1.Parental Consent Approaches for Newborn WholeGenome Sequencing–Based Screening

This study aims to develop a standardized informed consent system for whole-genome sequencing (WGS)-based newborn screening (NBS) in Korea through a policy design approach grounded in the domestic legal and policy context. We conducted an integrated literature review of international norms, domestic legal frameworks, guidelines, and major overseas WGS-based NBS programs using a common analytical framework. To develop a consent model feasible in the Korean context, we also analyzed stakeholder evidence from parent interviews and a clinician survey focused on consentrelated issues. The findings indicate that the current Korean legal and institutional framework does not adequately address long-term storage, reanalysis, recontact, and the transfer of decision-making authority as the child matures. Based on these findings, we propose a Korean informed consent model consisting of a staged consent pathway spanning antenatal, postnatal, and adulthood-related decision points; tiered consent options for the return of results; and safeguards against disadvantage or discrimination related to participation, non-participation, or withdrawal. The study further suggests that implementation will require broader legal and procedural clarification across the relevant regulatory framework, along with the continued development of education and counseling strategies.
2.Staged Wound Closure of the Anterolateral Thigh Flap Donor Site by Combination of Shoelace Approximation and Negative Pressure Wound Therapy

The anterolateral thigh (ALT) flap serves as a workhorse flap for reconstruction of large extremity defects. However, primary closure of the donor site is often not possible when the flap width exceeds 8–9 cm, and skin grafting is commonly required, resulting in unfavorable scarring and discomfort. Three patients received skin cancer or sarcoma resection surgery which required subsequent resurfacing with an ALT free flap. Instead of directly closing the donor sites, elastic sutures were employed in a zigzag shoelace configuration for gradual approximation of the wound edges. Subsequently, negative pressure wound therapy (NPWT) was applied to manage postoperative edema and stabilize the wounds. Complete donor wound closure was achieved between postoperative days 4 and 10. NPWT was continued in the form of incisional NPWT after definitive closure to stabilize the wound site, reduce edema, and facilitate recovery of skin pliability. Tension-releasing taping was also used together with the wound closure. No complications such as skin edge necrosis were observed. The scars were satisfactory with minimal discomfort. This staged combination of shoelace approximation and NPWT represents a simple, safe, and effective alternative to skin grafting for ALT donor sites that cannot be closed primarily, minimizing morbidity while providing aesthetically favorable outcomes.
3.Effect of nutrition support team on 28-day mortality in Korean patients with acute respiratory failure

Background:
Providing optimal nutrition to patients with acute respiratory failure is difficult because nutritional requirements vary according to disease severity and comorbidities. In 2021, the National Medical Center initiated a protocol for screening upon admission and regular monitoring by a multidisciplinary nutritional support team (NST), for all patients in the medical intensive care unit (ICU). This study aimed to evaluate the effects of routine NST monitoring and active intervention on the clinical outcomes of patients with acute respiratory failure.
Methods:
Patients with acute respiratory failure requiring high-flow nasal cannula, non-invasive ventilation, or mechanical ventilation were included. The primary outcome was 28-day mortality after ICU admission. Secondary outcomes included the supplied/target calorie ratio, supplied/target protein ratio on day 7, and complications.
Results:
In total, 152 patients were included in the analysis. The patients were divided into a pre-monitoring (n=96) and post-monitoring groups (n=56). More patients in the post-monitoring group received NST intervention and had earlier initiation of enteral feeding. In survival analysis, 28-day mortality was significantly lower in post-monitoring group (adjusted hazard ratio, 0.42; 95% CI, 0.24–0.74). The ratio of achievement for required calories and protein on day 7 was higher, but not significantly, in the post-monitoring group. No significant differences were observed in the incidence of complications.
Conclusions
Regular NST monitoring in the ICU could have contributed to a reduced risk of 28-day mortality in critically ill patients with acute respiratory failure.

Hangovers from alcohol consumption cause symptoms like headaches, nausea, and fatigue, disrupting daily activities and overall well-being. Over time, they can also lead to inflammation and oxidative stress. Effective hangover relief alleviates symptoms, prevents dehydration, and replenishes energy needed for daily tasks. Natural foods considered high in antioxidants and antiinflammatory properties may aid in the hepatic breakdown of alcohol. The study aims to investigate the impact of glutathione or its enriched yeast extract, which is recognized for its antioxidant characteristics, on alcohol metabolism and alleviating hangovers in a rat model exposed to binge drinking. In this study, glutathione and its enriched yeast extract controlled hangover behaviour patterns, including locomotor activity. Additionally, it enhanced the activities of alcohol dehydrogenase (ADH) and aldehyde dehydrogenase (ALDH) following ethanol ingestion (3 g/kg). Further, the incorporation of glutathione led to an increase in the expression of antioxidant enzymes, such as SOD and catalase, by activating the nuclear erythroid 2-related factor 2 (Nrf2) signaling pathway.This activation reduced the excessive production of reactive oxygen species (ROS) and malondialdehyde. Next, glutathione modulated the activity of cytochrome P450 2E1 (CYP2E1) and the protein expressions of Bax and Bcl2. Besides, in vitro and in vivo investigations with glutathione demonstrated a regulating effect on the pan-s-glutathionylation and its associated protein expression, glutaredoxin 1 (Grx1), glutathione-S-transferase Pi (GST-π), and glutathione reductase (GR). Together, these findings suggest that glutathione or its enriched yeast extract as a beneficial dietary supplement for alleviating hangover symptoms by enhancing alcohol metabolism and its associated Nrf2/Keap1 signalings.

7.Real‑world apixaban concentration in Korean patients with atrial fibrillation

Purpose:
Apixaban is recommended for patients with atrial fibrillation. Although routine monitoring of plasma concentrations is not typically advised, factors such as ethnicity, sex, and comorbidities can influence these levels. Our study analyzed the plasma apixaban concentrations (PAC) in patients to explore whether these levels, along with underlying conditions, offer enhanced insights for risk stratification.
Methods:
This study analyzed 49 patients with atrial fibrillation who had been taking apixaban for over a month, examined factor Xa levels within 6 h post-administration, and correlated PAC with clinical characteristics such as age, body weight, estimated glomerular filtration rate (eGFR), presence of heart failure, and bleeding events.
Results:
The mean plasma concentration of apixaban in all patients was 160.3 ± 77.5 ng/mL. Those taking apixaban 5 mg twice daily had higher plasma concentrations than those taking 2.5 mg twice daily (191.2 ± 75.3 ng/mL vs.137.2 ± 72.0 ng/mL, p = 0.014). Among the patients receiving a reduced dose, renal function and heart failure were significantly associated with plasma concentrations. No factors were associated with the plasma concentrations in patients receiving the standard dose. Notably, reduced-dose patients with heart failure had plasma concentrations comparable to those of individuals receiving the standard dose and exhibited a higher incidence of bleeding than the other groups.
Conclusions
PAC measurement revealed that apixaban dosages, classified based on age, body weight, and eGFR, were generally effective. Nonetheless, heart failure may increase plasma levels and correlate with an increased bleeding risk in Korean patients on reduced doses. Therefore, tailoring apixaban prescriptions to account for heart failure and other comorbidities may enhance treatment efficacy.
8.Individualized Fracture Prevention for Postmenopausal Women with Osteopenia

Osteopenia—defined by a bone mineral density T-score between −1.0 and −2.5—is more common than osteoporosis and accounts for most fragility fractures in postmenopausal women. Approximately 50% of Korean women aged ≥ 50 years have osteopenia. Despite this high prevalence, optimal therapeutic strategies remain unclear. This review summarizes the clinical significance and management of osteopenia. Clinical practice often categorizes osteopenia into mild, moderate, and severe based on specific T-score ranges. Studies indicate that women transitioning from normal or moderate osteopenia to lower bone density experience more fractures. One study reported that approximately 10% of women with normal BMD or osteopenia progressed to osteoporosis, mostly from moderate to severe osteopenia groups. Fracture risks, particularly for hip and vertebral fractures, are elevated mainly in moderate to severe osteopenia.The National Osteoporosis Foundation and the American Association of Clinical Endocrinologists incorporate FRAX® scores alongside T-scores and clinical history to guide pharmacological intervention. Accordingly, South Korean guidelines classify fracture-risk groups as low, moderate, high, or very high, allowing treatment of patients with osteopenia in the high-risk categories. Evidence supporting medications for fracture prevention remains limited. However, growing interest in preventing fractures has directed the ongoing studies to evaluate various drugs. Food and Drug Administration-approved treatments include menopausal hormone therapy, bisphosphonates, and selective estrogen receptor modulators. To reduce fractures in postmenopausal women, treatment should not be restricted to osteoporosis alone. Women with moderate to severe osteopenia may also benefit from medications used for osteoporosis. Clinicians should assess individual fracture risks and select preventive treatments based on risk level, fracture site, menopausal symptoms, patient preference, and cost-effectiveness.
9.Novel Deep Learning-Based Vocal Biomarkers for Stress Detection in Koreans

Objective:
The rapid societal changes have underscored the importance of effective stress detection and management. Chronic mental stress significantly contributes to both physical and psychological illnesses. However, many individuals often remain unaware of their stress levels until they face physical health issues, highlighting the necessity for regular stress monitoring. This study aimed to investigate the effectiveness of vocal biomarkers in detecting stress levels among healthy Korean employees and to contribute to digital healthcare solutions.
Methods:
We conducted a multi-center clinical study by collecting voice recordings from 115 healthy Korean employees under both relaxed and stress-induced conditions. Stress was induced using the socially evaluated cold pressor test. The Emphasized Channel Attention, Propagation and Aggregation in Time delay neural network (ECAPA-TDNN) deep learning architecture, renowned for its advanced capabilities in analyzing person-specific voice features, was employed to develop stress prediction scores.
Results:
The proposed model achieved a 70% accuracy rate in detecting stress. This performance underscores the potential of vocal biomarkers as a convenient and effective tool for individuals to self-monitor and manage their stress levels within digital healthcare frameworks.
Conclusion
The findings emphasize the promise of voice-based mental stress assessments within the Korean population and the importance of continued research on vocal biomarkers across diverse linguistic demographics.
10.Full mouth rehabilitation of skeletal class II patient with regaining occlusal vertical dimension: a case report

Class II malocclusions present with a different occlusal pattern to patients with Class I malocclusions, characterized by a large overjet and overbite in the anterior region, resulting in habitual mandibular protrusion during speech or mastication. When rehabilitating Class II patients, it is important to consider lateral and anterior guidance, to ensure balanced posterior guidance during protrusive movements before anterior contact, and to establish freedom in the intercuspal position. In this case, digital tools were used for the full-mouth rehabilitation of a skeletal class II patient. A virtual patient was created using facial scan data, digital facebow transfer and jaw motion tracking. Provisional restorations were fabricated based on the virtual patient. After identifying occlusal interference during anterior movement with the first provisional, the virtual patient was updated and the occlusal design was refined. For the final restorations, the virtual patient was updated again to reflect the functionally and esthetically satisfactory provisional restorations and their adapted occlusion. This digital approach facilitated accurate replication of the dynamic occlusal relationships, resulting in functionally and esthetically successful outcomes.
