1.Cordycepin Inhibits Fat Infiltration after Rotator Cuff Tear Injury by Regulating Wnt/β-catenin Signaling Pathway

ObjectiveTo investigate the effect and mechanism of cordycepin in inhibiting fat infiltration after rotator cuff injuries in rats by regulating the Wnt/β-catenin signaling pathway, providing a theoretical basis for clinical treatment of rotator cuff injuries. MethodsFifty SPF-grade female SD rats were used in this study, with 10 randomly selected as the blank group. A rotator cuff injury repair model was established by supraspinatus tendon and suprascapular nerve compression. The successfully modeled rats were randomized into model and low-dose (20 mg·kg-1), medium-dose (40 mg·kg-1), and high-dose (80 mg·kg-1) cordycepin groups. After 6 weeks of treatment, the gait analysis was performed to assess the limb function in rats. Oil red O staining and Masson staining were employed to observe pathological changes in the muscle tissue. Enzyme-linked immunosorbent assay (ELISA) was used to measure the levels of interleukin-1β (IL-1β), interleukin-6 (IL-6), and tumor necrosis factor-α (TNF-α) in the serum. Immunohistochemistry (IHC) was employed to detect the expression of peroxisome proliferator-activated receptor γ (PPARγ) and CCAAT/enhancer-binding protein α (C/EBPα), which are markers of adipogenesis. Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) and Western blot were employed to determine the mRNA and protein levels, respectively, of Wnt3a, Wnt10b, and β-catenin. ResultsCompared with the blank group, the model group showed decreases in stride length and paw print area (P<0.01), an increase in ratio of wet muscle mass reduction and a decrease in muscle fiber cross-sectional area (P<0.05), and decreased ratios of fat infiltration area and collagen fiber area (P<0.01). Additionally, the model group showed elevated levels of IL-1β, IL-6, and TNF-α (P<0.05), up-regulated protein levels of PPARγ and C/EBPα (P<0.01), and down-regulated mRNA and protein levels of Wnt3a, Wnt10b, and β-catenin (P<0.05, P<0.01). Compared with the model group, the low-, medium-, and high-dose cordycepin groups showed increases in stride length and paw print area (P<0.01), a decrease in ratio of wet muscle mass reduction and an increase in muscle fiber cross-sectional area (P<0.05), and increases in ratios of fat infiltration area and collagen fiber area (P<0.05, P<0.01). In addition, cordycepin lowered the serum levels of IL-1β, IL-6, and TNF-α (P<0.05, P<0.01), down-regulated the protein levels of PPARγ and C/EBPα (P<0.01), and up-regulated the mRNA and protein levels of Wnt3a, Wnt10b, and β-catenin (P<0.05, P<0.01). ConclusionCordycepin can improve the limb function, alleviate rotator cuff muscle atrophy, fat infiltration, and fibrosis, and inhibit inflammation in rats by regulating the Wnt/β-catenin signaling pathway.
3.Effect of Lingzhu Granules on PI3K/Akt signaling pathway in rats with polycystic ovary syndrome accompanied by insulin resistance

Objective To explore the regulatory effect of Lingzhu Granules on the PI3K/Akt signaling pathway in the rats with polycystic ovary syndrome(PCOS)accompanied by insulin resistance.Methods Fe-male Sprague-Dawley(SD)rats aged 3 weeks d old were selected to establish the model by using the DHEA method.The intervention treatment was administered by different doses of Lingzhu Granules and the homeostasis model as-sessment of insulin resistance(HOMA-IR)was calculated.The relative expression levels of PI3K,Akt,GSK-3β and GLUT4 mRNA in the ovaries was detected by RT-PCR.The expression levels of PI3K and Akt were detected by Western blot.Results Compared with the model control group,the rat body weight in the medium and high doses Lingzhu Granules groups was significantly reduced(P<0.05).The fasting blood glucose,fasting insulin and HOMA-IR index in the Lingzhu Granules low,medium and high doses groups were significantly lower than those in the model control group,and the differences were statistically significant(P<0.01).The mRNA relative expression levels of PI3K,Akt,GSK-3β and GLUT4 in the Lingzhu Granules medium and high doses groups all were higher than those in the model control group(P<0.05).The relative expression levels of PI3K and Akt in the ovarian tissue in the Lingzhu Granules low,medium and high doses groups were higher than those in the model control group,and the differences were statistically significant(P<0.05).Conclusion Lingzhu Granules could effectively improve the insulin resistance status of PCOS rats by enhancing the synthesis of PI3K and Akt.
4.Exploratory use of ddPCR in monitoring Epstein-Barr virus infection following allogeneic hematopoietic stem cell transplantation in children

Objective:This study aimed to introduce droplet digital PCR (ddPCR) for monitoring Epstein-Barr virus (EBV) infection after allogeneic hematopoietic stem cell transplantation (alloHSCT) in children and assess its viability as a complementary detection method in clinical settings.Methods:A total of 290 blood samples from 47 children undergoing alloHSCT were collected. Both ddPCR and real-time quantitative PCR (qPCR) were employed to detect EBV DNA load in plasma, with a comparison of detection efficiencies between the two methods. Continuous monitoring of 39 children was conducted to observe dynamic changes in EBV DNA load in plasma and analyze the merits and drawbacks of both methods.Results:The EBV positive detection rate of ddPCR was significantly higher than that of qPCR ( χ2=20.25, P<0.001), particularly in samples with low viral loads. Among the children monitored continuously for EBV DNA, 14 out of 39 exhibited positive ddPCR result. Notably, in two cases where patients displayed rash and fever symptoms with positive ddPCR result but negative qPCR result, ddPCR demonstrated heightened sensitivity in early EBV infection detection. Conclusions:ddPCR holds certain advantages in monitoring EBV infection post-alloHSCT in children, especially for samples with low viral loads. However, as this method is still in the exploratory stage of clinical application, further research and practice are needed to validate its utility.
5.Distribution characteristics and drug resistance of 267 strains in intensive care unit

Objective To explore the distribution characteristics and drug resistance of 267 strains of pathogenic bacteria in the intensive care unit.Methods 180 specimens sent for examination from 180 patients admitted to the intensive care unit of Yiwu Central Hospital who developed hospital-acquired infections from January 2021 to November 2023 were collected,and the sites of infection and the sources of specimens of patients with hospital-acquired infections were counted.The distribution characteristics of the patient's protozoa were analyzed,and the drug resistance of major Gram-positive bacteria(Staphylococcus aureus,Staphylococcus epidermidis)and Gram-negative bacteria(Escherichia coli,Pseudomonas aeruginosa,and Klebsiella pneumoniae)was analyzed in combination with drug sensitivity assessment results.Results Among the 180 patients with hospital-acquired infections,pulmonary and urinary tract infections accounted for a relatively high proportion,and sputum and urine specimens accounted for a relatively high proportion of the samples submitted for examination.180 specimens were cultured for pathogenic bacteria and 267 strains of pathogenic bacteria were detected,of which Gram-positive bacteria accounted for 29.59%,Gram-negative bacteria accounted for 67.04%,and fungi accounted for 3.37%.From 2021 to 2023,the detection rates of Gram-positive bacteria and fungi showed an increasing trend,and Gram-negative bacteria detection rates showed a decreasing trend.Staphylococcus aureus had high sensitivity to linezolid and Staphylococcus epidermidis had high sensitivity to moxifloxacin and linezolid.Escherichia coli and Pseudomonas aeruginosa had high sensitivity to piperacillin/tazobactam,and Klebsiella pneumoniae had high sensitivity to cotrimoxazole,imipenem,and piperacillin/tazobactam.Conclusion The distribution of pathogenic bacteria in patients with hospital-acquired infections admitted to the intensive care unit mainly consisted of Staphylococcus aureus,Staphylococcus epidermidis and Escherichia coli,Pseudomonas aeruginosa,and Klebsiella pneumoniae,and there were varying degrees of resistance to antimicrobial drugs.The subsequent selection of clinical drugs should be cautious,and antimicrobials with high sensitivity should be prioritized.

The comparison between traditional Chinese medicine Jinzhen oral liquid (JZOL) and Western medicine in treating children with acute bronchitis (AB) showed encouraging outcomes. This trial evaluated the efficacy and safety of the JZOL for improving cough and expectoration in children with AB. 480 children were randomly assigned to take JZOL or ambroxol hydrochloride and clenbuterol hydrochloride oral solution for 7 days. The primary outcome was time-to-cough resolution. The median time-to-cough resolution in both groups was 5.0 days and the antitussive onset median time was only 1 day. This randomized controlled trial showed that JZOL was not inferior to cough suppressant and phlegm resolving western medicine in treating cough and sputum and could comprehensively treat respiratory and systemic discomfort symptoms. Combined with clinical trials, the mechanism of JZOL against AB was uncovered by network target analysis, it was found that the pathways in TRP channels like IL-1β/IL1R/TRPV1/TRPA1, NGF/TrkA/TRPV1/TRPA1, and PGE2/EP/PKA/TRPV1/TRPA1 might play important roles. Animal experiments further confirmed that inflammation and the immune regulatory effect of JZOL in the treatment of AB were of vital importance and TRP channels were the key mechanism of action.
7.Association of ticagrelor with risk of infection:a two-sample Mendelian randomization study based on the GWAS database

OBJECTIVE To investigate the causal association between ticagrelor and risk of infection METHODS Two-sample Mendelian randomization was adopted. Genetic instrumental variables were selected based on the results of the largest genome-wide association analysis to in vivo exposure of ticagrelor and its major active metabolite AR-C124910XX. The causal associations of ticagrelor and its major active metabolite AR-C124910XX with drug indications (coronary artery disease, unstable angina pectoris, myocardial infarction, stroke and ischemic stroke)were analyzed by inverse variance weighted Mendelian randomization model as a positive control for genetic instrumental variables. The causal relationship between ticagrelor and bacterial infection, acute lower respiratory infection, bacterial pneumoniae, pneumoniae,acute upper respiratory infection and sepsis were furtheranalyzed by using this method, and the robustness of the results was assessed by using heterogeneity tests and horizontal 202002030415) pleiotropy tests. RESULTS The increase of area under the curve at steady state (AUCss) of the genetic surrogated ticagrelor significantly reduced the risk of coronary artery disease, myocardial infarction and unstable angina pectoris (P<0.001). AUCss genetic instrument variables of its main active metabolite AR-C124910XX failed to pass positive control. Further analysis showed that the increase of the genetic surrogated ticagrelor exposure suggestively reduced the risk of bacterial infection [OR(95%CI)=0.80(0.65,0.99),P=0.040] and sepsis [OR (95%CI)=0.84(0.73, 0.98), P=0.023]. The results of the heterogeneity tests showed that there was no heterogeneity in the causal association of the genetic surrogated ticagrelor AUCss with bacterial infection and sepsis (P>0.05). The results of horizontal pleiotropy tests showed that the causal association of genetic surrogated ticagrelor AUCss with bacterial infection and sepsis had no effects on horizontal pleiotropy (P>0.05). CONCLUSIONS Ticagrelor has a potential role in reducing the risk of sepsis and bacterial infections.
8.Exploring Mechanism of Renshen Guben Oral Liquids in Treating Renal Fibrosis Based on Metabolomics and Network Pharmacology

ObjectiveTo investigate the mechanism of Renshen Guben oral liquids(RGOL) in treatment of mice with renal fibrosis based on metabolomics and network pharmacology. MethodC57BL/6 mice were randomly divided into control group, model group and RGOL group, 12 mice in each group. Except for the control group, mice in the other groups were induced into unilateral ureteral obstruction(UUO) model by UUO. After preparation of the model, an aqueous solution of 4.2 g·kg-1 extract powder was administered by gavage to RGOL group for 14 d, and an equal amount of distilled water was administered by gavage to the control and model groups. After the last administration on the 14th day, urine was collected and detected by ultra-high performance liquid chromatography-triple quadrupole tandem mass spectrometry(UPLC-QQQ-MS/MS) with 0.1% formic acid aqueous solution as mobile phase A, and acetonitrile-isopropanol(70∶30) as mobile phase B for gradient elution(0-1 min, 5%B; 1-5 min, 5%-30%B; 5-9 min, 30%-50%B; 9-11 min, 50%-78%B; 11-13.5 min, 78%-95%B; 13.5-14 min, 95%-100%B; 14-16 min, 100%B; 16-16.1 min, 100%-5%B; 16.1-18 min, 5%B), column temperature of 40 ℃, flow rate of 0.4 mL·min-1, electrospray ionization(ESI), collection range of m/z 50-900. Through network pharmacology, the targets of components in RGOL and the targets of renal fibrosis were analyzed interactively, and the key components and key targets were screened by network topology analysis, and DAVID platform was used to predict the signaling pathways of RGOL for the treatment of renal fibrosis. ResultA total of 7 differential metabolites involving 8 metabolic pathways were identified in RGOL for the treatment of renal fibrosis. The network pharmacology revealed that 36 key components in RGOL were related to 7 differential metabolites, mainly ginsenosides, notoginsenosides and nucleotides. Based on the herbs-components-targets-pathways network, a total of 23 key targets related to the treatment of renal fibrosis by RGOL were highlighted, which together with the differential metabolites were involved in linoleic acid metabolism, arginine biosynthesis, tricarboxylic acid cycle(TCA), arginine and proline metabolism and other pathways. ConclusionBased on metabolomics and network pharmacology, this study preliminarily identified 7 differential metabolites, 36 potential pharmacodynamic components and 23 key targets and 4 key pathways in RGOL for the treatment of renal fibrosis, providing an experimental basis for the clinical application and mechanism study of this preparation.
9.Study on the clinical epidemiological characteristics of human rhinovirus infection in children with community-acquired pneumonia

Objective:To analyze the dominant genotypes and epidemic characteristics of human rhinovirus (HRV) in pediatric community-acquired pneumonia (CAP) in China.Methods:Between June 2017 to December 2019, throat swabs or nasopharyngeal aspirates were collected from pediatric CAP patients hospitalized in 6 medical institutions in Southern and Northern China (bounded by Qinling and Huaihe River), respectively.A total of 16 species of common respiratory viruses were screened using respiratory pathogen detection kits.Samples with positive HRV were genotyped for further epidemiological analysis.Results:The total detection rate of HRV in pediatric CAP (2 913 cases) was 12.2%(356 cases) in this study, which was 10.3%(145/1 410 cases) and 14.0%(211/1 503 cases) in Northern and Southern China, respectively.The detection rate of HRV in the Southern region was significantly higher than that in the Northern region, the difference was statistically significant( χ2=9.562, P=0.002). Epidemiological analysis showed that the distribution of HRV-positive cases in the Northern region was similar in all age groups, while 90.5% of positive cases in the Southern region were infants under 3 years.Classified by seasonal distribution, HRV-positive cases in the Northern region were mainly distributed in summer and autumn, while those in the Southern region were mainly distributed in spring and autumn.The coinfection rate of HRV and other pathogens was 40.7%, which was significantly higher in the Southern region than that of Northern region (49.0% vs.28.0%), the difference was statistically significant( χ2=15.801, P<0.001). The most common pathogens mixed with HRV were respiratory syncytial virus (RSV), human boca virus (HBOV) and parainfluenza virus type Ⅲ (PIV3) in the Northern region, with the mixed infection rate of 5.0%, 5.0% and 4.0%, respectively, which were RSV, PIV3 and human metapneumovirus (HMPV) in the Southern region, with the mixed infection rate of 9.0%, 7.0% and 6.0%, respectively.The genotyping results showed that all three HRV genotypes were detected.HRV-A (118/220 cases) was the dominant genotype, followed by HRV-C(82/220 cases). Conclusions:The detection rate of HRV in pediatric CAP cases is 12.2% in this study.There are significant differences in age and seasonal distribution of HRV-positive cases in the Southern and Northern regions.Coinfection of HRV and other pathogens is detected.The coinfection rate is significantly higher in the Southern region than that in the Northern region.HRV-A is the dominant genotype, followed by HRV-C.
10.Analysis of clinical features of 16 cases with Chlamydia psittaci pneumonia

Objective:To analyze the clinical characteristics of Chlamydia psittaci pneumonia and to investigate the correlation between serum inflammatory biomarkers and severity of the disease. Methods:Sixteen patients with Chlamydia psittaci pneumonia admitted to the Huizhou Municipal Central Hospital from January 2020 to July 2021 were selected as the study subjects, including 10 severe cases and 6 mild cases. Clinical data were collected and analyzed, such as baseline characteristics, clinical symptoms, laboratory inspection and chest imaging manifestations. Results:① Thirteen Chlamydia psittaci pneumonia were associated with a definite bird or poultry contact history. Common symptoms included high fever, chill, cough, fatigue, and anorexia (16 cases), dyspnoea (12 cases), and other systemic symptoms. ② Laboratory test results showed normal white blood cell count (WBC, 10 cases), decreased lymphocyte count (LYM, 13 cases), increased high sensitive C-reactive protein (hs-CRP, 16 cases), D-dimer (15 cases), lactate dehydrogenase (LDH, 13 cases), aspartate aminotransferase (AST, 16 cases) and alanine aminotransferase (ALT, 12 cases) levels, however, the albumin (Alb, 15 cases) lever was decreased. The numbers of CD3 + T cells and CD4 + T cells decreased in 10 patients. ③ The levels of D-dimer, interleukins (IL-2, IL-6, IL-10) in severe Chlamydia psittaci pneumonia were significantly higher than those in mild Chlamydia psittaci pneumonia [D-dimer (μg/L): 10 257±4 203 vs. 1 085±642, IL-2 (ng/L): 1.1 (0.8, 1.7) vs. 0.3 (0.1, 0.7), IL-6 (ng/L): 315 (182, 505) vs. 75 (18, 131), IL-10 (ng/L): 7.0±4.1 vs. 2.3±0.7], but the LYM was lower (×10 9/L: 0.4±0.1 vs. 1.1±0.4), with significant differences (all P < 0.05). ④ Chest imaging manifestations were exudative lesions and large consolidation of lungs. Large consolidation of both lungs can occur in some critically ill patients. Conclusions:Chlamydia psittaci pneumonia is mainly associated with a bird or poultry contact history. The clinical manifestations usually present high fever, dyspnea, normal or slightly increased leucocytes, and lung consolidation. The levels of LYM, D-dimer, IL-2, IL-6 and IL-10 in serum are expected to predict the severity of the Chlamydia psittaci pneumonia.
