1.A systematic review of quality assessment tools for pediatric palliative care based on COSMIN guidelines

Objective To evaluate the methodological quality and measurement attribute quality of the evaluation tool for pediatric palliative care quality assessment tools,and to provide references for medical staff to select the best assessment tools.Methods The PubMed,Embase,Cochrane Library,Web of Science,CINAHL,Scopus,China National Knowledge Infrastructure(CNKI),Wanfang Database,VIP Database,Chinese Biomedical Literature Database,GIN,NGC,NICE,NRAO,medlive,WHO,AAHPM,WHPCA,APHN were searched from inception to March 28,2024.Data were screened and extracted independently by 2 researchers.The consensus-based standards for the selection of health measurement instruments(COSMIN)checklist and quality criteria were employed to evaluate the methodological quality and psychometric properties of the included pediatric palliative care quality assessment tools.Finally,recommendations were formulated based on these evaluations.Results A total of 13 articles were included,involving 9 pediatric palliative care quality assessment tools.Among them,the PICU-QODD,PaPEQu and QCPCI demonstrated good content validity and internal consistency,and are recommended as Grade A.The remaining assessment tools are recommended as Grade B or C.Conclusion The PICU-QODD,PaPEQu and QCPCI are recommended for use,but further validation of their psychometric properties is still needed.

3.Construction of PD-1 overexpressing bacterial cytoplasmic membrane vesicles and evaluation of its targeting efficacy of mouse lung cancer xenograft tissue

Objective:To construct bacterial cytoplasmic membrane nanovesicles(BMV)with overexpressing programmed death 1(PD-1),denoted as BMV-PD-1 and evaluate the targeting efficacy of BMV-PD-1 towards transplanted lung tumor tissues in mice.Methods:The fusion plasmid ClyA-PD-1-EGFP fused by PD-1 and Cytolysin A(ClyA)was transferred into Escherichia coli BL21-Codonplus through plasmid transformation.Laser confocal microscopy,SDS-PAGE,and WB were used to detect the expression of the fusion protein ClyA-PD-1-EGFP.Bacterial membranes were extracted and processed with an extruder to generate BMV-PD-1.TEM and NTA were utilized to assess the morphology,size distribution,and zeta potential of BMV-PD-1,while WB was used to verify the presence of PD-1 protein.Laser confocal imaging was conducted to monitor the uptake of BMV-PD-1 by Lewis lung cancer cells.A C57BL/6J mouse subcutaneous transplant tumor model of LLC lung cancer cells was constructed,and the tumor targeting of BMV-PD-1 was evaluated by small animal imaging system.Results:Laser confocal microscopy images demonstrated that the plasmid ClyA-PD-1-EGFP was transferred into BL21-Codonplus and successfully expressed into protein.SDS-PAGE results suggested that ClyA-PD-1-EGFP was overexpressed in BL21-Codonplus.WB analysis indicated that PD-1 was expressed in bacteria and highly expressed in BMV-PD-1(P<0.001).NTA and TEM analyses revealed that BMV-PD-1 were spherical vesicles with a diameter of(145±14)nm and a negative surface charge.Laser confocal imaging showed that the high expression of PD-1 significantly increased the uptake of BMV-PD-1 by lung cancer cells(P<0.01).In vivo imaging of small animals further confirmed that the high expression of PD-1 can effectively improve cancer targeting of BMV-PD-1(P<0.01).Conclusion:In this study,bacterial plasma membrane nanovesicles BMV-PD-1 with high PD-1 expression are successfully constructed,and it is found that PD-1 overexpression markedly improve the mouse lung cancer xenograft tissue targeting specificity of BMV-PD-1,laying the groundwork for further development of BMV-PD-1 as a carrier for targeted drug delivery systems in tumors.
4.Discussing the new implications of"toxins damaging brain collaterals"from the pathogenesis of acquired cerebral amyloid angiopathy

Acquired cerebral amyloid angiopathy,as a cerebrovascular disease discovered in recent years,mainly spreads through iatrogenic factors.This paper focuses on acquired cerebral amyloid angiopathy and delves into the new applications and profound implications of the"toxins damaging brain collaterals"theory proposed by academician WANG Yongyan in stroke research.Starting from the perspective of"toxins damaging brain collaterals",it comprehensively analyzes the causes and pathological mechanisms of acquired cerebral amyloid angiopathy,emphasizing the importance of external toxins,latent toxins,and turbid toxins in understanding the pathogenesis of the disease.It reveals that"toxins damaging brain collaterals"is the core mechanism of acquired cerebral amyloid angiopathy.On this basis,it proposes a new connotation of"toxins damaging brain collaterals":first,"toxin"includes both internal and external toxins;second,the organic integration of"external wind theory"and"toxins damaging brain collaterals"guides the improvement of theoretical research,clinical treatment,and prevention strategies for stroke,and promotes a deeper understanding of the disease.This approach to understanding acquired cerebral amyloid angiopathy from the perspective of"toxins damaging brain collaterals"not only demonstrates the effective integration of traditional Chinese medicine theory and modern biomedical science,but also provides insights and references for the clinical diagnosis and treatment of the disease.
5.Needs of hospice care for end-stage AIDS patients: a meta-synthesis of qualitative research

Objective:To systematically evaluate the hospice care needs of end-stage AIDS patients.Methods:Qualitative research on the hospice care needs of end-stage AIDS patients was electronically searched on China National Knowledge Infrastructure, WanFang Data, China Biology Medicine disc, PubMed, Cochrane Library, PsychINFO, Web of Science, Embase, and so on. The search period was from database establishment to November 20, 2023. Quality evaluation criteria for qualitative research of the Joanna Briggs Institute Evidence-Based Health Care Center was used to evaluate the quality of literature. The Meta-synthesis method was used to integrate the results.Results:A total of 14 articles were included. Sixty-nine results were extracted and classified into six categories, resulting in five integrated results, including personalized support and care needs, psychological counseling and dignity maintenance, social assistance, spiritual sustenance, and information and decision-making needs.Conclusions:The end-stage AIDS patients have multi-dimensional needs for hospice care. Hospital and community staff should pay attention to the hospice care needs of end-stage AIDS patients and provide adequate care for patients.

7.Effect analysis of carotid endarterectomy for high bifurcation carotid artery stenosis

Objective:To explores the clinical efficacy and safety of carotid endarterectomy in patients with high bifurcation carotid artery stenosis.Methods:A total of 169 patients with carotid artery stenosis (147 patients with non-high bifurcation carotid artery stenosis and 22 patients with high bifurcation carotid artery stenosis), underwent carotid endarterectomy under surgical microscope in our hospital from January 2017 to January 2020, were included in the study. Patients with high bifurcation carotid artery stenosis were operated by cutting off the posterior abdomen of the digastric muscle to assist in exposing the distal end of the internal carotid artery plaque. Cervical CTA/DSA examination was performed within one week of surgery to confirm whether carotid artery stenosis was relieved; ultrasound examination of cervical vessels was performed 6 months and 1 year after surgery to determine whether restenosis was appeared in the carotid artery. The surgical efficacy, perioperative complications and re-examination results were compared between the two groups.Results:Postoperative carotid artery stenosis was relieved in both groups. The proportions of new cerebral infarction and temporary neurological impairment between the two groups (4.5% vs. 2.0%; 9.1% vs. 3.4%) showed no significant differences ( P>0.05). Carotid artery restenosis was not found in both groups. There was no obvious functional abnormality in the digastric muscles of patients with high bifurcation carotid stenosis. Conclusion:Cutting off the posterior abdomen of the digastric muscle during carotid endarterectomy can better expose the distal end of the internal carotid plaque in patients with high bifurcation carotid stenosis, and provide convenience for effective relief of carotid artery stenosis with high safety.
8.Relationship of Microchannels and Plaque Erosion in Patients with ST-Segment Elevation Myocardial Infarction: An Optical Coherence Tomography Study

Objective::Microchannels are associated with the progression of atherosclerotic vulnerable plaques. However, in patients with culprit optical coherence tomography (OCT)-defined plaque erosion, the knowledge of microchannels and culprit lesion vulnerability is limited. The aim of this study was to investigate culprit lesion characteristics in patients with ST-segment elevated myocardial infarction (STEMI) caused by plaque erosion with and without microchannels using OCT.Methods::In all, 348 STEMI patients with plaque erosion who underwent OCT of the culprit lesion at the 2 nd Affiliated Hospital of Harbin Medical University (Harbin, China) from August 2014 to December 2017 were included and divided into the microchannel group ( n= 116, 33.3%) and no-microchannel group ( n = 232, 66.7%). The clinical characteristics and OCT-derived plaque features were compared between both groups. Results::Among the 348 STEMI patients with plaque erosion, culprit lesions with microchannels had higher incidence of lipid plaque (59.5% vs. 45.3%, P= 0.012); calcification (41.4% vs. 24.6%, P= 0.002); spotty calcification (30.2% vs. 18.1%, P= 0.014); macrophages accumulation (72.4% vs. 45.7%, P < 0.001); and cholesterol crystals (32.8% vs. 14.2%, P < 0.001) than those without microchannels. In addition, minimal lumen area was smaller ((1.9 ± 0.9) mm 2vs. (2.8 ± 2.3) mm 2, P < 0.001) and lumen area stenosis was greater ((71.3% ± 13.4%) vs. (65.3% ± 19.3%), P= 0.001) in the microchannel group than in the no-microchannel group. Conclusion::In patients with STEMI caused by plaque erosion, one-third manifested typical microchannel characteristics, and those with microchannels were associated with more severe luminal stenosis and more vulnerable plaque features than those without microchannels.

10.Association between lempel ziv complexity of moderate to vigorous intensity physical activity time series and physical fitness among adolescents

Objective:
To investigate the variability of the time and frequency of Moderate to Vigorous Physical Activity (MVPA) during the school days and weekends of junior high school students, and to explore the correlation between MVPA and physical fitness.
Methods:
A cluster sampling method was used to investigate 79 students of grade 1 and grade 2 in Beijing Huilongguan Middle School. Adapted one day physical activity recording tool was implemented to investigate students’ physical activity from 8:00 to 23:00 a week, activity records were classified into daily MVPA parts and non MVPA parts based on MET values. Physical activity time series for 5 consecutive days and 2 weekends, as well as MVPA total time and the lempel ziv complexity of the sequence was calculated. Investigated the correlation between the MVPA situation and the results of national students’ physical fitness standard test.
Results:
There was no statistical difference in MVPA time between males and females during school days( t =-0.90, P >0.05). Females had greater variability in MVPA than males(58.9%, 53.5%). There was also no statistical difference in MVPA complexity between genders( t =-0.55, P >0.05). During weekends, there was no significant difference in the total MVPA time between men and women( t =-0.13, P >0.05). The CV of male MVPA time was lower than that of female(141.5%, 152.8%). There was also no significant difference( t =-0.63, P >0.05), and male MVPA complex CV was slightly higher than that of female ( 55.1% , 48.1%). The MVPA time CV of adolescents during weekdays was slightly lower for males (weekdays vs weekends: 53.5%, 141.5%) than females(58.9%, 152.8%). The MVPA complexity in weekdays among males was moderately correlated with BMI( r =0.41), and the MVPA time and weight of females ( r =-0.30) and BMI ( r =-0.31) were low. The complexity was related to body weight ( r =-0.31) and BMI ( r =-0.33) to a low degree( P <0.05).
Conclusion
Junior middle school students are more active on school days than weekend, but the MVPA time series are limited. Schools can effectively improve physical fitness by spreading out the activity period of MVPA in one day. MVPA complexity, as a more comprehensive indicator, may be more sensitive to BMI of junior middle school students.
