1.A Case of Oncocytic and Inverted Schneiderian Papilloma Originating from the Lacrimal Sac

Purpose:
To report a case of oncocytic and inverted Schneiderian papilloma originating from the lacrimal sac.Case summary: A 70-year-old male presented with a 5-year history of bilateral epiphora. Best-corrected visual acuity was 0.9 in the right eye and 1.0 in the left eye, and intraocular pressure was 12 mmHg in the right eye and 14 mmHg in the left eye. The findings of nasolacrimal duct probing, the irrigation test, and dacryoscintigraphy suggested partial obstruction of the right nasolacrimal duct. Computed tomography and magnetic resonance imaging revealed a mass measuring approximately 2.5 cm in the right lacrimal sac. Surgical excision of the mass was planned in collaboration with the otolaryngology department, followed by histopathologic examination of the resected specimen. The otorhinolaryngology team removed the lacrimal sac mass endoscopically through the nasal cavity and submitted the specimen for histopathologic analysis. Subsequently, endoscopic dacryocystorhinostomy was performed. Postoperatively, the patient’s epiphora resolved, and histopathologic evaluation confirmed the diagnosis of oncocytic and inverted Schneiderian papilloma.
Conclusions
Schneiderian papilloma can originate from the nasolacrimal duct and should be considered in the differential diagnosis of patients presenting with epiphora.

Background and Objectives:
The risk profiles, procedural characteristics, and clinical outcomes for women undergoing bifurcation percutaneous coronary intervention (PCI) are not well defined compared to those in men.
Methods:
COronary BIfurcation Stenting III (COBIS III) is a multicenter, real-world registry of 2,648 patients with bifurcation lesions treated with second-generation drug-eluting stents.We compared the angiographic and procedural characteristics and clinical outcomes based on sex. The primary outcome was 5-year target lesion failure (TLF), a composite of cardiac death, myocardial infarction, and target lesion revascularization.
Results:
Women (n=635, 24%) were older, had hypertension and diabetes more often, and had smaller main vessel and side branch reference diameters than men. The pre- and post-PCI angiographic percentage diameter stenoses of the main vessel and side branch were comparable between women and men. There were no differences in procedural characteristics between the sexes. Women and men had a similar risk of TLF (6.3% vs. 7.1%, p=0.63) as well as its individual components and sex was not an independent predictor of TLF. This finding was consistent in the left main and 2 stenting subgroups.
Conclusions
In patients undergoing bifurcation PCI, sex was not an independent predictor of adverse outcome.

4.A Case of Congenital Lacrimal Sac Diverticulum

Purpose:
We report the diagnosis and conservative treatment of a patient with dacryocystitis accompanied by a congenital lacrimal sac diverticulum.Case summary: An 11-year-old boy presented with itching and redness in his right eye. His best-corrected visual acuity was 0.8 in the right eye and 1.2 in the left. Physical examination revealed redness, swelling, and tenderness around the right lacrimal sac area. Probing of the nasolacrimal duct showed no obstruction. Additional tests were performed under the diagnosis of acute dacryocystitis. Computed tomography and magnetic resonance imaging suggested inflammation of the soft tissue with abscess formation around the right orbit, with inflammation extending to the lower eyelid. After 7 days of inpatient treatment, the symptoms improved. Dacryocystography performed after the symptoms improved showed a small amount of residual contrast in a space near the lacrimal sac, leading to the diagnosis of a right lacrimal sac diverticulum.
Conclusions
In patients with dacryocystitis without nasolacrimal duct obstruction, congenital lacrimal diverticulum should be suspected.
