Background and Objectives:
The risk profiles, procedural characteristics, and clinical outcomes for women undergoing bifurcation percutaneous coronary intervention (PCI) are not well defined compared to those in men.
Methods:
COronary BIfurcation Stenting III (COBIS III) is a multicenter, real-world registry of 2,648 patients with bifurcation lesions treated with second-generation drug-eluting stents.We compared the angiographic and procedural characteristics and clinical outcomes based on sex. The primary outcome was 5-year target lesion failure (TLF), a composite of cardiac death, myocardial infarction, and target lesion revascularization.
Results:
Women (n=635, 24%) were older, had hypertension and diabetes more often, and had smaller main vessel and side branch reference diameters than men. The pre- and post-PCI angiographic percentage diameter stenoses of the main vessel and side branch were comparable between women and men. There were no differences in procedural characteristics between the sexes. Women and men had a similar risk of TLF (6.3% vs. 7.1%, p=0.63) as well as its individual components and sex was not an independent predictor of TLF. This finding was consistent in the left main and 2 stenting subgroups.
Conclusions
In patients undergoing bifurcation PCI, sex was not an independent predictor of adverse outcome.

8.Associated factors of brain metastases and diagnostic yield of staging brain magnetic resonance imaging in anaplastic thyroid cancer

Background:
This study aimed to identify the associated factors for brain metastasis (BM) in patients with anaplastic thyroid cancer (ATC) and estimate the diagnostic yield of staging brain magnetic resonance imaging (MRI) during the initial evaluation of ATC.
Methods:
This retrospective, single-center study included patients newly diagnosed with ATC who underwent brain MRI, from May 2008 to July 2024. The patients were stratified into two groups (BM vs. non-BM). The clinical characteristics of ATC were compared between the two groups using the chi-squared test and multivariable logistic regression analysis. The diagnostic yield of initial staging brain MRI from 2010 to 2024 was calculated.
Results:
A total of 77 patients (61.52±13.19 years old, 37 men) were included. BM was observed in 19 patients (24.7%). The occurrence of BM was significantly associated with extracranial metastasis (p=0.004), especially lung metastasis (p=0.017), and neurologic symptoms (p=0.001). On multivariable logistic regression analysis, after adjusting for age, sex, and primary thyroid tumor size, the presence of extracranial metastases (odds ratio [OR]: 16.09 [2.39–360.70], p=0.019) and neurologic symptoms (OR: 5.95 [1.73–24.08], p=0.007) were independently associated with BM. The diagnostic yield of staging brain MRI during the initial evaluation of ATC was 6.1% (3/49). The diagnostic rate of BM was 26.5% (18/68) in patients with extracranial metastases and 38.9% (14/36) in those with neurological symptoms.
Conclusion
Routine staging or surveillance brain MRI in patients with extracranial metastasis is likely to offer significant benefits.

Background:
s/Aims: Reported incidence of extrahepatic bile duct cancer is higher in Asians than in Western populations. Korea, in particular, is one of the countries with the highest incidence rates of extrahepatic bile duct cancer in the world. Although research and innovative therapeutic modalities for extrahepatic bile duct cancer are emerging, clinical guidelines are currently unavailable in Korea. The Korean Society of Hepato-Biliary-Pancreatic Surgery in collaboration with related societies (Korean Pancreatic and Biliary Surgery Society, Korean Society of Abdominal Radiology, Korean Society of Medical Oncology, Korean Society of Radiation Oncology, Korean Society of Pathologists, and Korean Society of Nuclear Medicine) decided to establish clinical guideline for extrahepatic bile duct cancer in June 2021.
Methods:
Contents of the guidelines were developed through subgroup meetings for each key question and a preliminary draft was finalized through a Clinical Guidelines Committee workshop.
Results:
In November 2021, the finalized draft was presented for public scrutiny during a formal hearing.
Conclusions
The extrahepatic guideline committee believed that this guideline could be helpful in the treatment of patients.
10.Intralesional Methotrexate Injection for the Treatment of Epithelial Crateriform Tumor

Background:
Intralesional methotrexate injection (IL-MTX) is an appropriate strategy for treating epithelial crateriform tumors (ECTs) when surgical excision can result in functional or cosmetic defects; however, not all ECTs are responsive to this treatment.
Objective:
This study aimed to evaluate the effectiveness of IL-MTX for ECTs and to determine the differences in clinical response according to the pathological features.
Methods:
The medical records of patients treated with IL-MTX for their ECTs were retrospectively reviewed. Effectiveness was evaluated in terms of size reduction and flattening.
Results:
Twenty-five cases of ECTs with biopsy were included in this study. Eight cases of keratoacanthoma (KA) and 15 cases of squamous cell carcinoma (SCC) were identified, but 2 cases could not be clearly distinguished. Seventeen patients (68%) showed a response after injection, and response rate in KA and SCC were 75% (6/8) and 60% (9/15), respectively. Nine patients showed complete resolution with IL-MTX. Patients received 3 injections, and regression was observed in 7.56 weeks after the first injection. According to histopathological results, patients with KA and SCC received 2 and 3.33 injections, respectively, and complete resolution was observed after 7 and 7.67 weeks, respectively.
Conclusion
IL-MTX is safe and effective, and could be considered as a useful non-surgical treatment option for ECTs. Both KA and crateriform SCC showed good response; However, KA showed a better response.
