1.Correlation between serum ferritin and response rate and prognosis of lung cancer immunotherapy

Objective To study the correlation between the serum ferritin level and the pathological classification,tumor stage,PD-L1 expression,disease control rate(DCR),and survival period of patients with lung cancer.Methods A total of 85 patients with lung cancer admitted to our hospital from February 2020 to February 2021 were selected as the research subjects.All patients received immunotherapy,and the serum ferritin levels of patients before and after immunotherapy were detected.Meanwhile,according to the difference in serum ferritin levels before and after immunotherapy,the patients were divided into the increased group(31 cases)and the decreased group(54 cases).The correlation between the serum ferritin level and the pathological classification,tumor stage and PD-L1 expression level of patients was analyzed,and the DCRs and survival periods of patients with serum ferritin level≥300 ng/mL and<300 ng/mL before treatment were compared,as well as the increased group and the decreased group.The survival curve of patients in each group was analyzed by the Kaplan-Meier.Results Patients with tumor stage Ⅰ to Ⅲ showed significantly lower serum ferritin levels than patients with tumor stage Ⅳ(P<0.05).The DCR,3-year survival rate and progression-free survival(PFS)of patients with serum ferritin level≥300 ng/mL were significantly lower/shorter than those of patients with serum ferritin level<300 ng/mL(P<0.05).The DCR,3-year survival rate and PFS of patients in the increased group were significantly lower/shorter than those in the decreased group(P<0.05).Conclusion The increased serum ferritin level in lung cancer patients before treatment and the further increase after treatment are significantly associated with the reduced efficacy of immunotherapy and the shortened survival period.The combination of baseline serum ferritin levels before treatment and dynamic changes after treatment may serve as a clinical biomarker panel for predicting immunotherapy outcomes.
2.Protective mechanism of salidroside on retinal ganglion cells in diabetic rats

Objective To explore the protective effect of salidroside on retinal ganglion cells(RGC)in diabetic rats and its possible molecular mechanism.Methods Forty-five male SD rats were selected and randomly divided into the control group,the model group and the salidroside group,with 15 rats in each group.Diabetes models of rats were established in both the model group and the salidroside group.After successful modeling,rats in the salidroside group were given 50 mg/kg salidroside by gavage daily for 12 consecutive weeks,while rats in the model group received no further treatment.Rats in the control group were given the same amount of normal saline by gavage for 12 consecutive weeks.The fasting blood glucose of rats was detected every 4 weeks.After the experiment,the survival rate of rats in each group was calculated.Pathological HE staining was used to observe the pathological changes of retinal tissue in rats,and RGC counting was performed.The contents of reactive oxygen species(ROS),superoxide dismutase(SOD),malondialdehyde(MAD)and catalase(CAT)in the retinal tissue of rats were detected by ELISA.Western blot was used to detect the expressions of heme oxygenase-1(HO-1),Kelch-like ECH associated protein 1(Keap1),and nuclear factor E2-related factor 2(Nrf2)in the KEAP1-NrF2/HO-1 pathway.Results There was no statistically significant difference in the survival rate among the three groups of rats(P>0.05).The fasting blood glucose levels of rats in the control group fluctuated within the normal range at each time point.The fasting blood glucose levels of rats in the model group and the salidroside group were consistently higher than those in the control group(P<0.05).The fasting blood glucose level of rats in the salidroside group gradually decreased over time(P<0.05).Compared with the control group,the retinal thickness of rats in the model group became thinner,the cell arrangement showed obvious disordered phenomena,and there was obvious edema in the inner and outer nuclear layers and vacuole phenomena.The pathological changes of retinal tissue of rats in the salidroside group were improved compared with those in the model group.Compared with the control group,the contents of ROS and MAD and the protein expressions of Keap1 and Nrf2 in the retinal tissues of rats in the model group were significantly higher(P<0.05),while the contents of CAT and SOD,the number of RGC and the protein expression of HO-1 were significantly lower/less(P<0.05).After intervention with salidroside,the contents of each oxidative stress factor,the number of RGC,and the protein expressions of Keap1 and HO-1 of rats in the salidroside group were significantly reversed compared with the model group(P<0.05).Conclusion Intervention with salidroside can alleviate the oxidative stress response in the retina of diabetic rats and relieve RGC injury,which may exert its effect through the Keap1-Nrf2/HO-1 signaling pathway.
3.Mechanism of action of mitochondrial DNA in alveolar epithelial type Ⅱ cells in silicotic fibrosis

Objective To explore the mechanism of action of mitochondrial DNA(mtDNA)of alveolar epithelial type Ⅱ cells(AECⅡ)in silicotic fibrosis.Methods This study was divided into the animal experiment and cell experiment.Animal experiment:twenty-eight C57BL/6 mice were randomly divided into the saline group(n=14)and the silicosis model group(n=14),which were given 50 μL of sterile saline and 50 μL of 50 mg/mL of SiO2 suspension by tracheal perfusion respectively.HE staining and Masson staining were performed to observe the histopathological changes of mouse lung tissues after sampling with the exposure of 28 days and 56 days,and Western blot was used to detect the expression levels of functional proteins of pulmonary surfactant protein-D(SP-D),and Krebs von den Lungen-6(KL-6),and fibrotic protein of transforming growth factor-β1(TGF-β1)in mouse lung tissues.Cell experiment:AECⅡ cell line of A549 was exposed to different concentrations of SiO2 suspension.CCK-8 was used to detect cell viability,DAPI staining was used to observe cell apoptosis,Western blot was used to detect related protein expression,JC-1 was used to detect the changes of mitochondrial membrane potential,and RT-qPCR was used to detect mtDNA levels in whole cells,mitochondria,cytoplasm and supernatant.The expression levels of related proteins in AECⅡ were detected by Western blot after cleared mtDNA by DnaseⅠ.Results Histopathological examination of lung tissues showed that SiO2 exposure led to damage in mice lung tissues,with infiltration of inflammatory cell,silica nodules,and a significant increase of collagen fibers.After SiO2 exposure,the protein expressions of SP-D,KL-6 and TGF-β1 in lung tissues were up-regulated(P<0.05).DAPI staining and CCK-8 results showed a reduced activity and impaired apoptosis of AECⅡ after treated with 400 and 800 μg/mL SiO2.JC-1 assay results demonstrated that exposure to 800 μg/mL SiO2 caused a decrease of mitochondrial membrane potential in AECⅡ(P<0.05).RT-qPCR results showed that 800 μg/mL SiO2 exposure led to increased mtDNA levels in the cytoplasm and supernatant of AECⅡ(P<0.05).Western blot results showed that 800 μg/mL SiO2 exposure caused an increase in the levels of SP-D,KL-6,and TGF-β1 proteins,while which were ameliorated by Dnase Ⅰ treatment.Conclusion SiO2 exposure results in the escape and release of AECⅡ mtDNA and dysfunction of AECⅡ,thereby leading to the development of pulmonary fibrosis.
4.Regulatory effects of miR-205 on proliferation,migration,and epithelial-mesenchymal transition in colon cancer cells

Objective To investigate the expression of microRNA-205(miR-205)in colon cancer and its regulatory effects on the proliferation,migration,and epithelial-mesenchymal transition(EMT)of colon cancer cells.Methods The expression levels of miR-205 in colon cancer tissues,adjacent normal tissues,and colon cancer cell HT-29 were detected by qRT-PCR.HT-29 cells were divided into three groups,cells transfected with miR-205 mimics were named the miR-205 mimic group,cells transfected with miR-205 inhibitors were named the miR-205 inhibitor group,and cells treated only with Lipo2000 transfection reagent were named the Buffer group.The proliferative activity of cells in each group was detected by CCK-8 assay and EDU assay;the migration invasion ability of cells was detected by Transwell assay;the expressions of EMT-related proteins N-cadherin,Vimentin,and E-cadherin in each group were detected by Western blot.Results Compared with adjacent normal tissues,the relative expression level of miR-205 in colon cancer tissues was significantly down-regulated(P<0.05).Compared with the Buffer group,the relative expression level of miR-205 in the cells of the miR-205 mimic group was up-regulated,while that in the miR-205 inhibitor group was down-regulated,the differences were statistically significant(P<0.05).Overexpression of miR-205 significantly inhibited the proliferation activity and migration ability of colon cancer cells(P<0.05),while inhibition of miR-205 promoted the proliferation activity and migration ability of colon cancer cells(P<0.05).Compared with the Buffer group,the levels of N-cadherin and Vimentin were down-regulated and the level of E-cadherin was up-regulated in the cells of the miR-205 mimic group,while the levels of N-cadherin and Vimentin were up-regulated and the level of E-cadherin was down-regulated in the cells of the miR-205 inhibitor group,the differences were all statistically significant(P<0.05).Conclusion The expression of miR-205 in colon cancer tissues is significantly decreased,and miR-205 exerts its tumor-suppressive effects by downregulating the proliferation,migration,and EMT of colon cancer cells.

6.Application of transumbilical single-port laparoscopic technique in treatment of complex appendicitis in children

Objective To investigate the application effect of transumbilical single-port laparoscopic technique in the treatment of complex appendicitis in children.Methods A total of 106 children with complex appendicitis admitted to our hospital from January 2021 to August 2023 were selected as the research subjects and randomly divided into the observation group(53 cases)and the control group(53 cases).Transumbilical single-port laparoscopic-assisted appendectomy was performed in children of the observation group,and open appendectomy was performed in children of the control group.The perioperative indexes,Alvarado score,serum inflammation indexes and postoperative complications of children were observed and compared between the two groups.Results The intraoperative blood loss,length of hospital stay,time of the first postoperative exhaust,time of the first postoperative eating,and time of the first postoperative defecation of children in the observation group were significantly less/shorter than those in the control group(P<0.05);while there was no statistically significant difference in operation time of children between the two groups(P>0.05).There was no statistically significant difference in Alvarado total score,symptom dimension score,sign dimension score or laboratory index dimension score of children before and 3 days after surgery between the two groups(P>0.05).The serum levels of serum interleukin-6(IL-6),C-reactive protein(CRP)and procalcitonin(PCT)of children 48 hours after surgery in the observation group were significantly lower than those in the control group(P<0.05).The incidence of postoperative complications in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Compared with the traditional open appendectomy,transumbilical single-port laparoscopic-assisted appendectomy for the treatment of complex appendicitis in children has less trauma,faster postoperative recovery and fewer complications.
7.Multimodal MRI features of cerebral small vessel disease combined with type 2 diabetes mellitus

Objective To analyze the imaging features of cerebral small vessel disease in patients with type 2 diabetes mellitus by multimodal MRI.Methods The clinical data of 160 patients with cerebral small vessel disease admitted to our hospital from January to December 2020 were retrospectively analyzed.According to whether they were complicated with type 2 diabetes mellitus,they were divided into the diabetic group and the non-diabetic group,with 80 cases in each group.Both groups underwent multimodal MRI scans.And the severity of lacunar infarction,the severity of subcortical and periventricular white matter lesions,white matter integral and cerebral microbleeds of patients in the two groups were compared.Results The severity of lacunar infarction(χ2=34.076,P=0.001),subcortical white matter lesions(χ2=25.000,P=0.001),periventricular white matter lesions(χ2=22.895,P=0.001)and white matter integral(t=12.370,P=0.001)of patients in the diabetic group were significantly higher than those in the non-diabetic group.No cerebral microbleeds were detected in either group of patients.Conclusion Patients with cerebral small vessel disease and type 2 diabetes mellitus show characteristic multimodal MRI changes.The increase in the number of lacunar infarction lesions and the aggravation of white matter lesions can be used as the characteristic imaging basis for the diagnosis of type 2 diabetes mellitus related cerebral small vessel disease.
8.The improvement effect and influencing factors of microsurgical varicocelectomy on scrotal pain

Objective To investigate the improvement effect of microsurgical varicocelectomy with vein ligation on scrotal pain and the related factors affecting the improvement of postoperative pain.Methods A retrospective analysis was conducted on the clinical data of 110 patients who underwent microsurgical inguinal varicocelectomy in Central Theater Command General Hospital from November 2021 to June 2023.The degree of pain before and after the operation was evaluated by Numerical Rating Scale(NRS).The pain relief rate,the occurrence of postoperative complications and the overall satisfaction of patients were evaluated.The semen and hormone levels of the patients before and after the operation were analyzed.Univariate and multivariate linear regression analyses were performed to investigate the relationship between clinical variables and postoperative 7-day NRS score.Results The average number of ligated veins during the operation was(6.60±3.76);postoperative satisfaction of patients was(4.23±1.08)points;complications occurred in 7 patients after the operation;after 3 months of follow-up,the overall pain relief rate was 92.73%(102/110).Compared with those before the operation,the NRS score of the patients significantly decreased 7 days after the operation,and the levels of total sperm motility,prolactin and testosterone increased 3 months after the operation,while the levels of serum follicle-stimulating hormone(FSH)and luteinizing hormone(LH)decreased,the differences were statistically significant(P<0.05).The univariate and multivariate linear regression analyses showed that the Dubin-Amelar classification of varicocele(P=0.045),operation time(P=0.049),and number of ligated veins(P=0.024)were significantly correlated with the change of postoperative 7-day NRS score.Patients with 1 to 4,5 to 10,and>10 intraoperative ligation veins were analyzed,and pain relief rates 3 months after the operation were 77.78%(28/36),100%(58/58),and 100%(16/16),respectively,with statistically significant difference(χ2=17.732,P<0.001).Conclusion Microsurgical varicocelectomy can effectively relieve scrotal pain and improve semen quality.Moreover,the relief of postoperative pain is related to the Dubin-Amelar classification of varicocele,the sufficient number of ligated spermatic veins,and the surgical experience of the surgeon.
9.Application of remimazolam combined with nalbuphine anesthesia in painless gastroscopy

Objective To explore the anesthetic effects of remimazolam combined with nalbuphine on patients undergoing painless gastroscopy.Methods A total of 120 patients who underwent painless gastroscopy in Handan Central Hospital from August 2019 to May 2021 were selected and randomly divided into the observation group(60 cases)and the control group(60 cases).Patients in the observation group anesthetized with remimazolam and nalbuphine,while patients in the control group anesthetized with propofol and alfentanil.The vital signs,sedative effect,recovery quality,cognitive function,and adverse reactions of patients were compared between the two groups.Results The heart rate(HR),respiratory rate(RR),and the level of pulse oxygen saturation(SpO2)of patients in the observation group were significantly higher than those in the control group(P<0.05).Five minutes after the examination,the Narcotrend score of patients in the observation group was significantly lower than that in the control group(P<0.05).The time from the last administration to awakening,the time from gastroscopy removal to awakening,and the time of leaving the anesthesia recovery room of patients in the observation group were significantly shorter than those in the control group(P<0.05).Five minutes after the examination,the score of the mini-mental state examination(MMSE)of patients in the observation group was significantly higher than that in the control group(P<0.05),while the incidence of adverse reactions was significantly lower than that in the control group(P<0.05).Conclusion The application of remimazolam combined with nalbuphine anesthesia in painless gastroscopy has a relatively ideal effect,which can effectively reduce the adverse reactions of patients and promote rapid postoperative recovery.
10.Application of 3D printing technology in bone graft fusion surgery for cervical spondylotic myelopathy

Objective To explore the clinical effect of 3D-printed interbody fusion cage applied in anterior cervical decompression and bone graft fusion for the treatment of cervical spondylotic myelopathy.Methods A total of 100 patients with cervical spondylotic myelopathy who underwent anterior cervical fusion surgery in our hospital from June 2020 to June 2023 were selected as the research subjects,they were randomly divided into the 3D group and the control group according to the random number table method,with 50 cases in each group.Patients in the 3D group were implanted with a 3D-printed interbody fusion cage which was made of microporous metal materials,while patients in the control group were implanted with intervertebral fusion cage which was made of polyetheretherketone material.The surgery-related indicators,cervical imaging parameters,cervical spinal cord function,cervical axial function and surgical complications of patients between the two groups were compared.Results There was no statistically significant difference in the operation time,intraoperative blood loss or length of hospital stay of patients between the two groups(P>0.05).The height of the cervical fusion segments and the Cobb angle of the fusion segments of patients in both groups at each time point after the operation were significantly increased compared with those before the operation(P<0.05).The height of the cervical fusion segments and the Cobb angle of the fusion segments of patients 1 month,3 months,and 6 months after the operation in the 3D group were all greater than those in the control group(P>0.05).The subjective symptom score and the total score of the Japanese orthopaedic association(JOA)of patients 6 months after the operation in the 3D group were higher than those in the control group(P<0.05).The cervical axial function of patients 6 months after the operation in the 3D group was better than that in the control group(P<0.05).The incidence of surgical complications in the 3D group was lower than that in the control group(P<0.05).Conclusion The use of 3D-printed interbody fusion cage during the anterior cervical decompression and bone graft fusion for patients with cervical spondylotic myelopathy can significantly improve the clinical symptoms of patients,achieve better cervical axial function,and have fewer complications.
