1.Fluoroscopy-Guided Anterior Cervical Epidural Blood Patch for Incidental Durotomy Following Anterior Cervical Discectomy and Fusion

In patients with ossification of the posterior longitudinal ligament (OPLL), the risk of incidental durotomy (ID) during anterior cervical spine surgery is relatively high. However, the anterior surgical approach is technically demanding because of limited visualization and restricted operative space, which makes direct dural repair difficult. As a result, indirect repair techniques are typically employed, but these approaches can pose significant postoperative management challenges, particularly in cases of symptomatic cerebrospinal fluid (CSF) leakage. A 68-year-old male patient presented with right-sided symptoms involving both the upper and lower extremities. Radiological evaluation revealed C3–4–5 anterolisthesis, OPLL at the C4–5 level, and bilateral foraminal stenosis at C3–4 and C4–5. During anterior cervical discectomy and fusion (ACDF), an ID occurred at the anterior aspect of the C4–5 segment and was managed with primary indirect repair. Approximately 2 weeks postoperatively, the patient developed symptomatic CSF leakage. Under fluoroscopic guidance, a needle was precisely positioned adjacent to the interbody cage at the anterior aspect of the C4–5 segment—the site of the durotomy—to administer a targeted anterior epidural blood patch (EBP). The procedure was completed successfully without complications and resulted in effective resolution of the CSF leak. ID at the anterior aspect of the spinal canal during ACDF may lead to persistent symptoms due to CSF leakage. In such cases, a minimally invasive strategy, such as a targeted anterior EBP performed under fluoroscopic guidance, may represent an effective alternative to surgical re-exploration for symptom resolution.
2.Full-Endoscopic Paraspinal Foraminotomy for Lumbar Foraminal Stenosis

Transforaminal endoscopic lumbar foraminotomy (TELF) is widely performed as a full-endoscopic surgical procedure for the treatment of lumbar foraminal stenosis. The technique involves the use of a small-caliber endoscopic system introduced through Kambin triangle to accomplish the primary surgical steps. However, anatomical barriers are frequently encountered in the lower lumbar segments, particularly at L4–5 and L5–S1, which may limit the feasibility of the transforaminal approach. Although various advanced transforaminal techniques have been developed to overcome these anatomical barriers, these techniques often make the procedure more technically demanding and may prolong operative time. In this video presentation, we report 2 cases in which a full-endoscopic paraspinal lumbar foraminotomy was performed to achieve adequate decompression of the exiting nerve root (ENR) without being constrained by these anatomical limitations. We also outline the procedural details and technical characteristics of this approach. Both patients presented with lumbar foraminal stenosis at the L5–S1 level, where anatomical barriers such as a high iliac crest, large transverse process, and sacral ala were present. A full-endoscopic paraspinal foraminotomy was performed at this level using a large-caliber endoscopic system, allowing sufficient decompression of the ENR and resulting in marked relief of radicular leg pain. We report a surgical procedure for full-endoscopic paraspinal lumbar foraminotomy using a large-caliber endoscopic system that permits the use of instruments of various sizes and configurations. When applied in appropriate clinical scenarios, this technique may facilitate more convenient and expedited decompression of the ENR.
3.Real‑world Application of the International Anorectal Physiology Working Group Standardized Protocol and London Classification: A Multi‑country Cross‑sectional Survey of Anorectal Manometry Practice in Asia

Background/Aims:
The International Anorectal Physiology Working Group (IAPWG) has proposed a standardized protocol and the London classification to enhance the consistency and diagnostic accuracy of anorectal manometry (ARM). However, real-world adoption in Asian countries has not been systematically assessed. This study aims to evaluate current ARM practices and adherence to the IAPWG protocol across Asian centers.
Methods:
A cross-sectional, 50-item web-based survey was distributed to gastroenterologists and motility specialists practicing in Asian countries.
Results:
Thirty-one centers from 8 countries responded (20 in Korea, 3 in Japan, 2 in Taiwan, 1 in China, and 7 in other countries). High-resolution ARM was used in 80.6% of centers, primarily with solid-state catheters. While all centers performed rest and short squeeze maneuvers, only 58.1% conducted all maneuvers recommended by the IAPWG protocol.Considerable variation was observed in test methodology and interpretation, including definitions of resting pressure, squeeze duration, push maneuver repetition, and rectal balloon volume for rectoanal inhibitory reflex. For balloon expulsion and rectal sensory testing, centers differed in patient positioning, balloon type, inflation methods, and threshold definitions. Only 38.7% of centers reported having institutional normative values for test interpretation, and 64.5% applied the London classification.
Conclusions
Across Asian centers, ARM practice shows marked regional variation and incomplete implementation of the IAPWG standardized protocol and London classification, highlighting persistent gaps in standardization. Strengthening procedural guidance, regionally appropriate normative data, and interpretation criteria through coordinated education and international collaboration is needed to support more consistent and clinically meaningful use of ARM in routine practice.

Purpose:
Since 1995, the Korean Gastric Cancer Association (KGCA) has been periodically conducting nationwide surveys on patients with surgically treated gastric cancer. This study details the results of the survey conducted in 2023.
Materials and Methods:
The survey was conducted from March to December 2024 using a standardized case report form. Data were collected on 86 items, including patient demographics, tumor characteristics, surgical procedures, and surgical outcomes. The results of the 2023 survey were compared with those of previous surveys.
Results:
Data from 12,751 cases were collected from 66 institutions. The mean patient age was 64.6 years, and the proportion of patients aged ≥71 years increased from 9.1% in 1995 to 31.7% in 2023. The proportion of upper-third tumors slightly decreased to 16.8% compared to 20.9% in 2019. Early gastric cancer accounted for 63.1% of cases in 2023.Regarding operative procedures, a totally laparoscopic approach was most frequently applied (63.2%) in 2023, while robotic gastrectomy steadily increased to 9.5% from 2.1% in 2014.The most common anastomotic method was the Billroth II procedure (48.8%) after distal gastrectomy and double-tract reconstruction (51.9%) after proximal gastrectomy in 2023.However, the proportion of esophago-gastrostomy with anti-reflux procedures increased to 30.9%. The rates of post-operative mortality and overall complications were 1.0% and 15.3%, respectively.
Conclusions
The results of the 2023 nationwide survey demonstrate the current status of gastric cancer treatment in Korea. This information will provide a basis for future gastric cancer research.
6.Risk Factors of FEV 1 /FVC Decline in COPD Patients

Background:
Factors influencing the decline in forced expiratory volume in one second (FEV1 )/forced vital capacity (FVC) for chronic obstructive pulmonary disease (COPD) progression remain uncertain. We aimed to identify risk factors associated with rapid FEV1 / FVC decline in patients with COPD.
Methods:
This multi-center observational study was conducted from January 2012 to December 2022. Eligible patients were monitored with symptoms, spirometric tests, and treatment patterns over 3 years. Rapid FEV1 /FVC decliners were defined as the quartile of patients exhibiting the highest annualized percentage decline in FEV1 /FVC.
Results:
Among 1,725 patients, 435 exhibited rapid FEV1 /FVC decline, with an annual change of −2.5%p (interquartile range, −3.5 to −2.0). Rapid FEV1 /FVC decliners exhibited lower body mass index (BMI), higher smoking rates, elevated post-bronchodilator (BD) FEV1 , higher post-BD FEV1 / FVC, and a lower prevalence of Staging of Airflow Obstruction by Ratio (STAR) stage IV. Rapid FEV1 /FVC decline was not linked to the annual exacerbation rate, but there was an association with symptom deterioration and FEV1 decline. In multivariable analyses, low BMI, current smoking, increased modified Medical Research Council dyspnoea score, low post-BD FEV1 , low STAR stage, high forced mid-expiratory flow (FEF 25-75% ), accelerated FEV1 decline, and not initiating dual BD therapy were identified as independent risk factors for rapid FEV1 /FVC decline.
Conclusion
We identified the risk factors for rapid FEV1 /FVC decline, including BMI, smoking, symptoms deterioration, FEV1 decline, and adherence to standard inhaler treatment. Our findings underscore the potential benefits of maintaining consistent use of long-acting beta-agonist/long-acting muscarinic antagonist even in the presence of worsening symptoms, in attenuating FEV1 /FVC decline.
