1.Analysis on Construction of Whole-course Management Model for Panvascular Diseases

Panvascular diseases are systemic diseases with atherosclerosis as the pathological core, involving multiple vascular beds and target organs throughout the body. Due to their wide range and complexity, the traditional single-discipline prevention and treatment model struggles to meet the needs of systematic management, while clinical diagnosis often remains one-sided and insufficient, leading to delayed treatment. Literature reviews show that panvascular diseases involve a wide range of lesion sites, numerous influencing factors, and are prone to endangering life and health. It is urgent to construct a comprehensive and whole-course prevention and treatment management system, with vascular health as the goal and patients as the core. First, early screening and risk assessment should be conducted for high-risk groups. In terms of treatment decisions for patients, multi-disciplinary collaboration is needed to establish a scientific and standardized prevention and treatment path. Second, it is important to attach great importance to a people-centered approach, enhance patients' familiarity with the disease through cognitive intervention, and shift from passive treatment to active health care. Thirdly, it is needed to leverage the advantages of modern science and technology, promote the deep integration of artificial intelligence innovations and modern medicine, and help traditional diagnosis and treatment plans evolve towards precision, intelligence, and personalization. This will open up new paths for the modernization of the whole-course management of pan-vascular diseases. Fourth, efforts should be made to continue to carry forward and innovate the characteristics of traditional Chinese medicine, adhere to equal emphasis on modern and traditional medicine, promote complementary advantages and coordinated development of Chinese and Western medicine, and form a unique Chinese model for the whole-course management of panvascular diseases. Fifth, through the reintegration and redistribution of government, medical insurance, and medical resources, comprehensive talents in the broad vascular disciplines should be cultivated and an efficient hierarchical management model established, providing reference and guidance for the whole-course management of comprehensive diseases in the future.
2.Cross-sectional survey of healthcare-associated infection in 5 736 medical institutions across China in 2024

Objective To understand the current situation of healthcare-associated infection(HAI)in China,pro-vide data support and decision-making basis for formulating scientific and effective strategies for HAI prevention and control.Methods A nationwide cross-sectional survey on HAI was conducted among various types and levels of medical institutions in China according to a unified protocol of bedside surveys and case investigations.Results In 2024,a total of 5 736 medical institutions and 2 751 765 patients were surveyed.Among them,34 889 HAI cases were identified,with a prevalence rate of 1.27%.The number of HAI episodes was 38 032,and case prevalence rate was 1.38%.The prevalence rate of HAI in medical institutions in different regions of China ranged from 0.66%to 2.35%.Among medical institutions of different scales,those with a bed capacity of ≥900 had the high-est incidence of HAI,reaching 1.65%.The most common infection site was the lower respiratory tract(44.66%),followed by the urinary tract(12.94%),surgical site(9.32%),upper respiratory tract(7.02%),and bloodstream infection(5.78%).The top 3 departments with the highest HAI rates were the general intensive care unit(10.02%),department of neurosurgery(5.51%),and department(group)of hematology(5.34%).A total of 23 238 strains of HAI pathogens were detected,with 10 714 strains(46.10%)from lower respiratory tract speci-mens.The top 5 detected strains were Klebsiella pneumoniae(14.76%),Pseudomonas aeruginosa(13.33%),Escherichia coli(12.79%),Acinetobacter baumannii(9.23%),and Staphylococcus aureus(7.88%).231 944 pa-tients underwent class Ⅰ incision surgery were monitored,with 1 647 cases experienced surgical site infection,and the prevalence rate of surgical site infection was 0.71%.The number of patients who should undergo pathogen de-tection(patients receiving therapeutic and therapeutic combined prophylactic antimicrobial agents)was 715 179,while the actual number was 480 492,with a pathogen detection rate of 67.18%.425 225 patients received patho-genic detection before treatment,with a detection rate of 59.46%.Conclusion The overall HAI prevalence in Chi-na is lower,showing disparities among medical institutions of different regions and scales.Therefore,precise imple-mentation of measures is necessary for HAI prevention and control,with a focus on high-risk institutions and high-risk departments,key areas,and critical procedures.All levels of medical institutions should continuously reduce the incidence of HAI by strengthening monitoring,standardizing the use of antimicrobial agents,and reinforcing basic HAI prevention and control measures.
3.Prevalence and molecular characterization of Shiga toxin-producing Esch-erichia coli in domestic goats in the Chengkou District of Chongqing

This study investigated the infection status,drug resistance,and molecular characteristics of Shiga toxin-producing Escherichia coli(STEC)in domestic goats in Chengkou county,Chongqing.In August 2023,283 fecal samples were collected from households in Chengkou county.After enrichment with EC broth and inoculation onto selective media,samples that tested positive for stx1/stx2 were selected for further isolation.The positive strains were investigated with antimicrobial susceptibility testing and whole genome sequencing.According to the whole genomic sequences,the stx subtypes,serotypes,multi-locus sequence types,virulence genes,drug resistance genes,and phylogenetic relationships of the STEC strains were analyzed.Forty-six strains of STEC were isolated from 283 goat fecal samples,thus resulting in a detection rate of 16.25%.The 46 STEC strains were categorized into 12 O∶H serotypes,among which O76∶H19 and O8∶H7 predominated,each represented by 9 strains.Five STEC strains were identified as serotype O157∶H7.The 46 STEC strains were categorized into 11 sequence types(STs),among which ST675 and ST196 predominated,each represented by nine strains,accounting for a 19.57%proportion.The strains were categorized into 7 stx subtypes,among which stx1c(26/46,56.52%),followed by stx2k(9/46,19.57%)predominated.All nine Stx2k-STEC strains were identified as serotype O8∶H7 and sequence type ST196.In antimicrobial susceptibility testing,2 STEC strains were resistant to ampicillin,one strain was resistant to ampicillin/sulbactam,one strain was resistant to cefazolin,and one strain was resistant to cefoxitin.Nine Stx2k-STEC strains were found to carry the beta-lactam resistance gene blaEC-18.Antimicrobial sensitivity tests revealed that the nine Stx2k-STEC strains were sensitive to all 15 tested antibiotics.Moreover,phylogenetic analysis indicated that the 9 Stx2k-STEC strains were remarkably similar but showed high genetic diversity with respect to that of the Stx2k-STEC strains isolated from other regions in China.Goatsare an important animal reservoir for STEC in theChengkou district of Chongqing,and novel sequence type Stx2k-STEC strains distinct from those found in other regions of China were identified in this region.
4.Sealed percutaneous lung biopsy tract with different sealants:Comparison on complication incidence

Objective To observe the complication incidences after percutaneous lung biopsy and sealed the tract with different sealants.Methods A total of 129 patients with solitary pulmonary nodule who underwent CT-guided percutaneous lung biopsy were retrospectively included and divided into group A(n=37),B(n=47)and C(n=45).The biopsy tract was sealed with sealant A(1 g gelatin sponge particles mixed with 10 ml 50%glucose solution)in group A,with sealant B(1 g gelatin sponge particles mixed with 10 ml normal saline)in group B,while with sealant C(1 g gelatin sponge particles mixed with the coagulant enzyme from Bothrops atrox venom and 10 ml normal saline)in group C.The incidence rate of complications such as pneumothorax and hemoptysis were comparatively observed among groups.Binary logistic regression was performed to screen the independent influencing factors associated with complications of percutaneous lung biopsy.Results No significant difference of gender,age,proportion of smoking history nor emphysema,diameter of pulmonary nodules,depth of puncture into lung parenchyma nor times of puncture was found among groups(all P>0.05).Complications occurred in 43 cases(43/129,33.33%),i.e.27 cases in group B(27/47,57.45%),11 cases in group A(11/37,29.73%)and 5 cases in group C(5/45,11.11%),and the complication rates decreased order of group B,A and C(all P<0.05).Compared with sealant A,sealant B was associated with increased risk(OR[95%CI]=3.190[1.183,8.598],P=0.022),whereas sealant C was associated with reduced risk(OR[95%CI]=0.266[0.079,0.889],P=0.031)of complications.Conclusion After percutaneous lung biopsy,the complication incidences decreased sequentially when the needle tract was sealed with saline B,A and C.

6.Complete genomic sequence analysis of the G6P1bovine rotavirus BLL strain

Bovine rotavirus(BRV)is an important pathogen causing diarrhea in calves.To understand the genomic charac-teristics and genetic variations in bovine rotavirus,and to further enrich data on the biological characteristics of rotavirus,we aimed to amplify 11 gene segments of the isolated and cultured G6P[1]bovine rotavirus BLL strain,perform whole genome se-quencing,and analyze the molecular characteristics.MEGA7.0 and DNAMAN software were used for homology and typing a-nalysis,and the whole genome phylogenetic tree was constructed to analyze genetic evolution relationships.The complete geno-type of the BLL strain was G6-P[1]-I2-R2-C2-M2-A3-N2-T6-E2-H3.Phylogenetic analysis of the VP7 and VP4 genes of the BLL strain showed that the VP7 gene had the highest homology with RVA/Cow-wt/HB01/China/2021,and the VP4 gene of the BLL strain was in the same branch as RVA/Human-tc/ISR/Ro8059/1995.From the sequence alignment of VP8*amino acids,the sialic acid domain of the BLL strain was found to be similar to that in other P[1]strains,but different from those in other types of strains,except for residue 189,which was the same as that in Ro8059 but different from that in other strains.The results suggested that the BLL strain might potentially infect humans.Therefore,continued monitoring and study of the biological characteristics of this strain are necessary to provide more information and evidence supporting further research on the cross-species transmission of group A rotavirus in China.

8.Abdominal superficial ultrasound-guided interventional treatment for short-term efficacy and its impact on immune levels in patients with advanced pancreatic cancer

Objective:To investigate short-term efficacy of interventional treatment guided by abdominal superficial ultrasound and its impact on immune levels in patients with advanced pancreatic cancer.Methods:A total of 152 patients diagnosed with advanced pancreatic cancer from May 2018 to March 2020 in the People's Hospital of Yuechi were selected as study subjects,with 84 patients treated with combination chemotherapy and 68 patients treated with chemotherapy alone.After 1∶1 propensity score match-ing,a total of 60 patients were included in each group.Clinical data were compared between matched groups,and effect of different treatment methods on short-term efficacy and immune levels in advanced pancreatic cancer were evaluated by generalized estimating equations.Results:There were no statistically significant differences in age,BMI,tumor location,gender,tumor size and tumor stage between matched groups(P>0.05).When comparing efficacy of matched groups,local control rate(LCR)showed a declining trend after treatment(P<0.05),median durations of local progression-free survival(LPFS)and overall survival(OS)in chemotherapy group were higher than monotherapy group(P<0.05).There was no statistically significant difference in CA19-9 level before treatment between combination chemotherapy group and monotherapy chemotherapy group(P>0.05).In combiuation chemotherapy group,CA19-9 level showed a continuous downward trend after treatment,with a significant decrease at 6-month follow-up compared to be-fore treatment;in chemotherapy alone group,CA19-9 level showed an overall decrease followed by an increase,with the lowest point at 1 month,and a significant decrease compared to before treatment,while no significant differences in remaining follow-up periods(P>0.05).There was no significant difference of CA19-9 level at 7 days and 14 days after treatment between monotherapy chemotherapy group and combination chemotherapy group(P>0.05).However,at 1 month,3 months and 6 months after treatment,CA19-9 level in combination chemotherapy group was significantly lower than monotherapy chemotherapy group(P<0.05).Immune levels in combina-tion chemotherapy group showed an initial increase followed by a decrease,reaching the highest point at 1 month,and total T-cell count showed a significant decrease after 6 months of treatment compared to before treatment(P<0.05).CD3+T,NK,IL-2 and IFN-γ immune levels were still higher than before treatment(P<0.05),with no significant differences in remaining indicators compared to before treatment(P>0.05).CD4+T/CD8+T in chemotherapy alone group did not show a significant trend,and remaining indicators showed same trend as combined chemotherapy group(P<0.05);after 6 months of treatment,total T-cell count,CD3+T cell count,NK cell count and IL-2 level were lower than before treatment(P<0.05),with no significant differences in remaining indicators compared to before treatment(P>0.05).During follow-up period,combined chemotherapy group showed higher levels of total T cells,CD3+T cells,NK cells,IL-2,IFN-γ and IL-6 levels compared to single chemotherapy group(P>0.05).CD4+T/CD8+T at 14 days and 3 months of follow-up was significantly higher in combined chemotherapy group than single chemotherapy group(P<0.05),while differences were not significant at other follow-up times(P>0.05).Generalized estimating equation evaluation showed that all indica-tors were statistically significant.Conclusion:Interventional treatment guided by abdominal superficial ultrasound can effectively improve short-term efficacy and immune levels in patients with advanced pancreatic cancer.
9.Application of totally laparoscopic right thoracic esophagojejunostomy in adenocarcinoma of the esophagogastric junction (AEG) surgery

Objective:This article introduces a novel technique for totally laparoscopic, right thoracic approach, esophagojejunostomy for digestive tract reconstruction.Methods:A retrospective analysis was conducted on the clinical data of patients with adenocarcinoma of the esophagogastric junction who successfully underwent totally laparoscopic esophagojejunostomy via the right thoracic approach at Union Hospital, Tongji Medical College, Huazhong University of Science and Technology, between February 2022 and March 2022.The surgical procedure was performed as follows:(1)Following total laparoscopic resection of the gastric tumor and lymph node dissection, the specimen was transected distal to the tumor margin. The specimen was then placed into a retrieval bag and extracted through the umbilical observation port.(2)Dissection was continued through the esophageal hiatus to mobilize the esophagus. The tumor-bearing tissue, along with the esophagus, was delivered into the thoracic cavity via the esophageal hiatus.(3)The jejunum was transected 20 cm distal to the ligament of Treitz. The distal Jejunum was mobilized for 15-20 cm and subsequently delivered into the thoracic cavity through the esophageal hiatus.(4)A Roux-en-Y jejunojejunostomy was constructed 45-50 cm distal to the cut end of the distal jejunal limb; the mesenteric defect was closed, and the duodenal stump was reinforced.(5)The patient was repositioned into the left lateral decubitus position. Port placement was established as follows: the observation port at the 7th intercostal space (ICS) in the right midaxillary line, the main operating port at the 4th ICS in the anterior axillary line, and the assistant operating port at the 9th ICS in the scapular line.(6)The main operating port incision was enlarged. Using a purse-string instrument, the esophagus was clamped and transected at least 5 cm proximal to the upper tumor margin, and the specimen was removed. (7)The distal jejunum was delivered into the thoracic cavity via the esophageal hiatus. Under total laparoscopic visualization, esophagojejunostomy was completed.Results:Both patients who underwent totally laparoscopic esophagojejunostomy via the right thoracic cavity successfully completed the procedure without conversion to laparotomy, unplanned reoperation, or any intraoperative/postoperative complications. The patients recovered well postoperatively, with no evidence of abdominal or thoracic hemorrhage. Postoperative computed tomography (CT) scans of the chest and abdomen confirmed the absence of anastomotic leakage or other related complications.Conclusions:The esophagojejunostomy was performed totally laparoscopically via the right thoracic cavity. This approach overcomes the drawback of significant trauma associated with open surgery while ensuring safe esophageal resection margins and thorough lymph node dissection. This technique offers advantages including minimal invasiveness, accelerated postoperative recovery, and a reduced incidence of reflux esophagitis. To our knowledge, no similar method of digestive tract reconstruction has been reported in the literature. Its novelty and clinical potential may offer new therapeutic options for patients with Siewert type II adenocarcinoma of the esophagogastric junction (AEG).

Objective To observe the complication incidences after percutaneous lung biopsy and sealed the tract with different sealants.Methods A total of 129 patients with solitary pulmonary nodule who underwent CT-guided percutaneous lung biopsy were retrospectively included and divided into group A(n=37),B(n=47)and C(n=45).The biopsy tract was sealed with sealant A(1 g gelatin sponge particles mixed with 10 ml 50%glucose solution)in group A,with sealant B(1 g gelatin sponge particles mixed with 10 ml normal saline)in group B,while with sealant C(1 g gelatin sponge particles mixed with the coagulant enzyme from Bothrops atrox venom and 10 ml normal saline)in group C.The incidence rate of complications such as pneumothorax and hemoptysis were comparatively observed among groups.Binary logistic regression was performed to screen the independent influencing factors associated with complications of percutaneous lung biopsy.Results No significant difference of gender,age,proportion of smoking history nor emphysema,diameter of pulmonary nodules,depth of puncture into lung parenchyma nor times of puncture was found among groups(all P>0.05).Complications occurred in 43 cases(43/129,33.33%),i.e.27 cases in group B(27/47,57.45%),11 cases in group A(11/37,29.73%)and 5 cases in group C(5/45,11.11%),and the complication rates decreased order of group B,A and C(all P<0.05).Compared with sealant A,sealant B was associated with increased risk(OR[95%CI]=3.190[1.183,8.598],P=0.022),whereas sealant C was associated with reduced risk(OR[95%CI]=0.266[0.079,0.889],P=0.031)of complications.Conclusion After percutaneous lung biopsy,the complication incidences decreased sequentially when the needle tract was sealed with saline B,A and C.
