Objective To understand the status of body image disturbance and its influencing factors in patients with ankylosing spondylitis (AS), so as to provide a scientific basis for the clinical management of AS. Methods A total of 353 AS patients admitted from January 2022 to December 2024 were selected as research subjects. Chinese version of Body Image Disturbance Questionnaire (BIDQ) was used to investigate the body image disturbance in AS patients. Single factor analysis was performed by t test and analysis of variance, and multiple factors were analyzed by multivariate linear regression. Results The total score of BIDQ in 342 AS patients was (25.01±4.22). Multivariate linear regression analysis results showed that self-paid medical expense, nighttime VAS score and negative emotion PANAS score could positively predict body image disturbance in AS patients (standardized regression coefficient=0.413, 0.413, 0.460, P<0.05), and PSSS score, positive emotion PANAS score and exercise management CDSSM score could negatively predict body image disturbance (standardized regression coefficient=-0.245, -0.134, -0.247, P<0.05). Conclusion The body image disturbance in AS patients is worthy of clinical attention. Nighttime pain, negative emotion and self-paid medical treatment can increase the risk of body image disturbance. Positive emotion, social support and high self-management level of exercise behavior can reduce the formation of body image disturbance, which can provide new ideas for clinical management of AS patients.

Evidence-based treatment strategies for patients with cardiovascular disease and diabetes have been updated in recent years. However, substantial gaps remain between guideline recommendations and clinical practice, which justify the urgent need to improve the quality of care for patients with these conditions. The Chinese Society of Cardiology and the Chinese Society of Diabetes, in collaboration with the American Heart Association and the American Diabetes Association, designed the China Diabetes Cardiovascular project. The China Diabetes Cardiovascular project is a nationwide registry study aimed at improving the quality of care for patients with acute coronary syndrome and diabetes in China. Launched in 2021, this project has enrolled 36 hospitals across mainland China. Patients with a primary discharge diagnosis of comorbid acute coronary syndrome and diabetes will be eligible to participate. Pre-defined performance measures will be adopted to evaluate the quality of care for these patients. Multiple quality improvement strategies will be adopted, including providing monthly quality reports based on these measures, conducting a series of training courses, and distributing educational materials. A comprehensive dataset, encompassing patients' characteristics, medical history, treatment before and during the current hospitalization, and discharge medications for secondary prevention, will be collected through a web-based data collection platform. This project has the potential to improve the quality of care and reduce the care disparities in the management of patients with these diseases. Moreover, with its comprehensive data collection, this project will provide a strong foundation for exploring key clinical questions.

Lung cancer poses a serious threat to global public health security.Chemotherapy,as the main strategy for cancer treatment, faces challenges such as high toxicity and drug resistance.Anticancer peptides have the potential of being developed into new anticancer drugs due to their advantages of broad-spectrum anticancer activity, rapid action, and difficulty in generating drug resistance, but they also face shortcomings such as weak activity and strong toxic side effects. The weakly acidic microenvironment of tumors(pH 6.5-6.8)provides a good idea for the design of anticancer peptides of high-efficiency and low-toxicity.Previously,we designed the acid-sensitive antibacterial peptide pHly-1 using the wolf spider(Lycosa singoriensis)toxin Lycosin-Ⅰ as a template. In this study,we found that pHly-1 also had acid-sensitive anticancer activity.Further alanine scanning analysis of pHly-1 was carried out,and we ob-tained a mutant pHTP-2 with better acid sensitivity, whose IC50(half maximal inhibitory concentration)against A549 cells was 15.68 μmol/L at pH 6.6 and was greater than 100 μmol/L at pH 7.4.At pH 6.6,pHTP-2 could act on various lung cancer cell lines and induce the death of A549 cells by rapid ly-sis;at pH 7.4,500 μmol/L pHTP-2 had weak toxicity to red blood cells(the hemolysis rate was ap-proximately 38%)and primary myocardial cells(the inhibition rate was 49.7%,with P<0.05).Analy-sis of its charge, particle size, morphology, and secondary structure showed that at pH 6.6,the histidine in the sequence of pHTP-2 was protonated, increasing the positive charge(P<0.01),decreasing the hy-drated particle size(P<0.05)and forming an α-helical structure to induce membrane lysis of A549 cells. At pH 7.4,it was deprotonated, the positive charge decreases,a β-sheet structure was formed and self-aggregation occurred, limiting its effect on the A549 cell membrane and showing weak activity. In summary,pHTP-2 could respond to the weakly acidic microenvironment of tumors to exert selective cyto-toxic activity, effectively overcoming the shortcomings of anticancer peptides such as low efficiency and high toxicity. Our findings suggest that it is a high-quality lead molecule for anticancer drugs.

Aim To explore the relation between the protective effect of hyperoside(Hyp)on energy metab-olism and apoptosis in cardiomyocytes after myocardial infarction(MI)and the regulation of peroxisome pro-life rator-activated receptor α(PPARα)pathway. Methods Mouse myocardial infarction injury model was established by ligation of left anterior descending coronary artery. The mice that were successfully liga-ted were randomly divided into the following groups:MI group,Hyp(9,18 and 36 mg·kg-1)group,posi-tive fenofibrate group(120 mg·kg-1)and Hyp(36 mg·kg-1)+PPARα inhibitor GW6471 group. In addition,a sham group was set up,only threading with-out ligature. The mice were administered different kinds of drugs by gavage for seven days, once daily.ECG changes were recorded in mice 5 min before,5 min after surgery, and 1 hour after day 7 using the BL-420F biofunction system. The diagnostic ultrasound in-strument was used to examine the heart structure and function of each group of mice. The influence of myo-cardial histology was observed by hematoxylin and eo-sin (HE) staining and Sirius red(Sirius Red)stai-ning.The changes of creatine kinase isozymin-MB(CK-MB),cardiac troponin Ⅰ(cTnⅠ)and lactate de-hydrogenase(LDH)in mouse serum were assessed by ELISA kit.The expressions of PPARα signaling related proteins and apoptosis-related proteins were detected by immunofluorescence and Western blot. Results Compared with MI mice,Hyp could significantly im-prove the ECG abnormality of MI mice, increase the left ventricular ejection fraction(LVEF)and left ventricu-lar short axis shortening rate(LVFS),reduce serum cTnⅠ content,CK-MB and LDH activity, and reduce myocardial fibrosis, infarct area and cardiomyocyte ap-optosis in the MI area.Meanwhile, this research found that Hyp could activate PPARα signaling pathway and regulate apoptosis-related proteins. However, the car-dioprotective effect of hyperoside was reversed by the combination with the treatment of the PPARα signaling inhibitor,GW6471.Conclusions Hyp has an impro-ving effect on energy metabolism and apoptosis in mice after MI,and the mechanism may be related to its acti-vation of PPARα signaling pathway.
