1.Research progress of tenecteplase in acute ischemic stroke

Tenecteplase is a third-generation thrombolytic drug.Compared with alteplase,it has the advantages of longer half-life and higher fibrin specificity.The effectiveness and safety of tenecteplase in the treatment of acute ischemic stroke have been extensively studied in clinical research.The article summarized the research progress of tenecteplase in the treatment of acute ischemic stroke based on completed randomized controlled studies and related Meta-analyses,and explored the pharmacological characteristics,effectiveness,and safety of tenecteplase,as well as current guideline recommendations,aiming to offer a reference for choosing more appropriate diagnostic and therapeutic methods for patients with acute ischemic stroke.

Acute ischemic stroke(AIS)has become one of the most serious diseases causing death and disability globally,with ischemic stroke caused by large vessel occlusion having higher rates of death and disability and causing a heavy financial burden on the patient's family.Early endovascular treatment to open the occluded vessel is essential to save ischaemic brain injury.Endovascular treatment can provide significant clinical benefit to patients.Aspiration thrombectomies(ADAPT)and stent retrievers are used as common modalities for endovascular treatment,but there are no conclusive findings regarding the advantages and disadvantages of the two main treatment modalities.This article comprehensively analyzes the current research results and compares the efficacy and safety of aspiration thrombectomy and stent thrombectomy in order to provide a scientific basis for clinical decision-making.

5.Surveillance of Aedes populations in Jiangsu Province in 2020

Objective To investigate the seasonal Aedes population fluctuation and the resistance of Aedes populations to common insecticides in Jiangsu Province in 2020, so as to provide insights into vector-borne infectious diseases control.. Methods One village was randomly sampled from each of Xinbei District of Changzhou City and Zhangjiagang County of Suzhou City in southern Jiangsu Province, Hai’an County of Nantong City and Yandu District of Yancheng City in Central Jiangsu Province, and Suining County of Xuzhou City and Sihong County of Suqian City in northern Jiangsu Province during the period between May and October, 2020. A small ponding container was sampled, and larval Aedes mosquitoes were collected using straws once each in early and late stages of each month. All larvae were bred in laboratory to adults for population identification. In addition, larval breeding were observed in all small ponding containers in and out of 30 households that were randomly sampled from six surveillance sites, and the larval mosquito density was estimated using Breteau index. Larval A. albopictus mosquitoes were sampled around Cuiyuan New Village in Jintan District of Changzhou City, and bred in laboratory to the first offspring generation, and the susceptibility of adult female mosquitoes to deltamethrin, lambda-cyhalothrin, malathion, and propoxur was tested using the filter-paper bioassay recommended by WHO. Results A total of 1 165 larval Aedes mosquitoes were captured from small ponding containers in six surveillance sites of Jiangsu Province in 2020, and all were identified as A. albopictus following eclosion. The largest number of Aedes larvae captured was found in July. A total of 1 152 households were investigated in six surveillance sites, and the mean Breteau indexes were 9.58, 13.20, 13.71, 13.20, 12.18 and 5.58 from May to October, respectively, while a high Aedes transmission risk was seen in Xinbei District of Changzhou City, with a higher Breteau index than in Suining (H = 23.667, Padjusted = 0.001) and Sihong (H = 22.500, Padjusted = 0.003) counties. The field-captured A. albopictus from Cuiyuan New Village in Jintan District of Changzhou City remained sensitive to malathion, but was resistant to propoxur, and developed high-level resistance to deltamethrin and lambda-cyhalothrin. Conclusions A. albopictus was present in southern, central and northern Jiangsu Province in 2020, and the larval density peaked in July. A. albopictus captured from Cuiyuan New Village in Jintan District of Changzhou City has developed high-level resistance to pyrethroid pesticides.
6.Inhibitory effect of hyperbaric oxygen on growth of transplanted nasopharyngeal carcinoma in nude mice and possible mechanism involved

Objective:To investigate the inhibitory effect of hyperbaric oxygen (HBO) on the growth of transplanted nasopharyngeal carcinoma (NPC) in nude mice and its mechanism.Methods:After the transplanted tumor model of human NPC cell line (CNE-2) in nude mice was established, twenty tumor-bearing nude mice were divided into control and HBO group by using a random number table method, each with 10 mice. The nude mice in the HBO group were exposed to one-hour-session of HBO a day for two consecutive weeks. After two weeks treatments, the mice in both groups were sacrificed, and tumor mass and volume were measured to calculate the tumor growth inhibition rate. Real-time polymerase chain reaction (RT-PCR) was used to detect the mRNA expression levels of vascular endothelial growth factor (VEGF) and basic fibroblast growth factor (bFGF) in transplanted tumor cells. Immunohistochemistry analysis was used to detect the expression levels of VEGF and bFGF proteins and tumor microvessel density (MVD). Spearman correlation analysis was used to assess the correlation between VEGF/bFGF expressions and MVD.Results:After 14 consecutive days of HBO treatment, the inhibition rates of tumor mass and tumor volume in the transplanted tumor model mice of the HBO group were respectively 43.12% and 43.06%, both were significantly lower than those of the control group ( P<0.01). The expression levels of VEGF mRNA and bFGF mRNA in the transplanted tumor tissue of the HBO group were significantly lower than those of the control group ( P<0.01). Immunohistochemistry detection results showed that, regarding the VEGF and bFCG expressions in the cytoplasm of tumor cells, the cytoplasmic staining of positive cells was brownish yellow, and the nucleus staining was light blue. The expression levels of VEGF and bFGF in the HBO group were significantly lower than those in the control group ( P<0.01). The immunohistochemistry detection results also indicated that, regarding the CD-34 protein expression in vascular endothelial cells, the staining was brownish yellow. The microvessels were of different shapes and sizes, and no regular vascular lumen was formed. The MVD value of the HBO group (33.16±4.89) was significantly lower than that of the control group (45.69±5.26), with statistical significance ( P<0.01). Spearman correlation analysis demonstrated that VEGF protein expression was positively correlated with MVD expression ( r=0.862, P=0.001), and bFGF protein expression was also positively correlated with MVD expression ( r=0.745, P=0.013). Conclusion:HBO has a significant growth inhibitory effect on the transplanted NPC in nude mice, the mechanism of which might be related to the reduction of the expression levels of VEGF and bFGF.

8.Comparison of 1-week terbinafime hydrochloride cream, 1- and 4-week miconazole nitrate cream in the treatment of interdigital tinea pedis: a multi-center, randomized and double-blind study

ObjectiveTo compare the efficacy and tolerability of 1-week 1% terbinafine hydrochloride cream, 1- and 4-week 2% miconazole nitrate cream in the treatment of interdigital tinea pedis, and to observe the relapse in patients treated with these regimens. MethodsA multi-center, randomized, double-blind and parallel group study was conducted. By using a stratified randomization protocol, patients were divided into 3 groups to apply terbinafine cream twice daily for 1 week and inert cream(placebo) for the next 3 weeks (1week terbinafine group), miconazole cream twice daily for 1 week and inert cream(placebo) for the next 3 weeks (1-week miconazole group), and miconazole cream twice daily for 4 weeks (4-week miconazole group),respectively. Clinical and mycological assessment was made on week 1, 3, 4, 6, 9 and 12 after the initiation of treatment. ResultsA total of 152 patients with positive baseline mycological culture were eligible for the efficacy analysis. After 4-week treatment, the mycological cure rates were 94.7%, 87.8% and 82.6%, global effective rates 89.5%, 81.6% and 63.0%, respectively for the 1-week terbinafine group, 4-week miconazole group and 1-week miconazole group. On week 12, the mycological relapse rates in 1-week terbinafine, 4-week miconazole and 1-week miconazole group were 13%, 14% and 21% respectively, and the incidence of adverse reaction was 2.38%, 2.38% and 3.57%, respectively. ConclusionsAs far as the efficacy and recurrence in patients are concerned, the 1-week terbinafine cream regimen is similar to the 4-week miconazole cream regimen for the treatment of interdigital tinea pedis.
9.Mycological profile of cryptococcal meningitis in patients with non-acquired immune deficiency syndrome during treatment and follow-up

Objective To examine mycological profile of eryptococcal meningitis in patients with non-acquired immune deficiency syndrome (AIDS) during treatment and follow-up so that to support clinical therapy. Methods Data of 28 cuhure-confirmed cryptoeoccal meningitis patients with non-AIDS were retrospectively analyzed. Fungat smear, count, culture and latex agglutination test of cerebrospinal fluid (CSF) were done during treatment and follow-up. Initial treatment included intravenous amphotericin B plus oral flucytosine or f;uconazole for at least 6 weeks, and consolidation treatment included oral fluconazole and (or) itraeonazole for at least 2 months. All 28 patients were cured. The data were analyzed by rank-sum test. Results The positive rate of CSF fungal smear was 92.9% before treatment and gradually decreased, and the fungal count was significantly reduced over time after treatment. While fungal smears of some patients were still positive after initial treatment. Fungal growth time in culture was gradually extended, and fungal culture turned to be negative in all patients after 2 weeks of treatment. The positive rate of latex agglutination test of CSF was 100%. Cryptococcal antigen titer decreased steadily after treatment, which was not correlated with the decrease of fungal count. Conclusion Mycological tests of patients with eryptococcal meningitis should be interpreted comprehensively during treatment, and result of each test should be specifically analyzed.
