Objective:To explore the maintenance of sinus rhythm and the changes in functional tricuspid regurgitation(FTR) in patients with atrial fibrillation(AF) combined with FTR following the Mei Mini Maze Procedure, and to analyze the correlation between these two factors. Methods:We retrospectively included 180 patients with AF and FTR who underwent left-sided thoracoscopic AF ablation(the Mei Mini Maze Procedure) at Xinhua Hospital in Shanghai from January 2019 to December 2021. After propensity score matching to eliminate confounding factors, the patients were divided into two groups based on the severity of FTR before surgery: the non-significant FTR group(+ -+ +, 68 cases) and the significant FTR group(≥+ + +, 68 cases). Outpatient follow-up with 24-72 hour dynamic electrocardiogram and transthoracic echocardiography(TTE) was conducted at 3, 6, 12, 24, and 36 months post-discharge. The maintenance of sinus rhythm and changes in FTR were compared between the two groups. Results:All patients successfully completed the Mei Mini Maze Procedure. Compared to preoperative values, the severity of FTR improved in both groups. Non-significant FTR group: 59% of patients(40 cases) had no regurgitation, 27%(18 cases) had mild FTR, 11%(8 cases) had moderate FTR, and 3%(2 cases) had severe FTR; significant FTR group: 19%(n=13) had no regurgitation, 25%(17 cases) had mild FTR, 25%(17 cases) had moderate FTR, and 31%(21 cases) had severe FTR. The Kaplan-Meier(KM) curve revealed that the sinus rhythm maintenance rate at 3 years post-surgery was significantly higher in the non-significant FTR group(72.06%) than in the significant FTR group (57.35%), with a statistically significant difference( P=0.011). Multivariate analysis showed that postoperative FTR ≥ moderate( HR=11.469), preoperative significant FTR( HR=1.206), age over 65 years( HR=3.734), non-paroxysmal AF( HR=2.346), and longer AF duration( HR=1.151) were risk factors for AF recurrence in this cohort. Conclusion:The recovery of sinus rhythm in AF patients is closely related to the severity of FTR. Patients with preoperative significant FTR had a lower rate of sinus rhythm maintenance after surgery. Both preoperative and postoperative significant FTR are risk factors for AF recurrence in AF patients with FTR. Therefore, for patients with significant FTR, simultaneous treatment of FTR during the surgery, including a standard Cox-Maze Ⅳ procedure, may be more beneficial for maintaining postoperative sinus rhythm.

Objective To describe and analyze the epidemiological characteristics of scrub typhus in Huai'an, Jiangsu Province from 2006 to 2024 and explore the long-term incidence trend and distribution of high-risk areas, and to formulate targeted prevention and control strategies. Methods The scrub typhus case report data of Huai'an from 2006 to 2024 in the Chinese Disease Prevention and Control Information System were extracted for descriptive analysis. Results A total of 898 cases of scrub typhus were reported in Huai'an, with an average annual incidence rate of 0.96 per 100 000 from 2006 to 2024. There was a turning point in the incidence trend of scrub typhus in 2011. From 2006 to 2011, the annual percentage change (APC) was 47.09% (95% CI: 7.53 - 859.39), and the upward trend was statistically significant (P < 0.05). From 2012 to 2024, the APC was -2.12% (95% CI: -29.09 - 3.75), and the downward trend was not statistically significant. October and November were the high-incidence months, and the total concentration from 2006 to 2024 was 0.93, indicating that scrub typhus had strict seasonality. The circular distribution method estimated that the peak period of the epidemic was from October 11th to November 25th, and the peak day of incidence was November 3rd. Jinhu County was a high-incidence area. The ratio of male to female cases was 1.03. The age group with the highest reported incidence was 40 to < 80 years old. The occupation with the highest proportion was farmers, accounting for 78.03%. Conclusion From 2006 to 2024, scrub typhus in Huai'an shows a peak every 3 - 4 years. Middle-aged and elderly farmers are the key population at risk, and Jinhu County is a key area. In the future, targeted health education should be carried out to effectively control the prevalence of scrub typhus.

Objective To investigate the effect of LINC00839 on the malignant biological behavior of endometrial cancer cells by regulating the miR-625-5p/cytoplasmic polyadenylation element binding protein 4 (CPEB4) axis. Methods The cancer tissues and adjacent tissues of 46 patients with endometrial cancer were obtained, endometrial cancer cells of HEC-1B,Ishikawa and RL95-2 and human endometrial epithelial cells (HEEC) were cultured in vitro, and the expression levels of LINC00839,miR-625-5p and CPEB4 in tissues and cells were detected. HEC-1B cells were divided into the HEC-1B group (conventional culture),sh-Ctrl group (transfected with sh-Ctrl),sh-LINC00839 group (transfected with shRNA-LINC00839),anti-miR-625-5p group (transfected with shRNA-LINC00839 and miR-625-5p inhibitor),and anti-NC group (transfected with shRNA-LINC00839 and inhibitor-NC). The proliferation, apoptosis, migration and invasion abilities of HEC-1B cells in each group were compared, and the expressions of CPEB4 and epithelial-mesenchymal transition (EMT) related proteins were determined. The targeting relationships between LINC00839 and miR-625-5p,and miR-625-5p and CPEB4 were analyzed. Results The mRNA expression of LINC00839 and CPEB4,as well as the positive expression rate of CPEB4 protein in endometrial cancer tissues were higher than those in adjacent tissues, and the expression of miR-625-5p was lower than that in adjacent tissues (P<0.05). Compared with HEEC cells, the expression of LINC00839 and the mRNA and protein expression of CPEB4 in Ishikawa,RL95-2 and HEC-1B cells increased (P<0.05),and the expression of miR-625-5p decreased (P<0.05). Compared with the sh-LINC00839 group and anti-NC group, the protein expression of N-cadherin,Vimentin,and CPEB4,24-hour absorbance, and migration and invasion cell numbers of HEC-1B cells in the HEC-1B group,sh-Ctrl group and anti-miR-625-5p group increased (P<0.05),while the expression of E-cadherin protein and cell apoptosis rate decreased (P<0.05). There were binding sites between LINC00839 and miR-625-5p,miR-625-5p and CPEB4,with targeting regulatory relationships. Conclusion LINC00839 is related to the malignant biological behavior of endometrial cancer cells, interference with LINC00839 expression can inhibit the proliferation, invasion and migration, and promote apoptosis of HEC-1B cells, and its mechanism may be achieved by regulating the miR-625-5p/CPEB4 axis.

Purpose To investigate the differential expression of proteins secreted by diffuse-type gastric cancer-associated fibroblasts(DGC-CAFs)and intestinal-type gastric cancer-associated fibroblasts(IGC-CAFs)according to Lauren classification of gastric cancer. Methods Fresh surgery samples were acquired to extract primary cancer-asso-ciated fibroblasts(CAFs)to obtain the conditional medium, including three cases of diffuse-type gastric cancer(DGC)and three cases of intestinal-type gastric cancer(IGC).Then high performance liquid chromatography-tandem mass spectrometry(HPLC-MS/MS)was used to detect the differences in secreted factors between DGC-CAFs and IGC-CAFs.Additionally,a total of 1 174 cases were collected from the GEO and TCGA databases, and the distribution of stromal cells was assessed via ESTIMATE to derive stromal scores for both DGC and GC.Furthermore,Kaplan-Meier survival analysis was performed to evaluate the relationship between high stromal scores, elevated CAFs proportions, and patient prognosis.Subsequently,GO enrichment analysis was performed to investigate associated genes and their bi-ological functions. Results HPLC-MS/MS analysis showed that only 10 proteins were identified to be expressed in both DGC-CAFs and IGC-CAFs,namely SPARC,COL6A1,COL1A2,COL1A1,COL3A1,FN1,DCN,ACTG1,TIMP1 and SHROOM3.There were 8 proteins expressed higher in DGC-CAFs than IGC-CAFs in all cases with ratio>2,namely COL3A1(5.28),COL1A2(3.98),DCN(3.71),TIMP1(3.40),COL6A1(3.35),ACTG1(3.13),COL1A1(2.84)and SHROOM3(2.50).Two other proteins,TUBB and BASP1,were identified in all three cases of DGC-CAFs.The former was only identified in one case of IGC-CAFs,while the latter was not identified in all three cases of IGC-CAFs.The results of bioinformatics analysis showed that the stromal score of DGC was higher than IGC,which correlated with poor prognosis.Analysis of cell components revealed that the related genes were more enriched in ECM,collagen-containing extracellular matrix and cell-cell junction. Also, the biological process and molecular function was based on the components, which was consistent with HPLC-MS/MS analysis results. Conclusion There are differ-ences in secreted proteins of DGC-CAFs and IGC-CAFs,both on the types and the content. The differential proteins are mostly enriched in ECM-related signaling pathways.Presumably, the high content of CAFs in the stroma affects the prognosis of gastric cancer patients by influencing the gene expression and the function of receptor pathway of ECM.
