1.Research progress on pentacyclic triterpenoids in medicinal Ilex species and their pharmacological activities.

Traditional Chinese medicine(TCM) capable of clearing heat and removing toxin is most commonly used in clinical practice and has the effect of removing fire-heat and toxin. Studies have shown that most of the Ilex plants have the effect of clearing heat and removing toxin, among which the varieties of I. cornuta, I. pubescens, I. rotunda, I. latifolia, and I. chinensis are most widely used. These plants generally contain triterpenoids and their glycosides, alkaloids, flavonoids, phenylpropanoids, and other chemical components, especially pentacyclic triterpenoids. According to their skeletons, pentacyclic triterpenoids can be divided into the oleanane type, the ursane type, the lupinane type, etc. Among them, ursane-type components are the most abundant, and 136 species have been found so far. These components have been proved to have pharmacological effects such as anti-inflammatory, anti-tumor, hypolipidemic, anti-thrombosis, cardiomyocyte-protective, antibacterial, and hepatoprotective effects. Therefore, this paper systematically reviews the domestic and foreign literature on Ilex plants with a focus on the research progress on pentacyclic triterpenoids and their pharmacological activities, aiming to provide reference for the development of TCM resources with the effect of clearing heat and removing toxin.

2.Granuloma faciale and Takayasu arteritis in a child: a case report.

An 11-year-old boy presented with erythematous plaques over the bilateral mandibular and mental regions for 2 years, accompanied by cough and dyspnea for more than 2 months. Chest computed tomography angiography revealed marked stenosis of the right pulmonary artery, irregular aortic caliber, and aortic wall thickening. Histopathological examination of the skin lesion, including immunohistochemistry and special stains, confirmed a chronic suppurative inflammation. Whole-exome sequencing was negative. A final diagnosis of granuloma faciale and Takayasu arteritis was established. Combination therapy with systemic tocilizumab, prednisone, and methotrexate, along with topical 0.1% tacrolimus ointment, resulted in a favorable clinical response. This report summarizes the clinical features of a pediatric case of granuloma faciale and Takayasu arteritis and reviews the etiology, diagnostic approach, and current treatment strategies for the disorders, aiming to enhance clinicians' understanding of these conditions.

3.Correlation between Serum FGF-23, HPSE Levels and Early Renal Impairment in Patients with Multiple Myeloma.

OBJECTIVE:
To investigate the relationship between serum levels of fibroblast growth factor-23 (FGF-23), heparanase (HPSE) and early renal impairment (RI) in patients with multiple myeloma (MM).
METHODS:
A retrospective analysis was conducted on the clinical data of 125 MM patients who were initially diagnosed in the Department of Hematology of the First Affiliated Hospital of Baotou Medical College, Inner Mongolia University of Science and Technology from June 2020 to June 2023. The patients were divided into RI group (>176.80 μmol/L) and non-RI group (≤176.80 μmol/L) based on their serum creatinine levels when diagnosed. The baseline data and laboratory indexes of the two groups were compared. The relationship between serum FGF-23, HPSE and early RI in MM patients was analyzed.
RESULTS:
Among 125 newly diagnosed MM patients, 33 cases developed early RI, accounting for 26.40%. The proportion of light chain type, blood urea nitrogen (BUN), blood uric acid, lactate dehydrogenase, FGF-23, and HPSE levels in RI group were higher than those in non-RI group (all P <0.05). There was no statistical significant difference in other data between the two groups (P >0.05). Multivariate logistic regression analysis showed that BUN, FGF-23 and HPSE were associated with early RI in MM patients (all P <0.05). The serum FGF-23 level was divided into Q1-Q4 groups by quartile, and the serum HPSE level was divided into q1-q4 groups. The correlation analysis showed that with the increase of serum FGF-23 and HPSE levels, the incidence of early RI increased (r =0.668, 0.592). Furthermore, logistic regression analysis showed that after controlling for confounding factors, elevated levels of serum FGF-23 and HPSE were still influencing factors for early RI in MM patients (OR>1, P <0.05). According to Pearson's linear correlation test, there was a positive correlation between serum FGF-23 level and HPSE level (r =0.373).
CONCLUSION
There is a certain correlation between serum levels of FGF-23, HPSE and early RI in MM patients, and the incidence of early RI is higher in patients with abnormally high levels of both.

4.Oxymatrine, a novel TLR2 agonist, promotes megakaryopoiesis and thrombopoiesis through the STING/NF-κB pathway.

Radiation-induced thrombocytopenia (RIT) faces a perplexing challenge in the clinical treatment of cancer patients, and current therapeutic approaches are inadequate in the clinical settings. In this research, oxymatrine, a new molecule capable of healing RIT was screened out, and the underlying regulatory mechanism associated with magakaryocyte (MK) differentiation and thrombopoiesis was demonstrated. The capacity of oxymatrine to induce MK differentiation was verified in K-562 and Meg-01 cells in vitro. The ability to induce thrombopoiesis was subsequently demonstrated in Tg (cd41:enhanced green fluorescent protein (eGFP)) zebrafish and RIT model mice. In addition, we carried out network pharmacological prediction, drug affinity responsive target stability assay (DARTS) and cellular thermal shift assay (CETSA) analyses to explore the potential targets of oxymatrine. Moreover, the pathway underlying the effects of oxymatrine was determined by Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analyses, Western blot (WB), and immunofluorescence. Oxymatrine markedly promoted MK differentiation and maturation in vitro. Moreover, oxymatrine induced thrombopoiesis in Tg (cd41:eGFP) zebrafish and accelerated thrombopoiesis and platelet function recovery in RIT model mice. Mechanistically, oxymatrine directly binds to toll-like receptor 2 (TLR2) and further regulates the downstream pathway stimulator of interferon genes (STING)/nuclear factor-kappaB (NF-κB), which can be blocked by C29 and C-176, which are specific inhibitors of TLR2 and STING, respectively. Taken together, we demonstrated that oxymatrine, a novel TLR2 agonist, plays a critical role in accelerating MK differentiation and thrombopoiesis via the STING/NF-κB axis, suggesting that oxymatrine is a promising candidate for RIT therapy.
5.The effect of rutaecarpine on improving fatty liver and osteoporosis in MAFLD mice

Metabolic-associated fatty liver disease (MAFLD) and osteoporosis (OP) are two very common metabolic diseases. A growing body of experimental evidence supports a pathophysiological link between MAFLD and OP. MAFLD is often associated with the development of OP. Rutaecarpine (RUT) is one of the main active components of Chinese medicine Euodiae Fructus. Our previous studies have demonstrated that RUT has lipid-lowering, anti-inflammatory and anti-atherosclerotic effects, and can improve the OP of rats. However, whether RUT can improve both fatty liver and OP symptoms of MAFLD mice at the same time remains to be investigated. In this study, we used C57BL/6 mice fed a high-fat diet (HFD) for 4 months to construct a MAFLD model, and gave the mice a low dose (5 mg·kg-1) and a high dose (15 mg·kg-1) of RUT by gavage for 4 weeks. The effects of RUT on liver steatosis and bone metabolism were then evaluated at the end of the experiment [this experiment was approved by the Experimental Animal Ethics Committee of Institute of Medicinal Biotechnology, Chinese Academy of Medical Sciences (approval number: IMB-20190124D303)]. The results showed that RUT treatment significantly reduced hepatic steatosis and lipid accumulation, and significantly reduced bone loss and promoted bone formation. In summary, this study shows that RUT has an effect of improving fatty liver and OP in MAFLD mice.
6.Risk factors, diagnosis and treatment of perforation after endoscopic retrograde cholangiopancreatography

After 50 years of clinical development, endoscopic retrograde cholangiopancreatography (ERCP) has become the preferred method for the clinical diagnosis and treatment of cholangio-pancreatic duct diseases; however, the major postoperative complications of ERCP, such as pancreatitis, hemorrhage, and perforation, are still a difficult issue faced by clinicians, and postoperative perforation is associated with an extremely high risk of death. Therefore, it is very important to explore the risk factors for perforation after ERCP, make a definite diagnosis of perforation in a timely manner, and formulate precise prevention and treatment measures. By reviewing a large number of articles, this article summarizes the influencing factors for perforation after ERCP and related diagnosis and treatment measures.
7.Predictive value of combined detection of serum SDF-1, YKL-40, CA724 and PG on recurrence after ESD in patients with early gastric cancer

Objective:To explore the combined predictive value of serum stromal cell-derived factor-1 (SDF-1), chitinase 3-like protein 1 (YKL-40), carbohydrate antigen 724 (CA724) and pepsinogen (PG) on recurrence after endoscopic submucosal dissection (ESD) in patients with early gastric cancer.Methods:96 patients with early gastric cancer admitted to Yan 'an Hospital of Traditional Chinese Medicine from August 2021 to August 2023 were retrospectively selected and enrolled into the study. According to the recurrence after ESD, they were divided into recurrence group (31 cases) and non-recurrence group (65 cases). The differences in baseline data, serum SDF-1, YKL-40, CA724 and PG levels were compared between groups. The correlation between serum SDF-1, YKL-40, CA724 and PG levels and clinical indicators of patients was analyzed. Normally distributed quantitative data were expressed as xˉ± s, and independent sample t test was used for inter-group comparison. Counting data were calculated as percentages, and χ2 test was used for inter-group comparison. Spearman method was used for correlation analysis. Multivariate logistics regression analysis was used to analyze the factors affecting postoperative recurrence. ROC curve was used to analyze the single and combined diagnostic value of each serum indicator on recurrence after ESD. Results:The number of well-differentiated cases in recurrence group (32.26%, 10/31) was lower than that in non-recurrence group (70.77%, 46/65), the difference was statistically significant ( χ2=12.81, P<0.001). The levels of SDF-1 [(160.57±16.49) μg/L], YKL-40 [(208.49±22.69) μg/L] and CA724 [(10.49±1.91) kU/L] of recurrence group were higher than non-recurrence group[(136.57±14.56) μg/L, (157.53±20.46) μg/L, (5.11±0.83) kU/L)], and the differences were statistically significant ( t=7.23, 11.01 and 19.29, respectively, all P<0.05), while the PGⅠ/PGⅡ [(2.87±0.57) μg/L] was lower than that in non-recurrence group [(4.18±0.76) μg/L], the difference was statistically significant( t=8.514, P<0.05). Correlation analysis showed that SDF-1, YKL-40 and CA724 were negatively correlated with the degree of differentiation ( r=-0.545, -0.415, -0.718, respectively, all P<0.05), and the PGⅠ/PGⅡ was positively correlated with the degree of differentiation ( r=0.687, P<0.05). Multivariate logistics regression analysis suggested that SDF-1 ( OR=8.191, 95% CI=1.483-45.246, P=0.016), YKL-40 ( OR=9.843, 95% CI=1.782-54.387, P=0.009), CA724 ( OR=10.386, 95% CI=2.053-52.506, P=0.005), PGⅠ/PGⅡ ( OR=0.108, 95% CI=0.028-0.417, P=0.001) and differentiation degree ( OR=0.421, 95% CI=0.219-0.810, P=0.010) were influencing factors of postoperative recurrence. ROC analysis results showed that the AUCs of SDF-1, YKL-40, CA724 and PGⅠ/PGⅡ in predicting postoperative recurrence of early gastric cancer patients were 0.857, 0.956, 0.945 and 0.910, sensitivities were 80.65%, 90.32%, 83.87% and 83.87%, specificities were 78.46%, 76.92%, 81.54% and 80.00%, respectively. The AUC, sensitivity and specificity of combined prediction were 0.986, 96.77% and 73.85%. Conclusion:The levels of serum SDF-1, YKL-40 and CA724 in patients with early gastric cancer recurrence after ESD are higher while the PGI/PGII is lower, and the above levels are relevant factors for postoperative recurrence and can be used as clinical reference indicators to predict recurrence after ESD.
8.Analysis of erectile dysfunction and its associated factors in male patients undergoing peritoneal dialysis

Objective To investigate the prevalence proportion and influencing factors of erectile dysfunction in male patients receiving peritoneal dialysis(PD),and to provide a basis for clinical intervention.Methods A total of 170 male patients undergoing maintenance PD in Jinling Hospital from January to June 2025 were selected as the study subjects.And the international erectile function index-5 was used to evaluate sexual function of the patients.The general data,dialy-sis adequacy,nutritional status and other clinical indicators were collected.The logistic regression analysis was used to identify the influence factors of erectile dysfunction in the patients with PD.Results The incidence of erectile dysfunction in male patients with PD was 45.3%.And univariate analysis showed that age,education level,smoking history,dialysis-related symptoms and depressive status were significantly correlated with erectile dysfunction in male patients(all P<0.05).Ordered logistic regression analysis identified age,sleep duration,smoking history,albumin and total cholesterol as independent risk factors for erectile dysfunction in male patients with PD.Conclusion The incidence of erectile dys-function is relatively high in male patients with PD which is influenced by various physiological and psychological factors.Clinically,attention should be paid to patients' nutritional status,sleep quality,and other factors,with comprehensive in-terventions implemented to improve sexual function and overall quality of life.

Objective To investigate the distribution and antimicrobial resistance profiles of clinically isolated Haemophilus influenzae and Moraxella catarrhalis in hospitals across China from 2015 to 2021,and provide evidence for rational use of antimicrobial agents.Methods Data of H.influenzae and M.catarrhalis strains isolated from 2015 to 2021 in CHINET program were collected for analysis,and antimicrobial susceptibility testing was performed by disc diffusion method or automated systems according to the uniform protocol of CHINET.The results were interpreted according to the CLSI breakpoints in 2022.Beta-lactamases was detected by using nitrocefin disk.Results From 2015 to 2021,a total of 43 642 strains of Haemophilus species were isolated,accounting for 2.91%of the total clinical isolates and 4.07%of Gram-negative bacteria in CHINET program.Among the 40 437 strains of H.influenzae,66.89%were isolated from children and 33.11%were isolated from adults.More than 90%of the H.influenzae strains were isolated from respiratory tract specimens.The prevalence of β-lactamase was 53.79%in H.influenzae strains.The H.influenzae strains isolated from children showed higher resistance rate than the strains isolated from adults.Overall,779 strains of H.influenzae did not produce β-lactamase but were resistant to ampicillin(BLNAR).Beta-lactamase-producing strains showed significantly higher resistance rates to these antimicrobial agents than the β-lactamase-nonproducing strains.Of the 16 191 M.catarrhalis strains,80.06%were isolated from children and 19.94%isolated from adults.M.catarrhalis strains were mostly susceptible to both amoxicillin-clavulanic acid and cefuroxime,evidenced by resistance rate lower than 2.0%.Conclusions The emergence of antibiotic-resistant H.influenzae due to β-lactamase production poses a challenge for clinical anti-infective treatment.Therefore,it is very important to implement antibiotic resistance surveillance for H.influenzae and guide rational antibiotic use.All local clinical microbiology laboratories should actively improve antibiotic susceptibility testing and strengthen antibiotic resistance surveillance for H.influenzae.

Objective To understand the changing composition and antibiotic resistance of bacterial species in the clinical isolates from outpatient and emergency department(hereinafter referred to as outpatients)and inpatient children over time in various hospitals,and to provide laboratory evidence for rational antibiotic use.Methods The data on clinically isolated pathogenic bacteria and antimicrobial susceptibility of isolates from outpatients and inpatient children in the CHINET program from 2015 to 2021 were collected and analyzed.Results A total of 278 471 isolates were isolated from pediatric patients in the CHINET program from 2015 to 2021.About 17.1%of the strains were isolated from outpatients,primarily group A β-hemolytic Streptococcus,Escherichia coli,and Staphylococcus aureus.Most of the strains(82.9%)were isolated from inpatients,mainly SS.aureus,E.coli,and H.influenzae.The prevalence of methicillin-resistant S.aureus(MRSA)in outpatients(24.5%)was lower than that in inpatient children(31.5%).The MRSA isolates from outpatients showed lower resistance rates to the antibiotics tested than the strains isolated from inpatient children.The prevalence of vancomycin-resistant Enterococcus faecalis or E.faecium and penicillin-resistant S.pneumoniae was low in either outpatients or inpatient children.S.pneumoniae,β-hemolytic Streptococcus and S.viridans showed high resistance rates to erythromycin.The prevalence of erythromycin-resistant group A β-hemolytic Streptococcus was higher in outpatients than that in inpatient children.The prevalence of β-lactamase-producing H.influenzae showed an overall upward trend in children,but lower in outpatients(45.1%)than in inpatient children(59.4%).The prevalence of carbapenem-resistant Klebsiella pneumoniae(CRKpn),carbapenem-resistant Pseudomonas aeruginosa(CRPae)and carbapenem-resistant Acinetobacter baumannii(CRAba)was 14%,11.7%,47.8%in outpatients,but 24.2%,20.6%,and 52.8%in inpatient children,respectively.The prevalence of multidrug-resistant E.coli,K.pneumoniae,Proteus mirabilis,P.aeruginosa and A.baumannii strains was lower in outpatients than in inpatient children.The prevalence of fluoroquinolone-resistant E.coli,ESBLs-producing K.pneumoniae,ESBLs-producing P.mirabilis,carbapenem-resistant E.coli(CREco),CRKpn,and CRPae was lower in children in outpatients than in inpatient children,but the prevalence of CRAba in 2021 was higher than in inpatient children.Conclusions The distribution of clinical isolates from children is different between outpatients and inpatients.The prevalence of MRSA,ESBL,and CRO was higher in inpatient children than in outpatients.Antibiotics should be used rationally in clinical practice based on etiological diagnosis and antimicrobial susceptibility test results.Ongoing antimicrobial resistance surveillance and prevention and control of hospital infections are crucial to curbing bacterial resistance.
