1.Association between unilateral or bilateral hearing loss and multimorbidity among the oldest old in China

Objective To investigate the associations between unilateral or bilateral hearing loss and 12 chronic diseases as well as multimorbidity among the oldest old in China,and to identify disparities in these associations of left-and right-side hearing loss with chronic diseases.Methods Totally 7 437 people aged ≥80 years old were selected from the Chinese Longitudinal Health and Longevity Survey(CLHLS)2018 cross-sectional data.With 12 chronic diseases and multimorbidity as outcome variables,the hearing loss as explanatory variable,socio-demographic characteristics,family factors,and lifestyle as covariates,the correlations of unilateral(left-or right-side)and bilateral hearing loss with chronic diseases and multimorbidity were analyzed using multivariate logistic regression model,and the trend analyses were carried out.Results There were 205(2.76%),227(3.05%)and 3 598(48.38%)old people with left-side,right-side and bilateral hearing loss,respectively.After adjusting for confounders,the oldest old with left-sided or bilateral hearing loss had a greater risk of multimorbidity compared with those with normal hearing function,with odds ratio(95%confidence interval)of 2.14(1.58-2.90)and 1.27(1.13-1.43),respectively,while no association between right-sided hearing loss and multimorbidity was observed(P>0.05).Trend analysis showed that the risk of multimorbidity increased with hearing loss from none to unilateral and then to bilateral(P<0.001).Conclusion Hearing loss may be related to the increased risk of multimorbidity in the oldest old,and the risk of those with bilateral hearing loss is higher.More attention should be paid to the prevention and treatment of hearing loss in the oldest old.
2.Impact of adverse childhood experiences on mental health among young individuals:chain-mediated effect of mobile phone dependency and self-rated health

Objective To explore the mediating role of mobile phone dependency and self-rated health in the relationship between adverse childhood experiences(ACEs)and mental health among young adults.Methods A cross-sectional study was conducted using cluster random sampling among 1 611 young adults(mean age 26.30 years)from a region in Hainan Province.Participants completed the childhood trauma questionnaire(short form),the mobile phone addiction index,the depression-anxiety-stress scale(simplified Chinese version),and a self-rated health questionnaire.Pearson correlation analysis and mediation effect analysis were employed to examine the relationships among ACEs,mobile phone dependency,self-rated health,and mental health.Results ACEs,mobile phone dependency,and self-rated health were all significantly correlated with mental health(all P<0.01).ACEs had a direct negative effect on mental health(direct effect=0.221,95%confidence interval[CI]0.150-0.293).Furthermore,ACEs exerted indirect effects on mental health through 3 pathways:the independent mediation of mobile phone dependency(indirect effect=0.081,95%CI 0.035-0.130),the independent mediation of self-rated health(indirect effect=0.034,95%CI 0.011-0.062),and the chain mediation of mobile phone dependency and self-rated health(indirect effect=0.009,95%CI 0.004-0.015).Conclusion ACEs have a significant impact on the mental health of young adults,with mobile phone dependency and self-rated health serving as key mediators.Interventions aimed at reducing mobile phone dependency and improving health status may help mitigate the negative impact of childhood trauma on mental health,thereby promoting psychological well-being in this population.
3.Clinical Analysis of 30 Cases of Uterine Arteriovenous Fistula Diagnosis Ultra-sonography

Objective:Analyze the clinical manifestations,diagnosis,treatment,and prognosis of uterine arterio-venous fistula(UAVF).Methods:A retrospective analysis was conducted on the clinical data of 30 patients diag-nosed with UAVF by ultrasonography at The First Affiliated Hospital of Zhengzhou University from January 1,2019,to December 31,2022.Results:Among the 30 patients,the median age was 33 years.All patients had a his-tory of uterine surgery,and 3 cases(10.0%)were complicated by gestational trophoblastic disease.Vaginal bleeding was the presenting symptom in 26 cases(86.7%).All patients were diagnosed using color Doppler ul-trasonography.Additionally,22 patients(73.3%)underwent digital subtraction angiography(DSA)for definitive diagnosis.The treatments administered were as follows:8 patients underwent uterine artery embolization(UAE)to hysteroscopic lesion resection,10 patients received UAE only,4 patients underwent hysteroscopic lesion resec-tion only,4 patients required UAE due to massive bleeding during hysteroscopic lesion resection,1 patient under-went direct hysterectomy,3 patients received conservative management.Among the 22 patients who underwent UAE,19 cases(86.4%)experienced no recurrence of vaginal bleeding.Among the 26 patients who underwent fertility-preserving treatment,18(69.2%)regained normal menstruation.Of the 10 patients within this group who desired pregnancy,4 subsequently had successful deliveries.Conclusions:UAVF primarily manifests as irregular vaginal bleeding.Color Doppler ultrasonography can serve as the initial diagnostic tool,while DSA provides a de-finitive diagnosis.UAE and hysteroscopic lesion resection can be considered preferred treatment options for pa-tients desiring fertility preservation.
4.Analysis of short-term efficacy of drug-coated balloon in the treatment of atherosclerotic renal artery stenosis

Objective To explore the feasibility,safety and efficacy of paclitaxel drug-coated balloon(DCB)in the treatment of atherosclerotic renal artery stenosis(ARAS).Methods A total of 28 patients with ARAS were selected.Balloon angioplasty was performed using a paclitaxel DCB at the site of renal artery stenosis(RAS)in these patients.Subsequently,a follow-up study was conducted to monitor various parameters of the patients,including vascular restenosis,blood pressure,the types of antihypertensive medications and renal function.The feasibility,efficacy and safety of balloon angioplasty using DCB in the treatment of ARAS were analyzed.Results Twenty-eight patients underwent 33 DCB balloon angioplasty.In one patient,there was no significant improve-ment in the degree of RAS,and thus further treatment with renal artery stenting was administered.The remaining patients all achieved both anatomical and hemodynamic success,with the degree of vascular stenosis at the lesion site decreasing from(79.74±5.13)%to(8.32±4.67)%,and the surgical success rate was 96.97%.The systolic/diastolic blood pressure of the patients was(179.16±30.65)mmHg/(108.26±20.93)mmHg(1 mmHg=0.133 kPa)at 24 hours postoperatively,(131.11±12.99)mmHg/(80.11±7.12)mmHg at 3 months postoperatively,(134.16±11.37)mmHg/(78.68±4.79)mmHg at 6 months postoperatively,and(133.37±12.71)mmHg/(80.11±4.84)mmHg at 12 months postoperatively.In comparison with the preoperative blood pressure of(184.63±27.64)mmHg/(109.11±22.26)mmHg,there was no significant decrease in blood pressure at 24 hours postoperatively,and the difference was not statistically significant.However,at 3,6,and 12 months postoperatively,the patients'blood pressure was signif-icantly lower than that before the operation,and all the differences were statistically significant.The glomerular filtration rate(GFR)was(36.19±18.32)mL/min at 24 hours postoperatively,(35.96±18.51)mL/min at 3 months postoperatively,(36.23±19.30)mL/min at 6 months postoperatively,and(35.59±18.26)mL//min at 12 months postoperatively,which all elevated compared with the preoperative GFR of(28.31±14.67)mL/min,and the differences were statistically significant(P<0.05).At 3,6,and 12 months postoperatively,the vascular patency rate was 100%as indicated by multifunctional color Doppler ultrasound examination or renal artery computed tomography angiography(CTA).No relevant com-plications and postoperative adverse events,such as renal artery rupture or dissection,renal artery thrombosis and acute renal insuffi-ciency,occurred in all 28 patients.Conclusion The paclitaxel DCB is safe and reliable for the treatment of ARAS and has remarkable curative effects,and it can be used as an effective vascular treatment regimen for ARAS.

7.A case report of interventional treatment of massive hemorrhage from left ureteral iliac artery fistula

This article reports that a patient with bilateral ureterocutaneous stoma after radical cystectomy developed intermittent hematuria 2 years after surgery, and there were no abnormalities on bilateral renal artery and abdominal aortic angiography. After removal of the double J stent, a left common iliac artery ureteral fistula was seen on abdominal aortic angiography, and the patient immediately developed hemorrhagic shock. Abdominal aortic balloon occlusion and left common iliac artery stent grafting were performed in the emergency department. After the stent was deployed and the balloon deflated, no bleeding was observed, and the patient's heart rate and blood pressure stabilized. Follow-up angiography showed complete resolution of the fistula. This condition is rare and dangerous, but interventional treatment can quickly control bleeding, offering minimal surgical trauma and fast recovery.
8.Risk factor analysis of carbapenem-resistant enterobacteriaceae infection based on machine learning

Objective To explore the machine learning model and risk factor analysis for hospital infection caused by carbapenem-resistant enterobacteriaceae(CRE).Methods The clinical data of totally 451 patients infected with extended-spectrum β-lactamases(ESBL)producing Enterobacteriaceae treated in the hospital from 2018 to 2022 were retrospectively collected.The patients were divided into CRE group(115 cases)and sensitive group(336 cases)according to the susceptibility of carbapenem.Four machine learning methods in-cluding Logistic regression analysis,random forest,support vector machine,and neural network were used to build prediction models and receiver operating characteristic curve was used to evaluate.Based on the predic-tion model with the best performance,risk factors for CRE infection were analyzed.Results Random forest model had the best performance,with the area under the curve of 0.952 3.The risk factors for predicting CRE infection by the random forest model included 15 clinical data items,namely fever for more than 3 days,cere-bral injury,drainage fluid sample,trunk surgery,first-level or special-level nursing,ICU treatment,procalcito-nin,anti-anaerobic bacteria,the use of third-generation cephalosporins,age,pre-albumin,creatinine,white blood cell count,and albumin.Conclusion The CRE prediction model developed in this study has good predic-tive value and the risk factors have guiding significance for the early prevention and treatment of CRE infec-tion in clinical practice.
