Purpose:
Endoscopic submucosal dissection (ESD) has become a standard minimally invasive treatment for selected patients with early gastric cancer (EGC). This study presents the first nationwide survey of patients with EGC treated with ESD in 2023, conducted by the Korean College of Helicobacter and Upper Gastrointestinal Research.
Materials and Methods:
Data were retrospectively collected from participating referral centers across Korea using a standardized case report form covering patient characteristics, tumor features, procedural details, histopathological findings, and clinical outcomes.Descriptive and comparative analyses were conducted to summarize nationwide ESD practice patterns and outcomes.
Results:
Data from 5,460 ESD cases from 5,250 patients across 27 institutions were analyzed. The mean age was 67.4 years, with 74.1% males. Multiple synchronous lesions were identified in 3.7%. Most lesions were located in the lower third of the stomach (64.0%), and differentiated-type adenocarcinomas accounted for 87.8%. The en bloc and complete resection rates were 99.2% and 91.4%, respectively. Curative resection was achieved in 80.5%, whereas local non-curative resection (L-NCR) and surgical non-curative resection (S-NCR) were identified in 2.8% and 16.7%, respectively. Additional surgery was performed more frequently in patients with S-NCR than in those with L-NCR (59.3% vs. 24.7%). The bleeding and perforation rates were 3.6% and 0.9%, respectively, and were mostly managed conservatively or endoscopically. The median length of hospitalization was 4.0 days.
Conclusions
This first nationwide survey provides a comprehensive overview of the current practice of EGC treatment using ESD in Korea, demonstrating high technical success and safety, and establishing a baseline dataset for future longitudinal research.

3.Effect of Nationwide Postoperative Rehabilitation at Home Program after Lower Extremity Surgeries in Older and Younger People

A nationwide Rehabilitation at Home Care Pilot Program for patients undergoing lower extremity orthopedic surgeries has been ongoing since 2020. The program was designed to improve clinical outcomes through early mobilization and rehabilitation after discharge. This study aimed to analyze the interim data to assess its effectiveness and suggest improvements, particularly for older patients who are more vulnerable compared to younger patients.
Methods:
We analyzed the data of 872 patients from seven hospitals. Patients were divided into an older-age group (OG; ≥60 years) and younger-age group (YG; <60 years). The Berg Balance Scale (BBS) and Numeric Rating Scale (NRS) scores for pain were the main outcomes.
Results:
Participants were categorized into OG (n=801) and YG (n=71). The most common surgeries were knee joint replacement (63.7%) in OG and hip joint replacement in YG (77.5%). It took more days to enroll in the pilot program in OG (4.4 days) than YG (3.2 days). OG showed less improvement in BBS than YG for all surgeries (23.3 vs. 31.9) and hip joint surgery (19.8 vs. 33.5). In patients with hip joint replacement in OG, those with a higher number of comorbidities (≥3) showed less BBS improvement than those with fewer comorbidities (<3). In the pain domain, NRS scores decreased in both groups.
Conclusion
The postoperative rehabilitation education program appeared to be effective in improving physical function and reducing pain severity, though the improvement was less obvious in older patients who may require a more comprehensive approach compared to younger patients.

6.Comparison of Reliability and Satisfaction Between Face-to-Face and Remote Formats in the Secondary Evaluation of the Student Emotional and Behavioral Characteristics Test:Exploring the Feasibility of Remote Assessments

Objectives:
The Ministry of Education in South Korea conducts an annual Student Emotional and Behavioral Characteristics Test to facilitate early identification and intervention for mental health issues among adolescents. Students who exceed the screening cut-off undergo a secondary in-depth evaluation. In response to the increasing use of remote services following the coronavirus disease-2019 (COVID-19) pandemic, this study aimed to compare satisfaction and perceived reliability of face-to-face and remote formats of the secondary evaluation as reported by school personnel responsible for conducting the assessments.
Methods:
A total of 94 school personnel who oversaw the 2023 secondary evaluations in a metropolitan area participated in an anonymous online survey. The survey assessed their satisfaction with the evaluation period, perceived reliability of the evaluation process, and perceived reliability of the written reports. Subgroup analyses were conducted based on their experience with and preference for each evaluation format. Mann-Whitney U tests were used to compare differences between groups.
Results:
No significant differences were found between the face-to-face and remote groups in satisfaction with the evaluation period or the reliability of the written results. However, the face-to-face format was perceived as significantly more reliable in terms of the evaluation process itself. Among participants who had experience with both formats, the face-to-face format continued to receive higher reliability ratings, particularly among those who preferred face-to-face assessments. In contrast, those who preferred remote assessments showed no significant differences regardless of the format used.
Conclusions
Although some counselors express concern about the reliability of remote assessments, the findings suggest that remote formats can yield comparable results to face-to-face evaluations in terms of satisfaction and outcome reliability. While remote assessments may not fully replace in-person interactions, they can serve as a valid and efficient alternative, particularly for large-scale mental health screenings in school settings. Expanding the use of structured remote evaluations may enhance accessibility and responsiveness in school-based mental health services.
7.Early single session of hyperbaric oxygen therapy mitigates renal apoptosis in lipopolysaccharides-induced endotoxemia in rats

Sepsis-associated acute kidney injury (SA-AKI) is a prominent sepsis complication, often resulting in adverse clinical outcomes. Hyperbaric oxygen therapy (HBOT), known for its anti-inflammatory characteristics, antioxidant effects, and ability to deliver high oxygen tension to hypo-perfused tissues, offers potential benefits for SA-AKI. This study investigated whether HBOT improved renal injury in sepsis and elucidated its underlying mechanisms. Methods: A lipopolysaccharide (LPS)-induced endotoxemia model was established using 8-week-old C57BL/6 mice. Thirty minutes post-LPS administration, a group of mice underwent HBOT at a 2.5 atmospheric pressure absolute with 100% oxygen for 60 minutes. After 24 hours, all mice were euthanized for measurements. Results: Our results demonstrated that HBOT effectively mitigated renal tubular cell apoptosis. Additionally, HBOT significantly reduced phosphorylated p53 proteins and cytochrome C levels, suggesting that HBOT may attenuate renal apoptosis by impeding p53 activation and cytochrome C release. Notably, HBOT preserved manganese-dependent levels of superoxide dismutase, an antioxidant enzyme, compared to the LPS group. Furthermore, transforming growth factor beta (TGF-β)/Smad4 and alpha smooth muscle actin expressions were significantly reduced in the LPS + HBOT group. Conclusion: An early single session of HBOT exhibited renoprotective effects in LPS-induced endotoxemia mice models by suppressing p53 activation and cytochrome C levels to mitigate apoptosis. The observed TGF-β decrease, downstream Smad expression reduction, and antioxidant capacity preservation following HBOT may contribute to these effects.
8.Anastomosing hemangioma masquerading as renal cell carcinoma in a transplanted kidney:a case report

Anastomosing hemangioma (AH) is an exceptionally rare benign vascular neoplasm that can closely mimic renal cell carcinoma (RCC) on radiologic and histopathologic evaluation. We report a 63-year-old male kidney transplant recipient who developed a small enhancing renal mass during pretransplant assessment for a second kidney transplantation. The patient had received a deceased donor kidney transplantation in April 1994 and resumed maintenance hemodialysis in April 2025 due to chronic allograft failure. During evaluation for a planned living donor kidney transplantation from his son, abdominal contrast-enhanced computed tomography revealed a 1.4 cm well-circumscribed enhancing mass in the upper pole of the transplanted kidney, highly suspicious for RCC. Magnetic resonance imaging demonstrated a T2 hyperintense lesion with heterogeneous and delayed enhancement, and positron emission tomography-computed tomography showed no hypermetabolic uptake or metastasis. Transplant nephrectomy was performed to exclude malignancy. Histopathologic examination revealed anastomosing capillary-sized vascular channels lined by bland endothelial cells without atypia, consistent with AH. Immunohistochemical staining was positive for CD34 and ERG, confirming endothelial differentiation. This case emphasizes the diagnostic challenge of distinguishing AH from RCC in transplant recipients and highlights the importance of accurate pathologic diagnosis to prevent unnecessary delay in subsequent kidney transplantation.

This review article investigates solid organ transplantation-induced osteoporosis, a critical yet often overlooked issue, emphasizing its significance in post-transplant care. The initial sections provide a comprehensive understanding of the prevalence and multifactorial pathogenesis of transplantation osteoporosis, including factors such as deteriorating post-transplantation health, hormonal changes, and the impact of immunosuppressive medications. Furthermore, the review is dedicated to organ-specific considerations in transplantation osteoporosis, with separate analyses for kidney, liver, heart, and lung transplantations. Each section elucidates the unique challenges and management strategies pertinent to transplantation osteoporosis in relation to each organ type, highlighting the necessity of an organ-specific approach to fully understand the diverse manifestations and implications of transplantation osteoporosis. This review underscores the importance of this topic in transplant medicine, aiming to enhance awareness and knowledge among clinicians and researchers.
