AIM To investigate the protective effects of verbascoside on D-galactose-induced liver injury in mice and its underlying mechanisms.METHODS C57BL/6J mice were randomly assigned to the normal group, the model group, the vitamin E group(100 mg/kg),and the low-dose and high-dose verbascoside groups(40,80 mg/kg),with 10 mice in each group.Simultaneous administration of medicine and subcutaneous injection of D-galactose(600 mg/kg)went on among the groups except the normal group for 8 weeks.Serum ALT,AST,ALP activities, along with TBil levels were measured using biochemical kits.Hepatic GSH,MDA concentrations,as well as SOD and GSH-Px activities were quantified.Liver pathological morphology was evaluated by HE staining, while hepatic fibrosis area was assessed using Sirius red staining.Western blot analysis determined hepatic expression of IL-6,IL-1β,TNF-ɑ,TLR4,NF-κB p65,IκBɑ and p-IKBɑ proteins.RESULTS Compared to the model group, the groups treated with vitamin E or verbascoside demonstrated significantly reduced body weight(P<0.05,P<0.01);increased hepatic index(P<0.05,P<0.01);decreased serum activities of ALT,AST and ALP alongsided reduced TBil levels(P<0.05,P<0.01);attenuated pathological damage of liver tissue and fibrosis severity;reduced hepatic MDA level(P<0.05,P<0.01);and elevated GSH level with enhanced SOD and GSH-Px activities(P<0.05,P<0.01).Furthermore,the high-dose verbascoside group showed significantly decreased hepatic expressions of IL-6,IL-1 β,TNF-ɑ,TLR4,NF-κB p65,and p-IKBɑ/IKBɑ proteins(P<0.05,P<0.01).CONCLUSION Verbascoside improves D-galactose-induced liver injury through its antioxidant activity,anti-inflammatory effects, and suppression of the TLR4/NF-κB signaling pathway.

Objective:To summarize the clinical characteristics of plaque psoriasis patients with thrombocytopenia, and to evaluate the efficacy and safety of biologics in such patients. Methods:A single-center retrospective cohort study was conducted. Clinical data were collected from plaque psoriasis patients with thrombocytopenia at the Department of Dermatology, Peking University Third Hospital between January 2017 and October 2024. Comparative analysis was conducted on clinical data, such as platelet counts, before and after the use of biologics, and the efficacy and safety of biologics were evaluated. Results:Eleven patients (10 males, 1 female; age range: 33 - 72 years) had thrombocytopenia prior to biologic therapy. Thrombocytopenia was caused by hypersplenism secondary to liver cirrhosis in 7 patients, and the causes of cirrhosis including prior medications for psoriasis (5 cases) and viral hepatitis (2 cases) ; 3 patients were diagnosed with primary immune thrombocytopenia (ITP), and 1 patient with aplastic anemia. All the 11 patients achieved a Psoriasis Area and Severity Index (PASI) 90 response after biologic therapy. Only one patient experienced a transient episode of further decrease in platelet counts, which was considered potentially related to anti-tuberculosis drugs. The Wilcoxon signed-rank test showed no significant decrease in platelet counts after biologic therapy in the 11 patients (pre-treatment platelet counts M [ Q1, Q3]: 77 [55, 87] × 10 9/L, post-treatment platelet counts: 84 [65, 114] × 10 9/L, P = 0.083) ; notably, 3 patients with ITP showed an upward trend in platelet counts after treatment with interleukin (IL) -17A or IL-23 inhibitors. Conclusions:Thrombocytopenia may not be a contraindication for biologic therapy in patients with plaque psoriasis. Plaque psoriasis patients with ITP may obtain dual benefits from the use of IL-17A inhibitors or IL-23 inhibitors.

Objective:To summarize the clinical characteristics of elderly patients with stage Ⅰ diffuse large B-cell lymphoma (DLBCL) and analyze the factors associated with prognosis. Methods:A case series study was conducted by retrospectively collecting clinical data from patients aged over 60 years with newly diagnosed stage Ⅰ DLBCL across 20 medical centers in Jiangsu Province, China, between June 2010 and April 2023. The involved site, classification and treatment plan were summarized. The primary endpoints were progression-free survival (PFS) and overall survival (OS). Statistical analyses were performed using the Kaplan-Meier method, and Cox regression model. Results:The study included 255 patients with a median age of 69 years, of whom 130 (51.0%) were male, 66 (25.9%) were aged ≥75 years and 26 (10.1%) had a high Charlson Comorbidity Index (CCI) score of ≥2. Extranodal involvement was observed in 163 (63.9%) patients, with the stomach (37.4%, 61/163), intestine (19.0%, 31/163), testes (11.0%, 18/163), and breast (7.4%, 12/163) being the most frequently affected sites. The non-germinal center B-cell (non-GCB) subtype was prevalent in 63.7% of patients (142/223), with no significant difference between the nodal and extranodal groups ( P=0.681). Furthermore, 73.9% (184/249) and 11.7% (29/249) of patients received the R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, prednisone) and R-miniCHOP regimen, respectively. The overall 3-year PFS rate was 81.5%, and the 3-year OS rate was 85.6%. Patients aged ≥75 years ( HR=2.910, 95% CI 1.565-5.408, P=0.001) and/or with a CCI score ≥2 ( HR=2.324, 95% CI 1.141-4.732, P=0.020) had a significantly poorer PFS. Incorporating age ≥75 years and CCI score ≥2 into the stage-modified international prognostic index (sm-IPI) can better stratify the prognosis of elderly patients with stage Ⅰ DLBCL. The 3-year PFS rate was 48.7% in the high-risk group versus 85.7% in the low-risk group ( P<0.001). Conclusions:Our findings show that the elderly patients with stage Ⅰ DLBCL were predominantly characterized by extranodal involvement (particularly in the stomach and intestinal tract) and non-GCB subtype. Age ≥75 years and CCI ≥2 were identified as independent prognostic factors. The newly established sm-IPI-75-CCI incorporating these factors demonstrated superior prognostic discrimination compared to conventional risk assessment systems.

The interactions between microorganisms and medicinal plants are crucial to the quality improvement of medicinal plants. Medicinal plants attract microorganisms to colonize by secreting specific compounds and provide niche and nutrient support for these microorganisms, with a symbiotic network formed. These microorganisms grow in the rhizosphere, phyllosphere, and endophytic tissues of plants and significantly improve the growth performance and medicinal component accumulation of medicinal plants by promoting nutrient uptake, enhancing disease resistance, and regulating the synthesis of secondary metabolites. Microorganisms are also widely used in the ecological planting of medicinal plants, and the growth conditions of medicinal plants are optimized by simulating the microbial effects in the natural environment. The interactions between microorganisms and medicinal plants not only significantly improve the yield and quality of medicinal plants but also enhance their geoherbalism, which is in line with the concept of green agriculture and eco-friendly development. This study reviewed the research results on the interactions between medicinal plants and microorganisms in recent years and focused on the analysis of the great potential of microorganisms in optimizing the growth environment of medicinal plants, regulating the accumulation of secondary metabolites, inducing systemic resistance, and promoting the ecological planting of medicinal plants. It provides a scientific basis for the research on the interactions between medicinal plants and microorganisms, the research and development of microbial agents, and the application of microorganisms in the ecological planting of medicinal plants and is of great significance for the quality improvement of medicinal plants and the green and sustainable development of TCM resources.
