1.Perioperative immune dynamics and clinical outcomes in patients undergoing on-pump cardiac surgery

Objective: To characterize perioperative dynamic changes in immune-cell phenotypes and inflammatory cytokines in patients undergoing CPB (cardiopulmonary bypass) cardiac surgery, and to explore their associations with postoperative outcomes. Methods: In this prospective cohort study, 120 adult patients who underwent elective cardiac surgery under CPB at West China Hospital from May 2022 to March 2023 were enrolled. Perioperative immune-cell phenotypes and concentrations of 40 inflammation-related cytokines were measured. The primary outcomes were the sequential organ failure assessment (SOFA) score at 24 h after surgery and ΔSOFA (the peak SOFA score within 48 h after surgery minus the preoperative SOFA score). Secondary outcomes included major adverse cardiovascular events (MACE), acute kidney injury (AKI), respiratory failure, severe liver injury, and infection. Results: The mean age of enrolled patients was 57±10 years. Of these, 52% (62/120) were male and 90% (108/120) underwent valve surgery. Plasma analysis showed that multiple pro-inflammatory cytokines increased during CPB and remained elevated up to 24 h after surgery; five chemokines and the anti-inflammatory cytokine IL-10 peaked at the end of CPB. The SOFA score increased from 1 (1, 2) preoperatively to 7 (5, 10) at 24 h after surgery, with a ΔSOFA of 6 (4, 8). Within 30 days after surgery, 48 patients (40.0%) developed AKI, 17 (14.2%) developed infection, 4 (3.3%) developed severe liver injury, 3 (2.5%) developed respiratory failure, and 3 (2.5%) experienced MACE. During the 2-year follow-up, 8 patients (6.7%) experienced MACE and 5 (4.2%) died. Conclusion: Multi-organ dysfunction is common after cardiac surgery under CPB (median ΔSOFA, 6), accompanied by perioperative activation of multiple immune-cell subsets and upregulation of pro-inflammatory, anti-inflammatory, and chemotactic mediators. This study provides data-driven evidence and research clues for further investigation of the associations between CPB-related immune perturbations and postoperative organ dysfunction and clinical outcomes.
2.Correlation between serum FGF2,ACE2 and severity of chronic obstructive pulmonary disease patients and their predictive value for prognosis

Objective To investigate the correlation between serum fibroblast growth factor 2(FGF2)and angiotensin converting enzyme 2(ACE2)levels with the severity of chronic obstructive pulmonary disease(COPD)and their predictive value for prognosis.Methods A total of 114 COPD patients treated in Hainan Cancer Hospital from June 2020 to June 2022 were selected as the study group.According to the severity of the disease,COPD patients were divided into acute exacerbation stage and stable stage.Another 114 healthy subjects were selected as control group.Serum FGF2 and ACE2 levels were detected by enzyme-linked immu-nosorbent assay.Pearson method was used to analyze the correlation between serum FGF2 and ACE2 and pul-monary function indicators.Multivariate Logistic regression was used to analyze the influencing factors of poor prognosis in COPD patients,and receiver operating characteristics(ROC)curve was plotted to analyze the predictive value of serum FGF2 and ACE2 for poor prognosis in COPD patients.Results Serum FGF2 level in the study group was significantly higher than that in the control group(P<0.05),and serum ACE2 level,the ratio of exertional expiratory volume in the first second to predicted(FEV1%),the ratio of FEV1 to forced vi-tal capacity(FEV1/FVC),and peak expiratory flow rate(PEF)in the study group were significantly lower than those in the control group(P<0.05).Serum FGF2 level in acute exacerbation was significantly higher than that in stable stage(P<0.05),serum ACE2 level,FEV1%,FEV1/FVC and PEF were significantly low-er than that in stable stage(P<0.05).Pearson correlation analysis showed that serum FGF2 and ACE2 levels were negatively correlated(P<0.05),and serum FGF2 and ACE2 were correlated with FEV1%,FEV1/FVC and PEF(P<0.05).The serum FGF2 level in the poor prognosis group was significantly higher than that in the good prognosis group(P<0.05),and the serum ACE2 level was significantly lower than that in the good prognosis group(P<0.05).Multivariate Logistic regression analysis showed that FGF2 was a risk factor for adverse prognosis in COPD patients(P<0.05),and ACE2 was a protective factor(P<0.05).According to ROC curve,the combination of serum FGF2 and ACE2 predicted the AUC of poor prognosis in COPD patients better than that predicted separately(Z=2.514,2.610,both P<0.05).Conclusion Serum FGF2 level in-creased and ACE2 level decreased in COPD patients,both of which are closely related to the severity of the disease.Combined detection has certain predictive value for poor prognosis in COPD patients.
3.Research progress of female heterozygous G6PD deficiency and its clinical detection status

Glucose-6-phosphate dehydrogenase(G6PD)deficiency is one of the most common monogenic genetic diseases in the world.China is the high incidence area of this disease.G6PD heterozygote is the main form of G6PD deficiency in females,which belongs to X-linked incomplete dominant inheritance.Therefore,the enzyme activities of female heterozygotes vary widely,showing normal to mild to moderate enzyme activi-ties.There is no unified calibrators for G6PD enzymatic detection reagents commonly used in clinical practice,and the results of the same sample detected by different kits from different manufacturers are different,so there is a phenomenon of missed detection in clinical screening of female G6PD heterozygotes.Although G6PD gene detection can effectively improve the detection rate of female heterozygotes,its clinical application is lim-ited due to cost and other reasons.Therefore,the development of detection methods that can improve the de-tection rate of female heterozygotes and are suitable for large-scale screening and clinical application will be an urgent need to be solved in clinical practice.This article reviews the epidemiology of G6PD deficiency,genetic characteristics of female G6PD gene,clinical manifestations and detection methods of G6PD female heterozy-gotes.
4.Study on the relationship between biofilm formation ability and virulence of various drug-resistant strains of Staphylococcus aureus in bloodstream infections

Objective To explore the relationship between biofilm formation ability and virulence of vari-ous drug-resistant strains of Staphylococcus aureus(SA)in bloodstream infections.Methods A total of 42 strains of bloodstream infection SA were collected and identified and tested for drug sensitivity using the BDM50 automatic bacterial identification drug susceptibility system.The biofilm formation ability was deter-mined by crystal violet staining.The virulence was evaluated by detecting extracellular lipase,extracellular protease,and hemolysin,as well as conducting a toxicity test with Galleria mellonella larvae.Results Among the 42 strains of bloodstream infection SA,there were 16 strains(38.10%)of methicillin-resistant Staphylo-coccus aureus(MRSA).Among the 42 bloodstream infected SA strains,25 strains(59.50%)had the ability to form biofilms,among which there were 18 strains of methicillin-sensitive Staphylococcus aureus(MSSA)and 7 strains of MRSA.Among the 42 SA strains with bloodstream infection,37 strains(88.10%)produced hemolysin,18 strains(42.90%)produced extracellular protease,and no strains produced extracellular lipase.The Galleria mellonella larvae toxicity test showed that MRSA had stronger virulence than MSSA.Conclusion Compared with MSSA,MRSA has a higher proportion of extracellular enzymes produced,stronger virulence,and relatively weaker ability to form biofilms.
5.Epidemiological characteristics and relationship analysis of food intolerance in children in Zhuzhou area

Objective To investigate the prevalence of food intolerance among children in Zhuzhou area and its relationship with age,gender,systemic diseases,and food allergies,so as to provide a basis for the scientific adjustment of children's dietary structure.Methods A retrospective analysis was conducted on totally 1 592 children who underwent food intolerance and food allergen testing in the hospital,the positive rate and distri-bution of 14 kinds of food intolerance were assessed,and their correlation with various factors was analyzed.Results Among 14 kinds of food tested,milk and eggs had the highest positive rates of intolerance,at 82.22%and 55.78%,respectively.The majority of children were intolerant to 1 to 2 kinds of food,with a de-creasing trend in the number of children intolerant to multiple kinds of food.Among the 14 types of food,ex-cept for mushrooms and pork,there were statistically significant differences in the distribution of negative,mild,moderate,and severe intolerance in other foods(P<0.05).Children tended to have moderate or even se-vere intolerance to milk and eggs,while they tended to have mild intolerance to other foods.There was no sta-tistically significant difference in the overall food intolerance rate between boys and girls(P=0.654),but the positive rate of tomato intolerance in girls was slightly higher than that in boys(P=0.043).Except for pork,there were statistically significant differences in the positive rates of intolerance to 14 different foods among different age groups(P<0.05).The positive rates of intolerance to cod,mushrooms,and crabs increased with age,while the positive rates of intolerance to beef decreased with age.There was a statistically significant difference in the positive rate of milk intolerance between healthy children and children with skin allergies(P<0.05).There was a statistically significant difference in the proportion of individuals who were tolerant and not allergic to milk compared to hose who were intolerant and allergic to milk(P<0.05).There was a statistically significant difference in the proportion of individuals who were tolerant and not allergic to eggs compared to those who were intolerant and allergic to eggs(P<0.05).Conclusion The positive rate of food intolerance among children in Zhuzhou area is relatively high,with milk and eggs being the main intolerant foods.There are differences in the positive rate of intolerance among different gender and age groups,and in-tolerance to milk and eggs is associated with food allergies to some extent.
6.Value of serum MMP-16,GDF-15 in predicting the risk of postoperative deep venous thrombosis in elderly patients with tibial plateau fracture

Objective To explore the levels of serum matrix metalloproteinase-16(MMP-16)and growth differentiation factor-15(GDF-15)in elderly patients with tibial plateau fracture and its predictive value for postoperative deep veinous thrombosis(DVT).Methods A total of 107 elderly patients with tibial plateau fracture admitted to the First Affiliated Hospital of Gannan Medical University from February 2020 to Febru-ary 2024 were collected as study subjects.They were divided into DVT group(n=39)and non-DVT group(n=68)according to whether or not they had formed DVT at 7 d postoperative.The enzyme-linked immu-nosorbent assay was used to detect serum MMP-16 and GDF-15 preoperative,1 d postoperative and 3 d post-operative,and other clinical data of the study subjects were collected.Pearson correlation analysis was used to analyze the correlation between serum MMP-16,GDF-15 and venous thromboembolism risk assessment scale(Caprini)score.Logistic regression analysis was used to analyze the relationship between serum MMP-16,GDF-15 and postoperative DVT.The receiver operating characteristic(ROC)curve was used to evaluate the predictive value of the two indicators for postoperative DVT.Results The serum MMP-16 and GDF-15 level of two groups were first increased and then decreased at 1 d preoperative,1 d postoperative and 3 d postopera-tive(P<0.05).Serum MMP-16 and GDF-15 levels were higher in DVT group than in non-DVT group at 1 d postoperative and 3 d postoperative(P<0.05).Serum MMP-16,GDF-15 at 1 d postoperative and 3 d postop-erative were positive associated with Caprini score,respectively(both P<0.05).Elevated Caprini score,ele-vated D-D,elevated serum MMP-16 and GDF-15 at 3 d postoperative were independent risk factors for DVT(OR=3.445,1.652,1.841,1.715,all P<0.05).The area under the ROC curve of serum MMP-16,GDF-15 at 3 d postoperative and indicators combined to predict DVT was 0.798,0.792 and 0.895,respectively.The com-bined predictive value of indicators combined was greater than that of single indicator(Z=2.253,2.346,both P<0.05).Conclusion Serum MMP-16 and GDF-15 expression are upregulated after tibial plateau fracture in elderly patients,which the indicators combined can predict the risk of DVT.

It is proposed that the brain Xuanfu and the glymphatic system(GS)have commonality in physiological structure and similarity in functions such as transporting brain substances,transmitting brain information and clearing metabolites.It is believed that GS may be the substantial manifestation of the brain Xuanfu,and GS dysfunction is the key pathological link of depression.Xuanfu stagnation and Shenji dysfunction are the core pathogenesis of depression.Taking"Brain Xuanfu-GS"as the starting point,the patho-logical mechanism of depression is deeply explored,providing a theoretical basis for the clinical diagnosis and treatment of depression in traditional Chinese medicine.

Objective To investigate the factors that influence ocular surface biology and visual acuity in individuals with diabetic dry eye(DDE)and analyze how these factors contribute to changes in visual quality.Methods Based on the disease duration,fasting blood glucose(FBG),and glycated hemoglobin(HbA1c)levels of patients with type 2 diabe-tes mellitus(T2DM),the DDE patients were divided into different groups.Logistic regression analysis was used to identify influencing factors related to ocular surface biology and visual quality in each group of DDE patients.Tear film stability was evaluated based on the tear film rupture time(BUT),Schirmer I test(SIt),and ocular surface disease index(OSDI).Lip-iview? Surface interferometers were used to measure tear film lipid layer thickness(LLT),meibomian gland loss rate(MGP),meibomian gland opening number(MGYLS),and meibomian gland secretion score(MGYSS).Wavefront aber-rometry was used to measure corneal wavefront aberration values at 4 mm and 6 mm pupil diameters.Ocular response ana-lyzer(ORA)was adopted to analyze corneal hysteresis(CH)and corneal resistance factor(CRF).Moreover,ELISA ex-periment to evaluate the trend of changes in inflammatory factors in tears.Results Logistic regression analysis revealed that T2DM duration,smoking history,FBG,HbA1c,total cholesterol(TC),triglycerides(TG),OSDI score,LLT,BUT,SIt,MGP,MGYLS,MGYSS,total higher-order aberrations,spherical aberration,coma aberration,trefoil aberration,tumor necrosis factor-α,interleukin-6,matrix metalloproteinase-9,receptor for advanced glycation end products,and insu-lin were all influencing factors for the risk of DDE(all P<0.05).As the T2DM course prolonged and FBG or HbA1 c levels rose,tear film-related indicators(LLT,BUT,and SIt)and meibomian gland-related indicators(MGYLS and MGYSS)inpa-tients gradually decreased,while OSDI scores and MGP gradually increased(all P<0.05).As the T2DM course prolonged and FBG or HbA1c levels rose,the total higher-order aberrations,spherical aberration,coma aberration,and trefoil aber-ration in DDE patients under 4 mm and 6 mm pupil diameters gradually increased;Meanwhile,best corrected visual acuity,corneal hysteresis,and corneal resistance factor gradually decreased;The contents of tumor necrosis factor-α,interleukin-6,matrix metalloproteinase-9,receptor for advanced glycation end products,and insulin in tears all gradually increased,while mucin-5AC gradually decreased(all P<0.05).Conclusion With the prolongation of T2DM duration and the in-crease of FBG or HbA1c,the ocular surface inflammatory response in DDE patients gradually worsens,corneal biological function decreases,and visual quality deteriorates.Timely systemic and local interventions are of great significance for im-proving dry eye symptoms and visual quality in DDE patients.
9.Brain functional magnetic resonance imaging changes in patients with depression after acupuncture treatment: ALE meta-analysis

Objective:Through the meta-analysis of activation likelihood estimation (ALE), the functional magnetic resonance imaging research evidence of acupuncture in the treatment of depression was systematically integrated to analyze the effect of acupuncture on brain activity in patients with depression and systematically reveal its potential neural mechanism.Methods:The literatures which used functional magnetic resonance imaging to explore brain image changes of patients with depression after receiving acupuncture intervention were retrieved from CNKI, VIP database, Wanfang database, PubMed, Embase, EBSCO host MEDLINE and Web of Science database. The retrieval time is from the establishment of the database to March 2025. Data processing was performed using the ALE method by the Ginger ALE 2.3 software. The coordinates of the subjects included in the study were extracted, and the brain regions with abnormal spontaneous brain activity in patients with depression were integrated and analyzed.Results:A total of 28 articles involving 770 subjects were included. Depressed patients receiving acupuncture exhibited increased brain activity in the bilateral medial frontal gyrus and bilateral anterior cingulate cortex. Exploratory analysis revealed that depressed patients receiving acupuncture exhibited reduced activation in the left posterior cerebellar lobe (MNI: x, y, z = -6, -68, -21; x, y, z = -9, -81, -9) compared to healthy controls (voxel sum=5 824 mm 3,P<0.05). Conclusion:Patients with depression who received acupuncture intervention exhibited relatively consistent activation in brain regions compared to depressed patients who did not receive acupuncture and healthy individuals. Compared to healthy individuals, patients with depression who received acupuncture showed reduced activation in the left posterior cerebellum. It is suggested that acupuncture intervention may play a therapeutic role by regulating the functional state of emotion-related brain regions and brain networks.
10.The expression of the threonine and tyrosine kinase gene in eyelid basal cell carcinoma and its impact on the biological behavior of malignant tumor cells

Objective To investigate the relationship between the threonine and tyrosine kinase(TTK)gene and eyelid basal cell carcinoma(BCC).Methods Bioinformatics methods were used to screen the core gene(namely,TTK)associated with the occurrence and development of BCC from the Gene Expression Omnibus(GEO)database.Surgically removed eyelid BCC tissue specimens(BCC cells were divided into BBC Grade Ⅰ,Ⅱ and Ⅲ groups by tumor grade)and be-nign tumor tissue specimens(Control group)were collected from Ziyang Central Hospital for subsequent experiments.Cel-lular immunofluorescence assay(CIA)was used to detect the expression of the TTK gene in benign and malignant eyelid tumor cells.After knocking down TTK in BCC cells through transfection with lentiviruses(the cells transfected with LV-TTK-shRNA were taken as the TTK-shRNA group,and those transfected with LV-BBC-shRNA were taken as the BBC nega-tive control group),CIA was used to detect the expression of key proteins Bcl-2 and Bax in the apoptotic signaling pathway of each group of cells.Results The bioinformatics analysis showed that the TTK gene was the core gene associated with the occurrence and development of eyelid BCC.CIA detection results revealed that the fluorescence signal intensities in the tumor cytoplasm of Control,BCC Grade Ⅰ,Ⅱ,and Ⅲ groups were 1.03±0.07,1.28±0.11,1.58±0.13 and 1.92±0.17,respectively.The fluorescence signal intensity gradually increased,and the difference in fluorescence signal intensity among the four groups was statistically significant(all P<0.05).Compared with that in the Control group(1.02±0.05),the cell fluorescence intensity was increased in the BCC negative control group(1.74±0.12)and decreased in the TTK-shRNA group(1.31±0.09)(P<0.05).The difference in cell fluorescence intensity was significant among the Control,BCC nega-tive control and TTK-shRNA groups(all P<0.05).The fluorescence intensity of the anti-apoptotic protein Bcl-2 was 1.04±0.12 in the Control group,2.12±0.23 in the BCC negative control group,and 1.43±0.15 in the TTK-shRNA group.The fluorescence intensity of the pro-apoptotic protein Bax was 1.02±0.08 in the Control group,0.64±0.11 in the BCC negative control group,and 1.47±0.16 in the TTK-shRNA group.After TTK knockdown,the expression level of BcL-2 in BCC cells decreased,and that of Bax increased.The fluorescence intensities of BcL-2 and Bax were significantly different among the Control,BCC negative control and TTK-shRNA groups(all P<0.05).Conclusion The TTK gene plays a role in the regulation of eyelid BCC cell proliferation,and this effect is closely related to the PI3K-AKT-Bcl-2/Bax signaling path-way.
