Metabolic-associated fatty liver disease (MAFLD) and osteoporosis (OP) are two very common metabolic diseases. A growing body of experimental evidence supports a pathophysiological link between MAFLD and OP. MAFLD is often associated with the development of OP. Rutaecarpine (RUT) is one of the main active components of Chinese medicine Euodiae Fructus. Our previous studies have demonstrated that RUT has lipid-lowering, anti-inflammatory and anti-atherosclerotic effects, and can improve the OP of rats. However, whether RUT can improve both fatty liver and OP symptoms of MAFLD mice at the same time remains to be investigated. In this study, we used C57BL/6 mice fed a high-fat diet (HFD) for 4 months to construct a MAFLD model, and gave the mice a low dose (5 mg·kg-1) and a high dose (15 mg·kg-1) of RUT by gavage for 4 weeks. The effects of RUT on liver steatosis and bone metabolism were then evaluated at the end of the experiment [this experiment was approved by the Experimental Animal Ethics Committee of Institute of Medicinal Biotechnology, Chinese Academy of Medical Sciences (approval number: IMB-20190124D303)]. The results showed that RUT treatment significantly reduced hepatic steatosis and lipid accumulation, and significantly reduced bone loss and promoted bone formation. In summary, this study shows that RUT has an effect of improving fatty liver and OP in MAFLD mice.

Journal of Clinical Hepatology 2025;41(3):580-587
After 50 years of clinical development, endoscopic retrograde cholangiopancreatography (ERCP) has become the preferred method for the clinical diagnosis and treatment of cholangio-pancreatic duct diseases; however, the major postoperative complications of ERCP, such as pancreatitis, hemorrhage, and perforation, are still a difficult issue faced by clinicians, and postoperative perforation is associated with an extremely high risk of death. Therefore, it is very important to explore the risk factors for perforation after ERCP, make a definite diagnosis of perforation in a timely manner, and formulate precise prevention and treatment measures. By reviewing a large number of articles, this article summarizes the influencing factors for perforation after ERCP and related diagnosis and treatment measures.

According to the "Regulations on clinical application management of medical technologies", physicians intending to carry out restricted technologies must undergo standardized training and pass assessments in accordance with the clinical application management standards for the respective technology. As ventricular assist technology is classified as a nationally restricted technology, standardized training is one of the essential conditions for its application. This paper primarily explores the standardized training for the clinical application of ventricular assist technology in Shanghai, in light of its background, clinical application, and current training status. It proposes the training requirements for ventricular assist technology, animal training assessment standards, and clinical practice assessment standards in Shanghai, aiming to promote the standardized development and high-quality advancement of ventricular assist technology in Shanghai.

A 64-year-old woman was treated with Sindillizumab immunotherapy for classic Hodgkin lymphoma. After 7 cycles, the patient developed fever, fatigue, poor appetite, and other symptoms.Auxiliary examination after admission showed that hemoglobin, platelets, white blood cells, and fibrinogen decreased, liver enzymes increased, serum ferritin increased significantly(3 727.56 μg·L-1),and spleen enlargement, which was finally considered to be sindilizumab associated hemophagic cell syndrome. The patient was given methylprednisolone sodium succinate 60 mg·d-1 intravenously for 2 days,40 mg·d-1 intravenously for 4 days, and 30 mg·d-1 intravenously for 1 day,and their symptoms improved significantly, and the temperature, blood count, aminotransferase and other indicators gradually returned to normal. After discharge, she was changed to prednisone 30 mg·d-1 oral therapy, and the dose was reduced by 10 mg per week until withdrawal. The patient did not restart sindilizumab after discharge, and the serum ferritin gradually returned to normal level during follow-up,and no hemophagocytic syndrome-related symptoms appeared again. It was suggested that during the use of immune checkpoint inhibitors, patients with unexplained fever and decreased blood counts,in the absence of improvement in anti-infection,the possibility of hemophagocytic syndrome should be considered and relevant diagnostic tests such as serum ferritin should be improved as soon as possible, and glucocorticoid therapy should be turned on to avoid delay of the disease.

Objective:To develop a simple, rapid, and convenient analysis method for the identification of breast cancer and its molecular sub-types.Methods:A laser confocal Raman spectrometer was used to collect Raman spectrograms of normal breast cells and different molecular sub-types of breast cancer cells, and assign the material origin of the Raman spectral peaks. First, Savitzky-Golay smoothing (with a window size of 9) was selected to perform smoothing and denoising on the Raman spectrogram. Subsequently, an iterative adaptive weighted penalty least squares method was employed for baseline correction, and principal component analysis was used to eliminate outliers. The recognition model of normal breast cells and breast cancer cells and the recognition model of different molecular sub-types of breast cancer cells were established by using three algorithms with different principles, including partial least squares discriminant analysis (PLS-DA), K-nearest neighbor (KNN), and support vector machine (SVM).Results:The Raman spectrogram and Raman peak shifts of normal breast cells and breast cancer cells were similar, but there were significant differences in intensity. The results of the machine learning models showed that the recognition accuracy of PLS-DA and SVM algorithms for distinguishing between normal breast cells and breast cancer cells was above 92.03% and 90.67%, respectively. The recognition accuracy of PLS-DA and SVM algorithms for different molecular sub-types of breast cancer cells was (83.66 ± 2.77)% and (90.55 ± 0.06)%, respectively. Conclusions:The combination of Raman spectroscopy and machine learning algorithms can achieve accurate identification of normal breast cells, breast cancer cells, and different molecular sub-types of breast cancer cells.

Aim To investigate the mechanisms of Chaijin JieYu Anshen formula modulating the depres-sive behaviors and the synaptic plasticity of hippocam-pal neurons in insomnia-concomitant depression rats based on the histone deacetylase 5(HDAC5)/myocyte enhancer factor 2C(MEF2C)pathway.Methods A rat model of insomnia-concomitant depression was es-tablished by PCPA injection combined with chronic un-predictable mild stress(CUMS),and the experiment was divided into the control group, the model group, the high, medium and low dose group of Chaijin JieYu Anshen formula, and the positive drug group. The de-pression of rats was evaluated by sugar-water prefer-ence test, open field test and morris water maze. The levels of 5-hydroxytryptamine(5-HT)and dopamine(DA)in serum were measured by enzyme linked im-munosorbent assay(ELISA).The pathological damage of hippocampal neurons was observed by HE staining and Nissl staining. The damage of dendritic spines of hippocampal neurons was observed by Golgi staining, and the levels of HDAC5,MEF2C,postsynaptic densi-ty-95(PSD-95)and synaptophysin 1(SYN1)in hip-pocampus were measured by Western blot,immunohis-tochemistry and immunofluorescence. Results Com-pared with the model group, the Chaijin JieYu Anshen formula could increase the sugar-water preference rate of the model rats, reduce the immobility time in the open field experiment, increase the total activity dis-tance,shorten the evasion latency in the localization navigation experiment, and prolong the residence time in the quadrant where the platform was located in the space exploration experiment(P<0.05,P<0.01).Moreover,the Chaijin JieYu Anshen formula improved the hippocampal neuron and dendritic spine damage and increase the dendritic branch length and dendritic spine density of hippocampal neurons(P<0.01,P<0.01),restore the serum levels of 5-HT and DA in insomnia-concomitant depression rats(P<0.05,P<0.01),down-regulate the HDAC5 protein, and up-regulate the expression of MEF2C,PSD-95,and SYN1 protein(P<0.05,P<0.01 or P<0.001).Conclusions Chaijin JieYu Anshen formula may alle-viate the depression-like behavior of model rats by re-ducing the expression of HDAC5 protein, thus deregu-lating the inhibition of transcription factor MEF2C,promoting the expression of PSD-95 and SNY1 protein, and exerting a protective effect on hippocampal neurons and synapses.

Objective:To compare the influence of dronedarone and amiodarone on the bleeding risk of patients with atrial fibrillation treated with rivaroxaban anticoagulation. Methods:Clinical data of 81 patients with atrial fibrillation treated with rivaroxaban anticoagulation at Xi'an International Medical Center Hospital from January 2020 to July 2023, including 36 patients treated with dronedarone and 45 patients treated with amiodarone, were retrospectively analyzed. The effects of dronedarone and amiodarone on the anticoagulation of rivaroxaban were compared using the incidence of bleeding events, thrombosis events, and adverse reactions as outcome measures. Results:The total bleeding in the dronedarone group [22.22% (8/36)] was significantly higher than that in the amiodarone group [6.67% (3/45)] ( χ2 = 4.12, P < 0.05). The total bleeding of conventional-dose rivaroxaban in the dronedarone group was 30.00% (6/20), while the total bleeding of low-dose rivaroxaban was 12.50% (2/16), with no statistical significance ( χ2 = 1.58, P > 0.05). No thrombotic events or adverse reactions to dronedarone or amiodarone were observed in all patients. Conclusion:Compared with amiodarone, dronedarone significantly increases the bleeding risk of rivaroxaban anticoagulation in patients with non-valvular atrial fibrillation, and reducing the dose of rivaroxaban in patients using dronedarone does not reduce the bleeding risk.

This article reports a case of acute kidney injury (AKI) following near drowning with reduced urine output as the main manifestation. The patient was a 37-year-old middle-aged man. His renal function deteriorated sharply after accidentally falling into the water, and renal pathology showed acute tubular injury. After hemodialysis treatment, urine output increased significantly, and renal function and proteinuria improved significantly. AKI following near drowning lacks typical clinical manifestations and is prone to delayed diagnosis and treatment. The patients with a history of near drowning should be followed up to determine whether they are complicated by AKI.
