peer support service on the daily living ability and social function of patients with chronic schizophrenia, and to provide references for promoting the rehabilitation of patients with chronic schizophrenia. MethodsA total of 100 patients with chronic schizophrenia who were hospitalized in The Third People's Hospital of Tianshui from January 1 to December 31, 2020 and met the diagnostic criteria of the International Classification of Diseases, 10th edition (ICD-10) were selected as the study objects, and they were divided into the control group and the study group with 50 cases each by random number table method. Patients in both groups received routine treatment and nursing care, on this basis, the study group received peer support service once or twice a week for 12 weeks, and the control group received the same peer support service at the end of the study. Before and at the 1st, 2nd, 4th, 8th and 12th week of the treatment, Activity of Daily Living Scale (ADL) and Social Disability Screening Schedule (SDSS) were used to evaluate the activities of daily living and social function of the two groups. ResultsThe ADL scores of the study group were lower than those of the control group at the 8th week and 12th week of the treatment ( t=-2.420, -2.814, P<0.05 or 0.01) . The SDSS scores of the study group were lower than those of the control group at the 8th week an 12th week of the treatment (t=-2.057, -2.322, P<0.05) . ConclusionPeer support services may help improve the ability of daily life and social function of patients with chronic schizophrenia. 8.Investigation on the effect of myocardial bridge on ventricular repolarization in civil pilots

Objective:To investigate the detection of myocardial bridge in civil aviation pilots and whether there is abnormal ventricular repolarization in the pilots with myocardial bridge, and to explore the effect of myocardial bridge on ventricular repolarization.Methods:Retrospective study was used to analyze the data of submaximal treadmill exercise test in 2019 for the civil aviation pilots aged ≥40. Pilots with cardiovascular diseases affecting ventricular repolarization were excluded. The resting electrocardiogram before exercise and 1 min QT interval after exercise were measured, and the QT dispersion (QT d) was calculated. The pilots were divided into myocardial bridge group and control group according to whether myocardial bridge was detected. The positive rate of treadmill exercise test and the incidence of ventricular extrasystole were compared between the pilots in myocardial bridge group and control group. The QT/RR slope was statistically analyzed in the pilots received 24 h electrocardiogram monitoring. Results:A total of 243 pilots were enrolled, including 242 males and 1 female. Among the 243 cases, 27 cases of myocardial bridge pilots were detected, and the myocardial bridge was mainly found in the anterior descending branch of left coronary artery (25/27, 92.6%). The 24 h dynamic electrocardiogram was examined in 17 cases, 6 cases were in the myocardial bridge group and 11 cases were in the control group. The total flying hours of the myocardial bridge group was longer than that of the control group ( Z=2.29, P=0.022); the positive rate or suspected positive rate of submaximal treadmill exercise test and the incidence of ventricular extrasystole in the myocardial bridge group were higher than those in the control group, and the differences were significant ( χ2=52.74, 5.78, P<0.001, =0.016); the 1 min QT d in the convalescent phase of the treadmill exercise test in the myocardial bridge group was longer than that in the control group, and the difference was significant ( Z=-2.20, P=0.028). The average frequency of 24 h ventricular extrasystole, QT e/RR slope and QT a/RR slope in the myocardial bridge group were higher than those in the control group, and the differences were significant ( t=2.37, 4.26, 2.51, P=0.034, <0.001, =0.024). Conclusions:Pilots with myocardial bridge may have prolonged ventricular repolarization and are prone to ventricular arrhythmias. In order to ensure flight safety, attention should be paid to the physical examination and flight adaptability evaluation of the pilots with myocardial bridge, especially to strengthen daily health management of the pilots with ventricular repolarization abnormalities.

10.Echocardiographic features of isolated brachiocephalic artery branches and analysis of missed diagnosis or misdiagnosis

Objective:To explore the echocardiographic characteristics of isolated brachiocephalic artery branches, and to analyze the reason of missed diagnosis and misdiagnosis in order to improve the diagnostic accuracy.Methods:Echocardiographic features of isolated subclavian artery and brachibrachial trunk confirmed by operation or CT angiography (CTA) or digital subtraction angiography (DSA) were retrospectively analyzed in Hebei Children′s Hospital from May 2017 to July 2021. Related literatures of other rare types of isolated brachiocephalic artery branches retrieved in PubMed were reviewed, and the echocardiographic characteristics of such vascular malformations were summarized.Results:A total of 4 children with isolated brachiocephalic artery branches were included. Echocardiography correctly diagnosed isolated brachiocebrachial trunk in 1 case and missed or misdiagnosis in 3 cases, including isolated subclavian artery was missed in 1 case, isolated subclavian artery was misdiagnosed as aberrant subclavian artery in 1 case and patent ductus arteriosus in 1 case. A total of 105 cases of isolated brachiocephalic artery branches were reviewed, including isolated brachiocephalic artery in 31 cases (53.3%), isolated common carotid artery in 15 cases (29.5%), isolated common carotid artery in 15 cases (14.3%) and isolated internal carotid artery in 3 cases (2.9%). The characteristic of echocardiography was that the brachiocephalic branch of the aortic arch was disconnected from the aortic arch, often via the ductus arteriosus or directly connected to the bifurcation of the pulmonary artery.Conclusions:Isolated brachiocephalic artery branches have certain characteristic echocardiographic features and these kinds of vascular malformations are accompanied by different degrees of stealing blood phenomenon. Careful tracing of the origin of brachiocephalic artery branches and combined with cervical vascular ultrasound when necessary can improve the diagnosis rate of isolated brachiocephalic artery branch lesions.
