1.Effect of butylphthalide on the risk of post-stroke epilepsy in patients with cerebral infarction

Objective To explore the effect of sequential therapy with butylphthalide on the risk of post-stroke epilepsy(PSE)in patients with cerebral infarction.Methods The clinical data of patients with cerebral infarction hospitalized in Langfang People's Hospital from October,2016 to October,2022 were selected and analyzed.Patients receiving butylphthalide sequential treatment were included in the butylphthalide group,patients who were hospitalized at the same time with matching gender,age(±5 years),basic disease,clinical drug treatment scheme and follow-up period but did not receive butylphthalide treatment were included in the control group.The general clinical data,laboratory examination results on admission,and incidence of PSE in the two groups were compared,and multivariate Logistic regression was used to analyze the correlation between sequential therapy with butylphthalide and PSE occurrence in patients with cerebral infarction.Results A total of 1,200 patients were included,with 600 patients in each group.There was no significant difference in general clinical data such as sex,age,complications and living habits between the two groups(P>0.05).There were no significant differences between the two groups in the patients distribution of TOAST classification,NIHSS score(P>0.05).The incidence of PSE in butylphthalide group was significantly lower than that in control group(4.17%vs.7.83%,P<0.05).The application of sequential treatment with butylphthalide was a protective factor for PSE occurrence in patients with cerebral infarction[OR=0.352,95%CI(0.151,0.821),P=0.016].Conclusion Sequential treatment with butylphthalide can reduce the risk of post-stroke epilepsy in patients with cerebral infarction.
2.Quality analysis of medical record home pages based on DRG/DIP dual dimensions

Objective To analyze the quality of medical record home pages for discharged patients in a tertiary hospital in Guangzhou in 2023 based on the dual dimensions of Diagnosis-Related Groups(DRG)and Disease Intervention Program(DIP),and to provide a basis for improving the refinement of medical record home page quality management.Methods Dis-charge medical records from a tertiary general hospital in 2023 were selected.All medical records were grouped using DRG,while medical records under Guangzhou medical insurance and cross-province medical insurance implemented under DIP were simulated for DIP grouping.The writing defects in the discharge medical records were analyzed,as well as the DRG grouping,RW,and DIP standard scores of medical records with home page filling defects.Results Among 172 230 discharge medical records,171 786 were grouped into DRG,91 768 were grouped into DIP,and 91 604 were grouped into both DRG and DIP.A significant positive correlation was found between RW and DIP standard scores(r2=0.681,P<0.001).Among the 91 604 medical records grouped into both DRG and DIP,1 942(2.12%)had writing defects,including 837(0.91%)with home page defects.DRG groups with more home page defect records included NC15,HK2,EJ13,EB15,HJ13,and RE19.Conclusion Quality defects in medical record home pages impact the accuracy of DRG and DIP grouping.Systematic measures such as strengthening medical record writing training and establishing scientific reward and punishment mechanisms should be implemented to provide data sup-port for the further advancement of DRG/DIP payment reform.

4.Research on the relationship between the Myers-Briggs Type Indicator personality traits and empathy among medical students:the mediating role of emotional regulation

Objective:To investigate the relationship between the Myers-Briggs Type Indicator(MBTI)personality traits,empathy ability,and emotion regulation among medical students,and to advocate for medical students to conduct in-depth self-analysis using MBTI,and to enhance their empathy ability in a targeted manner,thereby more effectively building harmonious doctor-patient relationships.Methods:A total of 664 medical students were investigated using the MBTI,the Chinese version of the Emotion Regulation Questionnaire(ERQ),and the Chinese version of the Interpersonal Reactivity Index(IRI).The mediation analysis in SPSSAU employed the product coefficient test method,utilizing Bootstrap sampling for data analysis.Results:The MBTI personality traits of medical students were generally inclined to Introversion-Sensing-Thinking-Judging(ISTJ).Medical students tended to use cognitive reappraisal,and the level of empathy needed to be strengthened.Males exhibited higher expressive inhibition than females,while females experienced greater personal distress than males.Expressive inhibition was also higher in only children compared to non-only children,and only children showed lower emotional empathy than non-only children.Cognitive reappraisal played a partial mediating role in the relationship between introverted/extraverted personality traits and personal distress among medical students(mediating effect=-0.017,accounting for 14.217%of the total effect),and a complete mediating effect between introverted/extraverted personality traits and emotional empathy(mediating effect=0.024,accounting for 100%of the total effect).Cognitive reappraisal partially mediated the relationship between life attitude orientation and emotional empathy(mediating effect=0.017,accounting for 15.727%of the total effect),and served as a partial mediator in the relationship between life attitude orientation and cognitive empathy(mediating effect=0.171,accounting for 25.827%of the total effect).Conclusion:Cognitive reappraisal has a mediating effect on the relationship between medical students'MBTI personality traits and empathy,guiding medical students to improve empathy abilities using cognitive reappraisal.Medical students can further understand their strengths and weaknesses by identifying their MBTI types,and improve their empathy ability in a targeted manner,to better adapt to future medical work.

7.Risk factors of ultrafiltration failure in patients undergoing peritoneal dialysis

Objective:To investigate the incidence and risk factors of ultrafiltration failure (UFF) in patients undergoing continuous ambulatory peritoneal dialysis (CAPD).Methods:The clinical data of 65 patients undergoing CAPD at the Hubei Provincial Corps Hospital of Chinese People's Armed Police Forces and the General Hospital of Central Theater Command from January 2016 to December 2021 were retrospectively analyzed. The clinical data included patient history, smoking history, duration of peritoneal dialysis, incidence of peritonitis, levels of hemoglobin, albumin, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol, total cholesterol, and triglyceride. Univariate and multivariate regression analyses were conducted to investigate the correlation between UFF and various indicators in patients undergoing CAPD.Results:Among the 65 patients undergoing CAPD, the incidence of UFF was 35.4% (23/65). There were significant differences in duration of peritoneal dialysis, history of peritonitis, history of type 2 diabetes mellitus, serum albumin, low-density lipoprotein cholesterol, and triglyceride between patients with UFF and those without UFF ( t = -5.05, χ2 = 11.51, 6.83, t = 5.91, -3.28, -2.83, all P < 0.05). Multivariate regression analysis showed that albumin was negatively correlated with UFF ( r = -1.06, P < 0.05), while duration of peritoneal dialysis, level of low-density lipoprotein cholesterol, and peritonitis were positively correlated with UFF ( r = 0.43, 2.20, 1.67, all P < 0.05). Conclusion:Peritoneal dialysis duration, peritonitis, and low-density lipoprotein cholesterol are risk factors for UFF in patients undergoing CAPD, while albumin is a protective factor against UFF in these patients.
8.Study on the medication law of TCM compounds used in the treatment of non-small cell lung cancer

Objective:To explore the medication law of TCM compounds used in the treatment of non-small cell lung cancer (NSCLC).Methods:Clinical literature about TCM compounds in the treatment of NSCLC was retrieved from CNKI, Wanfang Data, PubMed and Web of Science core collection database from January 1st, 2003 to May 1st, 2023. The Python and the ancient and modern medical record cloud platform V2.3.7 were used to analyses frequency statistics and properties, and clustering analysis, factor analysis and association rule analysis were performed.Results:A total of 866 articles were included, with 660 effective TCM compounds, 647 kinds of Chinese materia medica. There were 36 kinds of high-frequency Chinese materia medica, such as Astragali Radix, Atractylodis Macrocephalae Rhizoma and Poria; the efficacy categories of Chinese materia medica mainly included tonics and phlegm resolving cough relieving and asthma relieving drugs. Cluster analysis obtained 4 clusters, factor analysis extracted 11 common factors, and association rule analysis obtained 37 highly supported combinations of Chinese materia medica. The most reliable triple drug combination among them was Astragali Radix-Atractylodis Macrocephalae Rhizoma-Hedyotis diffusa willd, and the double drug combination was Astragali Radix-Ligustri Lucidi Fructus.Conclusion:TCM for the treatment of NSCLC mainly focuses on strengthening the healthy qi, clearing heat, resolving phlegm and detoxifying toxins, supplemented by tonifying qi and activating blood circulation, dispelling dampness and resolving phlegm.
