1.Research on medication rules for treating diseases related to spleen and stomach in LAN Shi Mi Cang

Objective To analyze the medication patterns of LAN Shi Mi Cang in treating diseases related to the spleen and stomach.Methods Search for prescriptions for treating spleen and stomach-related diseases in LAN Shi Mi Cang,a total of 95 prescriptions and 150 traditional Chinese medicines were included.The usage frequency,four Qi,five Wei and channel tropism of traditional Chinese medicine were analyzed,and the correlation degree,core drugs and new formula combinations were analyzed by using correlation analysis,cluster analysis and complex network analysis.Medication rules of Li Dongyuan in treating diseases related to spleen and stomach were summarized.Results The top three most frequently used herbs are Danggui,Shengma,and Gancao.These herbs generally exhibit warm,neutral,or cold properties,with flavors predominantly pungent,sweet,or bitter,primarily affecting the spleen and stomach meridians.The most closely associated herb pair is Danggui-Shengma.Core herbs include Huangqi,Shengma,and Chaihu,et al.Cluster analysis identified four distinct herbal combinations.Conclusion Li Dongyuan's treatment of diseases related to the spleen and stomach mainly focuses on tonifying the spleen and stomach,harmonizing Qi and blood,sweet and warm to remove heat,and raising Yang to disperse fire.It also treats external infections and internal injuries,as well as symptoms of excess and root deficiency.
2.Over-expression of miR-101 alleviates ventricular remodeling in rat models with acute myocardial infarction by inhibiting JAK2/STAT3 signaling

Objective To explore the effect and underlying molecular mechanism of miR-101 on ventricular remod-eling in rats after acute myocardial infarction(AMI).Methods The AMI rat model was established using the left anterior descending coronary artery ligation method.The AMI rats were randomly divided into AMI group,agomir-NC group,miR-101 agomir group and coumermycin A1 group,another 12 rats were selected as sham group with 12 in each.The targeting relationship between miR-101 and JAK2 was analyzed by Target Scan 8.0 database and double luciferase reporter gene assay.The expression of miR-101 in rat myocardium was detected by RT-qPCR.LVESD,LVEDD,LVEF and LVFS were measured by ultrasonography.The level of IL-1β,IL-6 and TNF-α in rats serum was determined by ELISA.The myocardial tissue lesion and fibrosis were detected by HE staining and Mas-son staining.The expression of collagenⅠand TGF-β in rat myocardial tissue was detected by immunohistochemical staining.The expression of E-cadherin,N-cadherin,Vimentin,p-JAK2,JAK2,p-STAT3 and STAT3 proteins was detected by Western blot.Results Compared with AMI group and agomir-NC group,the myocardial tissue lesions and fibrotic area in miR-101 agomir group were significantly decreased(P<0.05),the level of LVESD,LVEDD,L-1β,IL-6,TNF-α,collagenⅠ,TGF-β,N-cadherin,vimentin,p-JAK2 and p-STAT3 decreased(P<0.05).The levels of miR-101,LVEF,LVFS and E-cadherin were increased(P<0.05).Compared with miR-101 agomir group,the myocardial tissue lesions and fibrotic area in coumermycin A1 group significantly increased(P<0.05),the level of LVESD,LVEDD,L-1β,IL-6,TNF-α,collagenⅠ,TGF-β,N-cadherin,vimentin,p-JAK2 and p-STAT3 was increased(P<0.05).The level of miR-101,LVEF,LVFS and E-cadherin was decreased(P<0.05).Conclusions miR-101 inhibits myocardial inflammatory lesions,myocardial fibrosis and epithelial-mesenchymal fransition(EMT)process after AMI with a mechanism targeting at JAK2/STAT3 signaling pathway,thus alleviates ventricular remodeling in rats after AMI.
3.Long-term protective efficacy of recombinant CHO cell-derived hepatitis B vaccine:A 26-year follow-up study in Zhengding county

Objective:To evaluate the long-term protective efficacy of the recombinant Chinese hamster ovary cell-derived hepatitis B vaccine(CHO-HepB)26 years post-vaccination in the rural China.Methods:Zhengding county,Hebei province was designated as a rural monitoring site for CHO-HepB efficacy. Study participants included individuals born between 1997 and 1999 who had completed the three-dose CHO-HepB primary series without booster doses. A cross-sectional survey was conducted in late 2024 using random sampling. Demographic and vaccination history data were collected via questionnaires,and hepatitis B virus(HBV)serological markers were detected using chemiluminescence. Historical surveillance data were integrated to infer infection statuses of HBsAg-positive individuals and evaluate longitudinal trends in anti-HBs seropositivity and antibody titers.Results:Among 178 participants(mean time since vaccination:26.2 years),the seroprevalence rates were 0.6% for HBsAg(95% CI:0.0%-1.6%),64.6% for anti-HBs(95% CI:57.6%-71.6%),and 1.1% for anti-HBc(95% CI:0.0%-2.7%). Compared to the pre-vaccination baseline HBsAg positivity of 11.3% in children under 10 years of age,the estimated vaccine protection rate was 95%. Two notable cases were identified:one with concurrent HBsAg and anti-HBc positivity and one with anti-HBs and anti-HBc positivity,suggestive of transient HBV exposure(1999—2009)without chronicity. Natural immune boosting was inferred for the latter case based on anti-HBs titer dynamics. Longitudinal analysis of four prior cross-sectional surveys(2005,2009,2013,and 2017)revealed no significant upward trends in HBsAg and anti-HBc positivity(both P>0.05)over 26 years,while anti-HBs seropositivity declined significantly( P<0.05)from 6 to 26 years post-vaccination. Conclusion:The CHO-HepB vaccine demonstrates sustained immunological persistence and robust long-term protection up to 26 years post-immunization. Continued emphasis on rigorous implementation of mother-to-child transmission prevention strategies is critical for future hepatitis B control.

6.Effect of mindfulness-based stress reduction combined with knowledge, attitude and practice nursing in patients with obstructive sleep apnea hypopnea syndrome

Objective:To explore the effect of mindfulness-based stress reduction combined with knowledge, attitude and practice nursing in patients with obstructive sleep apnea hypopnea syndrome (OSAHS) .Methods:From January 2019 to January 2020, convenience sampling was used to select 80 patients with OSAHS admitted to the Department of Sleep Medicine, Second Hospital of Shanxi Medical University as the research object. The control group received routine nursing, and the observation group conducted mindfulness-based stress reduction combined with knowledge, attitude and practice nursing on the basis of the control group, and the intervention period was 1 month. The daytime sleepiness, psychological resilience and quality of life were compared between the two groups before and after the intervention.Results:After the intervention, the Simplified Chinese version of the Epworth Sleepiness Scale score in the observation group was lower than that in the control group, and the total score and all dimension scores of the Connor-Davidson Resilience Scale and the score of each dimension of the Calgary Sleep Apnea Quality of Life Index were higher than those in the control group, with statistical differences ( P<0.05) . Conclusions:Mindfulness-based stress reduction combined with knowledge, attitude and practice nursing can reduce daytime sleepiness in patients with OSAHS, and improve their psychological resilience and quality of life.
7.Clinical features and gene mutation analysis of CDKL5 gene related early-onset epileptic encephalopathy

Objective:To investigate the clinical characteristics and gene mutation of seven cases of CDKL5 gene related early-onset epileptic encephalopathy diagnosed by next-generation sequencing.Methods:The clinical data of children with early-onset epileptic encephalopathy from February 2018 to December 2019 in the Department of Neurology, Children′s Hospital Affiliated to Zhengzhou University were retrospectively analyzed. The whole exome sequencing method was used to analyze the entire exome of the proband, and seven cases of CDKL5 gene mutation positive were screened out, and Sanger sequencing verification on family members was performed to identify the source and the characteristics of gene mutations were analyzed.Results:Among the seven children diagnosed with CDKL5 gene related early-onset epileptic encephalopathy, the ratio of male to female was 2∶5, and the age of onset was 15 days to five months of birth. The clinical phenotypes all included different degrees of developmental delay and repeated seizures, which were manifested as general seizures, myoclonic seizures, convulsive seizures or focal seizures; the outcome of use of antiepileptic drugs to control seizures was poor, and some applications of ketogenic diet had better effects. CDKL5 gene mutation sites were all denovo mutations, including NM_003159: c.772_776del (p.K258Efs *10) frameshift mutation, NM_003159.2 (exon: 9-15) heterozygous deletion, CDKL5 hemizygous deletion, NM_003159: c.268 (exon5) G>T (p.E90 *, 941) and NM_003159: c.2578C>T (p.Q860 *, 171) nonsense mutation, NM_003159: c.211A>G (p.Asn71Asp) and NM_001323289: c.545T>C (p.L182P) missense mutation. Among them, c.772_776del (p.K258Efs *10), c.268 (exon5)G>T and c.2578C>T (p.Q860 *, 171) have not been reported. Conclusions:CDKL5 gene related early-onset epileptic encephalopathy is an early onset epilepsy, which is more common in women, and has different forms of seizures. The early electroencephalogram is characterized as severe abnormal brain discharge, and the disease progresses in various forms. There are no specific changes in head magnetic resonance imaging. Different gene mutation sites may lead to different phenotypes and prognostic differences. Many anti-epileptic treatments are ineffective, and ketogenic diets are effective for some patients.
8. Feasibility study of modified RUSS to predict the stone free rate after flexible ureteroscopic lithotripsy

Objective:
To explore the feasibility of the modified RUSS (Resorlu-Unsal Stone Score) renal stone scoring system in predicting the stone free rate after Flexible Ureteroscopic Lithotripsy(FURL).
Methods:
Retrospective analysis was performed on 104 cases of renal stones treated by FURL from March 2017 to January 2019 in the second affiliated hospital of zhengzhou university, including 75 males and 29 females. Age 20-75 (47.79±13.21) years. BMI 18.2-38.3 (24.4±3.7) kg/m2; Crushed stone site: left 56 cases, right 48 cases. There were 23 cases of renal inferior calyces calculi, 81 cases of non-inferior calyces calculi, and 19 cases of renal inferior calyces calculi with IPA<45°; American Society of Anesthesiology (ASA) scores: 65 cases of grade Ⅰ, 39 cases of grade Ⅱ; There were 71 patients with urinary tract infection before operation. There were 6 cases of renal puncture fistula before operation. Preoperative indwelling ureteral stent in 26 cases; There were 32 cases with history of extracorporeal shock wave lithotripsy. There were 27 patients with a history of urolithiasis therapy. The same physician used preoperative urinary CT+ 3D reconstruction imaging data to measure the factors influencing the postoperative stone free rate. RUSS renal stone scoring system was used to score the stones of patients before operation, and the relationship between the scores and the stones free rate was analyzed. The RUSS renal stone scoring system was supplemented and improved by including staghorn stone, duplicate renal, caliceal diverticulum, renal malrotation, stone area, and CT value related indexes. The modified RUSS renal stone scoring system was used to score the preoperative stone condition of patients, and the relationship between the score and the stone free rate was analyzed. The receiver operating characteristic curve (ROC) was drawn and the area under the curve (AUC) was calculated to compare the advantages and disadvantages of the modified RUSS, the characteristics of the stones, and RUSS system.
Results:
The stone free rate was 69.2% (72/104) 4 weeks after the operation, and there were no postoperative complications. Single-factor analysis showed that stone area, CT value, number of renal calyx involved by stone, multiple stones, IPA, stone size grading, renal anatomic structure abnormality, staghorn stone were all related risk factors affecting postoperative patients. Multi-factor analysis showed that stone area, number of renal calyces involved by stone, multiple stones, IPA and stone size were independent risk factors affecting the stone free rate after FURL. RUSS scores ranged from 0 to 3 points, corresponding to stone clearing rates of 86.8% (33/38), 67.7% (23/34), 58.3% (14/24) and 25.0% (2/8), respectively. 9.Diagnostic value of mitral regurgitation jet volume in the quantification of mitral regurgitation severity by general imaging three-dimensional quantification

Objective To evaluate diagnostic value of mitral regurgitation jet volume(MRvol) in the quantification of mitral regurgitation severity by general imaging three-dimensional quantification (GI3DQ) using the guideline recommended 2D integrative method as a reference.Methods Ninety-three patients with MR were divided into central MR group(n =41) and eccentric MR group(n =52).The American Society of Echocardiography (ASE)-recommended 2D integrative method was used as a reference for MR grading and MRvol was directly measured by GI3DQ method.Results In central MR,as assessed by receiver operating characteristic (ROC) analysis,the area under the curve(AUC)was 0.87(P <0.0001), and MRvol by GI3DQ at a cutoff value of 16.2 ml yielded 96.2% of sensitivity and 63.6% of specificity to differentiate mild from moderate MR;the AUC was 0.98(P < 0.0001),and a cutoff value of 47.8 ml yielded 98.6% of sensitivity and 96.2% of specificity to differentiate moderate from severe MR. In eccentric MR,the AUC was 0.76(P =0.086),and MRvol at a cutoff value of 14.8 ml yielded 90.9% of sensitivity and 60.0% of specificity to differentiate mild from moderate MR;the AUC was 0.84(P <0.0001) and a cutoff value of 40.7 ml yielded 80.0% of sensitivity and 79.7% of specificity to differentiate moderate from severe MR.Conclusions MRvol measured directly by GI3DQ could more exactly evaluate MR severity,and have better sensitivity and specificity to differentiate moderate from severe MR in central MR.
10.Clinical efficacy evaluation of vitrectomy combined with autologous internal limiting membrane transplantation for large macular hole

Background Giant idiopathic macular hole (IMH) severely affects visual acuity and increases operative difficulty during the surgery,so modifying or optimizing the operation method is helpful for improving the prognosis.Objective This study aimed to evaluate the efficacy and safty of vitrectomy combined with free internal limiting membrane transplantation for large IMH.Methods A prospective serial cases-observational study was carried out under the informed consent of each patient.Forty-two eyes of consecutive 42 patients with IMH of mean diameter (814.31±112.95)μm were included in the Second Hospital of Hebei Medical University from January 2013 to November 2015.All the eyes received vitrectomy combined with free internal limiting membrane transplantation and 12% C3F8 filling.The best corrected visual acuity (BCVA) (LogMAR),inner segment/outer segment (IS/OS) defect range,external limiting membrane defect range,retinal thickness at macular fovea were measured with slit lamp microscope,indirect ophthalmoscope and spectral-domain optical coherence tomography (SD-OCT) before surgery and 1,3,6 and 12 months after surgery.Results IMH complete closure in 97.6% eyes (41/44) at 12 months after surgery.BCVA was improved after operation and showed a significant difference among various time points (F =28.032,P<0.001).The IS/OS defect range was (1 112.00±45.44),(859.00±84.55),(649.00±52.47),(486.00±46.88) and (320.00±45.13) μm before surgery and 1,3,6 and 12 months after surgery,showing a significant difference among different time points (F=38.761,P<0.001),and the IS/OS defect range was gradually shrinked after operation compared with that before operation (all at P<0.05).The mean defect range of external limiting membrane was (1 038.00 ±39.63),(748.00±64.12),(585.00±48.88),(438.00±42.84) and (265.00±28.97)μm before surgery and 1,3,6 and 12 months after surgery,with a significant difference among various time points (F=36.459,P<0.001),and the mean defect range of external limiting membrane was evidently reduced (all at P<0.05).The foveal retinal thickness value was increased at 3,6 and 12 months after surgery,which was significantly higher than that 1 month after surgery (all at P<0.05).The high reflect signal was faded away at 3 months after operation,indicating that implanted internal limiting membrane was decomposed and metabolized.Conclusions Vitrectomy combined with autologous internal limiting membrane transplantation seems to be safe and effective for large IMH.
